Abstract

Introduction Corona Virus Disease 19 (COVID-19) infection was first reported inWuhan, China, in December 2019. The author wants to know how the OncologySurgery Division elective surgery patient services before and during early pandemi cera of COVID-19 at Dr. M Djamil Padang Hospital. M ethod: This research is acomparative study with a quantitative approach to elective surgery patients in theOncology Surgery Division before and during early pandemic era of COVID-19 at Dr.M Djamil Padang Hospital. The sampling technique in this study was total sampling.Data analysis will be carried out using statistical analysis in accordance with theexisting data scale and using SPSS software. Results: In this study, 137 sampleswere obtained consisting of 80 samples before the COVID-19 pandemic and 57samples during early pandemic era of COVID-19. Of the 57 patients who underwentelective surgery from the Surgical Oncology Division during early pandemic era ofCOVID-19, all patients were not suspicious / confirmed. The time for oncologyservice assessment was 14.26 days before COVID-19 pandemic and 8.93 days duringearly pandemic era of COVID-19. Conclusion: More patients who underwent electivesurgery in the surgical oncology division were female, both before and during earlypandemic era of COVID-19. The mean age of patients before COVID-19 pandemicwas 48.72 years and 49.53 years during early pandemic era of COVID-19. There isa disparity between the length of time the assessment of elective surgery patients inthe surgical oncology division before and during early pandemic era of COVID-19pandemic.

Abstract

Introduction : Dr. M. Djamil Padang General Hospital (RSUP) still applies conventional methods as thyroidectomy treatment. Satisfaction and comfort to the scar is one of the issues because conventional thyroidectomy surgery scars are located in the neck area that will affect the confidence of patients, especially women. Method: This research was a quantitative research using cross sectional study design. The research was conducted at The Oncology Surgery Polyclinic of RSUP Dr.M. Djamil Padang with a sample of 50 respondents. Result: More than half of thyroid tumor patients found were women (56%), most patients had total thyroidectomy surgery (70%) with malignan tumor type (72%), and the most tumor size in the T4 group (52%). The average age of patients during surgery was 54.26 years. The average patient satisfaction score for post-thyroidectomy scars was 14.72 with a total score of 16, the lowest score was 6 and the highest score was 22. Significant relationships were found in variable tumor size (p= 0.000), type of surgery (p= 0.005), tumor type (p=0.004) and gender (p = 0.028), while there was no statistically significant relationship between age and patient satisfaction with postthyroidectomy scars (p=0,176). Conclusion: There were significant relationships between gender, type of surgery, type of tumor, tumor size to patient satisfaction with post-thyroidectomy scars, while age has no signifacnt relationship to patient satisfaction with post-thyroidectomy scars. A small number of PSAS score showed that the average patient was satisfied with post-thyroidectomy scar.

Abstract

Introduction : Inguinal herniation is a protrusion of peritoneal component through abdominal wall caused by the weakness of aponeurotica tissue of abdomen. Inguinal herniation is the most common herniation cases in which 291.145 patient registered in Indonesia at 2012 and 87 patients in General Government Hospital of M. Djamil Padang at 2017-2018. Herniation treatment is consisted of herniorrhapy laparoscopy using the TEP or TAPP techniques and both of them are related to the incidence of chronical post-operative pain that was defined by IASP as an uncomfortable sensation lasting for more than 3 months. Method: This research was using retrospective study design. The research was conducted at RSUP Dr. M. Djamil Padang in August – October 2020 with 54 samples. Result: The study found there was 54 cases of inguinal herniation fit to the inclusion criteria with peak age of 82 years old and average of 59.1 years old. Most patients were male (79.62 %). There were 2 patients having chronic pain (7.41%) after herniorrhapy using the TAPP technique and 1 more (3.71%) having chronic pain with TEP technique. Bivariate analysis was done by Fisher Exact Test and the study concluded there was no statistical significance in relationship of chronic post-operative pain incidence with the particular technique chosen at herniorrhapy laparoscopy (p=1.000). Conclusion:There was no significant relationship between the incidence of postoperative chronic pain in patients undergoing laparascopic hernioraphy and TEP or TAPP procedure.

Abstract

Introduction Diabetic foot is an erosion or injury to the epidermis or the distal sole of the foot and the base of the foot in people with diabetes mellitus (DM). Diabetic foot is a major cause of limb amputation in non-traumatic cases. Per year, as many as 2-3% of people with diabetes experience complications of diabetic foot. The prevalence of DM sufferers with diabetic foot is 20-40% in developing countries. In Indonesia, the prevalence of DM sufferers with diabetic foot is around 15% with 32% mortality rate. One factor plays an important role in predisposing the occurrence of diabetic foot is the lipidprofile. The aim ofthis study was to determine the correlation between lipid profile and Wagner Severity Level on diabetic foot patients at Dr. M. Djamil Central Public Hospital, Padang in the period of July 2020 to December 2020. M ethod: This research is an observational analytic study with cross sectional analytic design. The research sample of 47 cases was taken by non-probability sampling with the consecutive sampling method. This research is an observati onal analytic study with cross sectional analytic design. Result: There were 47 cases that met the inclusion criteria where the 51 -60 ye ars age group (55.3%) was the largest group in cases of diabetic foot. Of these groups, it was dominated by female patients, namely 31 patients (66.0%). Meanwhile, the Body Mass Index (BMI) was dominated by the group with BMI <25 kg/m2 with a total of 26 patients (55.3%). Diabetic foot patient at Dr. M Djamil Central Public Hospital Padang who underwent amputation was 25 patients (53.2%) where most of them, namely 40 patients (85.1%) had suffered from Diabetes Mellitus for> 5-10 years. The findings obtained based on the Wagner Severity Level were thatthe Wagner grade 3- 5 (heavy) group consisted of 24 people (51.1%) and Wagner 1-2 (mild) as many as 23 people (48.9%). The results of the bivariate test between lipid profile levels with Wagner Severity Level in diabetic foot patients showed a significant correlation as indicated by a p value <0.05. Conclusion: Wagner Severity Level which is getting higher in diabetic foot patients shows low HDL levels, high LDL levels andhigh levels of triglycerides in blood plasma.

Abstract

Introduction. Pandemic Covid-19 situation in Indonesia since March 2021, make hospitals to have adaption with condition. Therefore, there is un satisfaction for nurse profession, training, nurse assessment and ethical in emergency, this is known by pre survei in emergency. This research was aimed to study about nurse satisfaction in pandemic area, and see pandemic effect to nurse satisfaction, topic research is satisfaction for nurse profesion, training, nurse assesment and ethical nurse. Methods. Research method for this research is descriptive quantitative, where research use fact, situation and variable happen when the research on going. instrument used in this research is quisoner, sample used is nurse which work in Hospital X, Tangerang district with total sample 151 nurses. Results. Nurse satisfaction preview in hospitals X, Tangerang district is more than 90% nurses satisfied with profession of nurses, training more than, nurse assessment and ethical of nurse. Conclusion. All the nurse satisfied with their profession, training, assesment and ethical implementation, there is improvement room to reduce blame culture in nurse division and implement just culture in nurse division.

Abstract

Background: The incidence of breast cancer worldwide is still high. Surgery remains the top choice with other modalities of chemotherapy, radiation, and immunotherapy such as Artemisia vulgaris (AV). The study was aimed to demonstrate that administration of AV extract increased p53 expression and the apoptotic index in mammary adenocarcinoma.Methods: This study used a "Post-test only control group design" on 24 females C3H mice that were randomly selected and divided into four groups: group K (control), P1(chemotherapy), P2 (extract), and P3 (combination). Mammary adenocarcinoma comes from the inoculation of donor mice. Chemotherapy of Adriamycin 0,18 mg and Cyclophosphamide 1,8 mg were given in 2 cycles. AV 13 mg (0.2 ml) was given once daily orally. IL-12 and granzyme expression were evaluated by immunohistochemical staining.Results: Mean of IL-12 and Granzyme expression were found in groups K, P1, P2, P3 were 50,40 ± 1,56, 60,28 ± 1,54, 53,48 ± 1,35, 75,40 ± 1,46 dan 14,96 ± 0,61, 24,86 ± 1,21, 17,14 ± 1,02, 26,62 ± 0,70. The statistical analysis showed that there were significant differences in the IL-12 between groups of K vs P1 (0,001), P1 vs P2 (0,001), K vs P2 (0,028), P1 vs P3 (0,001), K vs P3 (0,001), P2 vs P3 (0,001) and in granzyme expression between groups of K vs P1 (0,001), P1 vs P2 (0,001), K vs P2 (0,010), P1 vs P3 (0,047), K vs P3 (0,001), P2 vs P3 (0,001). Correlation analysis between IL-12 and Granzyme expression were found significant correlation (p = 0,001 and r = -0,911).Conclusion: Artemisia vulgaris can improve the effectivity of AdriamycinCyclophosphamide chemotherapy on C3H mice with mammary adenocarcinoma in terms of elevated IL-12 and Granzyme expression.

Abstract

Chronic Kidney Disease (CKD) can cause growth problems in children. This condition will affect children’s social life and increase morbidity and mortality. Growth hormone (GH) is one of medication that has been used for treatment of impaired growth in children with chronic kidney disease. In Indonesia, this management has not been provided, because it is expensive and its effectiveness is uncertain. This research was conducted to determine the effectiveness of growth hormone in increasing linear growth in children with CKD. This study is a systematic review of the Randomized Controlled Trial (RCT) study to determine the effectiveness of growth hormone as a treatment for children with CKD. Literature search was conducted using five databases, namely Pubmed, Google Scholar, DOAJ, BMC and CENTRAL that matched the inclusion and exclusion criteria. The guide used in the literature selection was the PRISMA flow chart. A Total of 10 studies were included in the systematic review. Growth hormone is effective in increasing linear growth in children with CKD. Generally there was a significant increase in height (standard deviation (SD)), high velocity (SD or cm) in the therapy group compared to the control group. The dose of growth hormone that is given varies, but the most is 4 IU / m2 / day, once a day, by subcutaneous injection. The duration of growth hormone administration varied from 6 months to 5 years with an average of 1.7 years. Growth hormone did not aggravate kidney problems in children with CKD and can increase important factors in bone formation, namely alkaline phosphatase and osteocalcin.

Abstract

Background: Primary CNS Lymphoma (PCNSL) is rare cancer from the brain or spinal cord lymphatic tissue. The incidence increased in patients over 60 years, with an incidence rate of 0.5/100,000 per year. PCNSL associated with poor survival. The aim of the study was to determine the incidence of Brain Malignant Lymphoma based on age, gender, symptoms, imaging modalities, HIV examination, and histopathological results at dr. Kariadi Semarang.Methods: This is a retrospective descriptive study. We collected brain malignant lymphoma patient’s medical records who came to the ER and the Neurosurgery Polyclinic of Dr. Kariadi Semarang from period January 2016-December 2018.Results: There were 1,012 brain tumor patients, 6 patients were brain malignant lymphoma with the highest prevalence in the age range of 45-65 years (5 patients) and 66.67% male. The most common symptom was blurred vision 23.07%. The imaging modalities showed lesions on: right parietal lobe 14.29%, sella region 14.29%, parasella region 14.29%, cerebellum 14.29%, IV ventricle 14.29%, vertebrae sacrum 2-5 14.29 %, and the fronto-temporo-parietal region 14.29%. No HIV test data were obtained. According to histopathology, Diffuse large B cell Lymphoma Malignant non Germinal Center (DLBCL) and High grade, B cell Non Hodgkin Lymphoma (small type) each were 33.33%.Conclusions: The prevalence of malignant brain lymphoma cases was highest in the 45–65 year age group and male, with blurred vision. Most of the results of imaging modalities show an even distribution of brain malignant lymphomas and generally do not do HIV testing. The most common histopathological diagnosis was DLBCL

Abstract

Background. Antioxidants are defined as compounds that are able to delay, slow down, or inhibit oxidation reactions. One of the plants that has the potential as natural antioxidants is the telang flower. The purpose of this study was to determine whether the ethanol extract of telang flower (Clitoria ternatea L.) can be formulated in a losion form that meets the requirements of the physical evaluation test and to determine the ethanol extract of telang flower (Clitoria ternatea L.) has antioxidant activity. Methods. The extraction using maceration techniques with 96% ethanol solvent. The yield obtained from the maceration technique was 29.16%. Phytochemical analysis of telang flowers contains alkaloids, flavonoids, polyphenols, saponins and tannins. Determination of the levels of flavonoids obtained levels of 10.05% and for polyphenols of 15.33%. Results. Telang flower extract (Clitoria ternatea L.) can be formulated into losions and meets the requirements of the physical evaluation test in the extract concentration of 0.1%. The losion of telang flower extract (Clitoria ternatea L.) with a concentration of 0.1% telang flower extract has very strong antioxidant activity because it has an IC50 value <50 ppm, and the IC50 yield of a losion with a concentration of 0.1% telang flower extract (Clitoria ternatea L.) of 37.92 ppm which means that it has very strong antioxidant activity.Conclusion. The formula of the telang flower extract lotion has strong antioxidant activity and has the potential to protect the skin from free radicals.

Abstract

Breast cancer is a hormone-dependent disease that relies on the mitogenic effect of estrogen to increase tumorigenesis and tumor growth. Clinically significant levels of estrogen-α receptor (ERα) expression are seen in 80% of human breast cancers, whereas progesterone receptor is expressed in 55% of human breast cancers. These data are one of the bases for the development of endocrine therapy. Endocrine therapy is therapy that targets the pathway and synthesis of estrogen, by blocking it via receptors, reducing circulating levels of estrogen, or suppressing estrogen synthesis in the tissues of women diagnosed with breast cancer.
