Abstract

Pneumonia is a disease that attacks the lower respiratory tract by pathogens. The most common causes in toddlers and children are viruses and bacteria. This article is a form of implementing family doctor services for patients by identifying risk factors, clinical problems, management and prevention efforts for patients based on a patient-centered and family problem solving framework. This study is a case report using primary data obtained through alloanamnesis, physical examination, home visits to complete family and psychosocial data, as well as environmental conditions. The assessment is based on a holistic diagnosis from the beginning, process and end of the study both quantitatively and qualitatively. A 7 month old toddler patient with a diagnosis of pneumonia. In this case, the internal factor that played a role in the patient was the patient's age, which was 7 months. External factors, namely the lack of family knowledge about pneumonia, its complications and the environment that has trigger factors. These patients were then given pharmacological and non-pharmacological interventions according to evidence-based medicine. The evaluation results showed an improvement in the patient's clinical condition and increased family knowledge about pneumonia. The diagnosis and management of this patient have been carried out according to the guidelines. Changes in behavior and knowledge of patients and their families are seen after interventions that are patient centered and family approach.

Abstract

Hydatidiform mole is an abnormal condition in pregnancy with part or all of the chorionic villi undergo hydropic degeneration. Hydatidiform mole is divided into complete hydatidiform mole and partial hydatidiform mole. The incidence rate in Indonesia is around 1:80 normal deliveries. While the incidence of partial moles is rare, the incidence varies from 5:100,000 and 1:10,000 pregnancies. Mrs. A 37 years old came to the ER Abdul Moeloek Hospital with complained abdominal pain and vaginal bleeding. Result of general examination: moderate ill appearance, blood pressure was 148/89 mmHg, other sign examination were within normal limits. On obstetric examination, the uterine fundal height was two fingers above the umbilicus, the fetal heart rate was 155x/min and three contractions in 10 minutes, the duration was 20-40 seconds. Speculum examination result : opened portio, there is a fish eyed-like bubble, and active bleeding. Ultrasound examination revealed that she was pregnant 26 weeks with a partial hydatidiform mole. Complete blood count with severe anemia. Whereas immunological and serological examinations results: β-hCG 5,242,880 mIU/mL; T3 1.93 nmol/L; T4 157, 43 nmol/L; TSH 0.01 uIU/mL. So the diagnosis is G3P2A0 26 weeks of gestational age in active phase with a partial hydatidiform mole accompanied by hyperthyroidism and severe anemia with single live fetus intrauterine. Patient lead to spontaneous vaginal delivery were then treated with curettage, transfusion and postpartum care. Furthermore, the mole tissue was taken for Anatomical Pathology examination and the patient was planned to control for the β-hCG evaluation.

Abstract

Chronic kidney disease (CKD) is a disease that has a fairly high impact on morbidity and mortality in the community. The prevalence of CKD in the world reaches 8-16%, while in Indonesia according to basic health research data in 2018 at the age of 15 years the incidence of CKD is 0.64% and is increasing every year. Hypertension is one of the risk factors for CKD. Hypertension is the most common cardiovascular disease suffered by the community and according to WHO data in 2015 there were 1.13 billion people experiencing hypertension. The method used by the author in compiling this article is in the form of a literature review study covering the activities of collecting scientific sources both nationally and internationally and presenting a summary in the form of the most relevant publications. Hypertension has a relationship with the incidence of CKD. This is reinforced and in line with several studies that have been carried out and obtained the results that there is a significant relationship between hypertension and CKD.

Abstract

Background: Vitamin K is a fat-soluble vitamin that has an important role in activating substances that play a role in blood clotting. Neonates tend to have relatively lower levels of vitamin K and vitamin K reserves in the liver compared to neonates. This causes newborns to tend to have vitamin K deficiency. Objective: To describe the level of knowledge of midwives and nurses about giving vitamin K to newborns at the Puskesmas Mesuji Timur District in 2022. The type of research used is descriptive with a cross sectional design. The sample in this study were midwives and nurses at the Mesuji Timur District Health Center with a total of 65 people. Results: In this study, the frequency of sufficient knowledge level of respondents was 47 people with a percentage of 72.3%. The conclusion of this study is that midwives at the Puskesmas Mesuji Timur District have good knowledge of 16%, 38% enough and less 8%. Nurses at the Puskesmas Mesuji Timur District have good knowledge of 26,7%, enough 60% and less 13,3%.

Abstract

General anesthesia is the most widely used anesthetic and is the standard for most surgeries. However, general anesthesia has several disadvantages, including side effects of the drugs used, longer recovery time, and inadequate pain control. Therefore, the use of regional anesthesia is an alternative option to reduce the adverse effects of general anesthesia, one of which is spinal segmental thoracic anesthesia. In this case report, a 52-year-old woman that diagnosed with Stage IIb Acute Limb Ischemia (ALI) underwent embolectomy under thoracic spinal segmental anesthesia. The patient presented with ASA III physical status with type II DM, CHF, CAD3VD and increased coagulation factors. Thoracic spinal anesthesia with a paramedian approach at the level of the T10-11 vertebrae using a regimen of levobupivacaine 5 mg and bupivacaine 2.5 mg. The adjuvants used were dexmedetomidine 5 mcg, ketamine 15 mg, and fentanyl 25 mg. During the operation there were no hemodynamic fluctuations. Post-operative monitoring of the patient's condition was stable, pain control was good, and there were no complaints of headache, nausea, and vomiting. Segmental thoracic spinal anesthesia has several advantages such as the need for lower anesthetic doses, safer than lumbar spinal anesthesia, good pain control, faster recovery, and avoiding the side effects of general anesthesia. Therefore, this anesthetic technique can be an effective alternative choice for general anesthesia

Abstract

A wound is a disconnection of tissue continuity due to damage, or loss of tissue substance due to injury or surgery. wounds are the most common injuries found in our daily lives. there are 3 phases of wound healing process are inflammatory phase, proliferation phase and maturation phase. this process is influenced by many factors like patient factors, wound factors and local factors. in the wound healing process, there are several things that can accelerate wound healing are away from cigarettes, reducing stress, improving nutrition by adding supplements (protein, vitamins and minerals), control chronic disease, stop consume alcohol, prevent infections (wound dressings and antibiotik), and avoid medical drugs that can affect wound healing. by understanding the physiology of good wound healing, we can accelerate wound healing and minimize unwanted effects

Abstract

Stunting is an irreversible growth disorder that can be influenced by many factors, one of which is influenced by inadequate nutritional intake. The prevalence of stunting in Indonesia is still quite high, so various efforts have been made by the government to tackle the stunting problem. One of the efforts made is to socialize the use of yard land as an effort to prevent and deal with stunting. This article is a literature review compiled to determine the effect of nutritional intake from the home garden on the prevalence of stunted and non-stunted toddlers. The literature sources used in this article use national and international scientific articles published in 2013-2022. Nutritional intake which is the main cause is macronutrient intake, namely carbohydrate, protein and fat intake. Intake of macronutrients provides a large energy contribution to the level of daily energy adequacy. Energy deficit that occurs in toddlers aged 1-5 years is the biggest cause of stunting. Toddlers with stunting will experience chronic growth failure, if not treated immediately it will have a long impact until puberty. Utilization of the yard will affect the amount and type of variation in nutritional intake in toddlers, the diversity of commodities from the home yard will increase the nutritional intake of toddlers so as to avoid chronic malnutrition and stunting. Abstract

Changes in the level of welfare of the Indonesian people have led to an increase in the prevalence of non-communicable diseases, one of which is Chronic Obstructive Pulmonary Disease (COPD). The prevalence of COPD in Indonesia is 3.7% and in Lampung the prevalence is 2.04% and ranks number 7 of the 10 most common diseases found in Lampung. In addition, it is estimated that there are 1.28 billion people aged 30-79 years who have hypertension, of which two thirds live in developing countries, and about 46% are not aware of having hypertension. Less than half (42%) of the population with hypertension is diagnosed and treated. In this case report, it is reported that a 69-year-old male patient with COPD and grade II hypertension had concerns that his complaints would increase and not improve. The limited knowledge about the patient's health condition in the form of risk factors, disease prevention, treatment patterns and the lack of motivation and encouragement from the family to stop smoking and take the patient for routine health control are factors that cause problems that occur in patient and family. The management was carried out holistically using a family medicine approach and after evaluation it was found that the patient's symptoms were reduced, increased knowledge, and changes in the patient's diet and physical activity.

Abstract

Human Immunodeficiency Virus (HIV) infection remains a global problem, with nearly 38 million people infected worldwide. Tuberculosis (TB) is the most common opportunistic infection found in HIV patients. Malnutrition in children with HIV infection can be caused by various factors such as individual factors, namely the absorption and metabolism disorders of children. Reported a 10-year-old girl with complaints of fever, cough and weight loss. The patient has been diagnosed with HIV since 4 years ago. The HIV status of both parents is unknown. History of contact with adult TB, namely the patient's father. Physical examination found multiple submental and submandibular lymph nodes (KGB) measuring 1x1cm palpable soft. On auscultation of the lungs there were crackles in both lung fields. Nutritional status of malnourished patients with marasmus type. AP chest x-ray examination revealed a primary impression of TB. The medical therapy given was continuing antiretroviral drugs (tenofovir, lamivudine, and evapirenz), D5¼NS fluids 1200cc/day, antibiotics ceftriaxone 1.2g/24 hours, antipyretic paracetamol syrup 3x7.5ml, folic acid 1x5mg on the first day and 1x1mg on the next day, Vitamin C 2x50mg, vitamin B complex 1 tablet per day and salbutamol 3x1,5mg. Pulmonary TB was treated with rifampin 200 mg/day, isoniazid 140 mg/day, ethambutol 300 mg/day, and pyrazinamide 500 mg/day. Non-medical therapy in the form of oral nutrition, namely rice, side dishes, and milk with a total calorie of 55kcal/kgBW/day.

Abstract

   Dermatophytosis infections have spread throughout the world. The prevalence of this disease is 20-25% with the most common cause being dermatophyte fungi, while the most common in Asia is tinea corporis infection with a prevalence of 35.40%. Implement a holistic approach to family medicine and comprehensively in detecting internal and external risk factors and solve problems based on Evidence Based Medicine that is family-approached and patientcentered. This study is descriptive study about case report. The primary data obtained through anamnesis, physical examination and a visit to the home. Secondary data were obtained from medical records of patients. The patient complained of reddish spots on the right left groin since 4 days ago. The patient also admitted that she could easily sweat and not ooften change her clothes. Diagnosis and management of this patient has been doing holistically, patients center, family approach based on several theories and current research that not just focused on the patient but to the other member of her familiy so it would resulting in an increase of cognitive and life quality on tinea cruris and obesity patient.
