Abstract

Introduction: The diagnosis of dry eye disease (DED) especially in the early stages is important, but difficult. This is due to the lack of a gold standard and a poor correlation between the biochemical changes in tears and clinical signs. Tear biomarkers can be used in diagnosing and monitoring DED because they are non-invasive, and have a good correlation with biochemical changes in tears and disease progression. Purpose: This article will describe some of the most important tear biomarkers for DED, namely markers for lacrimal gland dysfunction, inflammation, oxidative stress, and contact lens intolerance, and its correlation with subtypes and disease severity. Method: The method used is literature study using the last 10 years journals obtained from search engines such as Sciencedirect, PubMed, Google Scholar and ClinicalKey. Discussion: Biomarkers for lacrimal gland dysfunction are characterized by changes in protein levels (lactoferrin, lysozyme, etc.), neuromediators (NGF, CGRP, NPY, Serotonin), and mucin ((MUC)5AC); while the inflammatory response is characterized by changes in the expression of cytokines, chemokines, MMP-9, and albumin. Oxidative stress is characterized by changes in lipid levels (HNE, MDA). Meanwhile contact lens intolerance is associated with changes in 1D1-secretoglobin, ?2-microglobulin, lacritin, secretoglobin 1A2, albumin, LPRR4, LCN-1, and PIP. Conclusion: MMP-9 and a combination of Mammaglobin-B, lipophilin-A, and B2MG are biomarkers with the highest sensitivity and specificity of other biomarkers. Some of these biomarkers can be used to diagnose DED, differentiate between Sjögren DED syndrome and non-Sjögren DED syndrome, ADDE from EDE, and determine the severity of the disease.

Abstract

Introduction: Chemical eye burns, thermal burns, and radiation represent cases that often occur in cases in the emergency department in the eye area. Exposure to body fluids and cyanoacrylates (super glue) also often occur. These conditions if not followed up quickly will result in a poor prognosis. Objective: To determine the initial diagnosis and treatment of eye burns. Method: This article was created using the literature review method, involving 22 libraries both national and international books and journals. Results: Chemical burns represent the most frequent and potentially blind eye injury, this is an eye emergency that requires immediate treatment and starting treatment. The majority of victims are young and exposure can occur at home, work and can also be related to crime. Discussion: Alkaline injuries occur more frequently than acid injuries. Chemical injury to the eye results in extensive damage to the surface epithelium of the eye, cornea, anterior segment, and stem cells resulting in unilateral or bilateral permanent vision damage. Rapid and appropriate management is perhaps the most important factor in determining the final outcome. Conclusion: This article reviews emergency management and new techniques for improving the prognosis of patients with eye burn injuries. Abstract

Saat ini preeklamsia (PE) masih menjadi penyebab utama komplikasi kehamilan serta kematian pada ibu dan anak. Data WHO menunjukkan bahwa angka kejadian PE berkisar antara 5-15% dari seluruh kehamilan di dunia. Modalitas terapi PE saat ini hanya bersifat simtomatis dan belum mampu mencegah komplikasi yang ditimbulkan. Profilaksis yang tersedia pun masih dipertanyakan efektifitasnya. Patogenesis PE ditandai dengan invasi trofoblas yang dangkal sehingga menimbulkan kondisi hipoksia persisten yang menyebabkan peningkatan HIF-1-a-ME merupakan metabolit estradiol yang mampu menginhibisi translasi HIF-1a. Supresi terhadap HIF-1a akan mencegah transkripsi gen yang diregulasi oleh HIF-1a seperti ET-1, TGF-b3, AT1-AA, SLFT-1 dan sENG yang berperan penting dalam PE. Sayangnya, 2ME memiliki waktu paruh yang singkat. Enkapsulasi pada hidrogel termodifikasi terhadap 2-ME mampu mempertahankan efek teraupetik 2-ME hingga 160 hari. Pasca administrasi, 2-ME mampu menurunkan level HIF-1a, meningkatkan sifat invasif, dan migrasi trofoblas yang dibutuhkan dalam perkembangan arteri uteroplasenta yang adekuat, serta menurunkan tekanan darah dan level albumin urin maternal. Melihat efek klinis yang sangat menjanjikan, kombinasi MEC-2ME dapat menjadi modalitas mutakhir dalam upaya preventif dan kuratif penanganan PE.

Abstract

ABSTRACT Background: Paracetamol is used for analgesics and antipyretics, its use can damage the liver. Herbs that prevent liver disorders are curcuma. Black cumin can increase antioxidants. Objective: This study aims to compare the effectiveness of black cumin extract and curcuma against the liver function of paracetamol-induced rats. Methods: A laboratory experimental study posttest only with control group design. A total of 4 groups were treated for 7 days. SGOT and SGPT levels were tested. Data analysis using one-way ANOVA post hoc Games-Howell.Results: K- and K+ groups in this study showed an increase in SGOT SGPT levels. There is an effect of giving a dose of paracetamol 500 mg/kgBB on liver damage, there is no differences and significant effect in the administration of black cumin extract and curcuma dose 500 mg/kgBB to the liver of rats that have been induced by paracetamol (p>0.05). Conclusion: Giving paracetamol has an influence on rat liver damage. And there was no difference in the effects of hepatoprotector in groups given black cumin extract and curcuma in paracetamol-induced rats. Keywords: black cumin, curcuma, paracetamol, SGOT, SGPT

Abstract

ABSTRACT Introduction: Osteoarthritis (OA) is a degenerative joint disease affecting 10-15% population of adults above 60 years old. Prevalence rate of the disease in Indonesia in 2013 is 8,1% over the populations. Osteoarthritis is a slow progressive erosion of cartilage components which caused decreased of physical activity level and quality of life. Treatment for osteoarthritis can prevent the progressiveness of the disease. Discussion: Degradation of cartilage joint and subchondral bonein OA patients iscaused by mechanical stress or trauma. Trauma and the overuse of jointscouldrelease oxidative stress and activate sinovial immune response,IL-1 and TNF-? specifically. Activation of immune response can cause the decreased of aggrecans, decreased synthesis of hyaluronic acid which produce lubricant of joints, and impairment of c-Jun NH 2-terminal Kinase (JNK) signalling pathway which cause cartilage degradation and may lead to significant pain. JNKpathway inhibitor potencytoprevent and treat OA can inhibit cartilage degradation. Several compounds which are used to inhibit those proinflammatory cytokines activation are melatonine, vildagliptin (DPP-4 inhibitor), epigallocathecin-3-gallate (EGCG)and Lycorine. Conclusion: Inhibition of JNK pathway which is involved in the regulation of differentiation and apoptosis after oxidative stress exposureare supposed to decrease cartilage degradation significantly, so that it could be developed as a novel therapy modality to prevent worsening the disease of patients with OA. Abstract

Introduction: Onychocryptosis is a nail disease characterized by pain, redness, swelling or pus formation due to growing nail digging into the paronychium. Onychocryptosis is often encountered in daily practice and a general practitioner must be able to perform nail avulsion as one of the treatment option. Nail avulsion is an effective treatment for onychocryptosis but this prochedure does not prevent recurrence. For this purpose, a matricectomy can be performed following a nail avulsion procedure. This prochedure, although effective, is not widely discussed. Discussion: Matricectomy works by destroying the nail matrix so that the nail does not grow cutting the paronychium. Several studies showed the superiority of nail avulsion followed by chemical matricectomy in preventing recurrence of onychocryptosis compared to nail avulsion procedures alone. Matricectomy can be carried out using chemical agents such as phenol, TCA or sodium hydroxide. These three agents will cause necrosis of nail matrix in the area where they are applied. Saveral studies showed that the use of these chemical agents in matricectomy has a high success rate in preventing recurrence of onychocryptosis. Conclusion: Matricectomy can be done following nail avulsion to prevent reccurence of onychocryptosis. It can be carried out using phenol, TCA or sodium hydroxide. Abstract

Introduction: Depression is a mood-swing disorder that changes in a person within a certain period of time. Someone who suffers from depression will experience changes in mood, easily agitated, sensitive, feeling helpless, fatigue, decreased in concentration, changes in appetite, difficulty sleeping and symptoms of cramps, pain or indigestion without apparent cause. There are internal and external factors that caused depression in adolescents. Parenting is one of the environmental factors that influenced the mental condition of adolescents, which in turn can be an inhibiting factor or risk factor for depression. Purpose: to determine the effect of parenting on the incidence of depression in adolescents. Method: This article was created using the literature review method involving citations from books and journal articles published nationally and internationally. Results: Parenting affects depression in adolescents. Based on the three types of parenting such as democratic, permissive and authoritarian, it is known that democratic parenting is the pattern of parenting which has the most positive influence on the prevention of depression in adolescents. Discussion: Parenting is a way of parenting parents for children which includes support and care for children, which affects their future. Parenting is categorized as democratic, permissive, and authoritarian. Parenting affects the incidence of depression in adolescents that can be known based on the form of support, motivation, affection from parents to children so that it will affect mental health and can be a factor causing depression when entering adolescence. Conclusion: Parenting affects the incidence of depression in adolescents and can be a negative influence if not controlled as early as possible. Keywords: adolescent, depression, parenting.

Abstract

Introduction: Diabetes mellitus (DM) is a non-communicable disease that has increased from year to year. Type 2 diabetes mellitus is not caused by lack of insulin secretion, but is caused by the failure of the body's cells to respond to the hormone insulin (insulin resistance). Insulin resistance was found to be a major contributor to atherogenic dyslipidemia. Dyslipidemia in DM risks 2 to 4 times higher than non-DM. Although dyslipidemia has a great risk for people with type 2 diabetes mellitus, this conventional risk factor only explains a portion (25%) of excess cardiovascular risk in type 2 DM. Discussion: In uncontrolled type 2 DM patients, LDL oxidation occurs faster which results from an increase in chronic blood glucose levels. Glycemic control as a determinant of DM progressivity is determined through HbA1c examination. HbA1c levels are associated with blood triglyceride levels. Meanwhile, triglyceride levels are associated with total cholesterol and HDL cholesterol levels. HbA1c levels are also associated with LDL cholesterol levels. Conclusion: There is a relationship between lipid profile and the progression of type 2 diabetes mellitus. Keywords: type 2 diabetes mellitus, dyslipidemia, HbA1c, glycemic control, lipid profile

Abstract

Background:Framingham Risk Score and QRISK2 are cardiovascular desease (CVD) risk prediction recommended by American Heart Association for early detection of CVD to reduce morbidity and mortality. The primary purpose of this research is to compare the result from FRS and QRISK2 for adult in productive age in Jakarta. Method:This research is conducted using a cross-sectional diagnostic test. We collected primary data from 173 respondents in Wijaya Kusuma, Grogol, Jakarta Barat from September 2018 to November 2018. The data collected are medical history and results from blood pressure, cholesterol, and BMI. All the data entry is calculated by FRS and QRISK2 online calculator, then further analyzed by SPSS program. Results:ROC value for FRS and QRISK2 are 0.60 and 0.69. The sensitivity, spesifity, positive predicting value, and negative predicting value for FRS are 31%, 90%, 35%, and 88%. In another hand, QRISK2 are 58%,81%, 34%, and 91%. This analysis shows that QRISK2 have a higher accuracy compared to FRS. Conclusion:This research shows that there is a difference between FRS and QRISK2 in CVD risk calculation. QRISK2 have higher sensitivity and specifity compared to FRS, but FRS have a better positive and negative predicting value compared to QRISK2. From this research we concluded that use FRS to predict CVD risk in primary care is better compared to QRISK2.

Abstract

Thyrotoxicosis is defined as a clinical manifestation of excess thyroid hormone in the circulation. Epidemiological investigations report 0.2% of thyrotoxicosis caused by hydatidiform mole. Mola pregnancy is one of the diseases of the Gestational Trophoblast Disease (GTD), with amenorrhoea and irregular bleeding in the presence of vaginal vesicles. However, this condition can rarely be associated with hyperthyroidism, which may be related to the clinical features of hyperthyroidism. The basic pathogenesis of thyrotoxicosis is the similarity of HCG subunits with TSH which results in excessive stimulation of thyroid hormone. The following is a report of a 23-year-old woman who experienced amenorrhea followed by irregular vaginal bleeding, thyromegaly and abnormal thyroid hormone levels. Transabdominal ultrasound shows signs of molar pregnancy. Evacuation with curettage is done after the serum ?-hCG levels are reduced and thyroid hormone levels are also reduced.
