Abstract

Mental retardation (MR), also known as Intellectual Disability (ID), is a lifelong mental disorder characterized by significant cognitive and adaptive impairments, often associated with structural or functional brain anomalies and diverse etiologies that frequently co-occur with conditions such as epilepsy. In Indonesia, a survey conducted in 2012 revealed that 2.45% of the population had disabilities, including individuals with mental retardation. The World Health Organization (WHO) estimates the prevalence of mental retardation in Indonesia to be around 1-3% of the population, with one-quarter of cases classified as moderate to severe mental retardation. The occurrence of mental retardation is closely linked to a child's developmental trajectory, influenced by genetic and environmental factors. In terms of pathogenesis, the onset can originate from prenatal, perinatal, or postnatal factors, with various contributing elements, including a history of seizures. The severity levels are classified as mild, moderate, severe, or profound mental retardation. Epilepsy is a common neurological disorder in children, often co-occurring with mental retardation, found in 25% of cases. The occurrence of epilepsy can either accompany or be the cause of mental retardation, leading to a classification known as Developmental and Epileptic Encephalopathy (DEE). This case report will present the case of a teenager with mental retardation who has a history of recurrent seizures, illustrating the complex relationship between these two conditions. Hence, there is a need for a comprehensive approach and increased awareness in diagnosing and managing children with concurrent mental retardation and epilepsy with the goal of providing optimal care.

Abstract

Based on data from the World Health Organization, it is estimated that around 15-20% of the world's population suffers from hypertension and data from the 2018 National Basic Health Research, there are 54.4% of the Indonesian population aged ≥ 15 years with cholesterol levels (≥ 200 mg/dl. Purpose of writing This is to apply the principles of family medicine services and carry out holistic management based on a framework for solving patient problems using a patient-centered, family focused and community oriented approach based on Evidence Based Medicine. This study is a case report. Internal risk factors were found, namely family history, unbalanced nutritional intake, and lack of knowledge of the disease. Ekxternal risk factors include lack of family knowledge about desease prevention and complication, lack of supervision over diet and medication. Patient Mrs. J, 59 years old, was diagnosed as grade I hypertension, dyslipidemia, and grade I obesity. In this case, diagnosis and treatment were carried out in accordance with theory and related journals. After the intervention was carried out, there was a reduction in clinical symptoms and an increase in patient and family knowledge. Holistic management can improve the knowledge, attitudes and behavior of patients and families in maintaining health.

Abstract

Pathogenic diseases are diseases that are an important problem for public health, especially in developing countries like Indonesia. This disease occurs when disease-causing microorganisms infect the host's body tissue and then multiply and react to the organism being attacked because of the toxins they produce. Several types of disease pathogens can be caused by viruses, bacteria, fungi and parasites. Detection of pathogenic bacteria as a source of infection still relies on isolation methods in pure media culture and microscopic analysis followed by biochemical reactions. However, when using existing techniques, it is difficult to isolate some bacteria in pure culture media, making it a challenge to develop other methods to identify the source of the infection. So far, identification and detection of pathogenic bacteria has only been carried out by observation based on clinical symptoms and history of disease, as well as morphological and physiological characteristics. However, this method is less efficient so a technique is needed that can detect pathogenic bacteria quickly and accurately. There are several types of methods that can be used to identify pathogenic bacteria, namely, methods that target nucleic acids, immunology-based methods, and biosensor-based methods.

Abstract

Gout arthritis is a progressive chronic inflammatory disease due to deposition of monosodium urate (MSU) crystals in the joints due to long-lasting hyperuricemia with several associated risk factors including excessive consumption of purine compounds and lack of physical activity. Gout arthritis needs to be managed comprehensively and holistically through the role of the family doctor in order to improve the patient's quality of life and prevent complications that can occur. Apply the principles of family medicine services to patients based on the problems found and carry out holistic and comprehensive management using the principles of patient-centered and family approach based on Evidence Based Medicine. This study is a case report. Primary data were obtained from anamnesis, physical examination, and home visits to assess the physical environment. Secondary data was obtained from the patient's medical record. Assessment based on holistic diagnostics at the beginning, process, and end of the study. Mrs. M, 67 years old, came with complaints of joint pain in both knees since 1 week ago. The patient was diagnosed with gout arthritis. In this case, diagnosis and treatment have been carried out according to the theory and related journals. Management has been carried out with the approach of family doctor Ny. M 67 years old with Gout arthritis adjusted based on initial holistic diagnostics. The interventions carried out have increased patient knowledge and improved the healthy lifestyle of patients and their families as indicated by improvements in the final holistic diagnostic.

Abstract

Iron is an important component of hemoglobin. The function of iron, which is as a source of energy for muscles so that it can affect physical endurance and the ability to work in adolescents. Iron deficiency can lead to hematological disorders of anemia. Anemia is a body condition in which the number of red blood cells and oxygen transport capacity is insufficient to meet the physiological needs of the body. Moringa leaves can be used as a substitute for iron tablets, because Moringa leaf powder contains 28 mg of iron. The method used in this writing, is literature review by collecting several research articles. This literature source search uses literature research sources in the form of google scholar and Pubmed. The inclusion criteria from the search, namely research articles using Indonesian and English, articles published within the last 10 years (2013-2023), and discussing the topic of moringa oleifera and anemia. The results of this literature search can be concluded that Moringa oleifera supplementation significantly increases serum iron levels.

Abstract

Fanconi anemia (AF) is a rare, multisystem genetic disorder that affects various physiological aspects, especially the bone marrow, and increases the risk of cancer, such as acute myeloid leukemia (AML) and head and neck cancer. This disease is caused by mutations in 23 genes of the FA/BRCA DNA repair pathway. This article presents the results of a literature study on FA, with a focus on etiology, pathophysiology, molecular pathology, diagnosis, risk factors, clinical symptoms, and management. Through literature study methods, the author collected information from national and international journal sources between 2012 and 2021. The etiology of FA involves mutations in 23 FA genes, with variations in carrier frequency depending on ethnic group. The pathophysiology of FA is related to the inability of cells to repair damaged DNA interstrand crosslinks (ICLs), which can lead to bone marrow failure. Molecular pathology involves the function of the FA/BRCA repair pathway and its impact on the genome. Diagnosis of FA requires clinical evaluation, molecular analysis, and bone marrow examination. Risk factors involve genetic aspects and specific carrier frequencies in ethnic populations. Clinical symptoms of FA involve anemia, infection, and somatic manifestations. Management of FA depends on the severity, with hematopoietic stem cell transplantation (HSCT) being the primary choice. HSCT from matched, nonmatched, or haploidentical donors has provided increasingly better results. This study underscores the importance of early diagnosis of FA to improve prognosis. Although HSCT is the primary treatment option, careful consideration of risks and benefits must be taken. The results of this literature study provide in-depth insight into FA, supporting appropriate monitoring and management efforts for individuals with this condition.

Abstract

Globally, cancer is the leading cause of death because its pathophysiology includes multifactorial components. In general, various things mediate its development, such as genetic mutations, toxin intake, radiation exposure, unhealthy food, and work pressure. To increase the chance of cure from cancer and the possibility of survival, early detection, and appropriate treatment are needed. In the last decade, the high content of bioactive compounds contained in nutraceutical components has been considered as a complement in food, food supplements, and medicines. The medicines here include appropriate treatment to fight cancer without worrying about anticancer side effects that can cause adverse responses. Compounds in nutraceuticals are natural bioactive ingredients that have anticancer properties, which can kill cells that have metastasized or have become cancerous without being toxic to surrounding normal cells. This effect is proven when these natural bioactive compounds are contained in conventional medicine and show that nutraceutical supplementation contributes to the success of cancer treatment as an anticancer. This review examines the current knowledge from various literature regarding several natural bioactive extracts that contain certain polyphenolic compounds. The activity of this compound can be seen from the inhibition of cell proliferation, anti-inflammatory action, and antioxidant activity. Therefore, nutraceutical supplementation can be recommended along with conventional treatment to obtain a better therapeutic response in cancer patients.

Abstract

Hepatitis virus infection is a global public health threat that causes substantial liver-related morbidity and mortality. Hepatitis is a liver infection that causes inflammation, causing swelling of the liver. This liver infection has various variants, all of which are detrimental to human health. In recent times, natural compounds have become popular to be developed as antiviral drugs to fight viral infections. These natural compounds can come from plant extraction or a combination of animal and plant extraction. Several of these compounds have shown significant effectiveness in their mechanism as antivirals against the hepatitis virus due to the presence of bioactive components in them. The mechanism of action of this bioactive component is demonstrated by disrupting the virus life cycle, replication, release, and interaction with the host. This literature study will explain strategies for treating hepatitis with these natural compounds. Recent research shows that bioactive components such as phenylpropanoids, flavonoids, xanthones, anthraquinones, terpenoids, alkaloids, and aromatics have antiviral activity against hepatitis. Therefore, based on a literature review of the latest research that reveals clinical trials of these bioactive components, this natural compound can be used as a potential treatment alternative in the management of hepatitis in the future.

Abstract

Leprosy is a chronic granulomatous infectious disease caused by the obligate intracellular bacterium Mycobacterium leprae (M. leprae). This disease comes from the Latin word "lepros" which means scale. M. Leprae bacteria enter the human body through 2 main routes, namely through the skin and the upper respiratory tract. Leprosy bacilli enter the human body through direct contact with the skin or nasal mucosa originating from droplets. Bacteria from droplets will survive for 2 days in a dry environment, even up to 10 days in a humid environment and low temperatures. The success of therapy for Morbus Hansen patients is expressed by RFT (Release from Treatment). RFT can be stated after the dose is fulfilled without having to undergo laboratory examination. PB (paucibacillary) patients who have received 6 doses (blister) of treatment within 6-9 months are declared RFT, without having to undergo a laboratory examination. MB (multibacillary) patients who have received 12 doses (blister) of MDT treatment within 12-18 months are declared RFT, without having to undergo a laboratory examination. The factors that play a role in the success of therapy for leprosy patients are age, gender, education, employment, knowledge, compliance with taking medication, and family support.

Abstract

Hypertension is a global health problem that requires serious attention. Basic Health Research (Riskesdas) in 2018 shows that the prevalence of hypertension in Indonesia with a population of around 260 million is 34.1%, this shows an increase compared to the prevalence of hypertension in Riskesdas in 2013, which was 27.8%. The estimated number of hypertension cases in Indonesia is 63,309,620 people, while the death rate in Indonesia due to hypertension is 427,218 deaths. Data from WHO shows that the number of people suffering from hypertension doubled between 1990 and 2019, from 650 million people to 1.3 billion people worldwide. The purpose of this case report is to explore the role of hypertension management in the context of family medicine holistically and comprehensively through a Patient Centered, Family Focused, and Community Oriented approach. Family medicine has a crucial role in detecting, preventing, and managing hypertension. This study is a case report. Primary data were obtained through autoanamnesis, physical examination, and home visits. Secondary data were obtained from the patient's medical record. Mrs. E, 32 years old, has been complaining of headaches for the past day. Internal risk factors in the patient are being overweight, not exercising regularly, often eating fast food, having a history of cancer in parent, feeling stressed at work, and a curative treatment pattern. External risk factors in the patient is that patient have work activities that are mostly sitting. Diagnosis and management of patient is carried out holistically, Patient Centered, Family Focused, and Community Oriented. There are changes in clinical and patient behavior after the intervention. The family medicine approach emphasizes preventive interventions, patient education, and long-term management. This approach is expected to enhance the understanding of patients and their families regarding the importance of hypertension management and encourage sustainable behavioral changes.
