Abstract

Introduction: Hypokalemic Periodic Paralysis (HPP) is a rare neuromuscular disorder characterized by acute, reversible muscle weakness associated with low serum potassium levels. It may occur due to genetic abnormalities or secondary causes, including insulin therapy, which remains an underrecognized trigger. Case Report: A 71-year-old man with well-controlled type 2 diabetes mellitus and hypertension presented with sudden bilateral lower limb weakness. Physical examination revealed flaccid paraparesis without sensory deficits. Laboratory findings showed hyperglycemia (320 mg/dL) and hypokalemia (K⁺ 2.96 mmol/L). The patient was on regular subcutaneous insulin therapy with no prior similar episodes. Muscle strength improved within 24 hours after oral potassium supplementation and metabolic correction. Discussion: Insulin promotes intracellular potassium shift through activation of the Na⁺/K⁺-ATPase, leading to acute hypokalemia and impaired skeletal muscle excitability. The absence of sensory deficits and rapid clinical improvement following potassium supplementation supported the diagnosis of secondary HPP. In elderly patients with multiple comorbidities, this presentation may mimic neurological disorders, making electrolyte evaluation crucial. Conclusion: HPP should be considered in diabetic patients receiving insulin who present with acute muscle weakness. Early recognition and prompt correction of hypokalemia are essential for rapid recovery and prevention of serious complications.

Abstract

Introduction: This case report aims to heighten awareness that acute, sudden-onset abnormal movements can signal an underrecognized ischemic stroke, even without classic BEFAST (Balance, Eyes, Face, Arms, Speech) symptoms. Swift stroke diagnosis is vital, as time critically influences prognosis; yet, the typical presentation of hemiparesis often overshadows atypical features such as movement disorders, risking misdiagnosis. Case Report: A 60-year-old woman presented to the emergency department with sudden abnormal jaw and left limb movements lasting one day, with no prior episodes or accompanying symptoms. Comprehensive physical exams, ancillary tests, and imaging revealed an ischemic stroke in the left corona radiata on MRI (Magnetic Resonance Imaging). Discussion: These hyperkinetic movements stemmed from stroke infarction at corticostriatal fibers region, which normally transmit excitatory glutamatergic signals from the cortex to basal ganglia structures like the caudate nucleus and putamen. This input inhibits the globus pallidus internus, disinhibiting the thalamus to enable smooth movement initiation. Lesions in this pathway, as seen here, provoke abnormal hyperkinetic disorders. Conclusion: Left corona radiata ischemia atypically manifested as isolated movement abnormalities due to corticostriatal pathway interruption. Clinicians must vigilantly consider ischemic stroke in patients with abrupt-onset movement disorders, regardless of the absence of traditional symptoms, to ensure timely intervention and better outcomes.

Abstract

Background:Chronic pain is increasingly recognized as a complex condition involving interactions between peripheral nociceptive sources and central nervous system amplification mechanisms. Traditional models that focus solely on tissue damage are inadequate to explain the persistence and widespread distribution of many chronic pain conditions. Methods:This narrative review synthesizes current theoretical frameworks and empirical evidence on how peripheral pain sources interact with central amplification mechanisms. Literature from clinical, experimental, and neurobiological studies was reviewed to explore mechanisms underlying central sensitization, neuroinflammation, and patterns of pain comorbidity. Results:Evidence suggests that central sensitization, marked by increased neuronal excitability in central pain pathways, is a key mechanism connecting various chronic pain conditions. Peripheral nociceptive inputs, such as tissue inflammation, nerve injury, and musculoskeletal problems, can trigger and maintain central sensitization. Neuroimmune interactions and changes in descending pain modulation also help amplify and sustain pain. Epidemiological studies show frequent overlap among chronic pain conditions, supporting the idea of shared mechanisms involving peripheral and central interactions. Conclusion:Chronic pain is best understood as the outcome of dynamic interactions between peripheral pain sources and central amplification mechanisms. Recognizing these bidirectional processes has vital implications for clinical assessment and multimodal treatment approaches. Future research should prioritize longitudinal studies, validated mechanistic biomarkers, and integrated models that encompass peripheral, central, and psychosocial factors. Abstract

Background: The digital transformation in healthcare has fundamentally altered how neurologists develop and uphold their professional reputation. Although digital professionalism frameworks exist for general medical practice, approaches tailored to neurology remain underdeveloped. Objective: This narrative literature review aims to synthesize evidence exploring the intersection of clinical authority, digital professionalism, and reputation management in neurology and to develop a structured, neurology-specific conceptual framework. Methods: A narrative literature review was conducted by searching PubMed, Scopus, and Google Scholar using terms such as "physician reputation," "digital professionalism neurology," and "online physician reviews,". Studies published from 2008 to 2025 were included if they addressed physician reputation, digital professionalism, online reviews, or health communication strategies relevant to neurological practice. A total of 37 sources were identified and synthesized across eight interconnected themes. Results: This review presents the Neurology Reputation Architecture Model, a three-part framework consisting of Core (clinical competence, ethics, trust), Signal (digital presence, health communication, content strategy), and Shield (risk management, professional boundaries, crisis response). The evidence shows that social media activity, online patient reviews, and digital content strategies significantly influence patient choices, referral patterns, and professional reputation. Neurologists face unique challenges in communicating diagnostic uncertainties and complex neurological concepts in digital settings. Significant gaps remain in neurology-specific empirical research, especially in low- and middle-income countries (LMICs). Conclusion: Effective reputation management in neurology requires blending traditional professionalism with strategic digital engagement. Future research should focus on neurology-specific studies, LMIC contexts, and interdisciplinary methods that combine behavioral science, marketing theory, and sociology of professions.

Abstract

Introduction : Postural Persistent Perceptual Dizziness (PPPD) is the most common disorder among chronic-onset nd vestibular syndromes, accounting for 37% of cases, characterized by persistent dizziness, unsteadiness, anon-spinning vertigo exacerbated by movement, visual stimuli, and upright posture. The Niigata PPPD Questionnaire (NPQ) assesses these domains, but an Indonesian version has not been validated. Objective : To conduct cross-cultural adaptation and evaluate the validity and reliability of the Indonesian version (NPQ-Ina). Material and methods : This analytical cross-sectional study included adults diagnosed with chronic-onset vestibular disorders based on ICVD criteria at the Neurology Outpatient Clinic of Dr. Cipto Mangunkusumo National General Hospital. Cross cultural adaptation followed WHO guidelines. Construct validity was assessed using item total correlation, and internal consistency reliability was evaluated using Cronbach’s alpha. Result : Of the 36 study participants, the majority were female (91.7%) with a mean age of 52.2 ± 10.5 years, a median symptom duration of 19.5 months, and the most common diagnosis was PPPD (47.2%). All NPQ-Ina items showed significant and strong item-total correlations (0.617–0.875). The overall Cronbach’s alpha was 0.894. The visual and postural factors demonstrated high reliability (α = 0.818 and 0.815), while the movement factor showed moderate reliability (α = 0.685). Discussion : Strong validity and reliability across domains support the conceptual integrity of the NPQ-Ina, while slightly lower reliability in the movement factor may reflect diagnostic heterogeneity. Conclusion : The NPQ-Ina is a valid and reliable instrument for assessing symptom exacerbating factors in chronic onset vestibular disorders in the Indonesian population.

Abstract

Introduction: Pain is a complex subjective experience that involves not only sensory-discriminative but also affective-motivational, cognitive-evaluative, behavioral and social dimensions. Contemporary perspectives on pain have shifted from simplistic linear models toward a conceptualization of pain as a multidimensional mental state generated by the brain. This narrative review aims to summarize the perspectives of pain within multidimensional aspects by understanding pain as an evolving framework associated with mental health, validated by assessment instruments, and its clinical implications. Material and Methods: A comprehensive literature search was conducted via PubMed and ScienceDirect with keyword related to pain, chronic pain, acute pain, mental status, psychological state, mental health, multidimensional, biopsychosocial model, cognitive, affective, and sensory. A total of fifteen literatures were reviewed and analyzed. Discussion: The literature indicates that pain is a brain-generated phenomenon integrating sensory-discriminative, affective-motivational, cognitive-evaluative, behavioral, and social dimensions. Strong associations were identified between chronic pain and depression, anxiety, and the mental state of pain patients. Validated multidimensional pain assessment instruments are available for adult and pediatric populations as well as for specific clinical conditions; however, gaps remain in cross-cultural validation and in assessments tailored to older adult populations. Conclusion: Pain should be understood as a multidimensional mental state that requires comprehensive assessment and integrated intervention strategies. A biopsychosocial approach combining pharmacological management, psychological interventions, and functional rehabilitation represents best practice in chronic pain management. Future research should strengthen cross-population validation, integrate neurobiological measures with longitudinal psychosocial data, and develop mechanism-based personalized models of pain management.

Abstract

Background: Headache is a universally experienced neurological condition; however, its representation and subjective interpretation vary considerably across cultural contexts. Cultural neuroscience provides an integrative framework for understanding how sociocultural influences shape headache and pain experiences through identifiable neural mechanisms. Objective: This narrative review synthesizes empirical and epidemiological evidence regarding cross-cultural variations in headache presentation, psychophysical pain profiles, and systemic clinical management barriers across diverse global populations. Methods: A structured literature search was conducted across the PubMed and ScienceDirect databases to identify peer-reviewed studies published between 2016 and 2026 focusing on cultural neuroscience, primary headache disorders, and cross-cultural pain perception. A total of 20 articles meeting eligibility criteria were selected for qualitative narrative synthesis. Results: Cross-cultural differences in headache expression are heavily driven by lived experiences of socioenvironmental stress and structural inequities. Chronic exposure to systemic discrimination, lower socioeconomic status, and acculturation stress function as physical stressors that accelerate central sensitization and the chronification of episodic headaches. Furthermore, culturally distinct illness models and expectations of clinical stoicism significantly influence symptom communication, contributing to diagnostic misclassification. Finally, structural healthcare barriers perpetuate profound inequities in patient access to novel, targeted migraine therapies. Conclusion: Nociceptive processing appears to be relatively consistent across populations, whereas cultural factors influence the affective and contextual modulation of headache through identifiable neural circuits. Clinical implications highlight the importance of culturally informed assessment strategies and personalized therapeutic interventions.
