Abstract

Introduction: Improper management of condyle fractures can lead to changes in the structure of the temporomandibular joint, which can impact the patient's oral function. Corrective surgery is necessary to restore the patient's function and quality of life. This case report aims to discuss the procedure of bilateral condylectomy in neglected condylar fracture, as well as the risks and post-operative complication management. Case: A 26-year-old female patient presented with mandibular hypomobility and occasional temporomandibular pain. The patient had history of a traffic accident around four months ago, and was managed in a public hospital but symptoms persisted. Case Treatment: After thorough examinations, the patient was diagnosed with neglected complete bilateral condylar fracture with displacement and managed via bilateral condylectomy. Post-operative, the patient presented with anterior open-bite and facial paralysis on the right side, and was treated accordingly. During 4-month follow-up, complications and chief complaints had all subsided. Discussions: Mismanagement of condylar fracture could result in malunion of fragments, which is difficult to reposition due to the changes that have already occurred in the temporomandibular structure. Condylectomy is often necessary to relieve symptoms, although bilateral condylectomy poses certain neurological risks. Prompt recognition and intervention of nerve injury is important to prevent permanent complication. Conclusion and Suggestions: Condylectomy could properly treat neglected bilateral condylar fracture in adult patients. Operators must know the associated risk and management of post-operative complications.

Abstract

Povidone iodine is an antiseptic that is often used for wounds after tooth extraction, but is often debated because it can cause damage to cells and tissues. African leaf (Vernonia amygdalina Del.) from the family Asteraceae has saponins as anti-microbial, anti-inflammatory and antioxidant flavonoids as that all may be able to accelerate wound healing. To investigate the activity of extracts of Vernonia amygdalina folia (VAF) to accelerate the process of wound healing after tooth extraction in rats. VAF extracted by maceration using methanol. Wistar male rats as much as 6 groups (n = 5 mice each group) performed tooth extractions, and then observed the reduction of gingival socket closure for 21 days after being given povidone iodine, distilled water and methanol extract group VAF by 4 concentration. The closure of the wound after tooth extraction in the group of povidone iodine rats occurred on the 8th day, the group aquadest cover the wound occurred on the 12th day, the extract group 1, 3, 5, 7% sequentially occur wound closure on day 6th, 5th, 4th and 4th. VAF methanol extract of the leaves can accelerate wound healing after tooth extraction of mice

Abstract

Anomali vaskular adalah kelainan kongenital dan abnormal neonatal bersifat benign lesions serta dapat terjadi di kepala-leher pada usia anak-anak dan dewasa dengan regio yang sering terjadi di lidah,bibir,mukosa bukal,dan palatum. Anomali vaskular diklasifikasikan menjadi dua klasifikasi utama,yaitu tumor vaskular dan malformasi vaskular. Penelitian dilakukan dengan studi deskriptif retrospektif dan dilaksanakan di Rumah Sakit Umum Pusat dr. Hasan Sadikin. Kriteria inklusi penelitian yaitu rekam medis pasien terdiagnosa anomali vaskular periode Januari 2019–Desember 2020. Terdapat 38 kasus anomali vaskular pada rongga mulut yang dilakukan perawatan di Kelompok Staff Medis (KSM) Bedah Mulut dan Maksilofasial RSHS. Kelompok mayoritas pasien berusia 13-24 tahun sebanyak 21 orang. Pasien perempuan adalah golongan yang sering berobat di RSHS yaitu sebanyak 27 orang. Tipe anomali vaskular yang umum terjadi adalah vena malformasi berjumlah 18 kasus atau 47% dari seluruh total kasus. Regio yang sering terjadi adalah di labii inferior superior berjumlah 14 kasus atau 37%. Terapi yang sering diberikan adalah injeksi scleroting agent dan eksisi sebanyak 34 kasus. Anomali vaskular rongga mulut adalah kelainan yang sering ditemukan di KSM Bedah Mulut dan Maksilofasial RSHS dengan terapi yang umum diberikan adalah injeksi scleroting agent dengan tujuan untuk minimalisir komplikasi setelah tindakan operasi.

Abstract

Osteotomi Le Fort I merupakan operasi yang dinilai relative aman dengan morbiditas dan mortalitas yang rendah. Meskipun demikian, Tindakan ini diasosiasikan dengan komplikasi yang jarang tetapi berarti. Saraf infraorbital merupakan salah satu saraf yang paling umum terkait dengan osteotomi Le Fort I. Hal ini terjadi karena tertekan atau tertarik secara tidak sengaja oleh operator selama pembedahan subperiosteal sehingga sering kali menimbulkan injuri berupa parestesia temporer dan gangguan neurosensori. Tujuan dilakukan penelitian ini adalah untuk mengetahui tata laksana dari komplikasi infraorbital nerve injury pasca osteotomi Le Fort I, beserta jenis dan penyebab terjadinya komplikasi tersebut. Penelitian dilakukan dengan menggunakan metode rapid review. Data didapatkan melalui pencarian sistematis pada database online PubMed, Scopus, dan Science Direct dengan memasukkan kriteria inklusi dan eksklusi. Proses penyaringan artikel dilakukan dengan pendekatan PRISMA dan kemudian dilakukan penilaian kualitas menggunakan The Joanna Briggs Institute Critical Appraisal Tools. Hasil dari 7 artikel terpilih didapatkan jenis komplikasi yang paling banyak ditemukan adalah paraestesia sebanyak 4 artikel dan diikuti oleh hipoestesia sebanyak 3 artikel. Kesimpulan penatalaksanaan yang dapat dilakukan terhadap komplikasi infraorbital nerve injury pasca osteotomi Le Fort I adalah dengan melakukan pemeriksaan medis rutin, evaluasi sensasi, pemberian obat-obatan farmakologis serta penghilangan faktor penyebab terjadinya komplikasi infraorbital nerve injury pasca osteotomi Le Fort I.

Abstract

Background: Condylar fractures are among the most common mandibular fractures resulting from maxillofacial trauma, primarily caused by traffic accidents, occupational injuries, and sports activities. These fractures can lead to complications such as pain, mandibular deviation, malocclusion, and pathological changes in the temporomandibular joint. This study aims to identify the characteristics of condylar fractures in the Oral Surgery Department of Dr. Hasan Sadikin General Hospital Bandung from 2020 to 2024.Method: A retrospective analysis was conducted using medical records of maxillofacial trauma patients. A total of 111 cases were analyzed based on gender, age, occupation, education level, fracture classification, trauma etiology, and management.Result: Results showed that the majority of patients were male (72%), aged 13–24 years (48,6%), students (51,3%), and had a bachelor’s degree as the educational background (49,6%). The AO classification predominantly involved condylar neck fractures (52,3%), with traffic accidents being the leading cause (51,4%). The most common management method was open reduction and interdental wiring (76,6%). Conclusion: These findings highlight the need for targeted road safety education, strict traffic regulations, and improved preventive measures to reduce the incidence of such injuries. Therefore, an effort must be made to improve the completeness of medical record data in order to support more accurate analysis and the implementation of future research involving more comprehensive data collection.

Abstract

Facial soft tissue trauma is a common case in the emergency department. Handling this case requires special attention because the face is a prominent part and a marker of identity. Thalassemia is a blood disorder that causes the sufferer to experience anemia, where thalassemia patients must undergo blood transfusions for life. This case will describe a 14-year-old girl who came to the Emergency Department of Hasan Sadikin Hospital Bandung with complaints of bleeding on the eyelids due to a motorcycle accident, from the patient's history it is known that the patient is a thalassemia patient who routinely undergoes transfusions every month. As a result of this hemorrhage caused a significant decrease in hemoglobin value in the patient, so that emergency management was carried out, namely suturing the patient's traumatized eyelid and blood transfusion. Emergency management of facial soft tissue trauma in children with thalassemia must be done carefully to control bleeding and restore the function and aesthetics of traumatized facial tissues, besides that we also collaborate with pediatric colleagues for the management of anemia due to thalassemia in this patient.

Abstract

Background: Mandibular condyle fracture is a fracture of the mandible that quite often occurs due to trauma. Complications that often ensue from condyle fractures in children are ankylosis and temporomandibular joint disorders. Mandibular fracture treatment can involve open reduction or closed reduction. Fracture treatment is simple and appropriate technique can prevent further complications. Purpose: The aim of this case report is to present the management of condylar fractures in pediatric patients resulting from trauma using the open reduction technique. Case: An eight-year-old female patient after experiencing a fall presented with complaints of not being able to open her mouth. Clinical and radiographic examination showed a type IIIb condyle fracture. Case management: The patient was then treated with the open reduction internal fixation (ORIF) technique. Two plates were installed in the patient’s left condylar area using a preauricular approach. The patient showed improved results on the 10th day of control. Conclusion: Treatment of mandibular condyle cases with the ORIF technique in children gives quite good results. Regular evaluation is needed.

Abstract

Fibrous ankylosis of the mandibular condyles restricts jaw movement due to fibrous tissue adhesions in the temporomandibular joint (TMJ), leading to impaired oral function. Surgical management, especially condylectomy and coronoidectomy procedures, remains the most effective method of improving jaw function. A 12-year-old girl with a history of facial trauma and previous surgical intervention who developed progressive limitation in mouth opening over last three years. Clinical examination showed complete inability to open the mouth. Cone Beam Computed Tomography (CBCT) confirmed bilateral fibrous ankylosis of the mandible. The patient underwent bilateral preauricular surgical approaches, with excision of fibrous adhesions, bilateral condylectomies, and unilateral coronoidectomy. Mersilene mesh was placed bilaterally. Intensive physiotherapy was initiated after the surgery. At six months follow-up, the patient demonstrated significant improvement in jaw mobility. Fibrous ankylosis of the mandibular condyles can severely limit jaw function, especially in growing children. Surgical management successfully released the ankylosis and restored mandibular movement. Placement of Mersilene mesh helped prevent re-ankylosis, while early and intensive physiotherapy played a crucial role in maintaining postoperative mouth opening. Comprehensive surgical management combined with intensive postoperative rehabilitation is essential for effective treatment in patients with bilateral fibrous ankylosis of the mandibular condyles
