Abstract

Background: Diabetes mellitus is a chronic disease with increasing global prevalence, including in Indonesia. Among the elderly, its management is more complex due to age-related physiological changes and comorbidities. This study aimed to identify and analyze risk factors associated with diabetes in the elderly population. Subjects and Method: This retrospective cross-sectional study analyzed medical records of 1,634 inpatients with type 2 diabetes mellitus at PKU Muhammadiyah Hospital Yogyakarta from January 2021 to July 2023. Total sampling was used. Data on demographics, comorbidities, and laboratory values were analyzed using Chi-square test. Results: Of 1,634 patients, 853 (52.52%) were aged >65 years. Significant risk factors associated with elderly diabetes included elevated erythrocytes (OR= 1.60; 95% CI= 1.32–1.96; p <0.001), urea (OR= 1.51; 95% CI= 1.23–1.86; p <0.001), lymphocytes (OR= 1.26; 95% CI = 1.04–1.53; p= 0.020), hemoglobin (OR= 1.38; 95% CI = 1.14–1.68; p <0.001), systolic blood pressure (OR= 1.33; 95% CI= 1.07–1.65; p= 0.009), stroke (OR= 1.59; 95% CI = 1.09–2.32; p= 0.016), creatinine (OR= 1.24; 95% CI= 1.02–1.51; p= 0.028), and hypertension (OR= 1.29; 95% CI = 1.03–1.63; p= 0.028). Conversely, cholesterol (OR= 0.89; 95% CI= 0.65–1.23; p<0.001), and glucose (OR= 0.65; 95% CI= 0.51–0.83; p <0.001) were inversely associated. Conclusion: Elderly diabetes is significantly associated with multiple clinical and laboratory indicators. These findings highlight the importance of comprehensive monitoring to improve elderly diabetes management.

Abstract

Pneumonia disebabkan infeksi akut pada paru, menyebabkan peradangan dan keterbatasan oksigen. Terapi utama pengatasan pneumonia adalah antibiotik. Penggunaan antibibiotik yang kurang tepat perlu dilakukan evaluasi untuk mencegah pengobatan kurang efektif dan timbulnya resisten. Tujuan penelitian untuk mengetahui profil penggunaan antibiotik pada pasien pneumonia di rawat inap RSU Purbowangi periode tahun 2020–2022 dengan cara mengevaluasi penggunaan antibiotik menggunakan metode ATC/DDD dan DU 90%. Metode penelitian deskriptif dengan metode pengambilan data rekam medis pasien pneumonia dari tahun 2020-2022. Hasil penelitian, antibiotik yang digunakan pada pasien pneumonia rawat inap Rumah Sakit Umum Purbowangi periode tahun 2020-2022 meliputi ceftriakson, levofloksasin, dan meropenem. Nilai DDD ceftriakson sebesar 58,5 DDD/100 patient-days pada tahun 2020; 30,3 DDD/100 patient-days pada tahun 2021; 53,1 DDD/100 patient-days pada tahun 2022. Nilai DDD levofloksasin sebesar 20 DDD/100 patient-days pada tahun 2020; 8,4 DDD/100 patient-days pada tahun 2021; 21,2 DDD/100 patient-days pada tahun 2022. Nilai DDD meropenem sebesar 3,3 DDD/100 patient-days pada tahu 2020; 14,9 DDD/100 patient-days pada tahun 2021; 21,4 DDD/100 patient-days pada tahun 2022. Antibiotik yang termasuk ke dalam segmen DU 90% yaitu ceftriakson dan levofloksasin. Sedangkan, yang masuk ke dalam segmen DU 10% adalah meropenem. Kesimpulannya sebagian besar pasien pneumonia di rawat inap Rumah Sakit Umum Purbowangi menerima satu dosis harian ceftriakson setiap harinya yaitu sebanyak 58,5% pada tahun 2020; 30,3% pada tahun 2021; 53,1% pada tahun 2022. Rekomendasi sebaiknya dilakukan penelitian kualitatif dengan menggunakan metode gyssens di rumah sakit dan penyakit yang sama.

Abstract

Myasthenia gravis (MG) is an autoimmune disease involving the neuromuscular junction, mainly influenced by antibodies to acetylcholine receptors. In addition, antibodies to Muscle-Specific Kinase (MUSK), Low-Density Lipoprotein Receptor-Related Protein 4 (LRP4). It is essential to determine publication trends regarding Myasthenia gravis because, currently, there are no studies that describe Myasthenia gravis publication trends related to genes in this field of study. Bibliometric analysis is used to see research trends, including frequently used keywords, most published journals, most cited publishers, author agencies, and collaboration between authors depicted in visualization using the VOSViewer application. Bibliometric analysis shows that the research trend is increasing from 1969 to 2023. The most frequently used keywords are myasthenia gravis, human, autoimmune disease, genetics, and article. The leading information related to the data is that the average annual publication is 5.89%, and the average citation for documents is 13.9%. Indonesia, as one of the countries experiencing Myasthenia gravis, has excellent potential to increase the number of research articles, which can be a means of exchanging knowledge, ideas, and technology and also has the potential for collaboration with other countries
