Abstract

Aim of the study: The study aimed to investigate the causes, risk factors, and incidence of caesarean sections in Diyala province during the research period. Patients and Methods: This prospective study was conducted at Al-Batool Maternity Teaching Hospital between October 2023 and December 2023. The study population consisted of 96 women who attended the labor ward for delivery after 32 weeks of gestation, irrespective of the delivery method. Patients' data were collected using an organized questionnaire. The data were analyzed using the Chi-square test for independence between variables.  Results: Study outcomes revealed that the percentage of cesarean sections increased significantly with age, peaking among women aged 20-35 and those over 35 (67.1% and 77.8%, respectively), in contrast to the group under 20, where most deliveries were vaginal (81.8%). An increasing number of previous deliveries (four or more) led to a higher chance of cesarean sections in subsequent pregnancies (76.9% compared to 58.8%) for those with two to four previous deliveries versus those who were primipara (52.6%). The most common indication for elective cesarean sections among participants was a scarred uterus (62.5%), followed by fetal indications such as malposition, abnormal presentation, post-date, and oligohydramnios (15%). The leading reasons for unplanned cesarean sections included obstructed labor (30%), followed by cephalo-pelvic disproportion (20%), and maternal factors (15%). Conclusion: study finding highlight key risk factors influencing delivery methods, suggesting the need for tailored obstetric care to optimize birth outcomes and reduce unnecessary CS procedures.

Abstract

Pharmaceutical services in hospitals must comply with the standards set forth in Minister of Health Regulation No. 72 of 2016 to ensure the safe, effective, and efficient use of medications. One crucial aspect of clinical pharmacy services is evaluating the appropriateness of drug use in accordance with the National Formulary (Fornas), particularly in antihypertensive therapy, which is widely used in clinical practice. This study aims to evaluate the conformity level of antihypertensive drug use with Fornas at Bandung-Based General Hospital and analyze its economic impact. This research is a retrospective descriptive study that collects data on antihypertensive drug use from various service units from August to October 2024. The analysis was conducted by calculating the percentage of conformity based on the number of drug units used compared to the Fornas list, as well as assessing the economic value of the drugs utilized. The results showed a very high adherence rate to Fornas, with the highest conformity percentage reaching 99.88% in September 2024 and the lowest at 99.81% in August 2024, surpassing the minimum compliance standard of 80% set by the Hospital Minimum Service Standards. These findings reflect the effectiveness of the National Formulary policy and the hospital's commitment to rational therapy. Collaboration among pharmacists, doctors, and the medical team, along with continuous education, is necessary to ensure safe, regulatory-compliant, and efficient therapy.

Abstract

Non-formulary drug utilization in Harapan Kita Children’s and Mother’s Hospital presents a substantial challenge in healthcare cost management, especially for patients enrolled in the BPJS health insurance program. Given its status as a national referral hospital, Harapan Kita Children’s and Mother’s Hospital encounters a high prevalence of complex maternal and child health cases requiring the use of medications beyond the scope of the national formulary. This research endeavor seeks to examine the patterns and consequences of non-formulary drug use within this context. This study adopted a quasi-experimental design, incorporating both quantitative and qualitative methods, to evaluate the impact of an sosialisation intervention and audit clinic on physician compliance with formulary drug use. A pre-test and post-test design was employed to measure changes in non-formulary drug utilization. Findings indicated a statistically significant decrease in non-formulary drug use from 18% to 13% post-intervention. The integration of a clinical pathway and tailored physician training was instrumental in achieving improved formulary adherence. Given these results, it is recommended to broaden training and surveillance programs, strengthen technological support through the implementation of formulary information systems, and undertake longitudinal research to assess the long-term sustainability of the intervention's effects..

Abstract

Low awareness of students about PHBS allows for a lack of knowledge obtained by students due to the lack of information obtained. Therefore, health education is important to be encouraged. One strategy that can be used in providing health education to students is peer education. To determine and analyze the effect of peer education on the scope of Clean and Healthy Living Behavior in students. The type of research used in this study is quasi-experimental, with a pretest-posttest control group design. Data analysis using variate and bivariate tests. The subjects of this study were students of Class VII-A and Class VII-B of SMP Negeri 3 Kutacane in the 2024/2025 Academic Year. The research instrument used was a test sheet. The results of the Mann-Whitney test showed a significant increase in students' understanding of each indicator including PHBS, except for the indicator of not smoking, although there was an increase but it was not statistically significant. This can be seen based on the average value of the experimental class at the time of the posttest which was greater when compared to the pretest, with a value of p = <0.05, while for the non-smoking indicator the value of p => 0.05. These results indicate the influence of health education with the peer education method on the understanding of the scope of PHBS in experimental class students. Peer Education is able to improve students' understanding of the scope of PHBS better, compared to students who do not receive peer education intervention.

Abstract

The implementation of Electronic Medical Records (EMRs) in hospital emergency units is essential to improve the speed, accuracy, and quality of services. However, various challenges hinder optimal system utilization. This study aimed to assess the adoption of EMR at the Emergency Unit of RSUD A.M Parikesit Tenggarong using the Human-Organization-Technology Fit (HOT-FIT) framework. A qualitative descriptive approach was employed involving seven purposively selected informants, including health professionals, administrative staff, and IT personnel. Data were collected through interviews, observations, and document reviews, then analyzed using the Miles and Huberman interactive model. The findings indicate that from the human aspect, the system was considered easy to use (85%), though user satisfaction remained moderate (67.14%) due to system errors and incomplete CPPT data entries. Organizationally, the hospital provided adequate infrastructure and support, though some policies require stronger enforcement. Technological aspects revealed limitations in system stability, lack of pharmacy integration, and low service quality due to frequent downtime. The study concludes that EMR implementation in the Emergency Unit has not yet achieved full effectiveness across all HOT-FIT domains. Improvements are needed in system reliability, information consistency, and inter-unit integration to optimize EMR benefits and enhance patient care delivery in critical settings.

Abstract

The implementation of Electronic Medical Records (EMR) in outpatient services aims to improve the efficiency, accuracy, and accessibility of clinical information. However, the effectiveness of such systems is highly dependent on user satisfaction. This study evaluated the satisfaction of health personnel using outpatient EMR at Sultan Fatah Regional Hospital, Indonesia, by employing the End User Computing Satisfaction (EUCS) model. The research addressed issues related to incomplete data, low system responsiveness, and interface usability that may affect the quality of service delivery. A qualitative descriptive method was used, involving in-depth interviews, direct observations, and document analysis from 21 informants selected purposively across multiple service units. The EUCS dimensions—content, accuracy, format, ease of use, and timeliness—were used to assess user experience. The findings indicate that users encountered problems such as incomplete patient data and duplicate records (accuracy), non-intuitive interfaces (format and ease of use), and delays in accessing medical information (timeliness). Observations and documentation review further revealed a reliance on manual backups and inconsistencies between digital and physical records. Overall, the study highlights the need for better data integration, enhanced user training, interface redesign, and system infrastructure upgrades to ensure real-time and comprehensive EMR usage. The study concludes that addressing technical and human factors through the EUCS model can enhance user satisfaction and system utilization in outpatient healthcare settings.

Abstract

Electronic Medical Records (EMRs) have become a fundamental component of healthcare digitization, offering improved accuracy, accessibility, and efficiency in patient data management. However, the adoption of EMR systems often encounters varied acceptance levels among healthcare workers, particularly in outpatient services where workflow integration is crucial. This study evaluates the implementation of an outpatient EMR system at RSAU dr. Siswanto, Indonesia, using the Technology Acceptance Model (TAM) framework. The research aims to explore users’ perceived usefulness, ease of use, attitude toward usage, behavioral intention, and actual system usage in daily operations. Employing a qualitative descriptive approach, data were collected through in-depth interviews, observations, and document analysis involving doctors, nurses, and medical record staff. The findings reveal that although the EMR system is generally perceived as beneficial for improving documentation and efficiency, several challenges persist, including incomplete feature utilization, limited training, and inconsistent user adaptation across roles. Additionally, network instability and technical downtime were noted as barriers to system reliability. Overall, the study underscores that perceived ease of use and continuous institutional support significantly influence EMR acceptance. These insights highlight the need for targeted capacity building and iterative system development to ensure effective EMR integration in outpatient settings.
