Abstract

Background and purpose: National strategy to increase the coverage of prevention of mother to child transmission (PMTCT) program in Indonesia is integrating the service into public health centres (puskesmas). In Bali, the integration has been started since 2014. After one year of program implementation, an evaluation was conducted in order to provide input and feedback for program improvement. Methods: Secondary data analysis and indepth interview with 13 key informants in Denpasar City were conducted to assess input, process and output of the integration implementation. Results: Of the 11,719 pregnant women targeted within the program, 43% were offered to undergo HIV testing and 98% of these agreed to have HIV test with 17 were found HIV positive. Interviews with key informants found that there were adequate resources both in terms of manpower, funds and equipment. Obstacles found in the implementation include different site of ANC and HIV testing facility. The other barrier was high number of pregnant mother who conducted ANC at private obstetrician which not yet included in the program. Conclusions: Integration of PMTCT into ANC services at public health centre (PHC) is an effective way to improve the uptake of HIV screening among pregnant women. However, there is a need of effective of referral systems from private midwives and PHC satellite services. The expansion of networks into private midwives and obstetrician will be instrumental in improving performance outcomes.

Abstract

Background and purpose: Bali was visited by more than 4.9 million foreign tourists in 2016. To some extent foreign tourists may need health services during their travel. This study aims to describe perception of preparedness of general practitioners (GPs) to provide health services for foreign tourists. Methods: Surveys were conducted among foreign tourists who visited Bali in 2012 and among GPs in 2013. Self-administered questionnaires on the level of satisfaction were completed by 108 foreign tourists. Structured questionnaires exploring the travel medicine knowledge and English language proficiency were completed by 109 GPs. The experience of foreign patients was used to triangulate the response from GPs. Result: Good satisfaction in terms of capacity of GPs to provide quality services reported by foreign tourists was varied between 76% and 85%. Assessment of level of knowledge among GPs regarding travel medicine showed that 33% GPs were having good knowledge, 55.1% fair and 11.9% poor. The proportion of correct answer of several items were low, which included recommended vaccine (11%), management of jetlag (13,8%), symptoms of barotrauma during diving (13,8%), sunburn and protection (31,2%), malaria prevention (21%), and diarrhea management (27,5%). Good satisfaction of English communication reported by foreign tourists was between 65% and 78%. Meanwhile, 60% GPs believed to have good English communication with their foreign patients. Conclusion: Preparedness of general practitioners to deliver health services and in English communication to foreign tourists was generally sufficient. Knowledge of GPs on several aspects of travel medicine was insufficient.

Abstract

Background and purpose: In Indonesia, cervical cancer has the second-highest prevalence of cancer cases after breast cancer. This study aims to determine the survival rate and to explore factors that affect the survival of cervical cancer patients at Wangaya Hospital, Denpasar, Bali. Methods: This was a retrospective cohort study. The samples were 165 cervical cancer patients who started therapy at Wangaya Hospital between 2016 – 2021 and met the research criteria. The variables retrieved from medical record were age, employment status, marital status, education level, parity, stage of cancer, type of treatment, comorbidities and time to event of death. Data were analyzed using Kaplan Meier, Log-rank test, and Cox Proportional Hazard. Results: The result shows the cervical cancer survival rate during the period of study is 67.27% with the average of follow up time was 38.4 months and a mortality rate was 7.2/1000 person months (IR=0.007). From a total of 165 patients who were sampled, 54 patients died (32.73%). Based on the results of the Cox Proportional Hazard model, it was found that the factors significantly affected the death of cervical cancer patients at Wangaya Denpasar Hospital were older age (HR=1.06; 95%CI: 1.03–1.08; p<0.001), patients who were employed (HR=2.32; 95%CI: 1.24–4.33; p=0.008), at stage 1 cancer (HR=3.94; 95%CI: 1.07–14.49; p=0.039), stage 2 (HR=5.86; 95%CI: 1.50–22.78; p=0.011), stage 3 (HR=11.73; 95%CI: 3.15-43.56; p<0.001), stage 4 (HR=18.95; 95%CI: 4.33-82.91; p<0.001) and had comorbidities (HR=2.11; 95%CI: 1.12-3.99; p=0.021). Conclusion: The survival of cervical cancer patients at Wangaya Hospital is quite good and needs to be improved. To increase patient survival, appropriate treatment is needed, especially in patients who have comorbidities and older age. Screening efforts also need to be improved to find cases at earlier stages.

Abstract

Background and purpose: Since 2016, Badung District, Bali has implemented a free HPV vaccination program for school children, with vaccination coverage reported to be over 90% in 2016-2018. This study aims to objectively assess the coverage of HPV vaccination among school children in Badung District. Methods: This survey used a cross-sectional design that conducted between March and June 2019. Sample selection was done by systematic random sampling, following the WHO guidelines. A total of 216 families met the sample criteria, with a total of 249 children from all families. Respondents in this study were eligible girls' mothers or families who knew the child's vaccination history. Variables of this study were sociodemographic characteristics of respondents and children, HPV vaccination recall and HPV child vaccination validation. The data collected were analyzed descriptively to calculate the coverage of recall and validation of HPV vaccination. Results: The majority of respondents were mothers (70.83%) with a high school education (51.85%). The median age was 43 years (IQR=8 years). Each family had a mean of 1.15 eligible children (SD=0.39). Of the 249 children, most were aged 16-19 years (43.37%), with age at vaccination mostly ≤12 years (46.52%). HPV vaccination coverage by recall was 82.32% (95%CI: 77.56-87.09), with HPV vaccination coverage by validation was 76.59% (95%CI: 70.74-82.43). Conclusion: After validation, vaccination coverage was lower than vaccination coverage by recall. The health office needs to improve the vaccination registration system, in particular the uniformity of registration and record keeping to ensure proper traceability of vaccination history.

Abstract

Traffic accident is an unexpected event that occurs on the highway usually at a productive age. Traffic accidents occur due to the level of violations committed by the driver. The severity of head injury patients was assessed from the GCS score, namely mild, moderate, and severe head injuries. This study aims to determine the effect of the violations on head injuries in traffic accidents cases in Denpasar City based on age, gender, GCS score, classification of head injuries based on GCS score dan morphology, Mechanism of Injury to assess the level of violations. This research is analytic with cross sectional approach. The data used is secondary data of patients head injuries due to traffic accidents registered in the Medical Records Installation of Sanglah Hospital Denpasar in 2020. The data collection technique by total sampling with the result that 68 data matched inclusion criteria. The results show that most common patient found in the age group 15-24 years(38.2%), male (83.8%), GCS Score 13-15(48.6%), minor violations(33.8%), mild head injury(48.5%) with bleeding morphology(79.4%), and a chi-square test result 0.024 in severe head injuries caused by violation. The conclusion is the most head injury patient due to traffic accidents in Denpasar City are aged 15-24 years, male, GCS score 13-15, commits minor level violations, caused minor head injury with bleeding morphology, and result of chi-square test show that level of violation affect head injury. The result of this study can be developed and become the basis for the other research with more varied variables. Keywords : Traffic Accident, Violation Rate, Head Injury

Abstract

Bullying merupakan bentuk perilaku agresif yang menyakiti orang lain dan memberi banyak dampak negatif. Ada banyak faktor risiko bullying, namun faktor risiko ini kerap tidak konsisten antar penelitian. Tujuan penelitian ini adalah untuk mengetahui prevalensi pelaku dan korban bullying di Denpasar, serta meneliti hubungan variabel pola asuh orang tua dan perilaku bermain game online yang mengandung kekerasan dengan bullying. Variabel lain yang diteliti di penelitian ini adalah usia, kelas, jenis kelamin, dan jenis SD. Penelitian ini menggunakan desain analitik dengan pendekatan cross sectional. Responden penelitian berjumlah 340 anak dari empat SD kelas 4-6 yang dipilih melalui cluster random sampling. Pola asuh orang tua diukur dengan Parenting Style and Dimenssion Questionnaire (PSDQ). Perilaku bermain game online diukur dengan Entertainment Software Rating Board (ESRB). Perilaku bullying diukur dengan Peer Interaction in Primary School (PIPS) Questionnaire. Analisis data dilakukan dengan analisis univariat dan bivariat. Hasil dari penelitian didapat prevalensi prevalensi pelaku bullying adalah 76,2%, sedaangkan prevalensi korban bullying adalah 67,4%. Perilaku bermain game online yang mengandung kekerasan berhubungan signifikan dengan perilaku bullying, sedangkan pola asuh orang tua tidak berhubungan signifikan. Peneliti menyarankan orang tua untuk mengarahkan anak agar tidak bermain game online yang mengandung kekerasan, serta memberikan pola asuh yang baik.

Abstract

Penyakit tidak menular (PTM) merupakan salah satu penyebab kecacatan, pembengkakan biaya kesehatan, dan kematian terbanyak di dunia. Skrining menjadi salah satu upaya untuk mencegah penyakit ini. Skrining PTM dapat dilakukan dengan melakukan pemeriksaan tekanan darah, glukosa darah, kolesterol darah, dan indeks massa tubuh. Akan tetapi pemanfaatan skrining ini masih rendah. Berdasarkan teori Lawrence Green, faktor predisposisi, pendukung, dan pendorong merupakan faktor yang memengaruhi perilaku kesehatan. Penelitian ini bertujuan untuk mengetahui perbedaan proporsi perilaku skrining PTM berdasarkan faktor predisposisi, pendukung, dan pendorong di kota Denpasar. Penelitian ini merupakan penelitian cross sectional yang dilakukan di Denpasar pada Oktobber – Novermber 2022. Jumlah sampel yang digunakan sebanyak 160 yang diambil dengan metode convenient. Data diambil dengan kuesioner dan dianalisis menggunakan uji chi–square. Dalam penelitian ini, 41,3% responden memiliki perilaku skrining yang baik. Perilaku skrining yang baik ditemukan pada kelompok lansia awal – manula (PR 1,900; 95% IK: 1,335 – 2,705), memiliki asuransi (PR 1,682; 95% IK: 1,075 – 2,637), dukungan keluarga yang baik (PR 3,204; 95% IK: 2,003 – 5,126), dukungan teman yang baik (PR 3,132; 95% IK: 2,077 – 4,723); dan dukungan petugas kesehatan yang baik (PR 2,564; 95% IK: 1,761 – 3,734). Dari penelitian ini, dapat disimpulkan bahwa terdapat perbedaan proporsi perilaku skrining berdasarkan faktor usia, kepemilikan asuransi, dukungan keluarga, dukungan teman, dan dukungan petugas kesehatan. Upaya yang dapat dilakukan untuk meningkatkan perilaku skrining PTM adalah pemberian edukasi, lebih menyebarluaskan informasi mengenai posyandu lansia dan posbindu PTM, bekerja sama dengan penyedia lapangan pekerjaan untuk mengadakan skrining di tempat kerja, dan memberikan dukungan pada keluarga untuk melakukan pemeriksaan PTM. Abstract

ABSTRAK PrEP merupakan pilihan pencegahan penularan HIV/AIDS dengan angka keefektifan yang tinggi sehingga berpotensi dalam mengisi kesenjangan target program pencegahan penularan HIV/AIDS di Indonesia. Di Provinsi Bali persentase cakupan penggunaan PrEP pada LSL hanya berada pada angka 53,25%. Sedangkan, angka prevalensi HIV pada kelompok LSL meningkat 3 kali lipat dalam kurun waktu 10 tahun terakhir. Oleh karena itu, penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan perilaku memulai penggunaan PrEP pada LSL di Kota Denpasar. Penelitian dengan desain cross-sectional ini melibatkan 109 LSL berstatus HIV negatif yang dipilih dengan metode convenience sampling. Data dikumpulkan dengan pengisian kuesioner menggunakan metode self-administered pada kuesioner online (Google Formulir). Analisis data menggunakan uji statistik deskriptif, uji regresi logistik sederhana, dan uji regresi logistik multipel. Hasil penelitian menunjukkan serapan memulai penggunaan PrEP pada kelompok LSL di Kota Denpasar mencapai 37,61%. Faktor penggunaan kondom kadang-kadang (aOR=5,46; 95%CI=1,99-14,94; p=0,001), orientasi homoseksual (aOR=2,88; 95%CI=1,05-7,88; p=0,04) dan dukungan petugas kesehatan yang baik (aOR=9,11; 95%CI=3,04-27,34; p<0,0001) merupakan variabel yang paling dominan berhubungan terhadap perilaku memulaipenggunaan PrEP pada LSL di Kota Denpasar. Petugas kesehatan perlu meningkatkan dukungan informasi yang komprehensif mengenai pentingnya penggunaan PrEP meskipun penggunaan kondom tetap menjadi gold standar dalam mencegah HIV. Peningkatan peran petugas lapangan yang berfokus pada penjangkauan LSL biseksual dan peningkatan dukungan teman sebaya LSL dalam memulai PrEP sangat dibutuhkan dalam optimalisasi pelaksanaan program PrEP di Kota Denpasar.Kata Kunci : HIV AIDS, PrEP, LSL, Kota Denpasar, Yayasan Kerti Praja ABSTRACT PrEP is an option for preventing the transmission of HIV/AIDS which has a high effectiveness rate that has the potential to fill the gap in the target of the HIV/AIDS transmission prevention program in Indonesia. Only 53.25% of MSM in the Province of Bali were covered for PrEP use. Meanwhile, the MSM groups HIV prevalence rate has tripled over the past ten years. Therefore, this study aims to determine the factors associated with PrEP initiation behavior among MSM in Denpasar City. This cross-sectional study involved 109 HIV-negative MSM selected sing convenience sampling method. Data was collected by filling out a questionnaire using the self-administered method on an online questionnaire (Google Forms). Data analysis used descriptive statistical tests, simple logistic regression tests, and multiple logistic regression tests. The results of the study showed that only 37,61% of MSM living in Denpasar City started using PrEP. The variables that were found to be most associated with the behavior of initiating use of PrEP in MSM in Denpasar City included occasional condom use (aOR=5,46; 95%CI=1,99-14,94; p=0,001), homosexual orientation (aOR=2,88; 95%CI=1,05-7,88; p=0,04), and good health worker support (a R=9,11; 95%CI=3,04-27,34; p<0,0001). Health worker need to increase comprehensive information support egarding the importance of using PrEP even though consistent condom use remains the gold standard in preventing HIV. Optimizing the PrEP programs implementation in Denpasar City still requires expanding the role of outreach workers who concentrate on outreach to bisexual MSM as well as strengthening MSM peers support.Keywords: HIV AIDS, PrEP, MSM, Denpasar City, Kerti Praja Foundation

Abstract

Background: Pre-eclampsia is multi-organ damage and occurs after the 20th week of pregnancy. The incidence of pre-eclampsia in Indonesia is very high compared to the global incidence, which is 10% in Indonesia compared to 2% globally. If not well managed, pre- eclampsia can lead to complications. Therefore, prevention is needed, which can be increased knowledge and early detection of pre-eclampsia. Purpose: To increase knowledge and understanding about preeclampsia in pregnant women. Method: The health promotion was conducted on 29 September 2023 by presentations using power point and interactive Q&A with the target all pregnant women in Kenderan village (40 people). Then, Blood pressure checks and prenatal exercises were conducted. Knowledge improvement was evaluated by comparing the pretest and posttest questionnaire responses. Results: Thirteen pregnant women (32.5%), attended this activity. The activity took place at the Kenderan village office (10.00-11.30 WITA). A comparison of pretest and posttest shows an average increase from 5.6 to 7.6 points. However, there are still correct answer that need improvement, specifically regarding the signs and symptoms of preeclampsia (76.9%) and pregnancy nutrition preeclampsia prevention (84.6%). Conclusion: The implementation of health promotion for preeclampsia among pregnant women has been successful. However, the number of participants who attended was lower than the target. Nonetheless, health promotion participants have shown an increase in their knowledge. The timing of the next activity should be better aligned with the availability of pregnant women's schedules. Additionally, the material should focus more on the signs, symptoms, and pregnancy nutrition related to prevention of preeclamsia.

Abstract

The 2017-2019 Bangli Regency Health Office data shows that Tuberculosis (TB) case finding in Bangli Regency was below the national target. An evaluation of TB surveillance found that the participation of Private health centers in reporting and finding TB cases remained low in Bangli. Based on this finding, a workshop on strengthening private health service networks in Bangli was conducted online. The topics delivered in the workshop included TB case finding through the public-private mix, the role of private health centers in TB case finding and the results of the evaluation of the TB surveillance system in Bangli Regency. The workshop was attended by 47 participants from various Private health centers in Bangli. The workshop was evaluated using a questionnaire measuring knowledge of TB and TB surveillance. The result shows that most respondents had good knowledge of TB and TB surveillance systems; however, the low participation rate which was only 50\% of the target, made it important to send a summary of workshop materials designed in the form of booklets. These booklets have been distributed to all Private health centers, especially those who did not participate in the workshop, to increase knowledge and willingness to participate in TB case finding in Bangli Regency.
