Abstract

ABSTRACTIntroduction: Stevens-Johnson Syndrome (SJS) is an acute hypersensitivity reaction that manifests on the skin, oral mucosa, ocular, gastrointestinal, genital and anal area. It is also potentially life-threatening in concern of dehydration and infection. Oral mucosal lesions due to SJS resulted in a significant decrease of patient’s quality of life. When the oral mucosa involved, the intake of nutrients and fluids is disrupted contributing to electrolyte imbalance that aggravates dehydration. Moreover, oral mucosal lesions have become an entry point for infection. Purpose: This case report describes the important role of oral medicine specialists in the management of oral mucosal lesions in SJS patient. Review: A 26-year-old female patient was referred from the Department of Dermatology and Venereology with a diagnosis of SJS et causa suspected paracetamol and/or amoxycillin. The complaints comprised of pain on the lips and oral cavity, difficulty in mouth opening, and pain when swallowing. The management for oral lesions included: history taking, external and intra oral examinations, dexamethasone mouthwash, nystatin oral suspension, and sodium chloride (NaCl) 0.9% solution. The patient showed improvement in oral mucosal lesions within 3 weeks of treatment that was provided by oral medicine specialist and medical team collaboration. Conclusion: Based on this case report, the role of oral medicine specialist is very important as part of the management team for SJS patient. Oral medicine specialist can reduce morbidity that results from oral mucosal involvement. Collaboration with oral medicine specialist since the beginning of treatment is the key to success in SJS management. Keywords: Oral medicine specialist, Oral mucosal lesion, Stevens-Johnson Syndrome.

Abstract

ABSTRACTBackground: Candida albicans is the main microorganism that causes denture stomatitis, thus denture soaking in cleansing solution is needed to protect them from Candida albicans contamination. The 0.2% Chlorhexidine gluconate is one of denture cleansers that induce side effects at prolonged use. An alternative ingredient that can be used as a denture cleanser is ulin stem bark extract. Objective: To determine the effect of ulin stem bark extract at 20%, 40%, 60%, 80%, 100% concentration on the growth of Candida albicans on acrylic resin denture plates. Method: True Experimental with post-test only with control group design was employed under 7 treatment groups consisting ulin stem bark extract at 20%, 40%, 60%, 80%, 100% concentration, 0.2% Chlorhexidine gluconate, and aquadest in a total of 28 samples. Acrylic resin samples that had been exposed to Candida albicans weresoaked in respective treatment for 15 minutes. Results: The average of Candida albicans colonies on acrylic resin denture plates after soaking in ulin bark extract at 20%, 40%, 60%, 80%, 100% concentration, 0.2% Chlorhexidine gluconate, and aquadest were 29.5 CFU/ml, 13.0 CFU/ml, 0 CFU/ml, 0 CFU/ml, 0 CFU/ml, 0 CFU/ml, and 155 CFU/ml. Based on Mann Whitney test, there was no significant difference when ulin stem bark extract at 60% concentration was compared to 0.2% Chlorhexidine gluoconate. Conclusion: Ulin stem bark extract at 20%, 40%, 60%, 80%, and 100% concentration have been proven to reduce Candida albicans colonies on acrylic resin denture plates, and the 60% concentration is equivalent to 0.2% Chlorhexidine gluconate.Keywords: Candida albicans,Eusideroxylon zwageri, Heat cured acrylic resin. Abstract

ABSTRACTBackground. Lymphadenopathy is a common clinical feeding an anomaly in the size and consistency of lymph nodes. Local bacterial infection causes cervical adenopathy when the lymph nodes responded to local infection. Cervical lymphadenopathy is a common problems in pediatric. Lymph node with more than 10 mm diameter is usually considered abnormal. Lymph node enlargement caused by a chronic infection is known as chronic lymphadenopathy. Objectives. Discovery management of chronic lymphadenopathy in children with poor oral hygiene.Case. A 9-year-old boy came to Pediatric Dentistry Specialist Gusti Hasan Aman Dental Hospital with his parents with a complaint of pain in his right tooth posterior mandible for the last week. The patient had received medication, however the pain still existed. Extraoral examination revealed symmetrical face, a node on the middle neck sizing 30 mm in diameter for 4 years. The lump was treated several times with no progress in size. Intraoral examination revealed a cavity in posterior lower left tooth and painful in the last 5 days without any treatment. Plaque score showed poor oral hygiene.Case Management. Based on extraoral examination, the nodule on the neck reduced in size from 30 mm to 7 mm after treatment with antibiotic, anti-inflammatory medication, dental care and control periodic for oral hygiene management. Conclusion. Early detection and accurate lymphadenopathy diagnosis is important for successful therapy. Oral hygiene improvement can help in the healing process of chronic lymphadenopathy which occurred for 4 years. Keyword:  Limphadenophaty chronic, Oral hygiene, Staphylococcus aureus

Abstract

ABSTRACTBackground: Panoramic radiography is one of the most commonly used radiographic diagnostic tools. Panoramic radiography exposure can increase the GCF flow because of increasing vascular permeability.GCF flow can also increase in periodontitis conditions. Objective: The purpose of this study was to analyze the effects of 1, 2, and 3 times exposure of panoramic radiography to the volume of GCF in wistar rats with periodontitis. Method: This research was true experimental with post test onlycontrol group design. Thesampling technique used simple random samplingthat obtained 25 wistar rats comprising of 5 wistars from each group in which the GCF samples of each group was collected from. Results: The average number of GCF inhealthy rats groupthat was not exposed to panoramic radiographywas 0.114 µL, the periodontitis rats group that was not exposed to panoramic radiography was 0.246 µL, the periodontitis rats group that was exposed1 time was 0.286 µL, the group of periodontitis rats exposed 2 times was 0.294 µL, and periodontitis group of rats exposed 3 times was 0.374 µL. The results of the One-Way Anova and Post Hoc Bonferroni tests stated that there is a significant increase in the volume of GCF between the control group and all other treatment groups, as well as between the non-exposed periodontitis group and the periodontitis group with 3 times exposure. Conclusion: Panoramic radiography can causeincrease the volume of GCF in wistar rats with periodontitis. Keywords: Gingival Crevicular Fluid,panoramic, periodontitis, wistar rats

Abstract

ABSTRACTBackground: Ramania leaf (Bouea macrophylla Griff) extract gel has secondary metabolites in the form of flavonoids, steroids, phenols and terpenoids which have a role as antioxidant. They will protect the body from excessive production of reactive oxygen species (ROS) by increasing endogenous antioxidants SOD, CAT and GPX, so that wound healing will not be inhibited and the process of collagen synthesis can run smoothly. Objective: To analyze the effect of ramania leaf extract gel that was applied topically with 5%, 10% and 15% concentration on collagen fibers density in incisional wound of male Wistar rats (Rattus norvegicus) on the 7th and the 14th day. Method: This research is a pure experimental study with a posttest-only control group design, using 24 rats which were divided into 4 groups: the treatment groups given ramania leaf extract gel of 5%, 10%, 15%, and the control group given placebo gel. The application of the extract gel was done once within 24 hours. The collagen level was measured with a spectrophotometer on the 7th and the 14th day. Results: Two-Way ANOVA test results on the 7th and the 14th day of each group showed a significant difference with p=0.000 (p<0.05). The Bonferroni Post-hoc Test showed a significant difference with p<0.05 between the placebo gel group and the groups of ramania leaf extract gel of 5%, 10%, 15% on the 7th and the 14th day. Conclusion: There is an effect of ramania leaf extract gel on collagen fibers density with the most effective concentration of 15%.Keywords: Antioxidant, , Collagen, Ramania Leaf Extract Gel

Abstract

ABSTRACTBackground: The most common disease in society is dental problems. People often care less about oral health. Dental hard tissue disease is considered as a mild disease, not contagious and not life-threatening, so people will visit the dentist if they feel disturbed, especially in terms of mastication, swallowing and speaking. Dental hard tissue diseases in conservative dentistry that often causes problems are reversible pulpitis, irreversible pulpitis, and pulp necrosis. Until now, there are no definitive data on the prevalence of dental hard tissue disease in conservative dentistry in Indonesia, as well as data on the prevalence of dental hard tissue disease in Banjarmasin, South Kalimantan. Purpose: To find out the prevalence of hard tissue disease in conservative dentistry in patients who came to Gusti Hasan Aman Dental Hospital, Banjarmasin, South Kalimantan in 2015-2018. Methods: This research was a descriptive study through a retrospective study. Samples in this study were taken by purposive sampling method, totaling 11,480 medical records of patients who came to the Conservative Dentistry Department at Gusti Hasan Aman Dental Hospital Banjarmasin in 2015-2018. Results: The data obtained showed the prevalence of reversible pulpitis as many as 6,700 (59%), irreversible pulpitis as many as 1,291 (11%), pulp necrosis of 3,234 people (28%), and others as many as 255 people (2%). Conclusion: The highest prevalence of dental hard tissue disease in the Conservative Dentistry Department of Gusti Hasan Aman Dental Hospital in 2015-2018 is reversible pulpitis, followed by pulp necrosis, and irreversible pulpitis. Keywords: Necrosis, Prevalence, Pulpitis,

Abstract

ABSTRACTBackground: Surface resistance between composite resin and dentin is currently one of the problems of restorative materials. Failure ofcomposite resin are still being reported in clinical studies with failure rates ranging between 5-45% based on observations for 5-17 years. Bioactive composite resin is a new type of composite resin that has mechanical and chemical properties similiar to teeth. The use of resins, bonding materials, drying time of the solvent, and type of solvent also affect the shear bond strength Purpose: The purpose of this study was to analyze the effect of air drying time bonding (self-etch) with ethanol as a solvent on the shear bond strength of bioactive composite resins. Methods: This study used 32 non-carious maxillary premolar teeth that were fixed using acrylic resin. Fiber glass with a diameter of 3 mm and thickness of 3 mm were fixed to dentin surface and applied using bioactive composite resin were divided into 4 treatment group, i.e. the group without air drying, 20 s, 40 s, and 60 s. Shear bond strength test using Universal Testing Machine. Results: One Way Anova and Post Hoc Bonferroni test showed significant differences with p=0,002 (p<0,05). The average shear bond strength value of the lowest bioactive composite resin in the group bonding without air drying with a value of 6.381 ± 2.818 MPa and the highest shear bond strength value of the bioactive composite resin in the 60 seconds bonding air drying group with a value of 11.873 ± 2.931 MPa. Conclusion: The air drying time of the bonding affects the shear bond strength of the bioactive composite resin.Keywords: Air drying time, bioactive composite resin, bonding, ethanol, shear bond strength.

Abstract

ABSTRACTBackground: Chronic periodontitis is an infectious disease that causes damage on periodontal ligament and alveolar bone. The severity of periodontitis is caused by several types of bacterial species which one of them is Porphyromonas gingivalis bacteria with a prevalence of 85% in oral cavity. The extract of kelakai leaf contained antibacterial in the form of flavonoid, alkaloid, tannin, and steroid. Flavonoid consists of some chemical compounds which is one of them is quercetin. The level of quercetin in kelakai leaf is 503.56 mgQE/g. From some secondary metabolites, kelakai leaf has inhibitory power toward gram negative bacterial, Porphyromonas gingivalis. Objective: This research was intended to know the activity of inhibitory power of kelakai leaf toward Porphyromonas gingivalis bacteria. Method: This research was an experimental research consisted of 5 experimental groups that were group of kelakai leaf extract on the concentrations of 100 mh/ml, 75 mg/ml, 50mg/ml, and 25 mg/ml and the control group (0.2% chlorhexidine). Each treatment was done in 4 repetitions. The test of inhibitory power used diffusion method by measuring the inhibitory zone around the growth of Porphyromonas gingivalis on Mueller Hinton Agar media. The data were analyzed by using One Way Anova 95% and then continued with LSD. Results: Based on the LSD test, it was known that the extract of Kelakai leaf had inhibitor power activity toward Porphyromonas gingivalis. The highest inhibitory zone was on the concentration of 100 mg/ml with inhibitory zone of 14.61 mm. Conclusion: The extract of kelakai leaf had inhibitory power activity toward Porphyromonas gingivalis bacteria in vitro. Keywords: 0.2% chlorhexidine, Diffusion method, Inhibitory power, Stenochlaena palustris extract, Porphyromonas gingivalis.

Abstract

ABSTRACTBackgrounds: The use of mouthwash daily may influence the mechanical properties of bioactive composite resin as a dental restoration, one of the mechanical properties is surface hardness. Herbal mouthwash can be used to minimize the side effect of mouthwash that contain alcohol. Mauli banana stem and basil leaf extracts can be produced into herbal mouthwash. Methods: Thirty specimens (10mm diameter x 2mm thick; n=5/group) bioactive composite resins were immersed in a mixed solution of Mauli banana and basil leaf extracts with concentration of 25%, 50%, 75 %%, 100%, the control group of chlorhexidine gluconate 0.2% and aquadest for 7 days in 37oC. Measurement of the surface hardness was using the Vickers Microhardness Tester with 100 gf load for 15 seconds. Results: One Way ANOVA and Post Hoc Bonferroni exhibited the significant differences (p <0.05) in hardness values between the specimens that immersed in the mixed solution Mauli banana and Basil leaf extracts, which in the concentration of 100% (4,49 ± 4,61 VHN) compared with 25% (38,20 ± 2,58 VHN), 50% (41,40 ± 3,84 VHN) and 75% concentration (40,40 ± 3,55 VHN). There was no siginificant difference (p >0.05) between specimens immersed in CHX, aquadest and the mixed solution of mauli banana and basil leaf extracts in all concentration. Conclusion: There is no change in bioactive composite resin surface hardness after immersed in the mixed solution of Mauli Banana stem (Musa acuminata) and Basil leaf (Ocimum basilicum) extracts.Keywords: Basil leaf extract, Bioactive resin, Mauli Banana stem extract, Surface hardness

Abstract

ABSTRACTBackground: Chronic periodontitis is an infectious disease caused by bacterial colonization of dental plaque. Bacteria that play a role in chronic periodontitis is Porphyromonas gingivalis. One of mouthwash that reduce the number of P.gingivalis colonies is chlorhexidine 0.2%. Long term use of chlorhexidine 0.2% can cause some side effects to the oral cavity, so we need an alternative mouthwash from natural ingredients that can reduce the side effects of chlorhexidine 0.2%. Ulin bark extract contains phenolic, flavonoid, tannin, alkaloid, terpenoid, and saponin which can be an alternative base for mouthwash besides chlorhexidine 0.2%. Purpose: It is to analyze antibacterial effectivity of ulin bark extract on the growth of Porphyromonas gingivalis. Methods: This study was using true experimental research and post-test only with control group design, that used 9 treatment groups with 4 replications, that were 5%, 10%, 20%, 40%, 60%, 80%, 100% concentrations, chlorhexidine 0.2%, and sterile aquadest against P. gingivalis. The total samples were as many as 36. Result: One Way ANOVA and Post-Hoc Games Howell tests showed that the average absorbance values has a significant difference, then Kruskall Wallis and Post Hoc Mann Whitney tests showed the number of colonies with significant differences. MIC in this study was at 5% concentration and MBC was at 20% concentration.. Conclusion: Ulin bark extract with 40% concentration has an absorbance value equivalent to 0.2% chlorhexidine and 20% concentration has a bactericidal effects equivalent to 0.2% chlorhexidine against the growth of P.gingivalis.Keywords:Antibacterial, Dilution method,Porphyromonas gingivalis, Ulin Bark Extract.
