Abstract

Cardiovascular dysfunction is a disorder of the blood vessels, such as hypertension, heart failure, and stroke. In 2016, cardiovascular disease was the leading cause of death worldwide, based on data from the World Health Organization (WHO). Coronary Heart Disease (CHD) is the leading cause of death, along with stroke, as shown by the Sample Registration System survey in 2014.This case involves a 64-year-old male, Mr. N, who presented to the emergency department of RSUD dr. Fauziah Bireuen with complaints of chest pain that developed gradually and worsened over the past month. The pain was located across the entire chest and radiated to the back and neck. The pain was described as severe, like being crushed by a heavy load, worsened during strenuous activities such as cutting grass and chopping wood, and relieved when the patient bent forward. The pain could occur at any time, with a Visual Analog Scale (VAS) score of 10 (severe pain). The patient also reported easy fatigue during heavy physical activity over the past month. Laboratory examination revealed elevated leukocytes, mean corpuscular volume (MCV), segmented neutrophils, troponin I, cholesterol, and triglycerides. The patient was diagnosed with extensive anterior ST-elevation myocardial infarction (STEMI) post successful fibrinolytic therapy.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by hyperglycemia resulting from impaired insulin secretion, insulin action, or both. Globally, DM continues to pose a growing health challenge, particularly type 2 DM, which is closely associated with insulin resistance. In this condition, body cells fail to respond effectively to insulin, so even though the pancreas initially produces sufficient or excessive amounts of insulin, its effectiveness gradually declines. Over time, pancreatic beta-cell function deteriorates, further exacerbating chronic hyperglycemia. This persistent condition may lead to long-term complications, including nephropathy, retinopathy, neuropathy, and cardiovascular diseases, through mechanisms such as oxidative stress, inflammation, and the formation of advanced glycation end products (AGEs). The prevalence of type 2 DM is expected to continue rising, including in Indonesia, highlighting the need for comprehensive management strategies. An important aspect of DM management is the assessment of health-related quality of life. Quality of life reflects an individual’s perception of their health status within social and cultural contexts. This can be evaluated using generic instruments such as the WHOQOL-BREF questionnaire. In this case report, the quality of life of a patient with type 2 DM was assessed using the WHOQOL-BREF to provide a comprehensive understanding of the disease’s impact on the patient’s daily life.

Abstract

Spinal anesthesia is one of the regional anesthesia methods that is often chosen in various surgical procedures, especially in the area of the lower extremities, pelvis, and lower abdomen. The most common complication encountered in this technique is the occurrence of hypotension, which can affect tissue perfusion and hemodynamic stability of the patient. This study aims to examine the incidence of hypotension in spinal anesthesia, including mechanisms, risk factors, and management. The method used in this study is literature review with a scientific source search process conducted through various databases PUBMED, Sciencedirect, Researchgate, and Google Scholar with a publication year range of 2016 to 2026. The results showed that hypotension arises as a consequence of an obstacle in the sympathetic nervous system, which provokes dilation of blood vessels as well as reduced venous return. Risk factors include age, body mass index, patient position, block height, duration of surgery, fluid status, Injection Point, and type of anesthetic drug used. Management is carried out by administering fluids, vasopressors, as well as strict hemodynamic monitoring. A good understanding of the risk factors and mechanisms of hypotension is expected to improve patient safety during spinal anesthesia..

Abstract

Drug availability at community health centers is an important indicator of health service quality. Drug shortages or excesses may affect service effectiveness and budget efficiency. This study aimed to determine the level of drug availability at the South City Community Health Center and the Piloloda'a Community Health Center in 2024 based on compliance with the formulary, demand, receipt, and drug availability categories. This study used a descriptive analytical method with a cross-sectional approach. Data were obtained retrospectively from the 2024 Drug Use Report and Request Sheet (LPLPO). Data analysis was conducted by calculating the percentage of compliance with the formulary, demand, and receipt, and by determining drug availability levels using the Indonesian Ministry of Health (2010) formula and the classification of Carolien et al. (2017). The results showed that formulary compliance was 82% at the South City Community Health Center and 67% at the Piloloda'a Community Health Center, both below the 95% standard. Drug demand compliance scores were 151% and 199%, exceeding the 100–120% standard, while drug receipt compliance scores were 71% and 56%, below the 100% standard. Drug availability categories varied from adequate and insufficient to excess stock, with most drug items classified as insufficient stock. In conclusion, drug management at both community health centers has not been fully optimal. Improved coordination between community health centers and pharmaceutical facilities is needed to maintain stable drug availability and support service needs.

Abstract

This community service activity aimed to improve students’ knowledge and awareness regarding the dangers of vaping, free sex, and drug abuse among adolescents. The activity was conducted at SMAN 3 Lamongan on April 13, 2026, involving 28 students. The implementation method consisted of health counseling through material presentation using presentation media, interactive discussions, and evaluation using pretest and posttest methods. The materials covered the dangers of vaping and smoking, risky sexual behavior, and drug abuse. The results showed an increase in students’ knowledge after the counseling session. The average pretest score was 84.24%, which increased to 98.57% in the posttest. The Wilcoxon test result showed a significance value of 0.000 (<0.05), indicating a significant difference between pretest and posttest scores. This demonstrates that health education was effective in improving students’ knowledge related to healthy lifestyles and prevention of risky behaviors. Therefore, this activity is expected to serve as a promotive and preventive effort in shaping a healthy young generation that is aware of the importance of maintaining personal health

Abstract

Entamoeba histolytica is an intestinal protozoan that can cause diarrhea through fecal–oral transmission due to the consumption of food or water contaminated with parasitic cysts. Based on data from the Badan Pusat Statistik in 2017–2019, Simpang Keramat District, particularly Paya Leupah Village, still had limited access to clean water and low ownership of permanent sanitary latrines, which may increase the risk of environmental contamination. This study aimed to analyze the characteristics of dug wells and the risk factors associated with Entamoeba histolytica contamination in community well water. An observational analytic study with a cross-sectional design was conducted on 50 dug wells selected using purposive sampling. Data were obtained through field observations and laboratory examinations of well water samples. The results showed that most wells had distances to septic tanks that did not meet health standards (58%) and inadequate well floor conditions (82%), while the majority of wells had depths that fulfilled the recommended standard (76%). All well water samples tested negative for Entamoeba histolytica cysts; therefore, the association between risk factors and contamination could not be determined.

Abstract

Background: Infectious diseases caused by pathogenic bacteria, including Staphylococcus aureus and Escherichia coli, pose significant public health challenges, necessitating the exploration of alternative plant-based therapies. Objective: This study aimed to evaluate the antibacterial efficacy of Talu Dagang (Austroeupatorium inulifolium) leaf extracts and fractions and to identify the most bioactive subfraction through preparative isolation. Methods: Dried leaves were macerated with ethanol and partitioned using n-hexane and ethyl acetate. Antibacterial activity was assessed by agar well diffusion (MIC) and dilution (MBC) methods. The most active fraction was further fractionated by preparative thin-layer chromatography (PTLC), and subfractions were screened for antibacterial activity. Results: The ethyl acetate fraction showed the highest antibacterial activity, with an MIC of 5 mg/mL and MBC of 2.5 mg/mL against S. aureus. PTLC yielded ten subfractions (refined to seven), with subfraction 6 exhibiting the greatest potency (inhibition zone 8.6 mm at 500 mcg/mL) against S. aureus. Activity against E. coli was limited across all preparations. Conclusion: Talu Dagang leaf extracts, particularly the ethyl acetate fraction and its subfraction 6, possess significant antibacterial properties supporting the plant’s traditional medicinal use and potential development as a natural antimicrobial agent.

Abstract

Antimicrobial resistance is a persistent threat to hospital care, particularly when empirical therapy relies on broad-spectrum antibiotics without continuous evaluation of local use and susceptibility patterns. This literature review aimed to synthesize evidence from four Indonesian hospital-based studies regarding antibiotic utilization, rationality assessment, and bacterial resistance profiles. A structured narrative review was conducted using four selected articles provided by the author. Data were extracted for study design, setting, population, antibiotic evaluation method, dominant antibiotic classes, rationality indicators, resistance profile, and stewardship implications. The four studies were descriptive and hospital-based, using retrospective records, concurrent observation, ATC/DDD with DU 90%, qualitative rationality criteria, and antibiogram data. The synthesis showed a consistent concentration of antibiotic use in broad-spectrum groups, especially third-generation cephalosporins, penicillins, quinolones, and macrolides. In a Bandung public hospital, total antibiotic consumption reached 95,719.01 DDD, with penicillins, cephalosporins, quinolones, macrolides, and sulfonamides included in the DU 90% segment. In intensive care, ceftriaxone was the most frequently used antibiotic and most rationality indicators were appropriate, although clinically significant drug interactions were still identified. In pediatric acute respiratory infection inpatients, cefotaxime and ceftriaxone dominated empirical therapy. Resistance mapping in Denpasar highlighted relevant Gram-positive and Gram-negative pathogens and recommended antibiotics according to susceptibility levels. Overall, the reviewed evidence supports an integrated antimicrobial stewardship model combining ATC/DDD-DU 90% surveillance, qualitative rationality evaluation, antibiogram-based empirical guidance, and periodic feedback to prescribers.

Abstract

Speech and language are often mistaken to be synonymous. Speech is the articulation and production of speech sounds within the mouth, whereas language encompasses comprehension, processing and production of communication. Speech delay can be caused by hearing loss, brain disorders (such as mental retardation, specific receptive and/or expressive language disorders), autism, or disorders of the oral organs that make it difficult for a child to pronounce words (known as articulation disorders). To diagnose the cause of speech delay, a thorough examination by a doctor is necessary, which sometimes requires a multidisciplinary approach involving a pediatrician, an ENT specialist, and a child psychologist or psychiatrist. The developmental and growth stages are sensitive periods in a child’s life. This term “sensitive period” is a more nuanced way of referring to the critical periods in a child’s growth and development. Language disorders, particularly speech delay, are a common developmental issue. Several studies in various countries indicate that approximately 6 to 8% of preschoolers have speech delay. Early detection and intervention can improve emotional, social, and cognitive functioning, thereby improving outcomes. If a speech delay is suspected in a child, parents should be informed, and the child should be referred immediately to a speech-language pathologist and an audiologist.

Abstract

The percentage of newborns aged 0–6 months in Aceh Besar who received exclusive breastfeeding in 2022 was 55.9%, still far below the national target of 80%. Perception, knowledge, education, and employment are examples of various internal elements possessed by a woman that influence the effectiveness of exclusive breastfeeding to her child. Meanwhile, the mother's external environment consists of the support she receives from her partner and family members, as well as assistance she receives from medical personnel. This study aims to collect data from the Darussalam Community Health Center (Puskesmas) to draw conclusions about the correlation between family support and the success rate of exclusive breastfeeding. This study used a quantitative cross-sectional design. The study population was 82 people. This study excluded some individuals due to several challenges that arose during its implementation. The questionnaire used was the Guttman scale, which provides twenty answer options: yes or no. The chi-square test was chosen as the data analysis method. This study was conducted from April 14, 2025, to May 16, 2025. The findings indicate a correlation between the success of exclusive breastfeeding in the Darussalam Community Health Center (Puskesmas) and the practice of exclusive breastfeeding. This variable is associated with other variables such as instrumental support, emotional support, evaluation support, information support, and family support. The effectiveness of exclusive breastfeeding depends on the study site's ability to conduct relevant monitoring and evaluation, improve health and education, and encourage family involvement.
