Abstract

Introduction: Interdental papilla is triangular soft tissues that fill the gaps between teeth andfunction as protective periodontal tissues and are important in terms of aesthetics, ifinadequate, they will form black triangles, which are blackish areas between teeth. Non-surgical treatment of black triangle is preferred by patients as it is painless. Injectable Platelet-Rich Fibrin (i-PRF) is a platelet concentrate derived from the patient's blood that does notcause immunological reactions and contains growth factors for remodeling the interdentalpapilla. Case Report: A 20-year-old female patient, systemically healthy, came to Prof.Soedomo UGM Dental Hospital with complaints that the gums of the middle part of her upperteeth had a blackish area and disturbed her confidence. An objective examination was carriedout including, gingival biotype, black triangle area, and radiographs, so it was concluded thatthe black triangle was classified as class I Nordland and Tarnow and interdental papillareconstruction to correct the black triangle could be planned. The patient refused surgery so i-PRF can be used as an alternative non-surgical treatment. The black triangle closed completelyafter the i-PRF injection was given. Discussion: I-PRF has the ability to accelerate cellproliferation and migration as well as angiogenesis so that it can modulate fibroblastregeneration in the interdental papilla area. Conclusion: The use of i-PRF can be an alternativenon-surgical option for the treatment of interdental papilla reconstruction.

Abstract

Introduction: Cephalometric analysis is used to help establish diagnosis and treatment plan in orthodontics. One of the cephalometric analysis is the Downs analysis which its standard values were measured in Caucasians. However, these values cannot be applied to other races and ethnicities because there are racial and ethnic variations. Hence, it is necessary to know the average value for Indonesians. Objective: This study aimed to evaluate Downs analysis on cephalograms at RSGM YARSI Jakarta, compare it with Downs’ norms and other ethnicities, and determine differences between gender. Materials and Methods: Measurements of 10 parameters of Downs analysis were carried out on 29 cephalograms. Data were tested statistically. Results: The mean of the facial angle are 85.17°±3.07; angle of convexity 3.62°±2.79; AB plane angle AB -2.65°±2.55; mandibular plane angle 29.82°±3,37; Y-axis 64.15°±3.34, occlusal plane angle 11°±2.86; interincisal angle 113.06°±7.64; LI-occlusal angle 28.41°±5.24; IMPA angle 10.24°±5.76; distance of UI-APog 8.05 mm±1.85. Discussion: The RSGM YARSI cephalometric value compared to the Downs’ norms shows that the chin towards the FHP is more retroposition, the face is more convex, the relationship of the lower and upper jaw to the NPog is more protrusive, the direction mandibular growth towards the FHP is more vertical, the direction of facial growth is more downward, occlusal growth towards the FHP is steeper, and the incisors are more protruded. The direction of growth in male is more vertical than in female, and there are differences in cephalometric values with other races. Conclusion: There is a significant difference between the Downs cephalometric values of RSGM YARSI and the Downs’ norms in all measurements. Between men and women at RSGM YARSI there are gender differences in the direction of growth. There are differences in measurement values with other races.

Abstract

Abstract Introduction: CRISPR (Clustered Regularly InterSpacedPalindromic Repeats) is a technology with demonstratedcapability in modifying, repairing, and substituting genecomponents (genome editing). The system relies on Cas9enzyme (CRISPR-associated protein-9 nuclease) and gRNA (guide ribonucleic acid). Various diseases tied to specificgenes in the human genome are linked to oral cavity conditions. Objective: A literature review was used in this study to explain the potential application of CRISPR/Cas9 technology as a therapy for oral diseases in the future.Discussion: The application of CRISPR/Cas9 could lead to the identification of gene impairments and the discovery of genes counteracting tumor-triggering ones responsible for oralcancer. Additionally, it has utility in cases of microorganism infection in the oral cavity, such as targeting the glycosyltransferase encoding gene in Streptococcus mutans.This interference disrupts EPS (extracellular polymericsubstance) matrix synthesis, impeding biofilm formation.Conclusion: CRISPR/Cas9's effectiveness in altering bacterialpopulations or faulty genes presents an opportunity to address intricate oral diseases, potentially revolutionizing dental care. However, despite its promise, clinical trials involving this technology remain at an early stage and are relatively obscure, especially within dentistry. Therefore, a comprehensive body of literature is essential to foster understanding and awarenessof this groundbreaking advancement. Sistem ini bergantung pada enzim Cas9 (protein-9 nuklease terkait CRISPR) dan gRNA (panduan asam ribonukleat). Berbagai penyakit yang berhubungan dengan gentertentu dalam genom manusia juga berhubungan dengan kondisi mulut. Tujuan: Tinjauan literatur digunakan dalampenelitian ini untuk menjelaskan potensi penerapan teknologiCRISPR/Cas9 sebagai terapi penyakit mulut di masa depan.Pembahasan: Penerapan CRISPR/Cas9 dapat mengarah padaidentifikasi kerusakan gen dan penemuan gen yang melawan gen pemicu tumor yang menyebabkan kanker mulut. Selain itu, memiliki kegunaan dalam kasus infeksi mikroorganisme dirongga mulut, seperti menargetkan gen penyandiglikosiltransferase pada Streptococcus mutans. Gangguan ini mengganggu sintesis matriks EPS (zat polimer ekstraseluler),sehingga menghambat pembentukan biofilm. Kesimpulan:Efektivitas CRISPR/Cas9 dalam mengubah populasi bakteri atau gen yang salah memberikan peluang untuk mengatasi penyakit mulut yang rumit, yang berpotensi merevolusi perawatan gigi. Namun, meskipun menjanjikan, uji klinis yang melibatkan teknologi ini masih berada pada tahap awal dan relatif tidak jelas, terutama dalam bidang kedokteran gigi. Abstract

Latar belakang : Gigi premolar pertama rahang atas memiliki variasi genetik dalam morfologi jumlah saluran akar dan posisi foramen apikal. Pergeseran angulasi horizontal tabung sinar–X ke arah mesial dapat memberikan gambaran saluran akar yang awalnya superimposed menjadi terpisah. Tujuan : Penelitian ini bertujuan untuk mengetahui perbedaan pergeseran angulasi horizontal pada teknik radiografi periapikal terhadap posisi foramen apikal gigi premolar pertama rahang atas. Metode : Desain penelitian ini adalah Cross-Sectional menggunakan 6 sampel gigi premolar pertama rahang atas yang telah diekstraksi. Pada setiap gigi dilakukan empat kali foto dengan angulasi horizontal 0ᵒ, 20ᵒ, 30ᵒ, dan 40ᵒ ke arah mesial. Hasil gambaran radiograf diamati oleh dua reviewer. Data yang diperoleh kemudian diuji dengan uji Cohen’s Kappa untuk melihat tingkat kesepakatan antara kedua reviewer. Selanjutnya dianalisis dan diinterpretasi dengan Uji Friedman untuk membuktikan hipotesis. Hasil : Nilai signifikansi Cohen’s Kappa sebesar 0,004 yang artinya terdapat kesepakatan antara kedua reviewer. Nilai signifikansi uji Friedman pada posisi foramen apikal sebesar 0,019 Asymp sig (<0,05) yang artinya terdapat pengaruh yang signifikan. Kesimpulan : Terdapat perbedaan jumlah saluran akar dan posisi foramen apikal gigi premolar pertama rahang atas terhadap pergeseran angulasi horizontal pada teknik radiografi periapikal. Abstract

Abstract Introduction: Central diastema is a space between two central incisors that can be caused by attachment of frenum to the interdental papillae of maxillary central incisor. Abnormal attachment of superior labial frenum can affect aesthetic aspect, damage periodontal tissue, hinder proper maintenance of oral hygiene and inhibit treatment of diastema closure, thus frenectomy procedure is needed. Frenectomy is a complete removal of frenum’s fibrous tissue including the attachment on bone surface. Case and management: A 24-years-old male patient referred to Periodontics RSGM UGM Prof. Soedomo with chief complaint of frenum’s attachment that hinder tooth movement during orthodontic treatment. Clinical findings showed superior labialis frenum’s attachment reached interdental papillae in the palatal side. Frenectomy with paralleling technique is planned, with incision using scalpel on both lateral side of the frenum ridge. Discussion: Paralleling technique provides better perception in terms of postoperative pain and minimize complication risk compared to conventional technique. Primary closure is also more achievable because of thin parallel incision. Conclusion: Paralleling frenectomy technique is a procedure to eliminate predisposing factor of periodontal disease caused of frenum abnormal attachment and to support orthodontic treatment of diastema closure. Abstrak Pendahuluan: Diastema sentralis adalah celah di antara dua gigi insisivus sentral yang dapat terjadi karena perlekatan frenulum yang mencapai maksila insisivus sentralis. Frenulum merupakan lipatan membran mukosa yang melekat pada bibir atau pipi di mukosa alveolar, gingiva dan periosteum di bawahnya. Perlekatan frenulum labialis superior yang tinggi dapat menyebabkan diastema sentralis yang mengganggu estetik, iritasi pada jaringan periodontal, menghambat dalam pemeliharaan kebersihan gigi dan mulut, menjadi penghalang dalam perawatan ortodontik pada gigi insisivus depan rahang atas sehingga membutuhkan tindakan frenektomi. Frenektomi adalah pengambilan seluruh jaringan fibrosa interdental pada bagian frenulum termasuk perlekatannya di bawah tulang. Laporan Kasus: Seorang pasien pria usia 24 tahun rujukan ortodontik datang ke RSGM UGM Prof. Soedomo dengan keluhan adanya perlekatan frenulum yang mengganggu pergerakan gigi selama perawatan ortodontik. Pemeriksaan klinis terlihat perlekatan tinggi frenulum labialis superior dengan eksistensi perlekatan mencapai papila palatina. Prosedur perawatan frenektomi dengan teknik paralel, insisi menggunakan blade dan scalpel pada kedua sisi lateral ridge frenulum. Pembahasan: Teknik paralel memberikan persepsi baik pada pasien dalam hal nyeri paska operasi dan komplikasi minimal bila dibandingkan teknik konvensional lainnya. Primary closure memungkinkan pada kasus ini, karena insisi paralel yang tipis. Kesimpulan: Frenektomi teknik paralel merupakan perawatan yang dapat dilakukan untuk menghilangkan faktor predisposisi penyakit periodontal akibat perlekatan frenulum tinggi serta bertujuan mengoreksi diastema sentralis dan mendukung proses perawatan ortodontik. Abstract

Introduction: The discoloration of the oral mucosa is influenced by the presence and degree of dilation of the subepithelial corium blood vessels and the amount of melanin pigment. Changes in the oral mucosa of smokers encompass various significant pathological conditions, including smoker’s melanosis and hyposalivation. Case Report: A 27-year-old male presented to the dental hospital with complaints of black discoloration on the lower jaw gingiva, persisting for the past two years. He exhibited widespread black spots on both the upper and lower jaws without pain. The patient also reported dry lips. He had a smoking history of seven years, with a consumption of 10 cigarettes per day. Examination revealed pigmented lesions, and the patient's stimulated salivary flow rate was 0.7 ml/min. Discussion: The oral cavity is a primary gateway for toxic substances from cigarettes, while saliva is the main biological fluid exposed to cigarette smoke, which contains various toxic compounds responsible for structural and functional changes in saliva. Exposure to cigarette smoke can lead to reduced salivary flow rate (hyposalivation), changes in salivary components that may result in malignancies, and changes in the color characteristics of the oral mucosa, such as the appearance of pigmented lesions known as smoker’s melanosis. Management of hyposalivation in patients includes providing education to improve hydration and encouraging smoking cessation. Conclusion: Smoking affects the condition of an individual's oral cavity, impacting both saliva and the oral mucosa. It is crucial for dentists to educate patients about the use of tobacco and its effects on oral health.

Abstract

Introduction: Infected periodontal pocket by microorganisms plays an important role in the occurrence of periodontal tissue necrosis and the formation of periapical abnormalities. Therefore, periodontal pocket treatment should be carried out with the aim of eliminating or reducing the microbial population and biomechanically removing necrotic tissue which is a medium for microbial growth and preventing re-infection. One of the medicinal plants from the Kalimantan forest that has great potential is Sarang Semut (Myrmecodia pendens). (12) Objective: This study aims to determine the response of gingival healing in the case of periodontitis in the Wistar rat model after the application of Sarang Semut (Myrmecodia pendens) extract. Material and Methods: Sarang Semut (Myrmecodia pendens) extract dilution to concentration of 2.5%, 5% and 10% was carried. This research is a laboratory experimental research and used post-test with control group design. Results: There are three concentrations that have been given: 2.5%, 5%, and 10%. The smallest concentration provides a healing area. Discussion: The results showed that ethanol extract of Sarang Semut can increase the healing response of the gingiva that has been induced by periodontitis bacteria. Conclusion: Gingival healing response in periodontitis case of Wistar rat model after application of Sarang Semut extract (Myrmecodia pendens) showed results similar to positive control on 2.5% Sarang Semut extract application. Clinical signs on the rat's gingiva that experienced discoloration, countours and bleeding on the 3rd day gradually improved and on the 7th day there was no pocket depth.

Abstract

Abstract Introduction: Oral cancer is a prevalent malignancy associated with high mortality and morbidity, primarily due to its poor prognosis. Recently, phytomedicines have attracted interest for their potential role in chemotherapy. Kaempferia galanga, a medicinal herb, has shown anticancer properties in various human cancer cell lines, mainly due to its active compound, ethyl-p-methoxycinnamate (EPMC). Objective: This in vitro study aimed to assess the cytotoxic effects of EPMC on the HSC-4 oral cancer cell line. Materials and Methods: HSC-4 cells were treated with various concentrations of EPMC for 24 hours, and cytotoxicity was measured using the MTT assay. Results: EPMC exhibited dose-dependent cytotoxicity, with an IC50 value of 0.032 mg/mL. Discussion: These findings suggest that EPMC is a promising anticancer agent. Further research should investigate its molecular mechanisms and potential as a chemosensitizer when combined with standard chemotherapeutic drugs. Conclusion: EPMC induces cytotoxicity in HSC-4 cells, positioning it as a promising candidate for further exploration in oral cancer treatment. Keywords: Cytotoxicity, EPMC, HSC-4, Oral Cancer

Abstract

Abstract Introduction: High labial frenulum attachment affects periodontal tissue to gingivitis and central diastema that interfere aesthetic function, so frenectomy treatment is needed. Frenectomy with convensional technique using blade had a wide wound because attraction of lip muscles and cause a lot of bleeding. Minimal bleeding can be achieved by modifying frenectomy with Incision Below the Clamp (IBC). The objective of this paper was to report frenectomy treatment with IBC technique can minimize bleeding. Case History: A 21-year-old woman came to the Periodontics Clinic of Prof. Soedomo Dental and Oral Hospital UGM with chief complaint of gapped upper front teeth that interfered appearance. After examination, there was aberration labial frenulum superior cause central diastema. Treatment planned before diastema closure was frenectomy with IBC technique using clamp with parallel positition and attached to labial mucosa, then incision under the clamp to base of vestibule and suturing in mucolabial fold. Discussion: Frenectomy with IBC technique made small wounds on the labial mucosa because the attraction of the orbicularis oris muscle laterally was hold back by clamp and suturing immediately after incision will hold muscle attraction after clamp was release and reduced bleeding. Surgery with few bloods will provide a sense of calm and comfort for patient and dentist. Conclusion: Frenectomy for aberrantia frenulum labialis superior using IBC technique completely remove the frenulum and orthodontic treatment can be continued. Abstract

Background: Human Immunodeficiency Virus (HIV) is a type of virus that attacks white blood cells and causes a decrease in the human immune system. The route of HIV transmission can be through sexual intercourse, shared syringe use, transmission through the placenta from HIV-positive mothers to children, and transfusion of blood infected with the virus. According to WHO data, about 39 million people are infected with HIV by the end of 2022. Oral manifestations are the initial symptoms in 30-80% of HIV-infected people and can be used to detect HIV/AIDS early. Objective: This study aims to identify oral clinical finding in HIV/AIDS patients at oral disease clinic of RSUD Kota Tangerang. Methods: This research was descriptive analysis using cross- sectional study design from 97 electronic medical records of HIV/AIDS patients at the Oral Medicine Clinic of RSUD Kota Tangerang. The data collected included the diagnosis of oral cavity findings. Results: The results showed that the three highest percentages related to oral manifestations found were chronic gingivitis (26.7%), then oral candidiasis (21.1%), coated tongue (16.2%), cheilitis (9.7%) and apthous like ulcer (6.5%). Conclusion: In HIV/AIDS patients at the Oral Medicine Clinic, there are many oral manifestations that vary.
