Abstract

Pembangunan basis data genomik nasional melalui Platform SatuDNA menempatkan informasi biologis warga di dalam satu infrastruktur yang dikelola negara, sedangkan hukum Indonesia belum mengenal kedudukan kerabat biologis yang informasi genetiknya tersingkap tanpa pernah menyerahkan sampel maupun memberikan persetujuan. Penelitian ini bertujuan memetakan pengaturan yang berlaku, mengidentifikasi kelemahan dan kekosongan normanya, serta merumuskan model pelindungan hukum bagi kerabat biologis. Metode yang digunakan adalah yuridis normatif bersifat preskriptif dengan pendekatan perundang-undangan, konseptual, perbandingan, dan futuristik, bertumpu pada bahan hukum primer berupa Undang-Undang Nomor 27 Tahun 2022, Undang-Undang Nomor 17 Tahun 2023 beserta peraturan pelaksananya, dan bahan hukum sekunder berupa literatur bereputasi, Regulasi Umum Pelindungan Data Uni Eropa, serta kerangka kebijakan Global Alliance for Genomics and Health. Hasil penelitian menunjukkan dua kerangka hukum bekerja berdampingan dengan objek pelindungan yang berbeda, yaitu undang-undang pelindungan data melindungi orang perseorangan yang datanya diproses sedangkan undang-undang kesehatan melindungi material biologis dan kedaulatan genetika negara, sehingga kerabat biologis jatuh pada celah di antara keduanya. Interpretasi atas Pasal 1 angka 1 dan Pasal 1 angka 6 Undang-Undang Nomor 27 Tahun 2022 memperlihatkan kerabat berpeluang memperoleh kedudukan hanya sepanjang dirinya dapat diidentifikasi, sedangkan inferensi risiko yang tidak mengidentifikasi berada di luar jangkauan norma. Jalur hukum alternatif berupa gugatan perbuatan melawan hukum, gugatan perwakilan kelompok, ketentuan pidana, sengketa informasi publik, dan pengaduan disiplin tersedia, tetapi seluruhnya bersifat menyusul kerugian dan tidak menghadirkan pelindungan pendahuluan. Pertentangan antara kewajiban retensi jangka panjang dan hak penghapusan terselesaikan secara formal melalui pengecualian kepentingan penelitian ilmiah, sehingga persoalan sebenarnya terletak pada ketiadaan pengaman atas pengecualian tersebut. Penelitian ini merumuskan model Pelindungan Privasi Genomik Keluarga Berbasis Hak Terbatas dan Tata Kelola Berlapis yang menempatkan kerabat biologis tingkat pertama sebagai pihak terdampak terbatas tanpa hak veto atas keputusan partisipan, dijalankan melalui lima lapisan tata kelola, sembilan perangkat operasional, dan tiga tahap pelaksanaan yang disesuaikan dengan kondisi kelembagaan Indonesia.

Abstract

Human immunodeficiency virus (HIV) infection in children causes immunological and hematological disturbances, most notably CD4+ T-lymphocyte depletion and thrombocytopenia. Antiretroviral therapy (ART) suppresses viral replication and may restore both, but pediatric data comparing these parameters remain limited. Objective. To compare platelet and CD4 lymphocyte counts before and after six months of ART in children with HIV. Methods. An observational analytic study with a retrospective cohort approach used medical records of children (aged 0–18 years) with HIV (ICD-10 B20) treated at Haji Adam Malik General Hospital, Medan, from 2020 to 2025. ART-naive children with platelet and CD4 results within seven days before ART and at month six were included by consecutive sampling. Values were compared using the Wilcoxon signed-rank test (SPSS version 26.0); p<0.05 was considered significant. Results. A total of 74 children were analyzed (56.8% male; 45.9% good nutritional status; 75.7% without opportunistic infection). The most common regimen was zidovudine + lamivudine + nevirapine (28.4%). Median CD4 count rose from 429 to 503 cells/mm³ (p=0.002), whereas median platelet count rose from 304,000 to 323,000/µL without reaching significance (p=0.116). A positive directional relationship between the two parameters was observed after therapy. Conclusion. Six months of ART significantly increased CD4 lymphocyte count but not platelet count, largely because most children had normal baseline platelets. Routine complete blood counts alongside CD4 monitoring remain useful, and platelet count may serve as a simple adjunct marker of immune response.

Abstract

Wanita pasca menopause mengalami penurunan estrogen yang mempercepat kehilangan massa tulang dan meningkatkan risiko gangguan muskuloskeletal termasuk nyeri pinggang bawah. Hipovitaminosis D sering ditemukan pada populasi ini dan diduga menjadi faktor yang dapat dimodifikasi terhadap disabilitas akibat nyeri pinggang, namun bukti pada wanita pasca menopause Indonesia masih terbatas. Menganalisis hubungan kadar vitamin D dengan derajat disabilitas nyeri pinggang berdasarkan Oswestry Disability Index (ODI) pada wanita pasca menopause. Penelitian observasional analitik dengan desain potong lintang dilakukan di Poliklinik Obstetri dan Ginekologi Rumah Sakit Prof. Dr. Chairuddin P. Lubis Universitas Sumatera Utara periode Februari–Juni 2026. Sebanyak 25 wanita pasca menopause yang memenuhi kriteria dilibatkan dengan teknik consecutive sampling. Kadar 25-hidroksivitamin D serum diukur dan derajat disabilitas dinilai menggunakan kuesioner ODI. Data dianalisis dengan uji independent t-test dengan batas kemaknaan p<0,05. Rerata usia subjek 59,4 ± 6,41 tahun, rerata usia menopause 50,8 ± 3,99 tahun, dan median lama menopause 7 (1–25) tahun. Rerata kadar vitamin D adalah 23,94 ± 6,37 ng/mL dengan rerata skor ODI 31,27 ± 10,7. Hipovitaminosis D ditemukan pada 22 subjek (88%). Disabilitas sedang merupakan kategori terbanyak (76%). Rerata kadar vitamin D pada kelompok disabilitas minimal, sedang, dan berat berturut-turut 22,52 ± 6,72; 24,77 ± 6,43; dan 18,8 ± 4,24 ng/mL (p = 0,413). Hipovitaminosis D sangat prevalen pada wanita pasca menopause, namun tidak ditemukan hubungan bermakna secara statistik antara kadar vitamin D dengan derajat disabilitas nyeri pinggang berdasarkan ODI. Nyeri pinggang pada populasi ini bersifat multifaktorial dan tidak dapat dijelaskan hanya oleh status vitamin D.

Abstract

Lingkungan kerja yang sesuai standar harus menciptakan rasa nyaman bagi pekerja di lingkungan industri, perkantoran, dan pelajar di ruang kelas. Tujuan dari penelitian ini adalah untuk menganalisis kenyamanan Thermal tenaga kerja akibat lingkungan kerja panas atau dingin di  ruang rumah sakit.  Penelitian ini bersifat observasional dengan desain cross-sectional.  Besar  sampel  adalah  keseluruhan  populasi  yaitu  semua ruangan di Rumah sakit X.  Data  didapatkan  dari  pengukuran  suhu udara ruang, kelembaban udara, kecepatan udara, insulasi pakaian, dan laju metabolisme. Hasil pengukuran kenyaman termal  menunjukkan bahwa rerata nilai suhu ruangan pagi hari 24,75°C, siang hari 24,05 °C, dan Malam hari 23,05 °C. Menurut peraturan Menteri Kesehatan Nomor: 829/Menkes/SK/VII/1999 masuk dalam kategori nyaman. Hasil kelembapan pagi hari 59%, siang hari 59,6%, dan malam hari 58,6%, masih dalam batas toleransi, dengan standar 40%-70%.  Hasil ukur kecepatan angin pagi hari 0,24 meter/detik, siang hari 0,39 meter/detik, dan malam hari 0,58 meter/detik. Untuk mempertegas hasil kenyamanan termal dilanjutkan dengan perhitungan PMV dan PPD dengan menghitung isulasi pakaian dan metabolisme (MET). Standar ASHRAE-55 yang dipergunakan dengan alat kenyaman termal software CBE. Hasil yang diperoleh tidak memenuhi standar dengan pagi hari angka PMV -3,6 dan PPD 99% dengan tingkat kenyamannya dingin. Untuk Siang hari angka PMV yang diperoleh  -1,01 ( sedikit dingin) dan PPB sebesar 27%. Untuk hasil malam hari angka PMV -1,64  dengan sensasi dingin, untuk PPDnya 59%. Angka yang diperoleh untuk PPD di siang hari mendekati 99% suasana yang tidak diinginkan. Sehingga dapat disimpulkan bahwa Angka kenyamanan termal bukan hanya ditinjau dari faktor lingkungan tapi juga faktor personal pekerja dilingkungan tempat bekerja.

Abstract

Adherence to antenatal care (ANC) visits among pregnant women is crucial for the early detection of pregnancy risks and the improvement of maternal and fetal health. However, non-adherence to ANC visit schedules remains a frequently encountered issue. The Four Pillars Approach is a comprehensive strategy encompassing healthy lifestyle habits during pregnancy, family social support, healthcare worker competence, and the professional behavior of healthcare workers. This study aimed to analyze the impact of implementing the Four Pillars Approach on ANC visit adherence among pregnant women at the Cogreg Community Health Center (Puskesmas), Bogor Regency, in 2025. A quantitative method with a cross-sectional design was employed. The study sample consisted of 56 third-trimester pregnant women selected via purposive sampling. Data analysis was conducted using the chi-square test. The results indicated that a healthy lifestyle during pregnancy had a significant influence on ANC adherence (p = 0.029). Meanwhile, family social support, healthcare worker competence, and professional behavior among healthcare workers did not show a statistically significant influence (p > 0.05), although they remained supporting factors. The study concludes that a healthy lifestyle during pregnancy is the dominant factor influencing ANC adherence. This study also discusses practical implications, strengths, and limitations, serving as a basis for recommendations regarding maternal health services and future research.

Abstract

Chronic Otitis Media (COM) is an inflammation of the middle ear that can lead to intratemporal complications such as mastoiditis. Radiographic evaluation of the mastoid bone plays a crucial role in identifying structural damage and assessing the extent of infection. Theoretically, the Schüller method axiolateral projection involves a 45° head rotation and a 25°–30° caudal beam angulation. However, preliminary observations at the Radiology Department of 'Aisyiyah Ponorogo General Hospital revealed the use of a 40° caudal angulation. This study aimed to examine the mastoid examination procedure and the rationale behind the 40° caudal angulation at the Radiology Department of 'Aisyiyah Ponorogo General Hospital. A qualitative case study approach was employed at the facility between December 2025 and June 2026, involving interviews with three radiographers and one radiologist. Data collection was conducted through direct observation, interviews, and documentation. Data analysis was performed narratively—involving data reduction and presentation—followed by a comparison with theoretical standards and the drawing of conclusions. The results indicate that the mastoid examination is performed without special preparation, other than the removal of metal objects from the head area. Patients are positioned erect in a true lateral position, with the side of the head being examined placed against the detector. A bilateral Schüller method axiolateral projection is used, with a centering point 2–3 cm posterior-superior to the external auditory meatus (EAM), a 40° caudal beam angulation, and exposure factors of 70–80 kV, 200 mA, 16 mAs, and an FFD of 100–120 cm. The 40° angulation was selected to produce an elongation effect on the mastoid, thereby minimizing superimposition with surrounding bony structures, allowing for clearer visualization of mastoid air cells, and facilitating a comparison of the bilateral mastoid conditions. The study concludes that the mastoid examination procedure for patients with chronic otitis media (COM) at 'Aisyiyah Ponorogo General Hospital utilizes the bilateral axiolateral projection (Schüller method) with a 40° caudal angulation and no special preparation. This angulation aims to achieve optimal elongation of mastoid structures, minimize superimposition, and enhance the diagnostic accuracy for COM.

Abstract

Bone age assessment is a radiographic procedure used to evaluate skeletal maturity in children with growth disorders. While the left posteroanterior (PA) projection is the standard method, the use of bilateral PA projections for specific conditions was observed at the Radiology Department of Sleman Regional General Hospital (RSUD Sleman). This study aimed to examine the bone age assessment techniques and the rationale for selecting either the left PA or bilateral PA projection for patients with growth disorders at the hospital. A qualitative case study approach was employed, focusing on two patients with growth disorders. The study was conducted at the Radiology Department of RSUD Sleman between September 2025 and June 2026. Data were gathered through observation, in-depth interviews with three radiographers, a radiologist, and a pediatrician, as well as through documentation and a literature review. Data analysis involved data reduction, data presentation, and conclusion drawing. The results revealed that bone age assessments utilized the left PA projection as the standard procedure, with bilateral PA projections reserved for specific conditions. The left PA projection was performed with the central point at the third metacarpophalangeal joint and a focal-film distance (FFD) of 100 cm, whereas the bilateral PA projection utilized a central point positioned between the two hands, also with an FFD of 100 cm. The left PA projection was selected because it met diagnostic requirements for bone age assessment and adhered to the ALARA (As Low As Reasonably Achievable) radiation protection principle. Conversely, the bilateral PA projection was selectively used for patients with suspected congenital anomalies, deformities, trauma, or asymmetrical growth to compare the ossification development of both hands.

Abstract

Radiation protection aims to safeguard patients by applying the principles of justification, optimization, and limitation, ensuring radiation doses remain as low as possible without compromising image quality. Pediatric patients exhibit high radiosensitivity because their body cells are actively developing, making them more susceptible to the biological effects of radiation. Pediatric emergencies often require rapid radiological examinations, such as in cases of suspected diaphragmatic eventration. Suboptimal use of exposure factors (kVp, mAs) and collimation settings can increase radiation doses received by pediatric patients. This study aims to describe the Entrance Surface Dose (ESD) values, analyze the influence of technical factors on ESD, and evaluate compliance with IAEA Diagnostic Reference Level (DRL) standards. A mixed-methods approach combining qualitative and quantitative methods was employed. The study subject was a 3-day-old pediatric patient undergoing an OMD (esophagogastroduodenal) examination during an emergency at the Radiology Department of Sleman Regional General Hospital (RSUD Sleman). Data were collected between September 2025 and June 2026 through observation and interviews with four informants: one radiologist, two radiographers, and one medical physicist; quantitative data were derived from Dose Area Product (DAP) values. The OMD examination involved five exposures—AP (first), AP (second), LPO, LPO (repeat), and RPO—resulting in a total ESD of 0.389 mGy. The total DAP was 508.7 mGy·cm², which remains below the IAEA DRL reference value of 760 mGy·cm² for the 0-year age group. Factors such as kVp, mAs, SID, and collimation area were found to directly influence ESD values. The use of a 42 × 42 cm collimation field was excessively large and suboptimal for a pediatric patient.

Abstract

A perianal fistula is an abnormal tract connecting the anal canal to the skin surrounding the anus. Fistulography is used to visualize the fistula tract, thereby aiding in diagnosis. At the Radiology Department of Banyumas Regional General Hospital (RSUD Banyumas), the procedure differs from standard theory; specifically, PA and lateral projections are used, no markers are placed on the preliminary image, and undiluted iodine-based contrast media is utilized. This study aims to examine the fistulography procedure for perianal fistula cases at RSUD Banyumas, including the rationale for the chosen projections, the omission of markers, and the use of undiluted contrast media. A descriptive qualitative method with a case study approach was employed from December 2025 to May 2026. Data collection involved observation, interviews, documentation review, and a literature study. Research subjects included three radiographers, one radiologist, one referring physician, and one radiology nurse. Data were analyzed narratively and compared against established theory. The study found that fistulography for perianal fistula at RSUD Banyumas is performed without special patient preparation. Equipment and materials included a DR X-ray unit, apron, 50 cc syringe, Abocath catheter, gloves, gauze, Betadine, and Hexiol (300 mgI/mL) iodine contrast medium (50 cc). The contrast medium is administered incrementally through the Abocath catheter. The examination involves a plain pelvic radiograph (PA projection) followed by post-contrast images in both PA and lateral projections. These projections were selected to visualize the fistula's tract and depth, with the PA projection serving to minimize radiation exposure to the gonads. Markers are not used because the fistula's location is identified via the placement of the Abocath catheter. Using undiluted contrast medium provides higher opacity, ensuring the fistula tract is clearly visualized.

Abstract

Cervical syndrome is a disorder of the cervical vertebrae that can cause neck pain and activity limitations. Cervical spine radiography serves as a supporting examination for establishing a diagnosis. According to Lampignano and Kendrick (2018), cervical spine radiographic examinations include AP axial, lateral, and oblique projections. Oblique projections can be performed using RPO/LPO or RAO/LAO positions. However, at the Radiology Department of Sleman Regional General Hospital (RSUD Sleman), the axial, lateral, and oblique (RPO and LPO) projections are performed with a perpendicular beam direction—without the standard 15°–20° cephalad angulation—creating a discrepancy with the technique described in theory. This study aims to determine the cervical examination procedures for clinical cases of cervical syndrome at the RSUD Sleman Radiology Department and to understand the rationale for using RPO and LPO projections with a perpendicular beam direction for these cases. This study employs a descriptive qualitative method with a case study approach, conducted at the RSUD Sleman Radiology Department from December 2025 to May 2026. Data were obtained from 27 patients with clinical cervical syndrome through observation, in-depth interviews with three radiographers and one radiologist, and documentation of radiographic results. Data analysis was performed qualitatively by comparing the actual radiographic examination procedures with established theory. The results indicate that cervical spine radiography for cervical syndrome cases at RSUD Sleman utilizes AP axial, lateral, and RPO/LPO projections. Regarding patient preparation, patients are required to remove metal objects from the neck and head area prior to the examination. For oblique projections, the patient's body is positioned at a 45° angle relative to the image receptor, with the beam directed perpendicularly toward C4.
