Abstract

Hematological assessment serves as a standard examination in supporting diagnostics in clinical practice. The advanced hematology analyzer instrument namely Mindray BC 760 has recently introduced several new hematological parameters which previously unavailable. This study set out to establish the value of the reference interval of those new parameters on the Mindray BC 760 device in the Indonesian adult population. This observational study took place at Yarsi Hospital, Jakarta from March to August 2023. A total of 352 subjects who underwent medical check-ups at pathology clinics laboratory in the hospital were enrolled. All participants comprised both females and males aged > 17 years and were confirmed to be healthy through examination. Hematological assessment using Mindray BC 760 device. A remarkable significant difference was observed between females and males (p< 0.05) in terms of Hb, RBC, MCH, MCHC, RHE, RET, PLT, PDW, PCT, PLCC, Monocyte and eosinophil count, and neutrophil%. Conversely, MCV, RDW-SD, RET%, IRF, LFR, MFR, HFR, NRBC, NRBC%, WBC, NEU, LYM, BAS, IMG, LYM%, BAS%, IMG%, MPV, IPF showed no significant differences based on gender. It is recommended to adjust of reference interval value according to gender for several novel hematological parameters assessed through the Mindray BC 760 instrument.

Abstract

Stunting atau tinggi badan anak tidak sesuai umur masih merupakan tantangan kesehatan di Indonesia. Keberadaan stunting adalah indikasi asupan gizi yang kurang secara kronis yang berdampak kepada keterlambatan pertumbuhan, kecerdasan, dan kerentanan mengalami penyakit seperti infeksi dan obesitas. Kekurangan gizi kronis berdampak pada jumlah donor metil yang diperlukan untuk mengatur ekspresi gen dengan memodifikasi DNA. Metilasi pada situs DNA yang menyandi IGF2 (insulin growth factor) pada lokus DMR (differential methylated region) berpotensi mengatur tingkat ekspresi gen IGF2. Untuk mengevaluasi adanya perbedaan metilasi DNA gen IGF2, studi retrospektif case control dilakukan dengan merekrut 23 anak stunting dan 23 anak normal di rentang usia 3-5 tahun. Dengan menggunakan MSP (Methylation Specific PCR), metilasi DNA pada gen IGF2 ditemukan lebih tinggi di DNA darah anak stunting dibanding anak normal (median 22% vs 17,83%, p value >0.05) namun tidak bermakna secara statistik. metilasi DNA IGF2 ditemukan lebih tinggi pada anak laki-laki dibanding perempuan yang bermakna secara statistik (median 28,68% vs 14,80%, nilai p 0,04). Perbedaan persentase metilasi DNA pada gen IGF2 perlu dieksplorasi lebih jauh untuk menjelaskan mekanisme terjadinya stunting akibat malnutrisi kronis.

Abstract

Background: The Delta variant of SARS-CoV-2 led to a surge in COVID-19 cases in Indonesia. This study aimed to assess the demographic profile and mortality rates of hospitalized COVID-19 patients in YARSI referral hospital, Jakarta, comparing pre-Delta and Delta wave periods. The study also investigated whether low CT values in RT-PCR tests indicated heightened viral transmission before the Delta wave surge. Method: A retrospective analysis was conducted on 1,457 COVID-19 patients hospitalized at YARSI (January-August 2021) and 25,279 RT-PCR test results from walk-in patients (April-August 17, 2021). Differences were evaluated using Chi-square or Fisher’s Exact Tests while binary logistic regression was used to assess mortality risk factors. Results: There were increased proportions of pregnant women, patients aged 20-29 and those with coronary artery disease during the Delta wave. This period also showed a significant increase in mortality rates, with the highest seen in patients >60 years old or those with multiple comorbidities. Notably, most of the deceased patients (131 of 139) were unvaccinated. Analysis of RT-PCR data showed rising percentages of positive results with low CT values (below 21 or 15) from April to June. Conclusion: The Delta wave saw a higher risk of hospitalization among young individuals and pregnant women, despite their low mortality risk. The unvaccinated and those with multiple comorbidities faced higher mortality risks. Increases in RT-PCR positivity with low CT values preceded the July COVID-19 case surge.
