Abstract

Recurrent diabetic foot ulcer is a major chronic complication of type 2 diabetes mellitus that contributes to increasedmorbidity, amputation rates, and reduced quality of life, particularly among older adults. Ulcer recurrence is influenced not only by clinical factors but also by medication adherence, psychosocial conditions, family support, and environmentalcircumstances. This case report aims to describe the application of a family medicine approach in managing recurrent diabetic foot ulcer in an elderly patient with multimorbidity and socioeconomic limitations. Mr. S, a 74-year-old man with a five-year history of type 2 diabetes mellitus, hypertension, peripheral neuropathy, and previous foot debridement, presented with a plantar ulcer on the left foot caused by a wooden splinter from the floor of his stilt house. Clinical evaluation revealed a 3 × 2 cm ulcer with moderate infection, HbA1c of 10.2%, poor medication adherence, high diabetes distress, and risk of malnutrition. Holistic assessment identified internal factors including poor glycemic control, chronic non-adherence, and diabetic neuropathy, as well as external factors such as a hazardous coastal home environment, an elderly caregiver, and limited family income. Management was carried out using patient-centered care, family-focused care, and communityoriented care through wound debridement, insulin therapy, antibiotics, intensive diabetic foot care education, medication adherence improvement strategies, home environmental modification, and home visits. Four-week follow-up demonstrated clinical improvement, with ulcer size decreasing to 1.5 × 1 cm, healthy granulation tissue formation, improved medication adherence, and better foot care practices. A comprehensive family medicine approach may improve clinical outcomes and help prevent recurrent diabetic foot ulcers among vulnerable older adults.

Abstract

Dengue Hemorrhagic Fever (DHF) remains a major public health problem in Indonesia, with high morbidity and mortality rates. Since the current management of DHF is primarily supportive, identifying effective complementary therapies has become an important research priority. Indonesia possesses extraordinary biodiversity, including numerous medicinal plants rich in bioactive secondary metabolites with potential therapeutic properties. This study aimed to review the potential of Indonesian medicinal plants as candidates for supportive therapy in DHF based on available scientific evidence. A literature review was conducted by analyzing national and international publications related to medicinal plants and dengue infection. The findings indicate that several Indonesian medicinal plants, including Phyllanthus niruri L., Psidium guajava, Carica papaya L., and Ipomoea batatas, demonstrate promising potential as supportive therapies for DHF. Their therapeutic effects are mainly attributed to bioactive compounds such as flavonoids, quercetin, saponins, tannins, alkaloids, and polyphenols, which exhibit antiviral, anti-inflammatory, antioxidant, and immunomodulatory activities. These compounds may also promote thrombopoiesis by stimulating granulocyte-macrophage colony-stimulating factor (GM-CSF) and interleukin-3, thereby contributing to platelet recovery. In addition, they help reduce oxidative stress, suppress excessive pro-inflammatory cytokine production, and preserve vascular endothelial integrity, potentially preventing disease progression and plasma leakage. Overall, Indonesian medicinal plants represent promising candidates for complementary supportive therapy in DHF. Nevertheless, further preclinical investigations and well-designed clinical trials are required to confirm their efficacy, safety, optimal dosage, and mechanisms of action before they can be recommended for routine clinical practice.

Abstract

Typhoid fever remains one of the most common infectious diseases affecting children in developing countries, particularly in areas with poor sanitation and hygiene. Meanwhile, hookworm infection continues to be a significant public health problem because it may cause chronic blood loss, iron deficiency anemia, malnutrition, and impaired growth in children. The coexistence of these two infections may aggravate clinical manifestations and requires a comprehensive diagnostic and therapeutic approach. This case report aims to describe the clinical presentation, diagnostic evaluation, and management of typhoid fever accompanied by hookworm infection in a child. A 5-year-11-month-old girl was admitted to the Emergency Department of dr. H. Abdul Moeloek Regional Hospital with a two-week history of fever accompanied by abdominal pain, vomiting, and diarrhea. Physical examination revealed fever, pale conjunctiva, and abdominal distension. Laboratory investigations demonstrated microcytic hypochromic anemia, leukocytosis, thrombocytosis, and hypoalbuminemia. Serological testing for anti-Salmonella IgM (TUBEX) yielded a positive result with a score of 6, while stool examination identified Ancylostoma duodenale eggs. The patient was diagnosed with typhoid fever and hookworm infection and was treated with intravenous antibiotics, an anthelmintic agent, supportive therapy, adequate fluid and nutritional management, and education regarding personal hygiene and sanitation. During hospitalization, the patient's clinical condition gradually improved, as evidenced by the resolution of fever, abdominal pain, vomiting, and diarrhea, along with improved tolerance to oral intake, and she was discharged in good condition. This case highlights the importance of considering coinfection in children presenting with prolonged fever, anemia, and poor sanitation-related risk factors. An integrated approach involving accurate diagnosis, appropriate antimicrobial and anthelmintic therapy, nutritional support, and preventive education is essential to improve clinical outcomes and reduce the risk of complications and reinfection.

Abstract

Water quality is a crucial factor in maintaining public health. The quality of water is crucial for daily life, from drinking to household, industrial, and health needs. Water that does not meet standards can cause various infectious and chronic diseases. This article uses a literature approach through a review of various studies related to water quality parameters and their impact on health. Water quality can be measured using the Water Quality Index (WQI). Parameters that determine water quality include physical, chemical, and biological factors. Low water quality is associated with high levels of heavy metals (Pb, As, Fe, Cd), chemical compounds (nitrate and fluoride), and microbiological contamination (Escherichia coli). Water quality can be influenced by natural factors, such as rainfall, watershed geography, the atmosphere, and geology. Furthermore, water quality is also determined by community behavior, which in turn influences community health, such as industrial, domestic, and/or agricultural activities. Community behavior in water treatment and storage also influences the increased risk of diarrhea, cholera, and nervous system disorders. Water quality and community behavior play a significant role in community health. Cross-sectoral collaboration between government, industry, and the community is needed to maintain water quality and prevent waterborne diseases, thereby improving public health.

Abstract

The rapid increase in social media use among university students has raised concerns regarding its potential psychological consequences, particularly anxiety disorders. This study aimed to determine the relationship between social media usage intensity and anxiety levels among medical students at the Faculty of Medicine, Tarumanagara University, in 2025. An analytical observational study with a cross-sectional design was conducted involving 212 respondents selected through consecutive sampling. Social media usage intensity was assessed using the Social Networking Time Use Scale (SONTUS), while anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS). Data were analyzed using the Spearman rank correlation test, and the strength of the association was estimated using the Prevalence Ratio (PR). The results showed that 65.6% of respondents had high to very high social media usage intensity, while 59.0% experienced moderate to severe anxiety. Spearman rank analysis demonstrated a statistically significant positive correlation between social media usage intensity and anxiety levels (r = 0.324; p < 0.001). Furthermore, students with high social media usage intensity had a 1.91-fold higher risk of experiencing moderate to severe anxiety than those with lower usage intensity. These findings indicate that excessive social media use may contribute to poorer mental health outcomes among medical students. Therefore, promoting awareness and providing education on responsible and mindful social media use is essential to reduce its adverse psychological effects, support healthy digital habits, and improve students' overall mental well-being.

Abstract

Obesity is a growing global health problem, including in Indonesia. One preventative strategy is through the habit of reading nutritional information labels on packaged food products, which provide information on energy, fat, sugar, and sodium content. However, this habit remains low and has not been directly studied in relation to obesity incidence. This study is a literature review of articles from the Google Scholar, PubMed, ScienceDirect, and GARUDA databases from 2015–2025. Keywords used include "obesity," "nutrition information label," and "food label reading habits" in both Indonesian and English. The articles described are full-text and relevant to the topic. Several studies have found that individuals who regularly read labels tend to have a normal Body Mass Index (BMI). However, most of the literature only examines behavioral aspects or knowledge of nutrition labels, without exploring their direct link to obesity. Factors such as nutritional literacy, label design, and education significantly influence the effectiveness of labels on consumption. The habit of reading nutrition labels has the potential to help prevent obesity if accompanied by understanding and educational support. However, there is still a gap between knowledge and practice. A multidimensional approach is needed, including communicative label design, such as FoPNL, and ongoing regulation. Further studies are needed to examine the direct relationship between label reading behavior and obesity incidence.

Abstract

Inflammation plays a crucial role in the pathogenesis of various chronic diseases, prompting continued interest in the development of natural anti-inflammatory agents with favorable efficacy and safety profiles. Coriandrum sativum L. (coriander) has been reported to exhibit anti-inflammatory properties through multiple biological pathways. This article aims to summarize and evaluate recent experimental evidence regarding the anti-inflammatory activity of Coriandrum sativum. A narrative review approach was employed, with literature searches conducted in PubMed and Google Scholar for publications from 2015 to 2025 using keywords related to Coriandrum sativum and inflammation. Included studies comprised in vitro and in vivo experiments assessing the anti-inflammatory effects of extracts, fractions, or active components of Coriandrum sativum, with extracted data covering the type of preparation, experimental model, dosage or concentration, and measured inflammatory parameters. The reviewed studies indicate that various preparations of Coriandrum sativum exhibit measurable anti-inflammatory activity, particularly polar extracts (ethanolic and methanolic) and essential oils rich in linalool, which were consistently reported to reduce pro-inflammatory mediators and cytokines and to improve biochemical and histopathological parameters in inflammatory models. Nevertheless, heterogeneity in methodologies, dosage variations, and limited phytochemical characterization were noted across studies. Based on the available evidence, polar extracts and essential oils of Coriandrum sativum appear to be the most promising preparations as natural anti-inflammatory agents; however, further studies focusing on extract standardization, toxicity evaluation, and early-phase clinical trials are required to support their potential application in humans.
