Abstract

Cleft palate patients experience various problems, especially those affecting their speech intelligibility, such as speech articulation disorder. The aim of this research was to prove the effect of cleft palate reconstruction with modified pushback z–plasty technique at the nasal mucosa on velopharyngeal function and articulation function in speaking. A research has been conducted on post-reconstruction cleft palate patients by using modified pushback z-plasty technique at the nasal mucosa. The methods were examining velopharyngeal components with cephalometric radiograph, assessing airflow released from the nose when the patients pronounce vowel ‘a’ using nasal emission device and assessing speech articulation results through Sehati Program at Harapan Kita Children and Maternity Hospital, Jakarta. The research results showed that there was a statistically significant effect of cleft palate reconstruction with pushback z-plasty technique at the nasal mucosa on velopharyngeal function, and there was a statistically significant effect of cleft palate reconstruction with pushback z-plasty technique at the nasal mucosa on articulation function in speaking. In addition, there was also a statistically significant correlation between velopharyngeal function and speech articulation function of post-reconstruction cleft palate patients with pushback z-plasty technique at the nasal mucosa based on the cephalometric radiograph, but the result was not significant statistically based on nasal emission assessment.

Abstract

Sleep apnea syndrome (SAS), a common disorder, is characterized by repetitive episodes of cessation of breathing during sleep, resulting in hypoxemia and sleep disruption. The consequences of the abnormal breathing during sleep include daytime sleepiness, neurocognitive dysfunction, development of cardiovascular disorders, metabolic dysfunction, and impaired quality of life. There are three types of SA: obstructive sleep apnea (OSA), central sleep apnea (CSA) and mixed sleep apnea (MSA). OSA is a prevalent disorder in which there are snoring, repetitive apneic episodes, and daytime sleepiness. The diagnosis of SA includes assessment of subjective symptoms and apneic episodes during sleep documented by polysomnography. Treatments of OSA include continuous positive airway pressure (CPAP), oral appliance, and surgery; patients with CSAS are treated with oxygen, adaptive servo-ventilation, or CPAP. With assessment and treatment of the SA, patients usually have resolution of their disabling symptoms, subsequently resulting in improved quality of life. As conclusion, the succesful treatment depends on the choice of treatment either surgery or non surgery by patient. Early examination and risk that maybe occure is very important to inform before treatment is performed.

Abstract

Buccal fat pad (BFP) sebagai graft pedikel awalnya digunakan untuk merekonstruksi defek intra oral dengan ukuranmedium. Penggunaan graft pedikel untuk menutup defek tulang palatal dan mencegah atau memperbaiki fistula telahpopular akhir-akhir ini. Secara anatomis, BFP terdiri atas badan utama dan empat perluasan. Vaskularisasi BFPbersumber dari cabang arteri maksilaris, arteri temporalis superfisial dan fasial. Vaskularisasi yang berlimpah ke jaringanlunak memungkinkan terjadinya epitelisasi dengan cepat. Kemudahan pengambilan BFP, mobilisasi graft, pasokan darahyang sangat baik dan komplikasi yang minimal membuat graft pedikel BFP sebagai metode yang nyaman dan dapatdiandalkan pada penutupan celah langit-langit. Sebagai kesimpulan, flep pedikel BFP dapat dipertimbangkan sebagaiprosedur alternatif untuk memperluas pilihan terapi. Graft BFP memberikan keuntungan pada rekonstruksi celah langitlangit.

Abstract

Dewasa ini, sindroma sendi temporomandibular sering muncul dalam dunia kedokteran. Setiap kelainan senditemporomandibular tidaklah sama, oleh sebab itu dokter gigi perlu mengidentifikasi gejala dan menegakkan diagnosayang akurat untuk memutuskan perawatan yang tepat. Secara umum, perawatan untuk kelainan sendi temporomandibulardibagi menjadi perawatan konservatif, bedah dan bedah minimal invasif. Tujuan tulisan ini adalah untukmemperkenalkan kecanggihan teknologi pada bedah minimal invasif sendi temporomandibular yaitu artroskopi danartrosentesis dalam menangani internal derangement. Kesimpulannya, kedua teknik tersebut terbukti efektif karenamampu mengurangi rasa sakit dan meningkatkan fungsi sendi dengan resiko minimal dan pemulihan yang cepat.

Abstract

Tooth extraction is a process of removing teeth from the alveolar bone. In wound healing, fibroblast are very important cells. The main purpose of this study was to determine the effect of mobe leaf 1% and 3% extract gel (Artocarpus lakoocha) on fibroblast proliferation in post extraction tooth socket wound healing. This research used 16 samples of wistar rats, divided into 4 groups, a positive control group, a negative control group and a 1% and 3% mobe leaf extract gel group. The left mandibular incisors were extracted, then 1% and 3% gels of mobe leaf extract were applied on day 1 to day 7. Data analysis was calculated using the Kruskal-Wallis test on clinical data and one way ANOVA test for microscopic. The result of the socket wound healing activity test for a good concentration of mobe leaf extract gel was 3%. This research shows significant resultith p-value of 0.018 (< 0.05) on the closure of the socket wound clinically which means the closure of the wound accelerates because of the mobe leaf 3% extract gel treatment. The distance of fibroblast on microscopically shows significant resultith a p-value of 0.002 (< 0.05), which means that there was an enlargement of the distance fibroblast at the socket wound closure with application of mobe leaf 3% extract gel. From the results of the study it can be concluded that mobe leaf 3% extract gel has the best ability to show acceleration the closure of the socket wound either clinically or microscopically.

Abstract

Proses menua (lansia) adalah proses alami yang disertai adanya penurunan kondisi fisik, psikologis maupun sosial yang saling berinteraksi satu sama lain. Perubahan secara biologis ini dapat mempengaruhi status gizi lansia yang berkaitan erat dengan menurunnya aktivitas biologis tubuhnya dan berpotensi menimbulkan masalah kesehatan secara umum maupun kesehatan jiwa secara khusus pada lansia. Tujuan program ini adalah untuk meningkatkan kualitas hidup lansia melalui perawatan gigi yang mengganggu seperti gigi goyang dan berlubang, serta pembuatan gigi tiruan. Program ini dilakukan dengan tiga tahap dengan masing-masing tahap dilakukan oleh pakar prostodonsia, psikologi, dan rongga mulut dengan prosedur sesuai bidangnya. Hasil dan capaian yang diperoleh pada program ini terpenuhi sesuai dengan permasalahan yang terdapat pada masing-masing bidang. Kesimpulannya, penyuluhan yang dilakukan memberikan pengaruh positif untuk pengetahuan tentang akibat dan perawatan kehilangan gigi, serta motivasi untuk memakai gigi tiruan menjadi ada.

Abstract

Background: One of the common frequently performed procedures in dentistry is tooth extraction. After tooth extraction, a series of biological events occurs in the alveolar socket that ultimately results in healing of the socket. Binahong (Anredera cordifolia) is an herbaceous plant that contains active compounds that may accelerate the wound healing process. Purpose: The aim of this study is to investigate the effect of 3% binahong leaf extract gel on the healing process of the postextraction wound. Methods: 3% binahong leaf extract gel was prepared with the composition of 1 g carbopol, 1 g hydroxypropilmethylcellulose (HPMC), 4 g glycerin, 3 g triethanolamine (TEA), 0.04 g Nipagin, 0.04 g Nipasol, 1.2 g binahong leaf extract and Aq ad 40 g. A total of 18 people were subjected to tooth extraction and were then divided into two groups, with nine people in the binahong group and nine in the control group. Following that, residual socket volume (RSV) was examined on days 3, 7 and 14 after the extraction in both the binahong and control groups. The RSV value was obtained by calculating the mesiodistal x bucolingual width x the depth probing and measured using a calliper and probe. Repeated analysis of variance (ANOVA) and Friedman test followed by Wilcoxon and dependent t-test (p<0.05) were used to analyse data. Results: Significant difference in the mean RSV between the control and binahong groups was observed on days 3, 7 and 14 post tooth extraction. The RSV value in the binahong group was lower when compared to the control group (p <0.05). Conclusion: 3% binahong leaf extract gel can accelerate the process of healing socket wounds.

Abstract

Background: Alveolar osteitis is a well-known complication that occurs following a tooth extraction when the clot within the socket breaks down too early, causing increased localised inflammation and extreme pain. Alveolar osteitis delays the wound healing process of the socket. The polyphenols in the cocoa bean (Theobroma cacao L.) can stimulate the wound healing process. Purpose: The aim of this study was to analyse the effect of an 8% cocoa bean extract gel on the healing of alveolar osteitis following a tooth extraction. Methods: This study is an in vivo experiment with a posttest-only control group design. Thirty-six male Wistar rats were divided into three groups: a negative control, positive control and an 8% cocoa bean extract gel. A tooth extraction was performed on the mandibular incisor, and alveolar osteitis was induced by the application of adrenaline using a paper point on the socket. On the 3rd, 7th and 14th days, the clinical wound size of the extraction socket was measured, and the rats were sacrificed to observe the number of macrophages, fibroblasts and osteoblasts microscopically. A two-way analysis of variance test and post hoc least significant difference test were used to analyse the data (p < 0.05). Results: The data analysis showed a significant difference in the clinical wound size of the extraction socket and the number of macrophages, fibroblasts and osteoblasts between the 8% cocoa bean extract gel and the control groups (p = 0.000). Conclusion: An 8% cocoa bean extract gel stimulates the healing of alveolar osteitis following tooth extraction in Wistar rats.

Abstract

Objective: To determine the effectiveness of using green betel leaf (Piper bettle Linn) extract hand sanitizer gel in reducing the number of bacterial colonies in the palms of hand. Method: In this experimental research with posttest only with control group de-sign using dental clinical clerkship students of Faculty of Dentistry Universitas Sumatera Utara. They were evenly divided into 5 groups. They were chlorhexidine gel (group 1), 1% green betel leaf extract (group 2), 3% green betel leaf extract (group 3), 5% green betel leaf extract (group 4), and use of placebo (group 5). Bacterial colonies isolated from the swabs were identified by stan-dard microbiological procedures. All statistical analysis methods were conducted using the one-way Anova and posthoc LSD tests. Results: The results of univariate analysis showed that sig <0.05, meaning that there was a difference in the number of bac-terial colonies between chlorhexidine, 1%, 3%, and 5% green betel extract gel. The one-way Anova test at 95% confidence level proved that the differences are significant at p-value 0,001. Conclusion: There is a difference in the effectiveness of using a hand sanitizer for green betel leaf extract on the number of bacterial colonies in the palms of hand. Hand sanitizer green betel leaf extract gel 5% more effective than 1% and 3% concentrations.

Abstract

Cleft is a congenital abnormal space or gap in the upper lip, alveolus, or palate. This congenital abnormality occurs between the 5th and 10th weeks of pregnancy. Clefts are divided into cleft lip, cleft palate, as well as cleft lip and palate. Cleft lip and palate are caused by the interaction of individual genes with certain environmental factors. Mothers’ knowledge about risk factor of cleft lip and palate may promote better health-related behavior in their pregnancy by increasing the understanding about the risk factor. Unfortunately, until now there is still limited data about this. The purpose of this study was to determine the knowledge of pregnant women about risk factor of cleft lip and palate. This was a descriptive study that used the survey method. This study was conducted using a questionnaire distributed to 67 pregnant women (n = 67). The questionnaire consisted of 13 validated questions. The results of this study found that 10.4% of the respondents had good knowledge, 32.8% of the respondents had moderate knowledge and 56.7% of the respondents had poor knowledge. The overall knowledge of the pregnant women about risk factor of cleft lip and palate at Puskesmas Mutiara Asahan fell in the low category.
