Abstract

Malaria merupakan masalah kesehatan utama di Indonesia dan menjadi endemis di hampir semua wilayah luar pulau Jawa - Bali. Provinsi Maluku Utara merupakan salah satu daerah di Indonesia dengan kasus malaria cukup tinggi. Berdasarkan laporan tahunan Sub.Din P2M Dinas Kesehatan Provinsi Maluku Utara tahun 2005, menunjukkan bahwa ditemukan 4.967 kasus malaria dengan 1.938 positif P. falciparum, 3.004 positif P. vivax, dan 25 positif P. malariae. Dari jumlah kasus tersebut di atas, terdapat 60,5% diderita oleh anak dan merupakan tingkat kematian terbesar pada anak. Standar pengobatan malaria telah ditetapkan oleh Depkes R.I. Penelitian ini bertujuan untuk mengevaluasi tatalaksana pengobatan malaria pada anak dan tingkat keberhasilannya diinstalasi rawat inap Rumah Sakit Dr. H.Chasan Boesoirie Ternate selama tahun 2005. Penelitian ini menggunakan rancangan penelitian potong lintang dilakukan melalui penelusuran data secara retrospektif terhadap rekam medik pasien malaria yang menjalani rawat inap di Rumah Sakit Dr. H. Chasan Boesoirie Ternate selama kurun waktu Januari hingga Desember 2005. Penelitian inimelibatkan seluruh kasus malaria yang memenuhi kriteria inklusi. Penilaian kesesuaian penggunaan antimalaria menggunakan standar pengobatan malaria dari Departemen Kesehatan 2003. Analisis data dilakukan secara dekskriptif untuk mengidentifikasi kasus malaria anak, penggunaan antimalaria, dan keberhasilan terapi antimalaria pada kasus malaria anak berdasarkanhasil pemeriksaan parasitologi. Hasil penelitian menunjukkan sebanyak 560 kasus malaria pada anak, yang terdiri dari malaria klinis 220 kasus (39,29%), malaria falciparum sebanyak 285 kasus (51,06%), dan malaria vivax sebanyak 51 kasus (9,11%). Kasus malaria terbanyak yaitu pada kelompok usia 1 - 4 tahun sebanyak 270 (48,21%). Penggunaan terapi antimalaria pada anakterbanyak yaitu menggunakan kina i.v sebanyak 34,64%. Ketidaksesuaian penggunaan antimalaria terbanyak terjadi pada kasus malaria klinis (26,96%). Keberhasilan penggunaan antimalaria berdasarkan hasil pemeriksaan parasitologi terjadi pada 288 kasus (84,7%).

Abstract

Oxydative stress is known as a major trigger in cardiovascular dysfunction that leads to hypertension, atherosclerosis, and chronic heart failure. Polymethoxyflavone such as tangeretin and nobiletin has been reported to have biological activity including reducing cholesterol level and anti inflammation activity. The critical point that mediates atherosclerosis is the formation of stress oxidative which causes endothelial cells dysfunction.This study was conducted to explore the effect of tangeretin in protecting H2O2-induced oxidative stress on HUVEC. Cell viability was determined by using MTT assay. Cells were treated with different concentrations of tangeretin (12.5; 100; and 200 µM) for 24h before exposed to 600 µM H2O2 for 4h. Cells viability analyses showed that at low concentration (12.5 µM), tangeretin inhibited H2O2-induced cytotoxicity. Furthermore, it induced cells proliferation. However, at high concentration, it failed to protect HUVEC from H2O2-induced cytotoxicity and increased the cytotoxicity of H2O2. Based on this result, tangeretin in low concentration has a potential to reduce oxidative stress on HUVECs.

Abstract

ABSTRACTCelery (Apium graveolens L.) is one of plants that initially only used for cooking. Based onthe research by Wolski, et al., 2002, celery contains chemical compound such as tannins andpolyphenols. Tannins may be useful as an anti-diarrheal because of the adstringent effect. Thisstudy aims to determine the antidiarrheals activity of celery leaf in male mice strain Balb-C that wasinduced by castor oil. The ethanolic extract of celery leaves divided into three dose group (5,10 and20 mg/kgBW). This research was done by two methods, protection of castor oil induced diarrheaand intestinal transit. After five hours treatment, frequency of defecation, number of soft/liquidfeses and weight of stools were calculated. The gastrointestinal transit rate was expressed as thepercentage of longest distance traversed by the charcoal divided by the total length of the smallintestine.From the protection of castor oil induced diarrhea method, ethanol extract of celery 20mg/20gBW doses decrease frequency of defecation, quantity of the soft / liquid stool and weight ofstool after 5 hours treatment in male mice that had been induced by castor oil. The effectiveness ofethanol extract 20mg/20gBW dose equivalent to loperamid 0.06mg/20gBW (one way anova test atthe α = 95%). Besides, the ethanol extract of celery can prolong intestinal transit of mice, but lowerthan loperamid 0.06mg/20g (one-way anova test at the α = 95%).Keywords: antidiarrheal, castor oil, celery, intestinal transit, taninns

Abstract

ABSTRACTAcetaminophen is a kind of analgetic-antipyretic drug which the usage in excessive dose willcause the liver damage. Pegagan herb (Centella asiatica, L) is one of plant which is used to protectthe liver from various damage of drug. A research to study the hepatoprotective effect of pegaganherb infusion on acetaminophen induced wistar rat has been conducted. This research was aimedto find how excellent pegagan herb protects the liver from the damages caused by acetaminophenand find the data of hepatoprotective of pegagan herb’s dose effect span in infussion form throughthe SGPT enzym and hyspatology of liver cell analysis. This research used the completed randomof unidirectoral pattern method, using wistar strain white rat as the tested animals, weight 200 g 10%, age 5-8 weeks. The way of attemp: 36 rat were divided into 6 groups were each groups had 6rat. For the treatment, Group I was controlled by aquadest. Group II was given acetaminophensuspention dose 2,5 g/kg BB. Group III-IV was given pegagan herb infussion 0,027; 0,054; 0,0108;0,216 g/kg BB once in a day for a week and 8 hours after that, on the seventh day was givenacetaminophen suspention dose 2,5 g/kg BB. Blood taking was conducted by orbitalis sinus beforeth treatment, 8 hours after the seventh day treatment and 24 hours after acetaminophen was given.SGPT data was analysed by ANOVA statistical test, where if found the significant difference, itwould continue by Tuckey Test with 95% as significant standar. Then continued by hyspatology teston liver cell of rat. The result of this research showed that pegagan herb infussion dose 0,027;0,054; 0,108; 0,216 g/kg BB had a hepatoprotective effect on acetaminophen inducted white ratsuccessively 34,19%; 62,52%; 78,62%; 85,69%. And the result of qualitative analysis by histologyanalysis showed that pegagan herb had hepatoprotective effect.Keyword: Hepatoprotective, Pegagan Herb,Acetaminophen

Abstract

ABSTRACTLocal white mice were used to evaluate the toxic and teratogenic effects of various dose levels0.0, 0.5, 1.0, and 1.5 mg/kg body weight of Bergapten on fetuses and pups of pregnant females.The oral administration of the different dose levels of Bergapten on days 6-15 of gestation has notproduced any toxic condition and morphological or skeletal changes in fetuses of mice.Key Word : Bergapten, Teratogenicity, mice.

Abstract

ABSTRACTVirgin Cocunut Oil (VCO) is a pure coconut oil, it has many advantages in health. It hasbeen circulated in market with unpleasant taste. Therefor, an innovation to find an acceptableformula by consumer should be carried out. The aimed of this research is to find out theinfluence of mix emulsifier toward physical stability of emulsion. Multiple emulsions w/o/wwere made by mix Span 80 with variation concentration 20%, 15%, 10%, and Tween 80. In orderto know the physical stability of emulsion w/o/w should be carried out viscosity test, separation inroom temperature, separation at 40ºC, separation due to centrifugation, creaming volume,paticle size, homogenity for 4 weeks storage, and respondent survey with 20 person. The testresult for 3 formulation emulsion for 4 weeks storage showed that formulation are nothomogeneous, but arter the formulas were shaken, it become homogeneous. In variousconcentration of Span 80, the longer preperations were storage the higher volume sedimentationand viscosity. Moreover, the longer preparations were storage, creaming volume and particle sizedecrease. Emulsifier combination between Span 80 with variation concentration 20%, 15%, 10%,and tween 60 in multiple emulsion w/o/w influenced physical stability multiple emulsi w/o/w.Moreover, this formulation also would be pleasant by consumen.Keywords: Multiple emulsion w/o/w, Span 80, Tween 60, Virgin Coconut Oil

Abstract

ABSTRACTAim of this research is to know factor influenced to antioxidative activity of chloroform extract oftea leaf ( Camellia sinensis) with -tocopherol in peanut oil system. Research was done by takedata of peroxide value, tiocyanic acid value dan barbituric acid value of the system addedantioxidant at various concentration. Variance analysis was investigated to the data by interpretingfactors consist of time, extract concentration and -tocopherol concentration followed bydetermination of regression model. Result showed that time, extract concentration and -tocopherolconcentration influence to the antioxidative activity and there was interaction effect of the factors.Optimum condition gained at time of 60 minutes, -tocopherol concentration of 110 ppm andextract concentration of 100 ppm. The model resulted from this analysis can be used to predictsynergistic effect between chloroform extract of tea leaf with -tocopherol.Key word: antioxidant, synergistic effect, extraction

Abstract

ABSTRACTThe aim of this study was to get cytomegalovirus viruria prevalence data in healthy adultpopulation. Urine samples were obtained from healthy individuals. Preparation of sample and sera(polyclonal antibody) were performed by standard method., and detection of cytomegalovirusantigen in urine sediment were done by Indirect Immunofluorescence Antibodi Technique (IFAT).The results were analyzed with chi square pearson. From 341 healthy individuals (16 ♀ and 125 ♂)age between 18 to 61 years. Cytomegalovirys viruria were found in 5 (3, 55%) of subjects. In groupof subjects less than 30 years of age, cytomegalovirus viruria were found in 3,82% subject whereasin group of subjects more than 30 years of age, cytomegalovirus viruria was not found; chi squarepearson statistic analysis did not show significant difference between two groups (p = 0,529). Ingroup of women, cytomegalovirus viruria were found in 6,25% subjects whereas in group of men,cytomegalovirus viruria were found in 3,2% subjects; chi square pearson statistic analysis did notshow significant difference between two groups (p = 0,535). Cytomegalovirus viruria prevalenceamong healthy adult population studied was relatively low (3,55%).Key words: Cytomegalovirus, viruria, prevalence, latent infection, Indirect ImmunofluorescenceAntibodi Technique (IFAT)

Abstract

Obat antiinflamasi non-steroid (AINS) banyak digunakan dalam pengobatan berbagai penyakit yang melibatkan proses inflamasi dan merupakan grup terbesar dari agen farmasetik yang digunakan secara luas di seluruh dunia. Beberapa studi telah melaporkan bahwa komplikasi pada saluran pencernaan merupakan reaksi obat yang tidak dikehendaki (ROTD) yang paling sering terjadi terkait penggunaan AINS. Penelitian ini dilakukan dengan tujuan untuk mengetahui kuantitas dan kualitas peresepan obat golongan AINS untuk pasien rawat jalan di salah satu rumah sakit. Penelitian dilakukan dengan menggunakan sampel data peresepan obat selama satu tahun, yaitu selama periode 1 Juli 2006 sampai 30 Juni 2007. Data penggunaan obat golongan AINSdikumpulkan dari catatan penggunaan obat di instalasi farmasi rumah sakit meliputi jenis obat, bentuk sediaan, kekuatan, serta jumlah penggunaan. Pengukuran kuantitas penggunaan obat AINS dilakukan dengan menggunakan satuan unit DDD (Defined Daily Dose) yang dinyatakan dalam DDD/1000 kunjungan pasien rawat jalan (KPRJ). Kualitas peresepan obat AINS diukurberdasarkan kriteria keamanan relatif obat AINS terhadap saluran pencernaan yang dianalisis berdasarkan data kuantitas penggunaan obat AINS menggunakan metode Drug Utilization 90% (DU90%). Hasil penelitian menunjukkan bahwa keseluruhan AINS yang digunakan meliputi 7 jenis dengan 22 nama dagang. Kuantitas AINS yang digunakan adalah sebesar 182,5 DDD/1000kunjungan pasien, yang artinya adalah setiap pasien mendapatkan AINS sebesar 1,8 DDD. Jenis AINS yang termasuk dalam segmen DU90% adalah asam mefenamat (62,8%), ketorolak (15,4%), ketoprofen (8,6%), dan diklofenak (8,5%). Berdasarkan tingkat keamanan obat golongan AINS terhadap saluran pencernaan, kualitas peresepan obat golongan AINS untuk pasien rawat jalan di rumah sakit ini sudah cukup baik, karena AINSs yang digunakan sebagian besar merupakan AINS dengan risiko moderat dan rendah terhadap gangguan saluran pencernaan, yakni asam mefenamat dan diklofenak dengan persentase penggunaannya sebesar 71,3%.

Abstract

Health is rights of every human. In Indonesia, poor people had difficulty to access health services. Therefore government creates a program called “Jamkesmas” which based on DRGs (Diagnosis Related Groups) to improve quality of health services. Physicians should prescribe medicines according to the diagnosis and Jamkesmas formulary. The compliance between prescription with Standard Treatment Guidelines (STGs) and Jamkesmas formulary is a must. The aim of this research was to know the prescriptions compliance to STGs and Jamkesmas formulary. This research was observational study using prescriptions of Jamkesmas’s outpatients . Furthermore, qualitative data with in-depth interviews was used to further analysis. Quantitative data were calculated by using drug use indicators for the percentage of prescriptions compliance. The percentage of prescriptions compliance with STGs in each of 10 diseases in once of hospital in Daerah Istimewa Yogyakarta (Hospital X), started from 24.3% in CHF to 82% in DM diseases, whereas prescriptions compliance with Jamkesmas Formulary was started from 49% in vertigo to 96% in hypertension diseases. Average number of drugs per prescription was 2.7 and the most of drugs (85.7%) were prescribed by its generic names; 23.3% of prescriptions contained antibiotic and 22.0% contained of injectable drug. Percentage of drugs prescribed from essential medicines list was 76.7%. The prescriptions of 10 diseases were not fully compliance to Standard Treatment Guidelines and Jamkesmas formulary.
