Abstract

Objective: This study aimed to analyze the relationship between social support, workload, and burnout among inpatient nurses at Hospital X Methods: This study employed a cross-sectional design conducted from December 2024 to May 2025 at Hospital X. The population consisted of 69 inpatient nurses, with a sample of 59 nurses selected using simple random sampling. Data were collected using self-administered questionnaires measuring social support was assessed using a Sarafino-based instrument consisting of emotional support, informational support, instrumental support, appreciative support. Workload using a Budiasa-adapted questionnaire, consisting of work target, working conditions, use of working time, and work standards. Burnout using the Maslach Burnout Inventory consisting emotional exhaustion, depersonalization, and reduced personal accomplishment. Data analysis included descriptive statistics, normality testing, and multiple linear regression with a significance level of α is 0.05. Results: The results showed that nurses experienced moderate levels of social support (mean=31.29), workload (mean=21.22), and burnout (mean=14.98). Multiple regression analysis revealed that social support had a significant negative effect on burnout (B=−0.151, p=0.010), indicating its protective role. In contrast, workload had a significant positive effect on burnout (B = 0.400, p < 0.001), identifying workload as a major risk factor. The regression model explained 33% of the variance in burnout. Conclusion: Social support significantly reduces burnout, while workload significantly increases burnout among inpatient nurses. Strengthening workplace social support and implementing effective workload management are essential strategies to mitigate burnout and improve nurses’ well-being and service quality at Hospital X.

Abstract

Health information systems are important in providing efficient and effective services in the era of rapid development of information technology today. This study analyzed healthcare workers' acceptance of the benefits of implementing the Integrated Outpatient Revisit Patient Service Information System (SI PENDEKAR) at the outpatient clinic in Dr. Soedono General Hospital. This study used quantitative analysis with a cross-sectional approach. The questionnaire used a Technology Acceptance Model (TAM) instrument to evaluate officer acceptance of SI PENDEKAR innovation. The research subjects included 30 healthcare workers related to the use of innovation on officers’ attitudes towards using (path coefficient = 0.578, p=0.001) and perceived usefulness on behavioral interest in using (path coefficient = 0.651, p =0.001). The healthcare worker’s perception of the benefits of this innovation will increase the attitude of acceptance and attitude of use, which can increase the speed of services in the outpatient clinic.

Abstract

Peningkatan kualitas pelayanan rumah sakit sangat dipengaruhi oleh efektivitas pengelolaan sumber daya manusia (SDM). Human Resources Scorecard (HRSC) merupakan pendekatan strategis yang digunakan untuk mengukur kinerja SDM melalui perspektif keuangan, pelanggan, proses bisnis internal, serta pembelajaran dan pertumbuhan. Penelitian ini bertujuan menganalisis implementasi HRSC dalam pengukuran kinerja SDM di rumah sakit berdasarkan hasil penelitian terdahulu. Metode yang digunakan adalah literature review dengan pendekatan deskriptif-kualitatif melalui penelusuran artikel pada Google Scholar, Portal Garuda, dan Elsevier tahun 2019–2024. Berdasarkan kriteria inklusi, diperoleh enam artikel yang relevan. Hasil kajian menunjukkan bahwa HRSC mampu membantu rumah sakit dalam menetapkan indikator kinerja yang terukur, meningkatkan efektivitas kerja karyawan, mengidentifikasi kelemahan organisasi, serta mendukung pengambilan keputusan strategis berbasis data. Dengan demikian, HRSC direkomendasikan sebagai instrumen evaluasi kinerja SDM untuk meningkatkan kualitas pelayanan dan daya saing rumah sakit.

Abstract

Menopause represents a natural stage in a woman’s life that may be accompanied by physical, psychological, and social symptoms requiring appropriate clinical management. This study aimed to conduct a preliminary assessment of the feasibility of establishing a dedicated Menopause Clinic as a flagship service at MedicElle Primary Clinic in Surabaya. A quantitative descriptive study with a retrospective cross-sectional design was conducted using medical records of women aged 40-60 years who met the inclusion criteria. A total of 58 eligible participants were selected through a total sampling approach. The majority of participants were in the perimenopausal period and presented with multiple menopausal symptoms, predominantly vasomotor complaints, sleep disturbances, psychological manifestations, urogenital symptoms, and musculoskeletal pain. Overall, 82.7% of the patients experienced at least one menopausal symptom, with a mean Complaint Index of 2.8. Indicators of service readiness demonstrated the availability of six healthcare professionals, a facility readiness score of 4.2 out of 5, a follow-up rate of 67%, and a medical record completeness rate of 93%. These findings suggest that the institution has demonstrated preliminary readiness and that there is a clear demand for menopause related healthcare services. However, the overall feasibility of establishing the proposed clinic cannot yet be fully determined because important aspects, including financial feasibility, patient satisfaction, healthcare provider perspectives, and long-term clinical outcomes, were not evaluated in this study.

Abstract

This literature review aims to examine the influence of service quality and physical evidence on revisit intention in healthcare settings. As patient loyalty becomes increasingly critical in competitive health service markets, understanding non-medical service factors is essential. This review systematically analyzes 20 empirical journal articles published between 2016 and 2024, focusing on how tangible environmental elements and frontline service delivery affect patients' likelihood of returning to the same healthcare facility. Data were collected from journal databases using keywords such as "service quality," "physical evidence," and "revisit intention." Most of the reviewed studies used quantitative methods, applying statistical tools such as PLS-SEM and multiple regression analysis. Results show that service quality—particularly empathy, assurance, and responsiveness—has a consistently strong positive effect on revisit intention. Physical evidence, including facility cleanliness, layout, and comfort, also influences revisit behavior, although its impact varies depending on context. This review highlights the need for hospitals to improve both environmental and interpersonal service dimensions to enhance patient retention. Limitations include a focus on selected studies and potential publication bias. Future research should explore moderating variables and conduct longitudinal or comparative studies.

Abstract

Abstract Background : Human resource (HR) management in hospitals faces competency mismatches in structural positions and disproportionate workloads. This phenomenon stems from weak organizational needs analysis and flawed bureaucratic structures. Aims : This study aims to provide a novel descriptive theoretical framework on the "right man on the right place" concept and work standards through a needs analysis approach and proper organizational structure design to generate accurate job descriptions.. Methods : This Systematic Literature Review utilized the PRISMA protocol. A comprehensive literature search was conducted on Google Scholar, PubMed, and ScienceDirect databases for the 2020-2026 period. Results : The synthesis indicates that unclear main duties and functions trigger multiple roles due to administrative staff shortages. Implementing job analysis competencies supported by transformational leadership proves to be a key factor in enforcing fair workload distribution. The use of instruments like Workload Indicators of Staffing Need (WISN) is effective in ensuring proper staff placement. Conclusion : The application of transformational leadership in strengthening organizational structure through the integration of job analysis and objective workload calculation is an absolute pillar to minimize human error and optimize patient care quality in hospitals. Keywords : Job Analysis, Patient Care Quality, Staff Placement, Structural Position, Transformational Leadership.   Abstrak Latar Belakang : Manajemen sumber daya manusia (SDM) di rumah sakit dihadapkan pada terjadinya ketidaksesuaian kompetensi pada jabatan struktural serta beban kerja yang tidak proporsional. Fenomena ini berakar dari kelemahan analisis kebutuhan organisasi dan struktur birokrasi yang keliru. Tujuan : Penelitian ini bertujuan untuk memberikan deskripsi teoretis baru mengenai konsep the right man on the right place dan standar kerja melalui pendekatan analisis kebutuhan serta desain struktur organisasi yang benar guna menghasilkan deskripsi pekerjaan (job description) yang akurat. Metode : Tinjauan literatur sistematis (Systematic Literature Review) ini mengikuti protokol PRISMA. Pencarian artikel dilakukan pada database Google Scholar, PubMed, dan ScienceDirect untuk periode 2020-2026. Hasil : Sintesis hasil menunjukkan bahwa penataan tugas pokok dan fungsi yang tidak jelas memicu fenomena peran ganda (multiple roles) akibat kekurangan staf administrasi. Implementasi kompetensi analisis jabatan yang didukung oleh kepemimpinan transformasional terbukti menjadi kunci utama penegakan keadilan distribusi beban kerja (work load). Kesimpulan : Penerapan kepemimpinan transformasional dalam penguatan struktur organisasi melalui integrasi analisis jabatan dan penghitungan beban kerja yang objektif merupakan pilar mutlak untuk meminimalkan human error dan mengoptimalkan mutu pelayanan pasien di rumah sakit.. Abstract

Background: Digital transformation in the healthcare sector has accelerated the implementation of Electronic Medical Records (EMR) to improve service quality and operational efficiency. However, the level of EMR acceptance among healthcare professionals still varies, indicating that technological readiness alone is not sufficient to ensure successful adoption.Objective: This study aims to analyze the determinants of EMR adoption based on a synthesis of empirical evidence using the Technology Acceptance Model (TAM) frameworkMethods: A systematic literature review was conducted following the PRISMA guidelines. A total of 20 empirical studies published between 2020 and 2025 were selected through systematic identification, screening, eligibility assessment, and quality assessment. The reviewed studies examined key TAM variables, including perceived usefulness (PU) and perceived ease of use (PEOU), as well as external factors such as management support, training, information technology infrastructure readiness, social influence, and digital literacy.Results: The synthesis reveals that PU and PEOU are the most consistent and significant determinants of both the intention to use and actual use of EMR systems. PEOU (Perceived Examination of Use/System Usefulness) often emerges as an initial determinant that influences PU (Perceived Use/User Usefulness), thereby indirectly reinforcing behavioral intent. Organizational factors generally do not have a direct influence on intention, but play an important role in shaping users' perceptions of the usefulness and ease of use of the system. Social factors and digital literacy were found to be increasingly relevant in collaborative and technology-based healthcare environments. Conclusion: Findings indicate that successful EMR implementation depends not only on technical readiness but also on organizational strategies and user capacity-building interventions. During the digital adoption transition phase, prioritizing system usability and ongoing training is essential to increase acceptance. Further research is recommended to integrate TAM (Technology Acceptance Model) with other behavioral theories, such as UTAUT (Uniform Teaching and Understanding/Information Technology), to better understand the dynamics of EMR acceptance in the context of mandatory policies, particularly in Indonesia following the implementation of PMK No. 24 of 2022.

Abstract

Patient-Centered Care (PCC) is increasingly evaluated alongside clinical accuracy, especially in inpatient wards where interactions and coordination demands are intensive. This study analysed challenges in implementing a PCC-oriented work culture in the inpatient ward of RSUD Ketapang Sampang, focusing on communication, patient/family involvement, interprofessional coordination, and contextual factors shaping everyday care. Using a qualitative Interpretative Phenomenological Analysis (IPA) design, data were collected through semi-structured in-depth interviews with two participant groups (health professionals and inpatients and/or accompanying family members), supported by field observations and relevant document review. Analysis followed IPA stages from repeated reading and initial noting to emergent and cross-case theme development, with rigor enhanced through triangulation, member checking, and an audit trail. Findings indicate that PCC is understood as a service orientation that respects patients as persons, yet its relational expression often narrows when workload intensifies and routines become task-driven. Patient and family involvement was present but did not consistently translate into shared decision-making, and was frequently experienced as receiving information rather than dialogue and clarification. The consistency of patient experiences was shaped by handovers, authority boundaries, and the extent to which interprofessional coordination became visible as coherent communication to patients/families. PCC practices strengthened when supported by key figures and clear work supports, but sustainability remained uneven across shifts and staff.

Abstract

Background: The implementation of the Indonesian Case-Based Groups (INA-CBGs) system involves a persistent tension between cost containment and quality of care that threatens the financial sustainability of both hospitals and the National Health Insurance (JKN) scheme. Objective: To synthesise current evidence on how INA-CBGs implementation affects hospital financing efficiency and quality of care in the JKN era. Methods: A systematic literature review was conducted following the PRISMA 2020 guideline. Google Scholar, PubMed, ScienceDirect, Scopus, and Garuda were searched for studies published between 2014 and 2026, using Boolean combinations of the terms INA-CBGs, cost analysis, pending claims, and clinical-pathway compliance. Methodological quality was appraised with the Joanna Briggs Institute (JBI) critical appraisal tools, and the twelve eligible studies were synthesised narratively using the synthesis-without-meta-analysis (SWiM) approach. Results: Three themes emerged. First, complex cases and class-3 inpatients showed a predominantly negative cost gap (an ischaemic-stroke deficit averaging IDR 214,049 per patient, equivalent to an extrapolated annual deficit of about IDR 1.26 billion). Second, pending-claim rates varied across hospitals (1.99%–4.4%) and were driven mainly by clinical (up to 60.9%) and coding (33.6%) factors. Third, clinical-pathway compliance was low (80% of hospitals below 50%), despite consistent evidence that greater adherence reduces cost, length of stay, and complications. Conclusion: INA-CBGs has not yet achieved an efficiency–quality balance. Strengthening clinical pathways through routine audit, joint training, evidence-based tariff adjustment, and claim digitalisation is needed to safeguard JKN sustainability (claim ratio ≈108%).
