Abstract

Breast cancer ranks among the most prevalent forms of cancer, with chemotherapy serving as the primary treatment modality. Oral chemotherapy has emerged as a more practical option for patients, making adherence essential to its effectiveness. This study aimed to evaluate adherence to oral chemotherapy among patients with breast cancer. Using a descriptive observational cross-sectional design, the study was conducted among breast cancer patients receiving oral chemotherapy at a regional general hospital in East Kalimantan, Indonesia. Data were collected through the Oral Chemotherapy Adherence Scale (OCAS) questionnaire completed by the patients. A total of 55 female breast cancer patients, mostly aged 46–64 years (70.9%), married (87.3%), and unemployed (69.1%), were studied. Most had been diagnosed for 1–3 years (60.0%) and were in stage III (56.4%). All participants were covered by Indonesian Healthcare Insurance (JKN). The most prescribed oral chemotherapy was Tamoxifen (67.2%), followed by Capecitabine (5.5%), Exemestane (2.5%), and Letrozole (1.8%). Overall, 80% of patients demonstrated good medication adherence, with high compliance (94.5%) and persistence despite side effects (99%). Common barriers included forgetfulness (23.6% occasionally; 27.2% rarely) and difficulties accessing healthcare (70.9%). Most patients followed proper drug administration practices. Tamoxifen is the most commonly prescribed oral chemotherapy for breast cancer. This study shows that breast cancer patients generally adhere well to their oral chemotherapy regimens, though a minority may still struggle with adherence.

Abstract

Breast cancer incidence is increasing annually, with the proportion of stage III cases reaching 47%. Neo-adjuvant chemotherapy is necessary for its treatment, but its success rate is relatively low. Tumor progression and chemotherapy resistance depend on the stage, internal variables, the type of chemotherapy used, and the patient's psychological state. This study aims to determine the effect of psychotherapy on the clinical response to chemotherapy in women with locally advanced breast cancer. This randomized controlled trial involved 48 subjects, randomly divided into two groups of 24 subjects each. The control group received neo-adjuvant chemotherapy. The treatment group received neo-adjuvant chemotherapy and psychotherapy intervention at Dr. Soetomo General Hospital, Surabaya. Each subject received an assessment by the surgical team, including tumor size measurement, and the psychiatric team, including a psychological profile after being diagnosed with breast cancer and requiring neo-adjuvant chemotherapy. The treatment group had 87.5% of subjects responding well to chemotherapy, with 3 (12.5%) having no response, with 75% having a partial response, 12.5% ​​having a complete response, and 12.5% ​​having stable disease. The control group had 66.7% of subjects not responding, and 33.3% having a good response to chemotherapy, with 75% having a stable disease and 33.3% having a partial response. Psychotherapy has an effect on the clinical response to chemotherapy in women with locally advanced breast cancer.

Abstract

SARS-CoV-2 is a virus that is causing the coronavirus disease 2019 (COVID-19) pandemic that is currently still ongoing. Clinical similarities between COVID-19 patients and symptoms suggest a definite association with SARS-CoV-2. Therefore, it is necessary to carry out an analysis based on the literature. Search articles based on keywords “COVID-19”, “SARS-COV-2”, IL-21, “Tfh cells with years 2019-2021. Here we stand about SARS-CoV-2 which may cause autoimmunity in COVID-19 patients. IL-21 is a pro-inflammatory cytokine that occurs in SARS-CoV-2 and autoimmune infections. IL-21 is capable of eliciting autocrine and paracrine features. When IL-21 observes NK cells, Treg cells, epithelial cells, mast cells, B cells and dendritic cells, it becomes paracrine. Autocrine features when IL-21 promotes NKT cells, Th17 cells, CD4 T cells, CD8 T cells and tfh cells. The increased autocrine and paracrine levels of IL-21 found in some COVID-19 patients have contributed to the autoimmune development of COVID-19 infection. The role of IL-21 has a hyperinflammatory reaction which is a key role in the occurrence of autoimmune in COVID-19 patients.

Abstract

The rupture rate in abscesses is still high, the presence of rupture in LA will increase the morbidity and mortality rate, the presence of rupture will affect treatment options and affect the length of stay in hospital as well as the potential cost burden for this disease. This study aimed to determine the predictive factors for rupture of liver abscess patients who were hospitalized in Dr. Soetomo Hospital in the period 2011-2017. This study uses a retrospective - single analysis. The target population was patients with liver abscesses who were hospitalized in Dr. Soetomo Hospital Surabaya since January 1, 2011 - July 31, 2017.  The study sample was 137 people, sampling was done by consecutive sampling. Analysis of rupture predictive factors was carried out by bivariate test using chi square test and then variables in the bivariate analysis, results p <0.25, entered into multivariate regression analysis logistics to get a rupture predictor factor model. From 138 subjects, LA was more about males than females (4.3:1), with an average age of 44.80 ± 14.98 years, comorbid factors with the most DM, etiology of PLA higher than ALA. The most common complaints of LA are stomach pain and fever. In the rupture group, the proportion of hepatomegaly, pleural effusion, leucocytosis, elevated serum transaminases, hyperbilirubinemia, hypo albumin, giant-single abscess in right lobe was more dominant when compared to the non-rupture group. The proportion of morbidity (sepsis) and mortality in the rupture group was higher than in non-rupture group. Obtained a mortality rate of around 5.79 %, where septic shock is the most dominant cause of death. The magnitude of LA rupture is around 26.81%. The most dominant clinical manifestation of rupture is empyema, peritonitis, pneumonia and peripancreatic abscess. The results of this study indicate the size factor of abscesses and leucocytosis are independent predictive factors for rupture LA

Abstract

Craniofacial growth is a complex biological phenomenon governed by genetic blueprints but significantly modulated by environmental factors. With global malocclusion prevalence reaching 56 percent, sole reliance on genetic explanations is insufficient. The interaction between genetics and external determinants, particularly living conditions and lifestyle, plays a pivotal role in shaping orofacial architecture and stomatognathic health. This literature review aims to synthesize contemporary evidence regarding the impact of environmental determinants on craniofacial morphology. It seeks to elucidate the mechanistic pathways, specifically nutritional intake and functional habits, through which external factors alter genetic expressions and contribute to developmental anomalies. Discussion: Findings indicate that socioeconomic status and physical living conditions fundamentally influence orofacial maturation. Nutritional deficiencies correlate with dimensional deficits, while soft modern diets reduce masticatory stimulation. Furthermore, poor air quality triggers habitual mouth breathing. This respiratory adaptation results in "adenoid facies," characterized by maxillary constriction and vertical facial elongation, as evidenced by skeletal comparisons showing distinct morphological shifts between pre-industrial and modern populations. The interplay of genetics and environment dynamically governs craniofacial development. Understanding the etiology of malocclusion requires acknowledging environmental stressors as decisive factors. Effective management demands integrating clinical intervention with public health strategies addressing root environmental causes.

Abstract

Cleft lip is a congenital craniofacial anomaly influenced by multifactorial genetic and environmental factors, and advances in prenatal imaging have significantly improved early detection and treatment preparation. This narrative review aims to synthesize recent evidence from 2015 onward regarding the etiology, early diagnosis, and multidisciplinary management of cleft lip, including the roles of surgery, dentistry, orthodontics, speech therapy, and psychosocial support. Current literature demonstrates that refined surgical techniques have enhanced facial symmetry, functional restoration, and long-term developmental outcomes. Coordinated dental and orthodontic interventions are essential for addressing maxillary deficiencies, dental anomalies, and occlusal development throughout growth. Speech therapy remains crucial in supporting velopharyngeal competence and improving communication skills, while psychosocial guidance plays a significant role in fostering emotional well‑being, social adjustment, and long-term quality of life. Studies consistently emphasize that successful outcomes depend on early diagnosis, continuity of care, and effective collaboration among specialists within a structured cleft team. Evidence supports that optimal cleft lip management is achieved through a comprehensive, individualized, and multidisciplinary approach that integrates surgical, orthodontic, speech, and psychosocial interventions to maximize functional results, aesthetic satisfaction, and overall patient quality of life.

Abstract

Background: Jaw growth disorders involve malformations of and/or deviations within the relationship of the facial skeleton, specifically the maxilla and mandible. They may be consequent to a disturbance in genetic regulation of the hormones responsible for facial skeletal development, as well as the presence of a variety of both genetic and trauma-induced craniofacial disorders. As such, the aforementioned factors contribute to abnormal growth, position and symmetry of the jaw bones, potentially affecting day-to-day function in speech, mastication, and aesthetics. Objectives: This literature review aims to integrate current knowledge on the types of jaw growth disorders and their developmental characteristics within the context of craniofacial growth. Discussion: This review categorizes jaw growth disorders into sagittal, vertical, transverse, and asymmetrical growth patterns, highlighting differences in underlying developmental mechanisms, while simultaneously, comparing results across studies, which reveal consistent associations between growth disturbances and genetic factors, and environmental modifiers. Conclusion: Understanding jaw growth disorders as part of a developmental spectrum enhances understanding on the subject matter and supports early identification and preventive strategies. Integrating developmental biology with craniofacial growth research provides a framework for future longitudinal and interdisciplinary studies.

Abstract

The mineralization of enamel and dentin is a critical process in oral growth and development, underpinning the structural integrity and functional capacity of teeth. This process is highly dependent on the coordinated roles of vitamin D, calcium, and phosphorus. Vitamin D functions as a key regulator of mineral homeostasis, enhancing intestinal absorption and systemic availability of calcium and phosphorus, which are essential for hydroxyapatite crystal nucleation and growth. Calcium serves both as a structural component and as a signaling ion, orchestrating biomineralization within the extracellular matrix of dental tissues, while phosphorus contributes to crystal lattice formation and stabilization. Deficiencies or imbalances in these nutrients during critical developmental periods can result in enamel hypoplasia, dentin defects, and increased susceptibility to dental caries, ultimately compromising oral health. This narrative review synthesizes current evidence on the interdependent roles of vitamin D, calcium, and phosphorus in enamel and dentin formation, emphasizing their physiological, biochemical, and clinical significance in supporting optimal dental development

Abstract

Tongue posture and tongue thrust are functional components of the stomatognathic system that have been widely discussed in relation to craniofacial development during childhood and adolescence. Altered resting tongue position and atypical swallowing patterns are frequently described alongside variations in dental arch form, occlusion, and oral cavity morphology. A narrative synthesis of existing literature is required to clarify how these functional patterns are conceptualized within developmental and clinical contexts. This narrative review aims to explore how tongue posture and tongue thrust are discussed in the literature in relation to craniofacial development, focusing on descriptive patterns and reported variations across international studies. The reviewed literature describes tongue posture in association with dental arch configuration, palatal morphology, and orofacial muscular balance, while tongue thrust is commonly discussed alongside occlusal characteristics and jaw growth during developmental stages. Structural and functional aspects of the oral cavity are portrayed as interconnected, with substantial variability across studies reflecting differences in age, growth stages, and methodological approaches. This narrative review highlights consistent descriptive themes and notable heterogeneity in the literature addressing tongue-related functional patterns and craniofacial development. The findings support the use of non-causal interpretative frameworks and emphasize the value of integrative perspectives in future literature-based research.

Abstract

Orofacial clefts (OFC) are among the most common craniofacial malformations, significantly affecting oral function, facial aesthetics, and quality of life. Neural crest cells (NCCs) play a role in craniofacial development, and disruptions in their behavior are closely linked to the pathogenesis of OFCs. This study employed a narrative literature review approach. This narrative review was conducted using articles from PubMed, ScienceDirect, and Google Scholar, focusing on NCCs, craniofacial development, cleft lip, cleft palate, and orofacial clefts. NCCs are essential for forming craniofacial bones, cartilage, connective tissue, and palatal mesenchyme. Disruptions in their specification, migration, or differentiation, influenced by genetic, epigenetic, or environmental factors, can impair facial fusion, leading to cleft lip and/or palate. Evidence from molecular studies and clinical observations supports NCC dysfunction as a central mechanism in OFCs. Neural crest cells are fundamental to craniofacial morphogenesis, and their dysregulation is a key factor in orofacial clefts. Insights into NCC biology may guide improved risk assessment, preventive strategies, and targeted therapeutic approaches, emphasizing the importance of multidisciplinary management for affected individuals.
