Abstract

Latar belakang : Swamedikasi atau pengobatan sendiri merupakan bagian dari upaya masyarakat untuk menjaga kesehatannya sendiri. Swamedikasi berarti mengobati segala keluhan pada diri sendiri dengan obat obat bebas dan bebas terbatas yang bisa dibeli bebas di apotik atau toko obat atas inisiatif sendiri tanpa nasihat dokter. Dalam pelaksanaannya, masyarakat harus memiliki pengetahuan yang memadai sehingga bisa berpraktik swamedikasi yang rasional sehingga bisa memperoleh manfaat serta terhindar dari risiko atau kerugian. Tujuan : Penelitian ini bertujuan untuk mengetahui profil swamedikasi di kalangan Mahasiswa S1 Farmasi Institut Ilmu Kesehatan Bhakti Wiyata Kediri Periode September 2020. Metode : Penelitian deskriptif observasional dengan rancangan survey cross sectional. Teknik sampling yang digunakan yaitu purposive sampling dengan sampel sebanyak 87 responden. Hasil : Profil swamedikasi pada penelitian ini adalah mayoritas responden berjenis kelamin perempuan (80,5%) dan berusia 17-19 tahun (50,6%). Alasan terbanyak melakukan swamedikasi adalah karena keluhan ringan (57,5%) dengan keluhan terbanyak demam (14,9%) dan flu (12,6%). Sumber informasi terbanyak adalah dari buku kuliah dan pengalaman belajar (42,5%), tempat memperoleh obat di apotek (87,4%), mayoritas membaca aturan pakai, efek samping obat serta kadaluarsa

Abstract

Background: Geriatric patients are elderly patients with multiple diseases. Geriatric patients have more comorbidities and often receive polypharmacy prescriptions. PIMs have been identified in many elderly patients. One of the explicit criteria that can be used to identify the incidence of PIMs in geriatric patients is the Beers Criteria. Objective: To determine the frequency of PIMs and the factors that can influence the occurrence of PIMs based on the Beers Criteria in hospitalized geriatric patients at RSUD Dr. Soedomo Trenggalek. Method: This research uses a type of research with a retrospective observational analytical approach. The sampling technique used was purposive sampling with a total sample of 312 medical records.  Inclusion criteria: age ≥ 65 years and exclusion: incomplete medical record data. Results: The most common characteristic profiles were male gender (62%), elderly (65-74 years) 69%, type of BPJS service (87%), main disease with the cardiovascular system (31%), number of diseases 2 (43%) and the number of drugs 6-10 drugs (50%). There was an incidence of PIMs of 78.53%. The factors gender, age, type of service, main diagnosis have a p value ≥ 0.05, while the number of diseases and medications have a p value ≤ 0.05. Conclusion: There is an incidence of PIMs in hospitalized geriatric patients of 78.53% with the number of diseases and number of medications influencing the incidence of PIMs in hospitalized geriatric patients.

Abstract

Abstrak Fraktur merupakan kondisi dimana tulang keras atau tulang rawan mengalami kerusakan. Salah satu jenis fraktur adalah close fraktur. Saat seseorang mengalami close fraktur akan mengalami nyeri. Pemberian obat analgesik merupakan salah satu cara untuk mengurangi rasa nyeri pasien close fraktur. Penelitian ini bertujuan untuk mengetahui efektivitas obat analgesik pada pasien rawat inap close fraktur di RSUD Gambiran Kota Kediri. Metode yang digunakan adalah deskriptif observasional dengan desain penelitian secara retrospektif. Pengukuran skala nyeri pada pasien menggunakan skala Visual Analog Scale pada data rekam medis. Data diambil dari 95 pasien. Dari hasil penelitian terdapat 3 janis pemberian obat analgesik pada pasien close fraktur yaitu metamizole dan ketorolac tunggal dengan rata-rata VAS sebelum pemberian obat 5 yaitu nyeri sedang sangat menyusahkan kemudian setelah pemberian obat menjadi 4 yaitu nyeri sedang menyusahkan, dan kombinasi metamizol + ketorolac dengan rata-rata VAS sebelum pemberian obat 6 nyeri berat yang masuk kedalam kategori sedang kemudian setelah diberikan obat menjadi 4 yaitu nyeri sedang menyusahkan. Penelitian ini menunjukkan bahwa terdapat keefektivitasan obat analgesik dalam menurunkan nyeri pada pasien close fraktur di RSUD Gambiran Kota Kediri dilihat dari derajat nyeri yang dirasakan pasien berkurang setelah pemberian obat analgesik dan hasil uji wilcoxon dengan nilai p-value 0,000 < 0,05.

Abstract

Diabetes mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia that occurs due to defects in insulin secretion, insulin action or both. Diabetes mellitus (DM) is currently a global health threat. Various epidemiological studies have shown an increase in the incidence and prevalence of DM in the world. Based on data from the profile of the East Java Health Office in 2021, it is estimated that DM patients in Madiun City rank fourth in the East Java region. . Objective: This research was conducted to determine the evaluation of the rationality of insulin in type 2 DM patients in the Outpatient Hospital of Madiun City Hospital for the period 2022. Methods: This research method is in the form of an observational research, retrospective data collection in the form of medical records of history of type 2 diabetes mellitus patients at Outpatient Hospital at RSUD Madison City. The total population in this study in the period January - December 2022 was 165 patients. Results: From the 62 type 2 DM patients who were outpatients at Madiun City Regional Hospital, it was found that 14.52% using single insulin and 12.90% used Detemir insulin. The use of the insulin combination was 85.48% and the Detemir – Aspart insulin combination was 58.06%. Conclusion: Evaluation of the rationality of insulin use showed that the correct drug was 100% and the correct dose was 93,55%.

Abstract

Kajian ini bertujuan menelaah secara komprehensif peran edukasi pasien oleh apoteker dalam mendukung praktik penggunaan obat rasional. Penelitian dilakukan dengan pendekatan kualitatif melalui studi pustaka terhadap sepuluh artikel ilmiah yang relevan dan terbit antara tahun 2010 hingga 2025. Proses penelusuran literatur mengikuti pedoman Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Pencarian dilakukan melalui basis data internasional, termasuk PubMed, ScienceDirect, dan SpringerLink. Dari total 139 artikel yang teridentifikasi, 25 artikel dihapus karena duplikasi dan 5 dinilai tidak layak secara otomatis, sehingga tersisa 109 artikel untuk proses penyaringan lebih lanjut. Setelah seleksi mendalam berdasarkan kriteria inklusi dan eksklusi, hanya 10 artikel yang memenuhi kriteria dan dijadikan dasar analisis. Data dianalisis secara tematik untuk mengidentifikasi pola kontribusi edukasi apoteker terhadap perilaku penggunaan obat rasional. Hasil analisis menunjukkan bahwa edukasi yang diberikan apoteker berperan signifikan dalam meningkatkan kepatuhan terapi, mengurangi kesalahan penggunaan obat, serta meningkatkan efisiensi biaya kesehatan. Pendekatan edukasi kolaboratif yang berpusat pada pasien terbukti lebih efektif dibandingkan penyampaian informasi satu arah. Temuan ini menguatkan konsep dalam Pharmaceutical Care dan Health Belief Model, terutama mengenai pentingnya komunikasi terapeutik dalam memfasilitasi pengambilan keputusan pasien yang aman, efektif, dan berkelanjutan terkait penggunaan obat.

Abstract

Background: The global prevalence of hypertension continues to rise, with projections indicating that approximately 29% of adults will be affected by 2025. Hypertension is often accompanied by comorbidities and complications. Hypertension treatment usually involves more than one drug or polypharmacy, increasing the risk of potential interactions. Objective: To examine the association between polypharmacy and potential drug-drug interactions in outpatient hypertensive patients. Material and Method: Retrospective observational study using a purposive sampling technique. A total of 96 patients were included from January-December 2023. The instruments used were medical records, the Drugs Interaction Checker application on the Drugs.com application, and Medscape. Results: Most patients were male (59.38%) and aged 56–65 years (37.50%). Polypharmacy was observed in the majority of patients, with major polypharmacy (>4 drugs) accounting for 48.96%. Potential drug interactions were identified in 65.63% of patients. Pharmacodynamic interactions were more common (58.57%) than pharmacokinetic interactions (41.43%), with moderate severity predominating (78.09%). The number of medications and comorbidities were significantly associated with potential drug interactions (p < 0.05). Conclusion: The number of drugs and the number of comorbidities affect the potential for drug–drug interactions in outpatients with hypertension at Dr. Iskak Tulungagung Hospital.

Abstract

Myocardial infarction (MI), a subtype of coronary heart disease (CHD), is still a major cause of morbidity and mortality around the world. Non-ST-segment elevation MI (NSTEMI) is more common than ST-segment elevation MI (STEMI). Antithrombotic therapy, including antiplatelet and anticoagulant agents, is essential in MI management to prevent thrombus formation and reduce ischemic complications. Although international and national clinical guidelines (PERKI and ACC/AHA) provide recommendations tailored to MI type and patient factors, discrepancies in clinical practice, especially in developing countries, may impact treatment outcomes. This retrospective descriptive study aimed to evaluate the use and appropriateness of antithrombotic therapy in NSTEMI and STEMI patients at Hospital X, Kediri City, in 2018. Data were collected from medical records of hospitalized acute myocardial infarction (AMI) patients. Aspirin combined with clopidogrel was the most commonly used regimen for NSTEMI, whereas STEMI patients frequently received aspirin, clopidogrel, and enoxaparin; fibrinolytics were administered in nine STEMI cases. Dosage evaluation showed compliance with guidelines in NSTEMI patients, while 36.84% of STEMI patients, particularly those receiving enoxaparin, were given incorrect dosages. These findings highlight the need for improved adherence to clinical guidelines to optimize antithrombotic therapy outcomes in MI management

Abstract

Metformin, a well-established first-line oral antidiabetic agent, has recently attracted interest in dermatology because of its anti-inflammatory, antioxidant, and regenerative properties. The skin, being vulnerable to aging and environmental damage, is a promising target for topical metformin therapy, although its clinical translation remains limited. This narrative review critically examines current evidence on topical metformin in dermatological applications and explores formulation approaches to optimize its cutaneous delivery. A thematic literature search was conducted in PubMed, Scopus, ScienceDirect, and Google Scholar for articles published between 2015 and 2025, focusing on studies of topical metformin for skin disorders. The final selection included 13 articles on wound healing, 5 on melasma, 1 on acne, 1 on psoriasis, and 1 on alopecia, all meeting predefined inclusion and exclusion criteria. Data from in vitro, in vivo, and clinical research were synthesized into three themes: pharmacological rationale, therapeutic efficacy, and formulation strategies. Promising results have been reported in melasma, acne, psoriasis, alopecia, and wound healing, where topical metformin has been shown to modulate melanogenesis, accelerate wound closure, reduce inflammatory cytokines, enhance hair regrowth, and support scar-free repair. Diverse formulations, including hydrogels, ethosomes, lipid nanoparticles, and niosomes, have been designed to improve skin penetration, drug retention, and therapeutic benefit while limiting systemic exposure

Abstract

Antibiotic that appropriately as used anti-infective has drug a great success in reducing morbidity and mortality rates resulted from various infectious diseases, leading its increasing usage. Antibiotic are generally eliminated through kidney. Patients with kidney disorder, dosage adjustment is recommended for certain antibiotics. This research aims at evaluating the use of antibiotics among patients with kidney disorder in the inpatient wards of PKU Muhammadiyah Hospital Yogyakarta. Data collection was done retrospectively based on medical record data of inpatients meeting both inclusion and exclusion criteria in PKU Muhammadiyah Hospital Yogyakarta. Data processing was done by using a descriptive analytical technique. Result of the study shows that there were 76 patients with kidney disorder using antibiotics and meet inclusion and exclusion criteria. In this study patients wasn’t shown an indications of infection is aproximately 75,00% (57 patients) and patient with indication infection were 25,00% (19 patients). From those who have an appropriate indication, it is just 21,05% (16 patient) have an appropriate medication, 23,68% (18 patient) is right patient and 22,37% (17 patient) have an appropriate doses. Clinical outcome of those 19 patient is good (14 patient) with inpatient duration less than 7 days for 12 patient. Total 76 patient with renal disorder who received antibiotics, 21,05% (16 patients) have inappropriate doses which is mean there is no doses adjusment according to the creatinine clearance of each patient. Abstract

Background: Stroke involves a rapid onset of neurological deficits that last for 24 hours or more or lead to death. While Atorvastatin is highly effective for secondary prevention in ischemic stroke patients, it may cause a diabetogenic side effect that can raise blood glucose levels. Objective: This study aims to assess the immediate changes in Random Blood Glucose (RBG) levels after starting Atorvastatin therapy in hospitalized ischemic stroke patients with a history of diabetes mellitus and hypertension. Methods: This was a retrospective, descriptive cohort study using purposive sampling. The sample included adult inpatients diagnosed with ischemic stroke, known diabetes mellitus, and hypertension who were prescribed Atorvastatin at Bhayangkara Hospital from January to June 2024. Patients with incomplete medical records were excluded. RBG levels were monitored over a short-term period of 3 days. Data were analyzed using descriptive statistics and a paired t-test to compare glucose levels before and after therapy. Results: Of the 28 eligible patients, most were male (16; 57.1%) and aged 55–64 years (15; 53.6%). The most common profile included ischemic stroke, diabetes mellitus, hypertension, and dyslipidemia (8 patients; 28.6%). The paired t-test showed no statistically significant difference between baseline and 3-day post-treatment RBG levels (p = 0.993; p > 0.05). Conclusion: In this small cohort, short-term (3 days) Atorvastatin therapy did not significantly increase RBG levels in ischemic stroke patients with diabetes mellitus. However, due to the small sample size (n=28) and short observation period, these results cannot be generalized, as long-term effects of statins on blood glucose usually take longer to manifest.
