Abstract

This study explores how communication rooted in local cultural values in Central Java influences adolescents' capacity to make appropriate decisions regarding the risks of sexually transmitted diseases and human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS). Adolescents in Central Java are highly vulnerable to the risks of sexually transmitted diseases and HIV/AIDS given the sociocultural dynamics that influence their sexual behaviour. This phenomenon is exacerbated by the shift in the age of sexual initiation to an increasingly early age, from 18-19 years to 17-18 years, based on data from the Indonesian Demographic and Health Survey. The increase in the prevalence of risky sexual behaviour among adolescents requires a comprehensive approach, particularly through culture-based communication, to improve the ability to make appropriate decisions in the face of the threat of STDs and HIV/AIDS. Abstract

Background: Reproductive justice links reproductive rights with social justice, highlighting how systemic inequalities shape reproductive autonomy. Midwives, especially in underserved areas, are frontline advocates who provide clinical care, emotional support, and community advocacy aligned with this framework. Purpose: This study explores midwives’ experiences in responding to requests for assistance with unwanted pregnancy termination, a central issue in reproductive justice. Methods: A community service approach was used through Focus Group Discussions (FGDs) in Sorong, Southwest Papua, Indonesia. Fifteen midwives from rural and peri-urban health facilities participated. The FGDs aimed to understand their dual roles as healthcare providers and advocates for women’s reproductive rights. Results: Findings show midwives in Eastern Indonesia are key actors in advancing reproductive rights by offering vital reproductive health services and supporting marginalized women. However, their ability is hindered by barriers such as limited knowledge of regulations, lack of resources, cultural stigma, and restrictive policies. These challenges restrict their capacity to address reproductive justice issues effectively. Conclusion: The study highlights the urgent need to integrate reproductive justice principles into midwifery education and training. Equipping midwives with knowledge, advocacy tools, and policy awareness will strengthen their role in protecting women’s reproductive rights in low-resource settings. This research adds to global discussions on reproductive justice and underscores the transformative potential of midwifery practice in advancing health equity in Eastern Indonesia.

Abstract

Background: Contraceptive discontinuation remains a critical challenge in reproductive health programs, threatening program sustainability and maternal health outcomes. Transforming patterns from discontinuation to sustained commitment requires innovative health promotion models that address multi-dimensional factors influencing contraceptive behavior. Understanding the pathway from discontinuation to continuity is essential for designing effective, context-specific interventions. Purpose: This study aimed to design a comprehensive health promotion model that transforms contraceptive discontinuation patterns into sustained commitment among acceptors in Malang Regency, Indonesia, utilizing Health Belief Model and Social Cognitive Theory as theoretical foundations Methods: An exploratory qualitative study using Grounded Theory methodology was conducted through Focus Group Discussions (FGDs) involving key stakeholders including heads of Population and Family Planning Department, health center directors, midwives, Family Planning Field Officers (PLKB), and community health workers in Malang Regency. Data analysis followed the three-stage Grounded Theory coding process: open coding, axial coding, and selective coding. Results The designed health promotion model encompasses five interconnected dimensions grounded in Health Belief Model and Social Cognitive Theory: Individual Factors (knowledge, attitudes, self-efficacy), Environmental Factors (social support, healthcare accessibility), Behavioral Factors (observational learning, reinforcement), Perceptions (perceived risks, benefits, barriers), and Sustained Participation in Contraceptive Services. The transformation pathway "from discontinuation to commitment" was achieved through innovative strategies including the "Susimilek" program (husband involvement in contraceptive selection), community health worker engagement, and data-driven policy interventions. The model demonstrates how addressing multi-level determinants can convert discontinuation patterns into sustained contraceptive commitment, particularly addressing cultural, geographical, and structural barriers specific to Indonesian context. Conclusion: The designed health promotion model provides a comprehensive theoretical and practical framework for transforming contraceptive discontinuation patterns into sustained commitment through evidence-based, multi-level interventions. This culturally-sensitive model offers practical guidance for contraceptive services to enhance continuation rates and build long-term commitment among acceptors in similar low-middle income country contexts.

Abstract

Background: Unintended pregnancy is a global issue that threatens women's physical and mental health, with 50% of pregnancies worldwide falling into this category, largely influenced by male dominance in decision-making. In patriarchal families, women often lose their reproductive decision-making rights, increasing health risks and gender-based violence. Objectives: The purpose of this scoping review is to analyze women's experiences in facing unintended pregnancies within patriarchal families. Methods: The method used involved searches across four databases PubMed, Wiley, Science Direct, and Google Scholar using the PRISMA ScR methodology. Data were collected based on research objectives, publication year, study design, respondent characteristics, and research context. Critical appraisal was conducted using the JBI Critical Appraisal Tools. Results: This scoping review analyzed five qualitative studies from South Asia (Bangladesh, Pakistan, Nepal) and Africa (Eswatini, Nigeria) related to unintended pregnancy in patriarchal families. Based on the assessment, two articles were categorized as Grade A, while three were categorized as Grade B. The analysis identified three main themes: male-dominated decision-making patterns, gender inequality in contraceptive use and gender literacy, and the role of families in either supporting or opposing unintended pregnancies, including abortion decisions. Conclution:Unintended pregnancy in patriarchal families reflects the complexities of male dominance, gender inequality, and the lack of family support, which undermine women's rights, autonomy, and well-being.
