Abstract

Streptococcus mutans is a major caries-causing bacteria, which can aggregate to form plaques. Nowadays, caries prevention approach in the cellular level can be done through molecular technology by using anti-bacterial, antibodies or addition of remineralization agent such as Casein Phospho Peptide-Amorphous Calcium Fluor Phosphate (CPP-ACFP) as an inhibitor of biofilm formation by Streptococcus mutans. The purpose of this study was to investigate the role of CPPACFP as an inhibitor to the growth of S. mutans, when used in low and moderate caries risk patient. The CPP-ACFP paste was applicated to patients for 60 days. Any caries inhibitory effects were evaluated as the difference of S.mutans count which was analyzed by S. mutans strip count (GC Corp) and colony count was performed with a bacterial culture. The result showed that there was no significant difference between before and after using CPP-ACFP in colony counting by S.mutans stripcount (GC Corp), pH, and buffer capacity from saliva. In conclusion, there was no effect after using CPP-ACFP since user didn’t use it regularly. To expect better result in using CPP-ACFP preventive treatment, we should improve patient’s compliance.

According to Household Health Survey in 2004 the prevalence of caries in Indonesia up to 90,05 %. This research was done to know parents role in taking care dental health of children, the dental health status and the relation between parents role with the dental health status. The type of this research is cross sectional study. The samples of this research were students and their parents. The numbers of sample were 167 students. Data sampling of children’s dental health status was being done by direct examination of oral cavity by using debris index according to Greene and Vermillion, deft index according to Klein and gingivitis index according to Ramfjord. Meanwhile data of parent’s role was being done by giving questionaires. The result of this research showed that father’s roles was still less in teaching their children how to brush teeth since 2 years old, notifying the time to brush teeth, accompanying children to dentist and checking oral health, yet there were well in taking care of children in brushing teeth till now, providing the toothbrush and toothpaste which is suitable for children and in taking care of the eating habits of children, while mother’s role was not much different. The mean of deft index of children was 4,20 ± 3,25, OHIS 0,39 ± 0,55 and gingivitis 0,03 ± 0,06. The result of the research showed that was relation between parent’s role with the mean of deft index, OHIS index and gingivitis index in children. In conclusion, the parents’ role in taking care of children’s dental health could affect the dental health status in children.

Abstract

Smoking is one of the most important risk factors for oral diseases. Tobacco smoking produces more than 4000 chemical materials. Gingival protects underlying tissue to oral environment. Cells of gingival epithelium are metabolic actively, and can be reacted to internal and external stimuli. The thickness of oral mucosa epithelium is related with the amount of cell or proliferation activity. This study aimed to investigate effect amount of cigarette smoking on thickness of gingival epithelium. This study was animal experimental laboratory study. This study used 40 males Rattus Norvegicus strain Spraque Dawley, and were divided into 4 groups. They were control, mild, moderate and severe smoker groups. The animal models were exposed 2 grams by pump and were euthanized with ketamine in the 7th day. Measurement of gingival epithelium thickness used hematoxylin eosin that was measured from basale to granular layer and 3 sites (in µm). Data were analyzed by one way anova. The results of this research showed that there was an increasing thickness of gingival epithelium of animal models. The highest of gingival epithelium thickness was in severe smoker groups. In conclusion, cigarette smoking influenced on gingival epithelium thickness.

Abstract

During cavity preparation and the removal of caries dentine, it is possible to accidentally expose the dental pulp. Direct pulp capping may be indicated for maintaining pulp health and function. The direct pulp capping material that used in this study was platelet rich plasma (PRP). Platelet rich plasma is on autologous growth factor containing of platelet derived growth factor (PDGF), transforming growth factor-β (TGF-β), vascular endothelial growth factor (VEGF), insulin growth factor (IGF) and epidermal growth factor (EGF). The aim of this study was to evaluate inflammation response of pulp tissue following direct pulp capping with PRP. Thirty sound teeth from Sprague Dawley were used and divided into two groups: group 1 as a control, teeth were capped with calcium hydroxide/Ca(OH)2 (n=15) and group 2 with PRP (n=15). After the 1st, 7th and 21st days, respectively, 5 teeth of each group were processed for light microscopic examination. Inflammation responses were assessed by haematoxylin eosin. The result showed a similar inflammation response of Ca(OH)2 (group 1) on the 1st and 7th days 40% necrose and 40% severe inflammation and the 21st day 60% samples were severe inflammation. Inflammation response of PRP (group 2) on the 1st day showed 40% necrose and 40% severe inflammation, on the 7th day, 100% severe inflammation and on the 21st day, 60% mild inflammation. After the 21th day observation period, Kruskal Wallis test showed that inflammation response was not significant in both groups (p>0,05). In conclusion, PRP and Ca(OH)2 as a direct pulp capping material had no different inflammation response.

Abstract

Erythema multiforme is an acute vesiculo-bullous disease on the skin and membrane mucous, self limiting, and it presents with clinical variation (multiforme). Patients who suffer EM will feel painful of the oral lesion, wich could interfere in intaking of food and nutrition, and further more can bring down the body resistance. Approximatelly 50% case of EM caused by medicamen hiperensitivity reaction, most often because the class of sulfonamide drugs, NSAIDs, and anticonvulsants. A male patient aged 41 years, suffering of painful blisters with spontaneous bleeding on the mouth, difficulty in eating and talking. Patient was taking carbamazepine and amitriptyline given by neurologist. Patient also finished taking amoxicillin provided by dentist two days ago. Extra oral shows erosion and ulcers, painful and easily bleeding on the red upper and lower lips. Most ulcers were covered by erythematous-brown crust. Intra oral showed erosion/ ulcers, painful, easily bleeding, and covered by yellow-white pseudomembrane. There were no abnormalities on the skin. In conclusion to stop using all drugs that suspected as the cause, systemic corticosteroid administration, topical anesthesia, supportive therapy, and education for patient responded positively to patient’s recovery.

Abstract

Tetracycline becomes one of intrinsic factors which causes tooth staining. Discoloration due to tetracycline is divided into 4 degrees of severity. Bleaching can be treated for the 1st or the 2nd degree. The 3rd and the 4th degree can not be treated but must be veneer. The 3rd and 4th degree are indicated by dark tooth color, and a ribbon-like picture showed in labial teeth. A 30 years female patient came to the Dental Clinic, dental examination found teeth color dark grayish yellow (C4 shade guide color) and in the central of labial there was a picture like white banded. Diagnosed of the case was the 3rd degree of discoloration. On the first visit, dental impression was performed for making bleaching tray. On the second visit, there was polished with pumice powder. Carbamide peroxide 10% as bleaching paste was applied to the labial bleaching tray, and inserted for 30 minutes. Once removed then patient gargled with warm water. Patients got instructions to do it at home twice a day for 2 weeks the result showed third. On the third visite the colour of the teeth look brighter, and there were no gingiva irritation and sensitive teeth. Patient was very satisfied. In conclusion, bleaching of the third degree tetracycline discoloration can make the teeth look brighte.

Abstract

One of a self-ligating bracket is Damon bracket system, most is the blending of passive self-ligating brackets and hightechnology archwires. Damon bracket system usually needs the absence of teeth extraction for treatment but in some Asian cases the treatment still need to extract teeth. Recently, more recognized that the othodontic treatment goals shifted from dentition to facial esthetic, not only the convexity of facial esthetic, but also how to create a nice smile arc. This case report presents two treatments of mild Class III severe crowded teeth with Damon bracket system, the extraction case and the non extraction case. The first case, female patient, 17 years old, had 0,5 mm overjet and 1 mm overbite. Both first premolars maxilla and mandibular had been extracted. The second case, female patient, 13 years old, had - 4 mm overjet and 3 mm overbite, none of teeth had been extracted. High torque Damon bracket system slot 0.022” was applied to the first patient and low torque with slot 0.018” to the second patient. Class III dental relationship was corrected with a positive overbite, overjet and good occlusion was achieved for both patients within 17 months. There were no more crowding teeth, profile is also improved. In conclusion, damon bracket system was given the fast and painless treatment with longer time interval of controls.
