Abstract

Background: Lumbar Spinal Stenosis (LSS) is a pathological condition characterized by narrowing of the spinal canal in the lower back. In Indonesia, comprehensive data on LSS is currently unavailable. This study aimed to determine the profile of LSS patients at Dr. M. Djamil Central Public Hospital. Methods: This descriptive cross-sectional study utilized medical record data from 122 LSS patients at Dr. M. Djamil Central Public Hospital between 2018 and 2022, selected via consecutive sampling. Inclusion criteria included age over 18 years and a confirmed LSS diagnosis. Exclusion criteria included incomplete medical records, lack of MRI results, and follow-up visits without treatment. Univariate analysis was performed using SPSS 25.0 to describe patient characteristics, causative factors, symptoms, location of stenosis, and management type. Results: In this study, 122 samples met the inclusion and exclusion criteria. LSS was mainly found in patients aged 60-69 years (33.6%) and in women (59%). Most LSS patients had obesity (BMI category I, 32%). Many LSS patients were housewives (42.62%) and reported light work activity (63.1%). The most common cause of LSS was degenerative factors (59%). The main symptom experienced by LSS patients was lower back pain (94.3%), frequently accompanied by radicular pain (79.5%). The most common location of stenosis was L4-L5 (43.4%). Most LSS patients in this study were managed both conservatively and operatively (70.5%). Conclusion: These findings contribute to a better understanding of LSS prevalence and distribution among patients. Further multicenter studies are needed to establish a comprehensive national LSS profile.

Abstract

Tuberculous spondylitis accounts for around 2% of all cases of Tuberculosis (TB) and around 15% of extrapulmonary TB cases. It has been reported that a 17 years old boy with a complaint of a bump on the lumbar region and felt low back pain since two years before admission with a history of back trauma. There was a decrease of body weight. There was no paraesthesia nor paralysis. Defecation and micturition were normal. Basic immunization was incomplete. On physical examination found palpable lymph nodes 0,5x0,5x0,5 cm, multiple et regio colli. There was no BCG scar. Impression nutritional status was undernourished. There was fixed palpable mass at back size about 5x4x5 cm, hard, no fluctuations, no rebound tenderness. Lung examination was normal. Tuberculin test showed induration sized 20 mm. Gene Xpert result Micobacterium Tuberculosis (MTB) not detected. On chest X-ray examination found L1-2 corpus destruction. MRI Spine was suggestive of compressive fractures and suggestive of a bilateral psoas abscess. Decompression and lumbar stabilization surgery were performed. The histopathology examination results were consistent to spondylitis TB characteristics. The patient was discharged on 6th hospitalization and given anti-tuberculous drug.Keywords:  bump, extrapulmonary, fracture, spondylitis, tuberculosis

Abstract

Scoliosis correction methods often involve orthopedic procedures such as implant placement to stabilize movement and correct spinal deformity. The choice of surgical method is highly dependent on the location and nature of the fracture. Fractures with significant damage require a different approach compared to cases of minor injuries. Three dimensional finite element model of C1–L6 spine was used to simulate conditions with single cylindrical implant fixation under a vertical downward loading force 50 N and cylindrical screw types made of titanium alloy. The human spine, encompassing the cervical, thoracic, and lumbar regions, exhibits a complex biomechanical response when subjected to physiological loads. Displacement that occurs due to axial force can be the result of pedicle screw movement with vertebrae of the spine. Installation of 4 (four) rows of pedicle screws reduces the release of pedicle screws from the spine. The use of more fixation can reduce the stress distribution on the vertebrae of the spine.

Abstract

Background: Vertebral fracture is one of the traumas that results in the highest rates of disability and death. Vertebral fractures can occur in various regions, but more than 50% of cases occur in the thoracolumbar region T10-L2. This can be caused by several factors, such as age, gender, and BMI. More than 30% of patients have symptoms of chronic back pain accompanied by neurological deficits. This study discussed the characteristics of patients with thoracolumbar fractures at Dr. M. Djamil Padang Central General Hospital. Methods: This research is an observational descriptive study with a cross-sectional design. Research data were obtained from patient medical records, and the total inclusion sample is 139 patients with thoracolumbar fracture at Dr. M. Djamil Padang Central General Hospital in the 2018-2022 period. The data were processed and displayed as frequency distributions. Results: This research showed that the incidence of thoracolumbar fractures is found in 72 patients, with 72 being women. Mainly occurred between 15-64 years of age (73.4%), with 61.2% of cases caused by low energy trauma. As many as 62.6% of the patients received conservative management accompanied by surgery. Conclusion: This study concluded that thoracolumbar fractures often occur in women of reproductive age. The most common etiology is a fall from a height of less than three meters.

Abstract

ABSTRAKLow Back Pain (LBP) adalah gangguan muskuloskletal dimana rasa nyeri dapat menurunkan produktivitas, mengganggu kehidupan sosial dan membuat penderita harus mendapatkan pengobatan. Tujuan dari penelitian ini adalah melihat pengaruh analgesik dalam menurunkan intensitas nyeri yang dirasakan penderita. Penelitian melibatkan 63 orang penderita dilakukan secara observasi dengan teknik wawancara menggunakan kuesioner yang telah tervalidasi. Pengolahan data memggunakan SPSS Versi 22 diketahui sebaran sosiodemografi penderita paling banyak pada rentang usia 34-41 tahun (22,2 %), wanita (65.1%), pendidikan SLTA (76.2%), pekerjaan IRT/Pensiunan (54%), diagnosa penyakit canal stenosis (19%) dan natrium diklofenak (87.3%) merupakan analgesik terbanyak yang digunakan. Hasil penelitian selanjutnya menunjukan natrium diklofenak dan parasetamol efektif dalam menurunkan intensitas nyeri dengan peningkatan respon pengobatan 24 jam secara bermakna masing-masing p 0.0000.05 dan p 0.0240.05 dan penurunan nilai nyeri masing-masing p 0.0050.05 dan p 0.0010.05. Kesimpulan penelitian ini adalah pemberian analgesik dapat menurunan nilai intensitas nyeri yang dirasakan, sehingga didapatkan kualitas hidup yang lebih baik bagi penderita LBP di Poliklinik Orthophedi RSUP. Abstract

Background: Idiopathic scoliosis can cause pulmonary dysfunction due to thoracic pressure that triggers hypoxia, theoretically increasing hemoglobin and hematocrit levels as a compensatory mechanism.  Objective: This study aims to describe the levels of hemoglobin and hematocrit in patients with idiopathic scoliosis at the Orthopedic Clinic of RSUP Dr. M. Djamil Padang. Methods: This research employed a descriptive retrospective method, collecting medical record data of idiopathic scoliosis patients from 2018 to 2022. The data analyzed included age, gender, hemoglobin levels, and hematocrit levels prior to surgery. Out of 17 patients recorded, nine met the inclusion criteria. Results: The results showed that all patients were in the adolescent age category (10–18 years), with the highest age range being 14–16 years. All patients were female. A total of 44% of patients had hemoglobin levels of 13–14 g/dl, while 55% had hematocrit levels of 40–42%. Conclusion: The conclusion of this study is that there was no significant increase in hemoglobin and hematocrit levels before surgery, indicating no clear evidence of hypoxia compensation through elevated hemoglobin and hematocrit levels in patients with idiopathic scoliosis at RSUP Dr. M. Djamil Padang.

Abstract

Background: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis which has long been known and still one of the top 10 causes of death in the world. Tuberculosis spondylitis is one of the most common extrapulmonary tuberculosis. This study aims to describe the characteristics of patients diagnosed with tuberculous spondylitis at RSUP Dr. M. Djamil Padang in 2018 – 2020. Methods: This study is an observational descriptive study with a retrospective approach using medical record data from patients diagnosed with tuberculosis spondylitis at RSUP Dr. M. Djamil Padang 2018 - 2020. The sampling method used total sampling technique. Results: The results of this study show that tuberculous spondylitis is mostly in the age group 17 – 25 years (25%), the ratio between male and female gender is 1:1, patients come from outside Padang (78.8%), occupation most were housewives (27.5%), and most had no family history of tuberculosis (80%). Local pain is the most common clinical manifestation experienced by as many as 70 people. The lesion location is most often in the thoracic region (45%). Conclusion: Tuberculous spondylitis is most common in the age group 17 – 25 years, comes from outside the field, is a housewife, and has no family history of tuberculosis. Men and women have the same number of cases. Local pain is the most common clinical manifestation and the thoracic lesion is the most frequently found location.
