Abstract

This study is based on problems that often occur in patients at NU Mangir Hospital with the aim of this study as an effort to detect several possibilities that occur so that it becomes a solution to minimize existing symptoms. This study uses quantitative research with the type of data collection taken based on random sampling techniques from existing patients and then analyzed using statistics with the help of SPSS version 20.00. Infection in patients with Water Seal Drainage (WSD) is a serious problem that increases morbidity and mortality. This study aims to identify demographic and clinical factors that influence the incidence of infection in WSD patients at NU Mangir Hospital, Banyuwangi. The method used is a cross-sectional approach with purposive sampling of 24 respondents. The results showed that systemic infections occurred most often (45.8%), and age factors, length of WSD installation, leukocyte levels, and comorbidities were significantly related to the incidence of infection. Improvement in the quality of care is needed to reduce the risk of infection.

Abstract

Children are the precious successors of the nation, so parents are expected to educate, protect and nurture their children to have a creative, intelligent, healthy and productive future. Aspects of development that can be affected by stunting can be clearly seen in aspects of children's cognitive and socio-emotional development. In this study, sampling of early childhood aged 24-59 months in the Alak Health Centre Area, Kupang City. Quantitative Research Method with cross sectional method research design with the results there is a significant effect of stunting on socio-emotional development because the sig value. Fisher's Exact Test 0.000 <0.05 and there is a significant effect of stunting on children's cognitive development because the sig value. Fisher's Exact Test 0.000 <0.05. In this study there is a significant influence on socio-emotional and cognitive development in children who experience stunting conditions.

Abstract

Coffee is one of the most widely consumed beverages worldwide, including in Indonesia. Coffee consumption has become part of people's daily habits, whether as a companion for work, relaxation, or as part of social culture. However, this habit is often associated with the potential for increased blood pressure due to caffeine, which is a stimulant to the central nervous system and can affect the function of the heart and blood vessels. This study aims to analyze the relationship between the frequency of coffee consumption and systolic and diastolic blood pressure in adults. The research method used a quantitative approach with a correlational design. The study sample consisted of 100 adult respondents selected through a purposive sampling technique based on certain criteria, such as not currently taking antihypertensive medication and having no history of serious cardiovascular disease. Data collection was carried out in two stages. First, respondents completed a questionnaire containing information on the frequency and habits of coffee consumption, including the number of cups per day and the type of coffee consumed. Second, blood pressure was measured using a digital sphygmomanometer to obtain accurate systolic and diastolic blood pressure data. The results of the statistical analysis showed a significant positive correlation between the frequency of coffee consumption and increases in blood pressure, both systolic and diastolic. Specifically, respondents who consumed more than two cups of coffee per day tended to have higher blood pressure values compared to those who consumed one cup or less. This finding has important implications for health promotion efforts, particularly in educating the public about wise coffee consumption. Recommendations include limiting coffee intake, choosing coffee with low caffeine content, and balancing this with a healthy diet, regular physical activity, and stress management as part of a healthy lifestyle.

Abstract

Hypertension is a chronic disease characterized by persistently elevated blood pressure. This condition occurs when the arteries narrow, obstructing blood flow and increasing pressure on the vessel walls. As a result, the heart is forced to work harder to pump blood throughout the body. If this condition is not properly controlled, it can lead to serious complications such as damage to the blood vessels, heart, kidneys, and even the brain. Therefore, effective management of hypertension is crucial, not only with pharmacological treatment but also through non-pharmacological interventions such as regular exercise. One recommended form of light exercise is brisk walking, which involves brisk walking at moderate intensity. This exercise is known to reduce peripheral resistance when the body's muscles contract, which in turn helps control blood pressure and increase cardiac output. This case study aims to determine the effectiveness of brisk walking therapy on the risk of decreased cardiac output in hypertensive patients. The intervention was carried out on hypertensive patients through home visits three days a week, with one meeting each morning. Patients were given brisk walking therapy with a specific duration and intensity according to their condition. Evaluation was carried out by measuring blood pressure before and after the intervention using a standard observation sheet. After three interventions, there was a significant decrease in blood pressure in patients. This indicates a positive effect of brisk walking on the cardiovascular system of hypertensive patients. Brisk walking is an effective and easy-to-do non-pharmacological therapy to help lower blood pressure and reduce the risk of decreased cardiac output in hypertensive patients. It is hoped that this therapy can become part of a self-help intervention carried out continuously in the patient's home environment.

Abstract

Recurrent aphthous stomatitis (RAS) is one of the most common oral mucosal diseases, characterized by recurrent and painful ulcerations of the mobile or non-keratinized oral mucosa. These lesions are often found on the tongue, labial, and buccal mucosa. Clinically, RAS is divided into three types: minor, major, and herpetiform. The minor type is the most common and has smaller lesions, while the major type has larger lesions and often leaves scars. The herpetiform type is characterized by numerous small lesions appearing simultaneously. Factors influencing the immunological response in RAS include genetic predisposition, viral and bacterial infections, food allergies, vitamin and microelement deficiencies, systemic diseases, stress, and hormonal disorders. Several studies have shown that these factors can trigger or worsen the occurrence of RAS lesions. In addition, emotional and physical stress can contribute to increasing the frequency or duration of ulceration. A 36-year-old female patient presented with pain on the inside of the lower lip that had lasted for 10 days. An intraoral examination revealed a unilateral, yellowish-white ulcer on the inner lower lip surrounded by a distinct, reddish erythema. The lesion was approximately 2-3 mm in size, single in size, soft in consistency, and symptomatic. Based on these findings, the treatment plan includes a complete blood count (CBC), which measures various blood components, including red blood cells, white blood cells, and platelets. This examination is necessary to obtain further information regarding the cause of RAS in this patient. RAS is a multifactorial lesion that can occur in the oral mucosa. Supportive tests such as a complete blood count (CBC) are very useful in establishing the diagnosis and providing further guidance in planning appropriate therapy. Abstract

Worm infestation due to Soil-Transmitted Helminths (STH) infection remains a public health problem, especially among elementary school children who frequently interact with contaminated soil. This condition is a major concern because it can cause long-term health problems, particularly in terms of physical growth and development of children. This study aims to identify the presence of STH worm eggs and larvae in soil and fecal samples of children in Bekoso Village, Pasir Belengkong District, Paser Regency, East Kalimantan. This study used a descriptive approach with a cross-sectional design. The samples used consisted of 23 soil points taken from oil palm plantations with sandy and loose soil criteria, as well as 16 fecal samples from elementary school children. Fecal examination was carried out using the Kato-Katz method to detect worm eggs, while the soil was examined using the Baermann technique to detect worm larvae. The results showed that 25% of children's fecal samples were infected with worm eggs, with 12.5% containing hookworm eggs, 12.5% Taenia sp. eggs, and 6.25% Ascaris lumbricoides eggs. In soil samples, 60.87% tested positive for Strongyloides stercoralis larvae and 26.09% tested positive for Ascaris lumbricoides eggs. These findings indicate a high risk of STH infection in children in the area. Environmental factors such as soil moisture, soil texture suitable for worm breeding, and poor personal hygiene practices are suspected to be the main causes of the spread of infection. This study emphasizes the importance of education on clean and healthy living behaviors (PHBS) and improving environmental sanitation to reduce cases of worm infection, especially in endemic areas such as Bekoso Village. More comprehensive prevention efforts are urgently needed to address this problem and improve public health, especially among elementary school children.

Abstract

Breakfast is an important habit that plays a major role in meeting children's energy and nutritional needs. The energy obtained from breakfast is essential for daily activities, including learning and playing, enabling children to achieve optimal performance both physically and cognitively. Furthermore, breakfast habits also contribute to children's nutritional status, reflecting the balance between nutritional intake and needs. However, the reality shows that in Indonesia, approximately 40% of children still do not have breakfast before going to school, potentially affecting their nutritional status. This study aims to determine the relationship between breakfast habits and the nutritional status of students at Muhammadiyah Kayen Elementary School. The study used a quantitative approach with a cross-sectional design and utilized primary data. The study population was all 225 students in grades I to IV. Of these, 112 students were selected through a purposive sampling technique. Data were collected using a questionnaire to explore breakfast habits and anthropometric measurements of height and weight to determine nutritional status. Data analysis was performed using the Fisher exact test. The results showed a p-value of 0.742 (p > 0.05), indicating no significant relationship between breakfast habits and students' nutritional status. A total of 79.5% of students had good breakfast habits, while 20.5% were classified as adequate. Meanwhile, the students' nutritional status consisted of good nutrition (57.1%), overnutrition (19.7%), obesity (11.6%), malnutrition (8%), and undernutrition (3.6%). Thus, it can be concluded that breakfast habits are not significantly related to the nutritional status of students at the school.

Abstract

Postoperative Total Knee Replacement (TKR) patients often face various clinical problems, including quadriceps muscle weakness, limited range of motion of the knee joint, swelling, and pain that can have a significant impact on functional movement ability, independence of daily activities, and overall quality of life. The condition demands the role of the physiotherapist in providing appropriate and comprehensive interventions to facilitate the patient's recovery. One approach that can be done is to provide a combination of Neuromuscular Electrical Stimulation (NMES) modalities and exercise therapy. This study aims to evaluate the benefits of NMES and exercise therapy combination interventions in improving quadriceps muscle strength, knee joint range of motion, and functional mobility ability in post-TKR patients. The research design used a case study method involving three patients who were in phase 2 (motion phase), namely 3 days to 6 weeks after TKR surgery. Patients show complaints in the form of weakness of the quadriceps muscles, limited range of joint movement, and decreased knee function function. Physiotherapy interventions were given as many as 8 meeting sessions with a frequency of 2 times a week. The intervention program consisted of administering NMES to the quadriceps muscle and functional exercises such as sit to stand exercise, gait training, strengthening exercises using resistance bands in the ankle joint, and stair climbing. The results showed a significant increase in quadriceps muscle strength, an increase in the range of motion of the knee joint, and an improvement in functional movement ability after all intervention sessions were completed. These findings confirm that the combination of NMES with exercise therapy can be an effective rehabilitation strategy for post-TKR patients. The implementation of this method has the potential to speed up recovery, reduce dependence on others, and improve the patient's quality of life in a sustainable manner.

Abstract

One of the key indicators of nation’s health development is life expectancy, which reflects the overall quality of public health. As life expectancy increases, the prevalence of non-communicable diseases (NCDs) also tends to rise, with Type 2 Diabetes Mellitus (DM) being one of the most common. In Indonesia, the number of individuals with Type 2 DM has reached approximately 10.7 million, and this figure is projected to increase significantly to 21.3 million cases by 2030, posing a serious public health challenge. This study aimed to describe the process of clinical nutrition care based on the Nutrition Care Process (NCP) for a patient diagnosed with Radiculopathy accompanied by Non-Insulin-Dependent Diabetes Mellitus (without complications), Essential (Primary) Hypertension, Spondylosis, and Spinal Stenosis, through the application of a regulated diabetic diet (DM RG) during hospitalization. The case study was conducted in November 2024 at RSI Ayani Surabaya and involved systematic monitoring of food intake and clinical outcomes during treatment. Findings indicated that within three days of observation, the patient’s dietary intake reached the target goals, demonstrating effective implementation of the prescribed meal plan. From physical and clinical monitoring, a notable decrease in blood pressure was observed on the second and third days, suggesting improvements in cardiovascular response to dietary management. Additionally, the patient’s temperature, pulse, respiratory rate, and oxygen saturation remained within the normal range throughout the treatment period, further supporting the effectiveness of the intervention. These results emphasize the importance of structured nutrition care in managing patients with multiple comorbidities, particularly those with diabetes and hypertension, as dietary regulation plays a central role in stabilizing health conditions. Overall, the study demonstrates that the application of the NCP framework combined with consistent monitoring can lead to positive patient outcomes and highlights its potential as a model for improving clinical nutrition management in hospital settings.

Abstract

This study aims to systematically examine the influence of transformational leadership and organizational culture on nurses' performance in hospitals. The method used is a Systematic Literature Review (SLR) with a qualitative approach. The findings indicate that transformational leadership has a significant impact on improving nurses’ performance, particularly through enhanced work motivation, job satisfaction, loyalty, and team effectiveness. Transformational leaders inspire nurses by providing a vision, encouraging innovation, and fostering a supportive environment that aligns with their values and goals. As a result, nurses exhibit increased commitment to their work, higher engagement levels, and a proactive attitude toward their tasks, leading to better overall performance. Organizational culture also plays a crucial role, especially values such as collaboration, human orientation, and integrity, which have been shown to support a positive work environment and nurse productivity. A culture that prioritizes teamwork, shared responsibility, and mutual respect enables nurses to work more cohesively and efficiently. Hospitals with a supportive and inclusive culture experience reduced turnover rates and higher job satisfaction, as nurses feel valued and respected within the organization. Additionally, when the culture encourages continuous learning and professional development, nurses are more likely to stay updated on best practices and provide high-quality care to patients. However, differences in findings across studies suggest the need for contextual understanding and consistent internalization of cultural values. Factors such as geographical location, hospital size, and leadership styles may influence the degree of impact that transformational leadership and organizational culture have on nurse performance. Therefore, it is essential to adapt leadership strategies and cultural initiatives to the unique needs and challenges of each healthcare setting.
