Abstract

Patient Mrs. N, a 48 year old woman, presented to the Lhoksukon Community Health Center’s geriatric clinic with a complaint of itching that began approximately 5 months ago and has worsened over the past month. Lesions were found on her hands and feet. The initial symptoms appeared after the patient was exposed to floodwaters; subsequently, the symptoms worsened, particularly after washing or performing household chores. Primary data were obtained through self-reporting, physical examination, home visits, completion of the family folder, and completion of the patient’s medical record. Patient assessment was conducted based on a holistic diagnosis encompassing quantitative and qualitative evaluations of the initial, ongoing, and final stages of the visit. Interventions provided included pharmacological therapy and education regarding the causes of Irritant Contact Dermatitis, the importance of maintaining skin cleanliness and moisture, and efforts to avoid triggering factors such as exposure to detergents, irritants, and prolonged contact with water. Education was also provided to encourage the patient to adopt clean and healthy lifestyle behaviors to prevent recurrence and worsening of the patient’s skin condition.

Abstract

Type 2 diabetes mellitus is a metabolic disorder with a steadily increasing global prevalence. Regulating blood glucose levels, particularly postprandial glucose, represents a critical strategy for the prevention and management of this disease. The inhibition of $\alpha$-amylase and $\alpha$-glucosidase activities is well-documented to delay carbohydrate digestion and subsequent glucose absorption. In recent years, herbal teas have gained significant attention as natural sources of bioactive compounds with potential antidiabetic properties. This literature review aims to evaluate the potential of various herbal teas, derived from both Camellia sinensis and non-Camellia sinensis species, in inhibiting $\alpha$-amylase and $\alpha$-glucosidase activities, based on in vitro studies published between 2016 and 2026. Relevant literature was gathered through systematic searches across PubMed and Google Scholar databases and further screened according to predefined inclusion criteria. A total of 26 articles were analyzed to evaluate enzyme inhibitory activities, the responsible bioactive compounds, and their reported mechanisms of action. The findings indicate that white and green teas exhibit potent $\alpha$-glucosidase inhibitory activity, primarily attributed to their catechin content, such as epigallocatechin gallate (EGCG) and epicatechin gallate (ECG). Furthermore, black, oolong, and dark teas, along with various polyherbal formulations, also demonstrated significant enzyme inhibition. Generally, polyphenols and other phenolic compounds serve as the primary constituents contributing to this biological activity. These findings suggest that herbal teas have the potential to be developed as an adjuvant therapy in the management of type 2 diabetes mellitus, although further in vivo studies and clinical trials remain essential to confirm their efficacy and safety.

Abstract

The pharmaceutical industry requires effective production equipment to ensure product quality consistency and timely distribution. This study aimed to analyze the Availability component within the Overall Equipment Effectiveness (OEE) framework through downtime evaluation of two blister machine lines at PT XYZ during the January–December 2025 period. A quantitative descriptive approach was employed using historical production data, with analysis focused on Planned Downtime (PDT) and Unplanned Downtime (UPDT). Dominant losses were identified using Pareto analysis and further investigated through root cause analysis. The results showed that the annual average OEE of Blister A was 78.4%, while Blister B achieved 71.51%, indicating a performance gap of 6.9%. PDT in both lines was primarily dominated by Lunches & Breaks, Cleaning, Inspection, and Lubrication (CIL), and Change Batch. Meanwhile, UPDT in Blister A was mainly caused by Unplanned Stops (1,132 minutes) and Reprocess, whereas Blister B was dominated by Unplanned Stops (5,117 minutes) and Breakdown (2,015 minutes). The findings indicate that Availability losses were influenced not only by mechanical factors but also by managerial aspects, digital system integration, and suboptimal process parameter standardization. Improving OEE requires optimizing operator rotation, implementing Single-Minute Exchange of Die (SMED), strengthening condition-based maintenance, and applying Statistical Process Control (SPC) to maintain sustainable process stability. These findings provide a data-driven basis for optimizing production capacity, prioritizing maintenance strategies, and supporting continuous improvement in pharmaceutical packaging operations.

Abstract

Guava (Psidium guajava L.) is a plant widely used in traditional medicine because it contains various bioactive compounds, such as flavonoids, phenolic compounds, tannins, alkaloids, terpenoids, lycopene, and vitamin C. These compounds are known to have various pharmacological activities that have the potential to support health. This opinion article aims to review and analyze the pharmacological activities of guava based on research results published in the period 2020–2025. The method used is a literature study of 11 national and international scientific journals selected according to the research topic. Data were analyzed descriptively by comparing findings regarding the biological activities of various plant parts, especially the fruit, fruit peel, and leaves. The results show that guava has various pharmacological activities, including antioxidant, antibacterial, antidiabetic, anticancer, anticholesterol, antimicrobial, and adaptogenic. Antioxidant activity is the most frequently reported effect, which is related to the high content of flavonoids, phenolic compounds, and vitamin C as free radical scavengers. Furthermore, several studies have shown that guava bioactive compounds can inhibit the activity of α-amylase and α-glucosidase enzymes, inhibit the growth of various pathogenic bacteria, and suppress the proliferation of cancer cells through various biological mechanisms. These findings indicate that guava has promising prospects for development as a raw material for herbal medicines and as a supportive therapy for various diseases. However, most of the available scientific evidence still comes from in vitro, in silico, and preclinical studies and therefore cannot be fully generalized to humans. Therefore, further research is needed, especially clinical trials with more comprehensive designs, to ensure the effectiveness, safety, optimal dosage, and potential use of guava in clinical practice.

Abstract

The determination of the active pharmaceutical ingredient is an essential step in pharmaceutical quality control to ensure the safety, efficacy, and compliance of drug products with the standards established by the Indonesian Pharmacopoeia. One of the analytical methods commonly used to determine the content of acetylsalicylic acid (aspirin) is the acid-alkalimetric method using back titration. This experiment aimed to determine the acetylsalicylic acid content in aspirin tablets using the acid-alkalimetric method and to evaluate its compliance with the requirements of the Indonesian Pharmacopoeia, Sixth Edition. The analysis was carried out by reacting a 60 mg aspirin sample with an excess of 0.1 N sodium hydroxide (NaOH), followed by back titration of the excess base with standardized 0.1 N hydrochloric acid (HCl) using phenolphthalein as the indicator. The experimental results showed that the HCl volume required for the blank solution was 15.8 mL, while the sample required 7.0 mL. The calculated acetylsalicylic acid content was 158.54 mg, corresponding to 379.43 mg per tablet or 379.4% of the labeled claim. This result exceeded the acceptable range specified in the Indonesian Pharmacopoeia, Sixth Edition, which requires the active ingredient content to be between 90.0% and 110.0% of the labeled amount. The discrepancy was likely caused by experimental errors, including inaccurate burette readings, improper endpoint determination, errors in solution standardization, or calculation mistakes. Therefore, careful execution of each analytical step is essential to obtain accurate and reliable assay results.

Abstract

Samanea saman (Jacq.) Merr. leaves contain various secondary metabolites with potential biological activities; therefore, preliminary toxicity testing is necessary to evaluate their safety and bioactive potential. This study aimed to determine the Lethal Concentration 50 (LC₅₀) value of the 80% ethanol extract of Samanea saman leaves using probit analysis and to classify its toxicity level. An experimental study was conducted using the Brine Shrimp Lethality Test (BSLT) with Artemia salina Leach larvae as the test organism. The extract was tested at concentrations of 10, 100, 500, 1,000, and 5,000 ppm. Larval mortality was recorded after the incubation period, and the data were analyzed using probit analysis to determine the LC₅₀ value. The results showed that larval mortality increased with increasing extract concentration, reaching 18.42%, 23.33%, 45.83%, 70.83%, and 88.89%, respectively. Probit analysis revealed an LC₅₀ value of 301.72 mg/L (ppm). Based on the toxicity classification, this value falls into the moderately toxic category, indicating that the 80% ethanol extract of Samanea saman leaves exhibits toxic activity against Artemia salina larvae. These findings suggest that the extract has promising potential as a source of bioactive compounds and warrants further investigation, including the isolation and identification of active constituents, cytotoxicity evaluation, and safety assessment in more complex biological systems.

Abstract

Anemia and undernutrition frequently coexist in early childhood and may reinforce one another through inadequate nutrient intake, impaired erythropoiesis, infection, and reduced appetite. This study analyzed the association between nutritional status and anemia among children under five years of age at Abdul Manap Regional Hospital, Jambi. An analytical observational study with a retrospective case-control approach used secondary medical-record data from 2019–2023. Ninety-two children were included, comprising 46 anemia cases and 46 non-anemia controls. Nutritional status was assessed using weight-for-height indicators based on World Health Organization child growth standards. Univariate analysis described participant characteristics, and the Chi-square test with an odds ratio estimated the association between nutritional status and anemia. Most participants were aged 13–36 months (55.4%) and male (63.0%). Undernutrition predominated in the anemia group (56.5%), whereas normal nutritional status predominated among controls (63.0%). Nutritional status was significantly associated with anemia (p=0.002); undernourished children had 4.31 times the odds of anemia compared with children with normal or excess nutritional status (95% CI: 1.79–10.39). These findings support integrated nutritional assessment, caregiver education, and routine anemia screening for young children.

Abstract

Elderly female rice farmers are vulnerable to chronic low back pain (LBP) due to prolonged bending activities, repetitive movements, and physical degeneration. This study analyzed the effects and the comparative effectiveness of infrared (IR) therapy and William Flexion Exercises (WFE) in reducing low back pain among elderly rice farmers. This study used a quasi-experimental design with a pre-test post-test control group design. The sample consisted of 30 elderly female rice farmers in Wukirsawit Village, Karanganyar (selected via purposive sampling), who were divided into the IR Group (n=15) and the WFE Group (n=15) for 12 sessions over 4 weeks. The measuring instrument used was the Visual Analog Scale (VAS), and the data were tested using the Wilcoxon Signed Rank Test and the Mann-Whitney U Test. Both interventions were proven to significantly reduce pain intensity (p<0.001). However, the between-group test showed a significant difference (p = 0.027), with the WFE group demonstrating a higher pain reduction effectiveness compared to IR therapy. These findings indicate that WFE can be prioritized as a primary active intervention for elderly rice farmers with LBP, while IR therapy can be used as a complementary modality prior to exercise.

Abstract

Breast cancer is one of the leading causes of death in women in Indonesia, with a tendency for patients to become younger. Early detection through Breast Self-Examination (BSE) plays a crucial role in reducing the risk of delayed diagnosis, but young women's knowledge and attitudes regarding BSE remain low. Video media is considered an effective health promotion strategy for increasing knowledge and shaping attitudes. This study aims to analyze the effect of video media on knowledge and attitudes about BSE among female students at SMA Negeri 1 Labuhan Deli. The method used in this study was quantitative with a pre-experimental research design with a one-group pretest–posttest conducted at SMA Negeri 1 Labuhan Deli. The sample consisted of 45 female students in grades 10 and 11 selected through purposive sampling. Data were collected using a questionnaire. Before the intervention, the majority of respondents had sufficient knowledge (53.3%) and attitudes (80.0%). After the intervention, all respondents (100%) had good knowledge and 97.8% had good attitudes. The Wilcoxon test results showed a p-value of 0.000 for both variables, indicating a significant effect of video media on improving female students' knowledge and attitudes about BSE. Video media has proven effective as a health promotion medium to increase awareness of young women in early breast cancer detection.

Abstract

The expansion of modern retail including hypermarkets, supermarkets, and minimarkets into pharmaceutical distribution enhances public access for self-medication, yet presents significant regulatory challenges regarding patient safety and product quality. This study compares the regulatory frameworks of Indonesia (Peraturan BPOM No. 5/2026) and Singapore (Health Sciences Authority guidelines) on supervising pharmaceutical products in modern retail. While both countries aim to safeguard consumers by restricting retail sales to high-safety, non-prescription drugs in original packaging, their regulatory strategies differ fundamentally. Indonesia employs a behavior-centric approach, mandating certified health support personnel at points of sale and educational warning labels on limited over-the-counter drugs. However, this model faces operational challenges due to high retail employee turnover and variable health literacy. In contrast, Singapore adopts a product-centric approach through the General Sale List (GSL), embedding built-in safety into product specifications via strict limits on active ingredient dosages and package sizes. This strategy minimizes retailer operational burdens and reliance on consumer literacy. Furthermore, Singapore features a dynamic, evidence-based reclassification mechanism and regulates innovative distribution channels like smart vending machines, whereas Indonesia maintains a static classification system focused on physical stores. In conclusion, both personnel oversight at point-of-sale and product specification standardization from manufacturers contribute significantly to establishing a safe and efficient drug distribution system in modern retail environments.
