Abstract

Whitening creams that supposedly contain mercury which is added with the aim to reduce freckles on the skin’s surface, but it gives a negative impact, for it was conducted analysis of the presence of mercury compounds qualitatively and quantitatively. This research is purely experimental research with the aim of identifying the content of mercury (Hg) is the bleach cream face on brand X circulating in Indonesia are on the label does not have the composition, number and batch number POM and determine levels of mercury (Hg) is the bleach cream face on brand X. Qualitative test using three reagents that NaOH, HCl, KI whereas quantitative testing using atomic absorption  spectrophotometry (SSA).The result of the research that brand X contains mercury (Hg). Brand X code A contains 2718,1273 bpm mercury (Hg) and code B contains 214,1661 bpm. Quantitative analysis results calculates using concentration obtained from a linear regression of whitening face cream brand X Code A contains 3239.9117 bpm mercury (Hg) and Code B contains 858,6798 bpm mercury (Hg) in pot.

Abstract

Drug management is a set of optimally conducted activities related to the aspects of drug planning, procurement, storing, distribution, and extermination. This was done to achieve the proper drug amount, types, supply, maintenance, and medication. Earlier observstion revealed problems in managing pharmaceutical stock and consumables, where some of the stocks are empty. This qualitative descriptive research focus on identifying pharmaceutical logistic stock and health supplies comprising drug planning, procurement, storing, distribution, and extermination in the pharmaceutical unit of Gorontalo. The primary data were obtained from questionnaires based on technical guidance by the health ministry while the secondary were obtained from related documents. Based on the findings, the pharmaceutical stock planning and storing process at the pharmacy unit did not meet the health ministry standards, which is opposite to the procurement, distribution, and extermination process.

Abstract

Chronic kidney failure is a condition where there is a gradual decline in kidney function. Chronic kidney disease is often followed by complications of anemia. The use of erythropoietin therapy in chronic renal failure patients undergoing hemodialysis requires no small amount of money. This affects the quality of life (utility). The purpose of this study is to measure costs and utility values using the EQ-5D-5L (EuroQol-Five Dimensions) instrument, VAS (Visual Analog Scale), and costs based on the patient's perspective at Prof.Aloei Saboe Hospital. This study is a prospective cross-sectional approach. Data was collected using the EQ-5D-5L and EQ-VAS questionnaires. The subjects of the study were hemodialysis patients as many as 45 met the inclusion criteria. The results showed that the most widely used Erythropoietin therapy was single Hemapo. The results of the calculation of the estimated cost of each hemodialysis patient from the perspective of a patient for one month of therapy are Rp. 7,112,000. The average expenditure of patient transportation costs is Rp. 50,111. The average cost of food for the patient is Rp. 25,000.Utility value based on average Indonesian set value 0.673 (sufficiently feeling well)and VAS score 71.4 (health status is quite healthy).

Abstract

Hypertension and diabetes mellitus are degenerative diseases where various therapies are certainly used so that the effectiveness of each drug and the costs incurred need to be considered. The more treatments used, the more costs will be incurred.  Therefore, a pharmacoeconomic analysis needs to be carried out to assist decision making in performing therapy. This research was a non-experimental research with descriptive analysis design.  Data collection was retrospective by tracing previous documents sourced from medical records of patients with hypertension and Type 2 Diabetes Mellitus (DM) at the Outpatient Installation of Toto Kabila Regional General Hospital for the period of October to December 2020. Samples were taken by purposive sampling technique and then the data obtained were analyzed by calculating the direct medical costs and drug effectiveness. Data analysis from this study used Cost Effectiveness Analysis (CEA) and was continued in the form of ratio, namely Average Cost Effectiveness Ratio (ACER) or Incremental Cost Effectiveness ratio (ICER). The results showed that in hypertension, drug therapy used were Amlodipine (47.8%) and Candesartan (52.2%), while in Type 2 DM therapy were the combination of Metformin and Glimepiride (68.9%) and the combination of Insulin-Aspart and  Insulin Detemir (31.1%). The direct medical cost of hypertension therapy with Amlodipine was Rp. 184.921, and Candesartan was Rp. 158.159.  As for the Type 2 DM therapy, the cost of Metformin-Glimepiride combination was Rp.  168.253,226 and the combination of Insulin-Aspart and Insulin-Detemir of Rp. 1.077.259. The results of the cost effectiveness calculation show that Candesartan is more cost-effective than Amlodipine, while the combination of Metformin-Glimepirid is more cost-effective than the combination of Insulin-Aspart and Insulin-Detemir.

Abstract

National formulary is a list of medicines compiled by a national committee established by the Minister of Health based on cutting-edge, efficacious, safe, and affordable scientific evidence referred for drug use by national health insurance. This study aims to determine the evaluation of drug prescribing in the Hospital formulary. This retrospective study involved subjects qualified for the inclusion criteria i.e. 73 sheet recipes in August - December 2019. some prescribed drugs that are not listed in the hospital formulary in the following percentage: August (100%), September (60%), October (50%), November (81%), and December (65%) with the average of 71%. This indicates that the prescription does not conform to the hospital formulary regulation of 100%. In conclusion, there are some drugs prescribed by the doctor that is not conformed to the hospital formulary, which means tight supervision for the prescribing is required.

Abstract

Tuberculosis is an infectious disease caused by bacteria known as mycobacterium tuberculosis. In Indonesia, tuberculosis is the third common public health problem the prevalence of the disease in this country is among the highest in the world after China and India. Sufficient knowledge of diseases and medicines correlates with the result of therapy in general. Knowledge about drugs is important for patients since it helps them to consume drugs properly (which result in maximum therapy) and to avoid complications. The purpose of this descriptive study is to determine the level of knowledge of patients suffering from tuberculosis in the tuberculosis treatment program at Bulango Utara Puskesmas (Community Health Center) by a questionnaire. It applied a cross-sectional approach with primary data obtained from the result of a sample questionnaire totaling 24 patients taken by saturated sampling. The result of this present study reveal that the level knowledge of patients with tuberculosis in the area is still relatively low at 75%, while only 25% is classified as moderate.

Abstract

Low Quality Assurance in Pharmacy Award Ipilo Gorontalo include pharmaceutical, security, procurement, storage, and distribution or drug delivery, drug management, on a prescription drug services, drug information services, as well as drug development, medicinal materials and traditional medicine. The impact of the implementation of activities in the pharmacy pharmacy services Anugerah Ipilo errors may occur treatment (medication errors) in the service process. Quality Assurance (QA) is one dimension that is used in the measurement of quality of service, where the application of QA in pharmacies Ipilo Award aims to provide quality assurance of pharmaceutical products and the quality of services provided to patients. This research is descriptive, the source of primary data obtained from the questionnaires with QA standard indicator framework on pharmaceutical services in pharmacies Anugerah Ipilo. The assessment results then classified based on criteria appropriate, relatively appropriate and not appropriate. The results showed that the Quality Assurance (Quality Assurance) on the quality of the equipment and the safety assurance of Pharmaceutical Services in Pharmacy Award Ipilo there are seven questions (70%) had a very good criteria, three questions (30%) had a good criterion. Quality Assurance (Quality Assurance) on the Process of Pharmaceutical Services in Pharmacy Award Ipilo there are 4 questions (40%) had a very good criteria, one question (10%) had a pretty good and 5 questions (50%) had less good. Pharmacies merely focused on the administration and management of medication alone is not the overall pharmacy services, in addition because the pharmacy Anugerah Ipilo prefer the function of the economy (business) rather than its social function, which pharmacies are required to gain / profit in business.

Abstract

Herbal medicine is a medicine made from natural ingredients, either plants, animals, or minerals. Herbal medicines are widely used by the people of Indonesia to treat certain symptoms or diseases. Although this medicine is made from natural ingredients and has been used for a long time, it is necessary to know how to take safe herbal medicines to avoid side effects. This service aims to build the independence of the Taula'a Village community in terms of treatment using herbal medicines. Independent in terms of sources of herbal medicines, independent in terms of management, and independent in terms of their use. The methods used are lectures, education, and direct practice on how to cultivate medicinal plants in the yard, how to cultivate, and how to use herbal medicines. Evaluation of the success of community service based on increasing knowledge after education and being able to process herbal medicines with raw materials based on spices to improve health.

Abstract

Rosella flower is a plant with an antioxidant effect because it contains anthocyanins included in the flavonoid group. Therefore, it is essential to develop rosella flower extract in the form of soap preparations. This present work formulated and evaluated transparent solid soap preparations with 70% ethanol extract. This laboratory experimental research analyzed transparent solid soap preparations that were made of three formulas with different concentrations of active substances, namely FI (0.5%), FII (1%), FIII (2%). In addition, the evaluation of solid soap preparations involved organoleptic observations (smell, color, shape), pH test, foam height test, and irritation test on volunteers' skin. The results of quality inspection of organoleptic preparations indicated that the higher the concentration of the active substance in solid soap preparations, the less transparent the soap looks; the pH test produced 10.63-11.43; the initial foam height test resulted in 80 mm, and the final rest got 70 mm, 60 mm, and 50 mm; skin irritation tested to volunteers showed a negative reaction. The formulation in this study was formula 1, which produced the best transparent soap. It is concluded that FI, FII, and FIII soap with 70% ethanol extract of rosella flowers can be formulated as transparent solid soap.

Abstract

Chronic Kidney Disease (CKD) is a condition in which there is a decrease in kidney function due to the presence of chronic and irreversible renal parenchyma. Patients with chronic disease often require a combination of several drugs. The administration of drugs combination is generally expected to have a beneficial effect. However, in some cases, drug interactions resulted in ineffective treatment and the emergence of unexpected events. This research aimed to determine drug interactions in patients with chronic kidney disease who are undergoing hemodialysis in the Inpatient Installation of Dr. M.M Dunda Limboto Hospital. This research was observational using a cross-sectional design and retrospective data collection. The sampling technique used was a purposive sampling technique with a total of 51 patient who met the inclusion criteria during the July-Desember 2020 period. Drug interaction data analysis was based on the Drug Interaction Checker and Drug Interaction Facts. The results showed that the most widely used drugs in patients with chronic kidney disease at Dr. M.M Dunda Limboto Hospital were anti-hypertensi drugs (17%), vitamins and minerals (13.30%), and gastrointestinal (11.32%). Drugs interactions with pharmacodynamic mechanisms were 31 cases (53.45%), pharmacokinetics were 21 cases (36.21%), and 6 cases were unknown (10.34%). The level of clinical significance in patients with chronic kidney disease was 75.86% moderate, 13.80% minor, and 10.34% major.
