Abstract

Integration of Primary Health Services is a priority program in primary health care facilities to support health transformation in Indonesia. The priority issues at community health centers (Puskesmas) within the Bantul Health Office area include a lack of understanding among health workers regarding health service risk management, and the use of manual forms for risk management reporting, which does not utilize information technology. Risk management plays a crucial role in preventing program failure, particularly in the implementation of Integration of Primary Health Services at Bantul Community Health Centers, which began in 2024. The Community Service (PKM) activities comprised risk management training and the dissemination of research results for a prototype risk mitigation calculator application based on an Early Warning System (EWS) dashboard, conducted through lectures, practical exercises, and discussions. Overall, the PKM implementation proceeded smoothly and in an orderly manner. The pre-test and post-test results of 27 participants showed an increase in the level of community empowerment in the knowledge aspect with an average score of 21, where the average initial score of 69.9 during the pre-test increased to 90.9 during the post-test. Participants were also able to simulate the use of the application, including installation, data input, output interpretation, and form printing, according to their respective access levels. Through this increased empowerment of partners, it is expected that Bantul community health centers will be able to properly document their risk management processes, which will enable the optimal implementation of the Integration of Primary Health Care program.

Abstract

Stunting has long-term impacts on a child's growth, development, and overall quality of life. Prevention efforts starting from the preconception phase, particularly among prospective brides and grooms (catin), represent a strategic approach to breaking the cycle of stunting. This community service program aimed to strengthen the capacity of health cadres in educating and conducting early detection of stunting risks among catin. The program was conducted over eight months (May–December 2025) using an empowerment approach through focus group discussions, socialization, training, and mentoring activities. A total of 15 participants were involved, consisting of 12 cadres from Kranji Village and 3 health personnel from the local health center. Results showed an increase in the average knowledge score of cadres across three main topics: Ideal Reproductive Health for Catin by 6.2 points, Stunting Concepts (Detection and Prevention) by 5.14 points, and Nutrition for Catin by 2.67 points. These findings indicate that empowerment-based interventions are effective in forming cadres capable of playing an active role in stunting prevention starting from the preconception phase.

Abstract

Adolescent reproductive health, particularly among adolescent girls, continues to require considerable attention from various stakeholders. In 2021, the number of adolescent girls in Cilacap Regency reached 956,682, with 1.03%–1.24% experiencing dysmenorrhea. The occurrence of dysmenorrhea among adolescent girls is influenced by multiple factors, and the symptoms frequently disrupt their daily activities. One of the complementary therapies that can be administered to reduce dysmenorrhea is abdominal exercise and Kegel exercise. These exercises help improve blood flow to the uterus, thereby inhibiting the release of prostaglandins, as excessive prostaglandin production contributes to pain. The training and mentoring activities on abdominal and Kegel exercises were conducted from June to August 2025. The program consisted of several stages: problem identification through interviews, a socialization stage conducted via lectures and discussions, a training phase, and finally, an evaluation stage to measure the program’s effectiveness. The training and mentoring were implemented with 10 adolescent girls who are members of the Adolescent Posyandu of RW 11, North Cilacap District, Cilacap Regency. The results showed a reduction in menstrual pain intensity, with 70% of participants achieving the category of mild pain. It is expected that, following the training and mentoring provided, the participants will consistently engage in abdominal and Kegel exercises to further reduce menstrual pain.

Abstract

The Maternal Mortality Rate is one of the indicators to assess the degree of public health, because of its sensitivity to improving health services, both in terms of accessibility and quality. The high maternal mortality rate is partly due to delays in early detection of the risk of complications. The purpose of this community service was to increase maternal knowledge in the prevention of complications and preparation for childbirth from the pregnancy period through the provision of health education based on booklets and websites. This community service method was health education and participatory discussions, with 13 pregnant women and 5 health cadres in Lebosari Village, Kangkung Kendal District. The activities carried out include health checks for pregnant women, provision of pre-tests of knowledge, education used website media and booklets, knowledge post tests, pregnancy exercises and oxytocin massage. The service was carried out once on June 25, 2025, and evaluation monitoring on August 30 and October 21, 2025. This activity increased the knowledge of pregnant women to detect early pregnancy conditions and hoped that pregnant women and cadres can find health information through the website or from booklets to increase knowledge about pregnancy, so that the sooner they recognize the risk of pregnancy, the better prepared they can face childbirth, including the ability to recognize emergency situations so that mothers can be referred to higher health service facilities when they occur Complications.

Abstract

Stunting is a chronic nutritional problem that affects children’s physical growth, cognitive development, and long-term quality of life. Prevention efforts require integrated interventions, including nutrition education and the strengthening of emotional management skills for mothers of children under five. This community service program aims to improve mothers’ knowledge through mother classes combined with coping skill training (CST) as a strategy to monitor child growth and detect stunting early. The program was implemented through several stages: planning, coordination with posyandu cadres, pretest-Posttets, health education using leaflets, posttest, data analysis, and evaluation. The participants were mothers of children under five at Posyandu Mawar Merah, Kedungkelor Village, Warurejo District, Tegal Regency. The results showed an increase in knowledge among 29 respondents, while 1 respondent experienced a decrease, with a statistically significant difference between pretest and posttest scores (p = 0.000; p < 0.05). These findings demonstrate that enhancing the capacity of mothers with toddlers through the Toddler Mothers Class combined with coping skill training for stunting prevention in Kedung Kelor, Tegal is effective in improving mothers’ knowledge of emotional management in monitoring child growth and development for stunting prevention. The program is recommended to be further developed with varied counseling methods to enhance sustainability and effectiveness.

Abstract

Denpasar City, the prevalence of hypertension and its related complications increased by 2.14% between 2023 and 2024. The incidence of complications among hypertensive patients is largely attributed to poor self-management. This community service program (PKM) aimed to enhance the knowledge and self-management behaviors of hypertensive patients through the application of the Health Belief Model approach. The program was implemented at Puskesmas I West Denpasar in October 2025 and consisted of health education sessions and progressive muscle relaxation exercise training for 25 Prolanis participants. The program produced two main outputs: a self-management guidebook and an educational video for hypertensive patients. Among the 25 participants, the mean knowledge score increased from 6.30 (pretest) to 9.40 (post test), indicating an improvement of 3.10 points. The mean score for relaxation exercise behavior increased from 6.10 to 9.50, reflecting an improvement of 3.40 points. Meanwhile, the mean blood pressure decreased from 150/95 mmHg to 138/88 mmHg, showing a reduction of 12/7 mmHg. These findings demonstrate that the intervention effectively improved participants’ knowledge and self-management behavior, accompanied by a significant reduction in blood pressure. It is recommended that similar community service programs be implemented more comprehensively by involving various related sectors, particularly with the support of health workers in providing relaxation exercise training for hypertension patients.

Abstract

Synthetic pesticide exposure remains a significant environmental health concern in Semarang Regency, particularly among farmers who rely heavily on chemical inputs. Although some farmers in Ketapang Village have begun adopting organic practices, their understanding of healthy farming and pesticide-related risks is still limited. This study aimed to strengthen the capacity of the Al Barokah Farmer Group through education and training on healthy farming practices, health risk mitigation related to chemical pesticides, and basic concepts of GMOs. The intervention included presentations, discussions, learning modules, and Focus Group Discussions to explore participants’ perspectives. Effectiveness was evaluated by comparing pre-test and post-test scores and analyzing changes across question components. Results showed an increase in average knowledge scores from 67.64 to 77.92. Eleven of Eightteen items (61.11%) demonstrated improved correct responses, particularly on pesticide use, local microorganisms (MOL), and liquid organic fertilizer (POC). Statistical testing yielded a p-value of 0.011, indicating a significant improvement in participants’ knowledge. These findings highlight the effectiveness of structured education in enhancing farmers’ understanding and support the adoption of safer and more sustainable agricultural practices. The results also identify key topics requiring further emphasis in future training programs.
