Abstract

Background: Chronic Obstructive Pulmonary Disease (COPD) represents a significant global health burden, contributing substantially to long-term morbidity and mortality rates worldwide. Purpose: To evaluate the effectiveness of diaphragmatic breathing exercises combined with facial cold stimulation on dyspnea perception, respiratory rate, and peak expiratory flow in COPD patients. Method: A quasi-experimental study was conducted using a pre-test and post-test control group design, involving 46 participants divided into intervention and control groups. Data collection tools included questionnaires, respiratory rate observations, and a peak flow meter. For data analysis, inferential statistics were applied: the Independent t-test and Paired t-test were used for normally distributed ratio data, while the Wilcoxon Signed Ranks Test and Mann-Whitney U Test were employed for ordinal data. Results: The combination of diaphragm breathing exercises and cold facial stimulation significantly reduces dyspnea perception in COPD patients. This intervention was also found to be effective in lowering the respiratory rate of COPD patients. Additionally, the results revealed a significant improvement in Peak Expiratory Flow Rate (PEFR) in the intervention group compared to the control group. Conclusion: Peak Expiratory Flow Rate (PEFR) significantly improved in the intervention group compared to the control group, indicating the combined intervention's efficacy in enhancing respiratory function in COPD patients.

Abstract

Background: Diabetes is a non-communicable disease. The number of diabetes cases and the prevalence of the disease have increased continuously in recent decades. Diet and physical activity are considered to be determining factors and are closely linked to the development of diabetes. Purpose: To analyze dietary patterns, physical activity, and medication adherence to glycemic stability in a community with type 2 diabetes mellitus. Method: This quantitative study employed analytical observation with a cross-sectional approach. The population comprised all patients with type 2 diabetes mellitus in the Montong Betok Community Health Center in December 2023. Sample inclusion criteria included residents aged 40 years and above who were willing to participate and had suffered from type 2 diabetes mellitus for more than 1 year and had been taking medication according to the recommended dosage with random blood sugar levels between 100 and 400 mg/dL. Simple random sampling was used to select 199 respondents. The analysis conducted in this study included univariate analysis, bivariate chi-square with SPPS version 20 for Windows, and multivariate analysis using logistic regression test. Results: The logistic regression analysis obtained a significance level of <0.05 for the variables of diet, physical activity, and medication adherence. This indicates that diet, physical activity, and medication adherence significantly influence respondents' blood sugar stability. Of the three variables analyzed, medication adherence had the largest Exp (B) coefficient, namely 1.346. This indicates that medication adherence is the variable that most dominantly influences blood sugar stability compared to diet and physical activity. Conclusion: Eating habits, physical activity, and medication adherence have a significant impact on blood sugar stability in type 2 diabetes mellitus patients. However, medication adherence variables are an important factor that influences type 2 diabetes mellitus patients.

Abstract

Background: Chronic diseases are a global health challenge that has a significant impact on individuals and health systems. Purpose: To analyze the relationship between family support and motivation for recovery in patients with chronic diseases. Method: A descriptive analytical design with a cross-sectional approach involving 315 patients at Syiah Kuala University Teaching Hospital. Data were analyzed using the chi-square test with SPSS. Results: The results showed that patients with good family support had higher motivation to recover (40.2%) compared to those with less support (4.5%). Conversely, low motivation to recover was more often found in patients with less family support (48.2%). Statistical analysis showed a significant relationship between family support and encouragement to recover (p = 0.001). Conclusion: Family support plays an important role in increasing the motivation for recovery of patients with chronic diseases.

Abstract

Background: Acute Respiratory Infection (ARI) is one of the diseases with high morbidity, especially in coastal communities. Fishermen face a high risk of ARI due to the work environment, poor sanitation, and limited access to clean water. Purpose: To examining the relationship between the availability of clean water and the incidence of ARI in fishermen in Aceh Jaya District. Method: An analytical observational study with a cross-sectional design was conducted on 200 fishermen in two villages using a total sampling technique. Data were analyzed using the chi-square test and logistic regression using STATA. Results: There is a significant relationship between the availability of clean water (p=0.000), water source (p=0.000), water condition (p=0.000), ease of access (p=0.000), and the availability of clean water within a certain period of time (p=0.000) with the incidence of ARI. Conclusion: Access to clean water plays an important role in preventing ARI. Efforts to increase the availability of clean water and sanitation are needed to reduce the incidence of ARI in fishing communities. Suggestion: To examine other factors contributing to the incidence of ARI, such as air quality.

Abstract

Background: Type 2 diabetes mellitus (DM) prevalence continues to increase in rural areas, which is of particular concern due to limited access to health services in these areas. Purpose: To identify various risk factors for the incidence of type 2 DM in rural areas for the development of effective prevention programs. Method: Systematic review is based on PRISMA guidelines. Searches were conducted in Scopus, Science Direct, and SINTA for 2020-2025 articles. Data were extracted and analyzed narratively based on the type of risk factors that were found. Results: Out of 856 articles, 20 studies met the inclusion criteria. Dominant risk factors identified included obesity (OR range 2.15-3.05), family history of DM (OR range 2.13-4.46), and low physical activity (OR range 2.94-7.43). Additional risk factors include hypertension, unhealthy food environment, and unhealthy diet. Several studies reported an increased incidence of type 2 DM in younger populations in rural areas. Conclusion: Obesity, family history of DM, and low physical activity are the dominant risk factors for type 2 DM in rural areas. These factors are important targets for health policy interventions, especially given the increasing incidence in younger populations in rural areas.

Abstract

Background: Improper management of all types of hospital waste has become a significant problem relevant to public health issues. However, even though there are regulations regarding waste management, the implementation still varies between hospitals. The most neglected factor is the waste management supervision frameworks.  Purpose: To identify the emerging concern of supportive supervision in hospital waste management. Method: This formative delphi study employed qualitative methods and focus group discussions (FGDs). Each FGD lasted 60 minutes. The study incorporated the concept of stakeholder engagement in hospital waste governance, focusing on factors driving stakeholder participation, knowledge-sharing structures, information exchange, decision-making processes, and the institutionalization of various centers of power and decision-making to diversify waste management oversight. Results: Participants were most likely to decide to improve waste management. Of the 18 questions, three received a higher number of "yes" responses (53.8%). The first question concerned managers in the environmental, occupational safety, and health department providing recognition/appreciation for staff performance or achievements in waste management at the hospital. The second question concerned information provided by managers to staff regarding open and periodic performance appraisals. The third question concerned the use of audit results to improve the existing waste management system. Conclusion: Supervision is an activity aimed at improving service quality. It requires a conditional and open environment to accept problem-solving. To enhance the quality of service, supervision must be systematic and require continuous follow-up.

Abstract

Background: Depression is prevalent in hemodialysis patients and may contribute to anemia, yet its impact on hemoglobin levels remains underexplored. Purpose: To examined the association between depression severity and hemoglobin levels in Indonesian hemodialysis patients. Method: A cross-sectional study was conducted at a single-site hemodialysis unit from October to December 2024. A total of 101 patients were selected using purposive sampling. Depression severity was assessed using the Beck Depression Inventory-II (BDI-II), and hemoglobin levels were measured via venous blood samples. Spearman’s correlation and linear regression analysis were used to analyze the association. Results: Depression severity was negatively correlated with hemoglobin levels (r = -0.62, p < 0.01). Patients with severe depression had hemoglobin levels nearly 3 g/dL lower than those with minimal depression. Regression analysis showed depression severity significantly predicted hemoglobin levels (Adjusted R² = 0.38, p < 0.01). Conclusion: Higher depression severity is associated with lower hemoglobin levels in hemodialysis patients. Routine mental health screening should be integrated into nephrology care, with nurses playing a key role in early detection and intervention. Future multi-center studies are needed to enhance generalizability.

Abstract

Background: Patients with chronic diseases generally experience repeated hospitalizations. Cultural diversity can affect communication and engagement from health care providers and recipients. An interactive culturally sensitive telenursing application is needed as a form of continuous nursing intervention when patients are at home. Purpose: To build and test the culturally sensitive telenursing application of Suster Kita. Method: Using the ADDIE (Analysis, Design, Development, Implementation, Evaluation) process design. The research was conducted from August 2022 to February 2024 and tested on 20 respondents in the DKI Jakarta area. The evaluation of the effectiveness of the Suster Kita culturally sensitive telenursing application at the implementation stage was carried out using a questionnaire on the use of the application developed by the research team. The final results of the application are evaluated 2 weeks after the respondent uses the application. Results: the use of the application showed that the average knowledge of chronic diseases and their treatment consisting of 10 knowledge components increased before and after the trial of the use of the culturally sensitive application of Suster Kita. Conclusion: the intervention is acceptable to users and is worth using. Application suggestions are integrated with laboratory results.

Abstract

Background: Mental health is an important aspect of human life. Traumatic experiences in childhood are associated with increased mental health morbidity. Factors such as lack of parental support, low self-esteem, and social difficulties play a role in increasing the risk of risky behavior in adolescents. Purpose: To determine the determinant factors that influence the incidence of depression in adolescents living in orphanages in Central Aceh Regency. Method: Used a Cross Sectional design in four orphanages in August 2023. The sample consisted of 302 adolescents aged 8-18 years, selected by purposive sampling. The analysis used logistic regression with 95% confidence. Results: A total of 121 respondents (40.07%) experienced depression. Related factors were education, region of origin, family visits, social support, and social discrimination. Social support was the most dominant factor (AOR=5.64; p=0.000). Conclusion: Social support plays a major role in preventing depression in adolescents in orphanages, so interventions that increase social support need to be prioritized. Suggestion: Explore other psychosocial factors such as quality of relationship with caregivers, trauma experiences, and adolescent coping strategies.

Abstract

Background: Bullying in schools is a growing global problem that affects victims, perpetrators, and the surrounding environment. Environmental and individual factors, such as family, mass media, and social pressure, play a significant role in the emergence of bullying behavior. In Indonesia, this phenomenon is still often overlooked, even though it has long-term consequences for students' psychological and academic well-being. Purpose: To analyze the factors that contribute to bullying behavior in schools. Method: A descriptive qualitative design with an exploratory focus was conducted in April 2025 at State Vocational High School 1 Banda Aceh. A total sampling technique was utilized, resulting in the selection of four participants, consisting of key informants including teachers and the headmaster. Data collection was carried out through triangulation, integrating multiple data sources, methods, and theoretical perspectives to enhance the credibility and validity of the findings. In-depth interviews served as the primary data collection method. The collected data were analyzed using thematic analysis, allowing for the identification of recurring patterns and the extraction of core meanings from the participants' narratives. Results: Bullying behavior among students is influenced by family, media, and peer environments. A lack of support or harsh treatment at home can lead children to imitate negative behavior at school, while exposure to violent media and poor parental supervision further contribute to aggression. Peer pressure also plays a significant role, as teenagers often follow group behavior to feel accepted. Conclusion: Preventing bullying requires a comprehensive approach involving families, schools and communities.
