Abstract

Female Sexual Dysfunction (FSD) has received less attention from doctors, beside very rare complained from patients.This is a contrast situation compare to male with diabetes, for example in general, erectile dysfunction is a common problemreported and decrease patient?s quality of life. A cross sectional study has done, enrolling 97 type 2 diabetes female at outpatientclinic Sanglah Hospital Denpasar. FSD was assessed using FSFI (the female sexual function index) with 19 items of questions.Based on the score of the answer, FSD cathegorized into four categories, including desire disorders, arousal disorder, orgasmicdisorder, and sexual pain disorder. In this study, mean age was 58.8 years (SD 8.9), age range between 34 ? 80 years; and meanduration of diabetes was 8.6 years (SD 8.0), range between 0 ? 30 years. Sexual function index of type 2 diabetes female werelow, arousal domain was the lowest score: 2.13 or 35.5% of maximum 6 score of this domain. Other domains also have low scoreunder 50%, including desire, lubrication, orgasm, and pain: 41.83%, 42.0%, 39.5%, 48.5%, respectively. Only satisfaction hasmore than 50%. There was significant association between each domain. Significant and strong negative association were foundbetween all domain and total score with age, followed by duration of diabetes, and number of children. While obesity, glycemiccontrol, lipid profile were not associated with female sexual function index in type 2 diabetes.

Abstract

Erectile dysfunction (ED) is a commonly reported condition among men with diabetes, however little is known aboutthe predictive factors associated with diabetic ED. To examine the predictors related to ED among type 2 diabetes outpatient, aconsecutive cross sectional study at Internal Medicine Outpatient Clinic, Sanglah Hospital was conducted, enrolled 137 type 2diabetes men aged between 35 ? 77 years old. We found 79.5% of type 2 diabetics with ED. Erectile function score was significantlycorrelated with other component of International Index of Erectile Function 15 (IIEF 15) including orgasmic function(r=0.622, p<0.001), sexual desire (r=0.782, p<0.001), intercourse satisfaction (r=0.911, p<0.001), and overall satisfaction (r=0.842,p<0.001). Erectile function was also correlated with age (r=0.315, p<0.001). Chi square analysis among diabetic complication,ED were significantly higher among patient with albuminuria/proteinuria (p=0.025, prevalence ratio 7.43 (95% IC 0.95 ? 58.15).Other factors such as duration of diabetes, central obesity, cigarette smoking, alcoholism, diabetic control (HbA1C and lipidprofile), diabetic complications, and hypertension were not significantly correlated with ED. ED were high among type 2 diabeticsand erectile function were correlated with orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction.ED also correlated with age and albuminuri/proteinuria but not with other predictors.

Abstract

Hepatitis C Viral (HCV) and Human ImmunodeÞ ciency Virus (HIV) infection are global health problems. Co-infectionHIV-HCV in intravenous drug users about 53 99%. Despite very high prevalence of this co-infection, there were controversiesabout impact of hepatitis C virus in CD4 response of HIV patients receiving anti retroviral therapy. We conduct a retrospectivecohort to Þ nd out the impact of hepatitis C virus in CD4 response after 6 months of Þ rst line ART therapy. We collect data fromVCT-CST clinic patient!s record. Inclusion criteria for this study were HIV patients more than 12 years old, receiving Þ rst lineof ART treatment, and with anti HCV result.Sixty two patients enrolled for this study. Twenty eight patients found positive anti HCV and 34 negative anti HCV result.Mean of age was 29 ± 4.44 years old in anti HCV (+) group and 34 ± 7.335 in anti HCV (-) group, male dominant (92.9% and85.3% respectively). There were no signiÞ cant differences in total lymphocyte count, hemoglobine, and transaminases (AST andALT) between the two groups. The increasing of mean CD4 after 6 months ART treatment higher in anti HCV (-) group than inanti HCV (+) group, but statistically not signiÞ cant.

Abstract

The mortality rate of Dengue Haemorrhagic Fever is still high. It is still very difficult to predict shock. The commonlaboratory feature to predict shock are low platelet count, high hematocrit level and the positive result of serologic test (IgG andIgM antiDHF), but the validity remains controversial. This researh was determine many factor to predict shock in denguehemoragik fever patient . This research was case-control study. Data was collected from secondary data based on hematologic andserologic findings of patients with DHF who were hospitalized in Siti Hajar Islamic Hospital Mataram in the period of June 1,2005 until June 1, 2006. The samples of this study were patients that diagnosed as DHF based on WHO criteria and performedwith rapid serologic test Pan Blow? made in America, for Ig G and Ig M anti DHF. DHF patients that suffering shock werecatogorized as case group (n=47), and DHF patients without shock were control group (n=94). The total number of control groupwas two times of case group. Risk factor of shock were analyzed using chi-square test and multiple logistic regressions with levelof significance of < 0.05. The number of samples used in this study were 141 patients diagnosed as DHF, there were 47 patienstsDHF with shok and 94 DHF patients without shock. There was 70% patient with thrombositopenia and 44% patient had high rateof hematokrit more than 41.5 %. Shock was more frequent among pasients age more than 25 years, male, hematokrit level lessthan 40% and platelet count 50,000-100,000/ml. Based on univariate analysis, age, gender, high hematocrit value andtrombositopenia were not significantly different to predict shock. On multiple logistic regression methode, primery infection(IgM+ and IgG-) and secondary infection (IgG positif and IgM+/-) were not significantly different to predict shock. The conclusionof this study is high hematocrit value, platelet count and serology IgM or and IgG antiDHF there were no factor that couldpredict shock in DHF patients.

Abstract

Anthropometric measurements are part of the nutritional assessment which is an important component of health care.Measuring the individual!s stature with accuracy is very important because it is a basis for estimating basal energy expenditureand consequent nutrient needs and to calculate indices of nutrient status. However, common methods of nutritional anthropometricassessment are not entirely satisfactory for all elderly persons. Certain measurements such as stature may be difÞ cult to obtainbecause of postural changes, thinning of the disks of the spinal column, and diminution in the height of the vertebrae, all of whichare associated with aging.This study conducted to investigate the correlation between thoracic hyperkyposis with the difference of height calculatedwith WHO formula and actual height measurements in elderly patients among geriatric outpatient clinic, Sanglah hospital. Thisis a descriptive study at August 2008. Height was calculated with WHO formula. For men 59.01 + (2.08 x knee height) and forwomen 75 + (1.91 X knee height) - (0.17 X age) and compared with actual height measurements. Data analyzed with analyticdescriptive Spearman!s correlation.There were 91 elderly patients included, male 38 (41.8%), female 53 (58.2%). Mean of age was 70.61 years ± 5.75 SD,actual height measurements was 155.30 cm ± 9.37 SD, height calculated with formula was 160.35 cm ± 8.59 SD, knee heightwas 49.60 ± 3.39 SD, thoracic hyperkiposis was -17.210 ± -11.7 SD and the difference between height formula and actual heightmeasurements was 4.99 cm ± 5.84 SD.There were no correlation between thoracic hyperkyposis and the difference of heightcalculated with WHO formula and actual height measurement with r = 0.001, p = 0.993. Abstract

Spontaneous Bacterial Peritonitis (SBP) is a frequent and severe complication in cirrhotic patients with ascites. It occursin 10-30% of cases and assosiated mortality rate is 20-40%. Spontaneous Bacterial Peritonitis is diagnosed base on the presence ofa Polymorphonuclear leucocyte count greater than 250/mm3 or positive monomicrobial ascitic fluid culture. The key to successfultreatment of SBP is knowledge of appropriate antibiotic regimen and an understanding of the setting in wich infection develops,particularly those individuals at high risk of infection. This study was conducted to know prevalence of SBP and to evaluatepossible risk faktors for this complication such as: severe cirrhosis (Child C), upper gastrointestinal bleeding, low protein ascites(<1g/dL) and low platelet count (<98.000/mm3). Sixty two consecutive hospitalized patients with cirrhotic and ascites were followup for manifestations of SBP. Prevalence of SBP was 30.6%. Multivariate analysis and Logistic regression showed that onlysevere cirrhosis (Child C) is the risk faktor of SBP. (OR= 5.297; 95% CI=1.036-27.079).

Abstract

Atopic allergic disease related with genetic factor and environment. Some studies found parents and environment factorhave an important role on emerging of clinical manifestation. This study try to evaluated wheather atopic parents andenvironment can inuanced clinical manifestation of allergic disease in their children. This information is important foreducation and in intervention of treatment for patients with allergic disease. This study is an observasional analytic casecontrol study involved all student range from junior until high school level and their parents at Surabaya region. Datawas taken by quisioner, physical and Skin Prick Test (SPT). Statistical analyses using multivariat logistic regression test.Threehundreed forthy eight student was enrolled where 91 (26.1%) with positive SPT have atopic father 26 (56.9%) andatopic mother 25 (54.3%) on the other hand 249 student with negative SPT have atopic father 22 (43.1%) and atopicmother 21 (45.7%). Logistic regression test found that sequency number of birth and explanation dust mate assosiatedwith occurence of clinical manifestation of atopic allergic disease in children.

Abstract

Elevation of IL-8 was found in many viral infection include dengue infection and related to the increases of illnessgradation. The aim of study is to know the secretion of IL-8 and the relation with the severity of the disease that caused by dengueviral infection that hospitalized at Sanglah hospital. There were 58 patients that diagnosed with dengue haemorrhagic fever basedon WHO criterion. Those samples consist of 38 patients of DHF grade I (mild) and 20 patients of DHF grade II, III and IV(severe). Serum was taken and then the level of IL-8 was examined by ELISA method with microplate reader in 450 nm wavelength (bioMerieux Reader 250). The data was analize statistically with non parametric measurement. The result showed thatthere were significant differences of IL-8 level between healthy people (control) and samples, and between mild and severepatients. We conclude that increase of IL-8 level related to severity of dengue viral infection

Abstract

Asthma as one of the allergic diseases continuos to be a major public health problem in developed anddeveloping countries. We carried out a population-based survey to search the prevalence of atopic asthma inschool children of Tenganan, Manggis-Karangasem. Asthma was diagnose by quisioner and bronchodilator test.Atopic asthma define as asthma patients with total IgE > 100 IU/ml and or skin prick test 3 mm. Five hundredfourteen (91.5%) students out of of 562 students were were invited, participated the study. Thirty six (7%)students were diagnosed asthma based on peak expiratory flow variability after brochodilator test, stratified bysex there were 13 (36.1%) boys and 23 (63.9%) girls and 9 (25.0%) primary school children, 14 (38.9%) juniorhigh, 13 (36.1%) high school, respectively. Mean of age, serum IgE total and wheal diameter was 13.42 ± 3.26year, 1551.83 ± 2008.19 IU/ml, 22.9±3.4% and 3.58 ± 0.99 mm, respectively. Out of 36 asthma patients, 35(97.2%) diagnosed atopic asthma consist of 12 (34.3%) boys and 23 (65.7%) girls, and 1 (2.8%) diagnosednonatopic asthma.

Abstract

Calciphylaxis, also called calciÞ c uremic arteriolopathy, is a rare abnormality which characterized by medial calciÞ cation of the small arteries and ischemia of the subcutaneous tissue, often leading to necrosis of subcutaneous fat and skin. It affects mainly woman with ESRD.  A 48 year old woman with ESRD had a history of long standing ESRD which was being treated by hemodialysis for 13 years and peritoneal dialysis for 12 months. Two weeks prior to admission, she complained of a constant dull aching pain accompanied by multiple cutaneous ulcers on both distal extremities. The symptoms did not resolve with antibiotic and analgesic treatment. The skin of the leg had multiple violaceous lesions which were tender on palpation. There was multiple painful lesions with diameter 6 ! 8 cm, necrotic ulcerated lesions surround by erythema. Laboratory data revealed serum calcium was 10.4 mg/dl, phosphate was 8.5 mg/dl and parathyroid hormone level 2,164. Plain Photo of abdomen and both extremity revealed calciÞ cation soft tissue. Skin biopsy was consistent of CalciÞ c Uremic Arteriolopathy (CUA). Histopathology studies showed metastatic calciÞ cation sub cutis (cutaneous calciphylaxis) and arteriolosclerosis within superÞ cial dermis.
