Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

This study was done to determine the suppression index of Cetirizine to the skin prick test results to obtain a correction constant or factor that can be used to assess the results of the skin prick test in patients who cannot stop the use of antihistamines (Cetirizine). This pre and post test study design clinical trial involved 22 atopic allergy patients who seek medical treatment at the Allergy and Immunology Outpatient Clinic Dr. Soetomo Hospital. Skin prick tests were done twice (SPT1 and SPT2) using house dust mite allergen extract to all study subjecs. The first (SPT1) were done after washout of all antihistamine for 1 week prior the test. All study subjects were then given Cetirizine 10 mg once daily for 5 days and on day 6 we performed the second test (SPT2). Cetirizine suppresion index and correction factor were calculated by comparing the wheal area of SPT1 and SPT2. All 22 study subjects (6 males and 16 females) were sensitized to house dust mite allergen. Mean serum total IgE levels were 176.42 + 352.5 IU/dL. Mean wheal area generated by the positive control (histamine 1 mg/mL) in SPT1 was 7.53 + 7.31 mm2, and in SPT2 was 1.08 + 1.46 mm2. Mean wheal area generated by house dust mite allergen in SPT1 was 43.57 + 36 mm2, and in SPT2 was 10.28 + 8.47 mm2. Cetirizine suppression index for positive controls (histamine 10 mg/mL) was 94.63 + 7.90% (p=0.000), while the Cetirizine suppression index for house dust mite allergen is 72.31 + 13.96% (p=0.000). There was no significant influence of serum total IgE levels to Cetirizine suppression index (p=0.381). The correction constant based on the calculation was 1.9. In conclusion, Cetirizine suppression index to the mean wheals area generated by house dust mite allergen was 72.31% and the correction constant was 1.9. In allergic patients who cannot stop their antihistamine drugs, Cetirizine 10 mg once daily can be used as a replacement and they still be able to undergo skin prick tests. The actual wheal diameter (or area) of the skin prick test results can be calculated by multiplying the measured wheal diameter (or area) under the Cetirizine administration with the correction constant.

Abstract

Tuberculosis (TB) is a chronic infectious disease caused by Mycobacterium tuberculosis that are often associated with uneffectiveness of the BCG vaccine and the high worm infection. The objective of this study was to determine the differences in the activity of Limfosit T type Th1 (IFN-g) and Th2 (IL-4) in lung tissue on Heligmosomoides polygyrus nematode and Mycobacterium tuberculosis sequential co-infection. This research using 49 mice were divide into 7 groups treated with infection by Mycobacterium tuberculose inhaled and Heligmosomoides polygyrus orally within 8 and 16 weeks. The levels of IFN-g in peripheral blood serum (89.929 + 3.533 pg/mL) resembles the pattern of the percentage of lymphocytes T CD4+ Th1 in lung tissue (3.246 + 0.519%) and peripheral blood (4.950 + 0.237%), while the levels IL-4 in the peripheral blood serum (20.782 + 4.043%) resembles the pattern of the percentage of lymphocytes T CD4+ Th2 in intestinal tissue (1.048 + 0.359%) and peripheral blood (1.196 + 0.557%). In conclusion, there is difference in the activity of lymphocytes T type Th1 and Th2 but it does not affect the immune response to Mycobacterium tuberculosis infection.

Abstract

Indonesia has a variety of herbal plants used as herbal/traditional medicines. Singawalang (Petiveria alliacea) is one of the herbal plants usually used in the United States as a cough medicine because of its expectorant effect. Singawalang plants are also widely used to cure tuberculosis. However, the investigations on the effects of toxicity on this plant leaf extract has not been done. This study aims to investigate the effects of active compounds in singawalang against Mycobacterium tuberculosis using a variety of solvents. The active compound of ethanol extract was obtained by maceration using ethanol solvent. The extract was then fractionated using column chromatography method, and using gradual eluent to produce fraction. The doses used were 0.5 mg/ml; 1 mg/ml; 2 mg/ml; 70% ethanol, h-hexane, benzene, chloroform, ethylacetate, silica gel 60GF254, Middlebrook 7H9, and 7H10. The reactions using Singawalang leaf extract with various solvents resulted in a reduction of the Mycobacterium tuberculosis colony growth, compared with the reaction on control treatment, treatment using DMSO 1%, and treatment using 70% ethanol. As a conclusion, the various solvents used did not make a significant difference. However, control treatment, treatment using DMSO 1%, and treatment using 70% ethanol had significant results.

Abstract

Up to the end of 2012, Social Security showed the occurence of 103.074 work accident cases. 91.21% of the accident casualties were recovered, 3.8% were disabled, 2.61% partially disabled, and the rest died (2.419 cases) and were totally-permanently disabled (37 cases), with the average of 282 work accident cases per day. Due to the increasingly tight corporate climate, the company tries to make production cost efficiency (cost of production). One solution that may be taken is by applying outsourcing system. With this system, the company may save expenses in funding human resources (HR) who work in the company concerned. If the aspects of the application of Occupational Safety and Health (OSH) are not properly managed, the possibility of work accidents and environmental pollution that have a negative effect on the corporate image and the continuity of company product supply will increase. This may affect the company's loss, both material and corporate image. The role and responsibility of the safety officer is very influential on the achievement of zero accident, so many safety programs are developed, such as safety briefing and safety patrol, with the aim to improve the aspect of OSH. However, some employees have not complied with the provisions of OSH implementation in the company they work at.

Abstract

The objects contained in crime scene and its surroundings have significance in the examination of forensic identification. The most commonly used specimens in the examination for identification are blood/blood spots, semen patches, vaginal swabs, buccal swabs and bones, including items often used by the perpetrator/victim the last time. For example, a mobile phone hearing aid (headset/earphone). In the use of earphones attached to the outer ear skin so it is suspected there is a serumen attached to the tool. One factor that can affect the quality of DNA is the prolonged exposure. Until now in Indonesia the effect of long exposure to room temperature on the quality of DNA on the DNA material of earphone swabs through DNA analysis has not been widely known. The type of study was laboratory experimental. Used earphones are exposed at room temperature within 1, 7, 14 and 20 days. The results of this study indicate that the environmental effect, ie the duration of exposure, affects the decrease in DNA content significantly from day 1 to 20. Detection of PCR mtDNA D-Loop HVS I visualization results 143 bp nt: 16268 16410 shows positive detection results (+) Only at day 1 exposure to room temperature [4 sample/66,67%] and day 7 [3 sample/50%]. Visualization of PCR results mtDNA D-Loop HVS II 126bp nt: 34 -159 was performed on day 1 day room exposure [2 sample/33,37%] and day 7 [6 sample/100%]. Conclusion, the duration of exposure to room temperature affect the quality of DNA from earpiece swab material. Decreased levels of earphones DNA swabs showed a significance value (p <0.005) against the effect of long time exposure to room temperature.

Abstract

Mucormycosis infection is an invasive, acute and rapidly progressing fungal infection with high mortality rate unless it was identified promptly. Oral manifestations of these infections, some times appeared as palatal ulcer and can be triggered by tooth extraction on maxilla or mandibula in which the organ is not cured but even damage the gingival and alveolar bone. Hyperbaric oxygen (HBO) therapy is the administration of 100% pure oxygen in a pressurized room more than 1 atmophere that can be used for therapy of mucormycosis infection in oral cavity. The aim of this study is to analyze the use of hyperbaric oxygen therapy on mucormycosis infections in oral cavity in 30 journals and 1journal of suspected case of mucormycosis infection post dental extraction from 1980 until 2016. Hyperbaric oxygen therapy was used as the last and adjunctive therapy after antifungal and surgery in 30 jounals but in 1 suspected case, hyperbaric oxygen therapy was given earlier. The need for consideration to provide early HBO therapy in case of mucormycosis infection in oral cavity will improve prognosis and survival rate of the patient.

Abstract

Escherichia coli (E. coli), a major cause of male genital tract infections, is asymptomatic and may result in male infertility. We have succeeded in isolating and characterizing proteins of E. Pili coli isolates from semen of infertile men who function as adhesin with a molecular weight (MW) 32.2 kDa. This study aims to prove the ability of IgG results adhesion proteins induced pili of E. MW coli 32.2 kDa as opsonin to determine the value of the activity and phagocytic capacity and as an anti- adhesion by calculating the average number of E. coli that attached to human spermatozoa. E. coli infertile men's semen were cultured using standard bacteriology. Peritoneal macrophages were isolated from mice. Spermatozoa from donors were prepared using Sil with Select Plus. IgG was obtained from mice immunized with (1) PBS (control), (2) E. coli pili adhesion protein isolated from infertile men semen with MW of 32.2 kDa and (3) weakened E. coli isolated from infertile men's semen. Phagocytic activity value was determined by counting the number of cells activated macrophage phagocytosis process in 100 cells. Phagocytic capacity value was determined by counting the number of bacteria ingested by 25 macrophages. Anti-adhesion test was done by counting the number of bacteria attached to 100 spermatozoa. The results of this study showed difference (p=0.000) in phagocytic activity and phagocytic capacity (p=0.000) between treatment (1) and (2), and between treatment (1) to (3). However, treatment (2) and (3) did not differ neither in phagocytic activity (p=0.693) nor in phagocytosis capacity (p=0.125). Anti-adhesion test produces difference (p=0.000) in the number of E. coli that attached to human spermatozoa between treatments (1) and treatment (2), and between treatments (1) and (3). The number of E. coli that attached to human spermatozoa between treatment (2) and treatment (3) was not significantly different (p=0.371). In conclusion, IgG from the induction of E. coli pili adhesion protein of infertile men semen isolates with MW of 32.2 kDa can increase phagocytic activity and capacity as well as serve as an anti- adhesion. Thus, IgG from the induction of E. coli pili adhesion protein of infertile men semen isolates with MW of 32.2 kDa is protective against in vitro E. coli infection, so that it can be used as material to prevent male reproductive tract infections due to E. coli.

Abstract

Spontaneous aneurysms of the superficial temporal artery (STA) are rare. Although STA aneurysms have a relatively benign course, when compared with aneurysms of larger caliber arteries, they may occasionally lead to severe hemorrhage and be associated with a multitude of bothersome symptoms. The objective of this study was to report a case of 72-year old male presented with spontaneous STA aneurysm. A 72 years old male presented with a lump on his right temporal side without any previous history of trauma and there was history of long standing hypertension. The lump had first been noticed 2 years before and it had been gradually increasing in size especially within last 6 months. On physical examination there was a pulsatile mass around 2 cm in diameter just above right ear. There was no thrill and bruit. CT angiography showed findings compatible with a STA aneurysm. The patient underwent proximal and distal ligation of the superficial temporal artery and excision of the aneurysm. Histopathological examination confirmed a true aneurysm of STA. The post operative period was uneventful. In conclusion, spontaneous STA aneurysm was best managed by surgical procedure. It consists of ligation and excision. It is simple, safe, and avoids recurrence.

Abstract

Broken nerves could regenerate when exposed to simple injuries by using a nerve conduit that has appropriate physiological and mechanical ability to support the nerves regeneration around the fissure of trauma. One of the biopolymer for the conduit composition is chitosan because it is biocompatible, biodegradable, non-toxic, and has similarity structure as natural glycosaminoglycans. The aim of research is to synthesize chitosan with variation of Degrees of Deacetylation (DD) and characterize the DD influence on mechanical properties and biocompatibility. Research design is prospective observational. Chitosan was treated with a decrease in the DD method and an increase in the temperature with the strength of alkaline solution, which was NaOH solution with concentrations of 5%, 20%, 35%, and 50% within 2 hours with a heating temperature of 95°C. The results of each DD variation were 23.24, 46.55, 53.48, and 55.06. It was characterized by tensile test with tensile strength values of 0.25 - 1.18 MPa. The degradation test results tend to decrease with the increasing concentration of NaOH proving that samples are biodegradable. The surface morphology of samples shows a pore range of 61.52 μm - 220.3 μm. The best result is the chitosan sample with 35% NaOH because due to the tensile characteristic and a pore in accordance with normal standard. Tensile strength is around 0.41 MPa - 3.69 MPa and pore size around 40 μm – 250 μm to accelerate nerve regeneration. The results are expected to provide alternative solution of nerve conduit development for peripheral nerve defects.

Abstract

Monitoring level of methotrexate is not only aimed at monitoring effectiveness, but also safety aspects of the administration of high-dose methotrexate because the use of high-dose methotrexate is one of the problems associated with toxicity in various organs. In the use of high-dose methotrexate, measurement of methotrexate is important to identify patients with a high risk of toxicity, particularly nephrotoxicity as well as for dosing guidelines for leucovorin therapy. The aim of this study was to analyze the safety of high-dose methotrexate on kidney function during chemotherapy consolidation phase in children with acute lymphoblastic leukemia. This was a longitudinal, observational prospective study conducted to determine the safety profile of high-dose methotrexate on kidney function during chemotherapy consolidation phase in children with acute lymphoblastic leukemia. Patients who met the inclusion criteria were given high-dose methotrexate according to the 2013 Indonesian ALL Chemotherapy Protocol. Measurement of methotrexate level and kidney function was done 3 times on each cycle of chemotherapy consolidation phase. Measurements were made on the 0, 24 and 48 hours after the first drop of high-dose methotrexate. This study had been reviewed by Ethics Committee of Dr. Soetomo Hospital Surabaya. There were 12 patients who met inclusion criteria and 6 patients among them had finished their chemotherapy consolidation phase completely. There was no significant change in kidney function after the administration of high-dose methotrexate compared to baseline (p>0.05) and there was no significant correlation between serum level of methotrexate versus creatinine clearance. In conclusion, methotrexate was safe to use in children with acute lymphoblastic leukemia.
