Abstract

Heart failure is a major cause of morbidity and mortality worldwide. Based on data from the World Health Organization (WHO), one of the most non-communicable diseases in the world is cardiovascular disease, whose spectrum is heart failure. To diagnose heart failure, a clinical history, physical examination and investigations are required. This study aims to determine the characteristics of heart failure patients at the heart polyclinic of RSUD Dr. M. Haulussy Ambon City March 2022. This research is a descriptive study. The number of samples in the study was determined using a categorical descriptive formula and the samples were taken using a simple consecutiv sampling technique. The data obtained were analyzed using the Statistical Package for the Social Science (SPSS). The results showed that from 74 samples of heart failure patients, 37 people aged >60 years (50%), 42 men (56.8%), 55 people had a history of coronary heart disease (74.3%). , as many as 21 people had a history of hypertension grade 1 (35.1%), as many as 7 people had a history of diabetes mellitus (9.5%), as many as 27 people took diuretic drugs (36.5%), as many as 34 people had a history of smoking (45.9%), as many as 31 people had a history of hospitalization (41.9%), as many as 26 people had a history of arrhythmias (35.1%), as many as 8 people had a history of valve abnormalities (10.8%).

Abstract

Waist circumference is a method used to determine central obesity, which is a condition of excess fat accumulation in the abdominal area, resulting in abnormal waist circumference in women >80 cm and men >90 cm. This study aims to determine the difference in effectiveness between prone plank exercise and bicycle crunch exercise in reducing waist circumference in adolescents. The research method used a quasi-experimental pretest-posttest two-group design. The sample consisted of 24 participants, divided into Group I (prone plank exercise) and Group II (bicycle crunch exercise), with each group consisting of 12 participants. Both groups underwent exercise therapy for 4 weeks, with a frequency of 5 times per week, with an intensity of 30 seconds per set, repeated 3 times. The measurement tool used was a calibrated midline. The results showed that Group I (prone plank exercise) experienced a reduction in waist circumference of 5.75 cm, while Group II (bicycle crunch exercise) had a reduction of 7.91 cm, indicating that both exercise therapies have an effect on waist circumference reduction. The difference test between groups resulted in p>0.05 (p=0.747), meaning there is no significant difference between the two exercise types.

Abstract

Employee work motivation is a crucial element in determining the quality of public services, especially in the health sector. This study aims to analyze factors related to employee work motivation at the Health, Population Control, and Family Planning Office of Marowali Regency. This study uses a quantitative approach with a cross-sectional design. The sample consisted of 39 respondents selected through the purposive sampling method. The research instrument is in the form of a structured questionnaire, and data analysis is carried out bivariously using the chi-square test and Fisher's Exact Test if the test assumptions are not met. The results showed that the work environment did not have a significant relationship with work motivation (p = 0.129). In contrast, the benefits variable showed a significant relationship (p = 0.009), and workload also had a significant effect on work motivation (p = 0.004). These findings indicate that employees' work motivation is significantly influenced by benefits and workload factors, while the work environment does not have a significant impact. The practical implication of this study is the need for managerial attention to a fair incentive system and a proportionate distribution of workload to increase employee work motivation. Increasing work motivation is expected to encourage the efficiency and effectiveness of public services in the health sector. This research contributes to the development of evidence-based human resource policies in the regional health service sector.

Abstract

Background: Dysmenorrhea is a common menstrual pain experienced by adolescent girls, often interfering with daily activities. Curcuma xanthorrhiza contains curcuminoids with anti-inflammatory and analgesic properties, making it a potential non-pharmacological treatment for menstrual pain. A preliminary study in Batuwarno Village showed a high prevalence of dysmenorrhea among adolescents. Objective: To determine the effect of Curcuma xanthorrhiza consumption on the reduction of dysmenorrhea in adolescents in Batuwarno Village. Methods: This study used a pre-experimental design with a one-group pretest-posttest approach. The sample consisted of 24 adolescent girls with dysmenorrhea, selected using consecutive sampling. The intervention involved administering 200 ml of boiled Curcuma xanthorrhiza twice a day for one day. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Data were analyzed using the Wilcoxon test. Results: The average pain score before the intervention was 6.54, and after the intervention, it decreased to 1.50. The Wilcoxon test yielded a Z value of -4.448ᵇ, where the negative Z value indicates that the more the intervention is given, the more the experienced pain decreases. The resulting p-value was 0.000 (p < 0.05), indicating a significant effect of Curcuma xanthorrhiza administration on the reduction of dysmenorrhea. Conclusion: Curcuma xanthorrhiza is effective in reducing dysmenorrhea in adolescents and can be considered a non-pharmacological alternative for managing menstrual pain.

Abstract

Background: Pregnancy is the implantation of the fertilized egg and sperm in the endometrium, and can cause discomfort, one of which is nausea and vomiting, especially in the first trimester. This symptom is common and can cause decreased appetite, paleness, weakness and decreased body fluids. The prevalence of nausea and vomiting in Indonesia is 50-90% of pregnant women. This condition occurs in 60-80% of primigravida pregnant women, and 40-60% of multigravida pregnant women. Objective: This study aims to determine the effect of ginger administration on reducing nausea and vomiting in pregnant women in the first trimester. Method: The method used is a pre-experimental method with a one-group pretest-posttest approach. The sample consisted of 22 respondents who experienced nausea and vomiting at the Simo Community Health Center, Boyolali. The sampling technique used a non-probability method with a consecutive sampling technique. Data analysis used the Wilcoxon Signed Rank test. Assessment of nausea and vomiting using the Pregnancy Unique Quantification of Emesis (PUQE-24) before and after administration of ginger for 4 days. Results: The majority of respondents before the intervention experienced moderate nausea and vomiting (54.5%), while after the intervention, the majority experienced mild nausea and vomiting (59.1%), and those who did not experience nausea and vomiting (31.8%). The Wilcoxon signed rank test showed that nausea and vomiting before and after ginger administration resulted in a p value of 0.000 (<0.05). Conclusion: Ginger administration has a significant and effective effect on reducing the level of nausea and vomiting in pregnant women in the first trimester.

Abstract

Elderly individuals are a group vulnerable to sleep disorders, with over 50% of them reporting a decline in sleep quality. Sleep disturbances in the elderly can affect their quality of life, which in turn impacts their physical and mental health. One non-pharmacological intervention considered safe, simple, and effective for improving sleep quality in the elderly is foot soaking in warm water. This study aims to evaluate the effect of foot soaking in warm water on the sleep quality of elderly individuals in the working area of Slawi Health Center. The research design used was pre-experimental with a one-group pre-test post-test design. A total of 34 elderly participants were selected using total sampling technique. The intervention consisted of soaking feet in warm water at a temperature of 37°C–39°C for 10 minutes before sleep, conducted for three consecutive days. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. The data obtained were analyzed using Paired Sample T-test for normally distributed data. The results showed that the average sleep quality score before the intervention was 10.12, which decreased to 7.85 after the intervention. Statistical analysis showed a significant difference in the mean scores before and after the intervention with a significance value of 0.0001. This indicates that foot soaking in warm water significantly impacts the improvement of sleep quality in the elderly. This intervention can be recommended as an effective, practical, and easy-to-implement independent nursing action to improve sleep quality in the elderly. Based on these findings, foot soaking in warm water can be a good option to improve sleep quality in the elderly, especially in community healthcare services.

Abstract

Hemodialysis is a medical procedure used to correct blood biochemical abnormalities caused by impaired kidney function, with the aid of a hemodialysis machine. One of the most commonly used accesses in this procedure is the Arteriovenous Fistula (AVF), which is an anastomosis between the artery and vein in the arm or other parts of the body to facilitate the cannulation process. However, during the AVF cannulation procedure, patients often experience pain, which can cause discomfort and anxiety. Cold compresses are one of the non-pharmacological techniques that can be used to alleviate this pain. This study aims to examine the effect of cold compresses on reducing pain during AVF cannulation in hemodialysis patients. This research uses a Quasi-Experimental design with a Pretest-Posttest Control Group Design model, involving 116 hemodialysis patients at RSI Sultan Agung Semarang. Patients were divided into two groups: the intervention group, which received cold compresses, and the control group, which did not receive treatment. Pain intensity was measured before and after cannulation using a visual analog scale (VAS). The results of the study show that the administration of cold compresses significantly reduced pain intensity, with a p-value of 0.000 (< 0.05). In addition, there was a significant difference in effectiveness between the intervention group and the control group, with the same p-value. In conclusion, cold compresses proved to be an effective, simple, safe, and easy-to-apply non-pharmacological intervention to reduce pain in hemodialysis patients during the AVF cannulation procedure. Therefore, the use of cold compresses can be recommended as a method to improve the comfort of hemodialysis patients.

Abstract

This study aims to determine the effectiveness of progressive muscle relaxation techniques in reducing headache pain in hypertensive patients in the inpatient ward of Harapan Anda Hospital in Tegal City. Hypertension is often accompanied by complaints of headaches, which is a significant problem for patients. One non-pharmacological approach that can be used to alleviate headaches caused by hypertension is progressive muscle relaxation therapy. In this study, 55 respondents with hypertension and complaints of headache were involved. The research used a quasi-experimental design with a quantitative approach. Before and after the intervention, patients underwent progressive muscle relaxation therapy for three consecutive days, with a duration of fifteen minutes per session. Pain levels were assessed using a pain scale before and after the intervention. Data analysis was conducted using the Wilcoxon test. The results showed that 53 patients (96%) experienced a decrease in pain levels after the therapy. The average pain scale significantly decreased, with moderate pain levels reducing from 89.1% to 3.6%, while mild pain increased from 10.9% to 85.5%. The Wilcoxon test results indicated a p-value of 0.000 (p < 0.05), indicating that progressive muscle relaxation therapy is effective in reducing headache pain in hypertensive patients. This therapy has been shown to be a simple, safe method that can be applied both independently by patients and in healthcare services. Therefore, progressive muscle relaxation can be an effective alternative in managing headaches in hypertensive patients.

Abstract

This study explains the analysis of the relationship between nurses' knowledge and compliance in implementing fall risk prevention in the inpatient ward of RSI Sultan Agung Banjarbaru. Fall risk is one of the most common patient safety incidents in hospitals, with significant impacts on the physical, psychological, and financial conditions of patients. Data from RSI Sultan Agung Banjarbaru in 2024 recorded three fall incidents, while a preliminary survey showed that 40% of nurses had insufficient knowledge of fall risk prevention and 60% were not compliant with existing preventive procedures. This study aims to determine the relationship between nurses' knowledge and compliance in reducing patient fall risk. The study design used a quantitative approach with a cross-sectional method, involving 97 nurses selected through simple random sampling. The instruments used were questionnaires designed to measure nurses' knowledge and compliance. Data were analyzed using Somers’ D test. The results showed that the majority of nurses had good knowledge (87.6%) and high compliance (90.7%). Additionally, a very strong and significant relationship was found between nurses' knowledge and compliance in implementing fall risk prevention (Somers’ D = 0.844; p = 0.000). This indicates that good knowledge of fall risk prevention procedures is strongly related to nurses' compliance in applying preventive measures. Therefore, it is essential for hospitals to provide ongoing education and training to enhance patient safety. With improved knowledge and compliance among nurses, fall risks are expected to be minimized, and patient safety can be better maintained.

Abstract

Patient safety is a crucial aspect of hospital service quality, with one of the main indicators being the prevention of fall risks. Fall risks among patients often lead to serious injuries and impact the quality of care, making preventive measures essential. Nurses play a central role in implementing these preventive actions. However, the level of nurse compliance in following preventive procedures is often influenced by work motivation. This study aims to analyze the relationship between work motivation and nurse compliance in implementing fall risk prevention at the Sultan Agung Islamic Hospital in Banjarbaru. This quantitative study used a cross-sectional design, involving 97 inpatient nurses as respondents. The instruments used in this study included a 20-item Likert scale questionnaire to measure motivation and an observation sheet with 10 items to assess nurse compliance in applying fall risk prevention procedures. The data collected were analyzed using the Somers’ D test to determine the strength and direction of the relationship between the two variables. The results showed that the majority of nurses had high motivation (92.8%) and high compliance (90.7%) in implementing the prevention procedures. The Somers’ D test revealed a significant relationship between work motivation and nurse compliance in fall risk prevention (p = 0.003; d = 0.866), indicating that higher work motivation leads to higher compliance. The conclusion of this study is that work motivation is closely related to nurse compliance in fall risk prevention. Therefore, enhancing work motivation should be an important strategy for hospital management in developing a better patient safety culture.
