Abstract

ABSTRACTBackground: Squamous Cell Carcinoma (SCC) is the most common case of oral cancer which often occurs laterally on the tongue and rarely develops on the dorsal surface of the tongue. More than half of the cases are diagnosed late, thereby reducing the survival rate of the patients. Objective: This report was intended as an evaluation for the management of squamous cell carcinoma under multidisciplinary approach between oral surgery and other departments, as well as the provision of further post-operative treatment. Case Report: The author presents a case of 68-years-old female patient with a lump and an ulcer on her tongue. Around six months prior to the visit, patient complained of tongue ulcer followed by the emergence of a lump in a size of a corn seed. The lump was gradually enlarged with constant widening of the ulcer. Pain on the tongue was also perceived. The patient was then referred to Hasan Sadikin Hospital for further treatment. Case Management: Patients underwent hemiglossectomy and Selective Neck Dissection (SND) surgical procedures performed by Oral and Maxillofacial Surgeon in collaboration with Surgical Oncologist. Furthermore, after surgery, the patient was consulted to the Hemato-Oncology Division of Internal Medicine Department for chemotherapy treatment. Conclusions: The exact diagnosis was made based on the histopathological biopsy results of the tongue tissue. Management of tongue cancer must be done multidisciplinary. Some things that must be considered in handling such cases are the eradication of the tumor, the return of oral cavity function, and the aesthetic/functional aspects of the patient. Keywords: Oral cancer, Squamous cell carcinoma, Tongue cancer

Tooth extraction is removing the whole tooth or root from its socket which involves hard tissue and soft tissue in the oral cavity. One important factor that influences the wound healing process after tooth extraction is the patient's compliance with the instructions given by the dentist. This study aimed to analyze the correlation between dentist instructions, patient’s knowledge, and patient’s experience on the level of patient’s compliance after tooth extraction. Methods: The study used an analytical method with a cross sectional approach with a non-probability sampling technique, purposive sampling with a total sample of 33 people. The inclusion criteria were: post-tooth extraction patients at the Oral Surgery Dental Clinic of Gusti Hasan Aman Dental and Oral Hospital, aged 18-60 years, maximum tooth extraction in the last 3 months, patients extracted with the intra alveolar method, and patients without systemic diseases. A questionnaire was used and the data results were analyzed using SPSS with correlative analytical methods. Results: The instructions given by the dentist were very clear by 32 patients (97%); the knowledge of the patients was very good, as many as 31 patients (93.9%); the patients had very good experience, as many as 26 patients (78.9%), and the level of patient compliance with post-extraction instructions was from 33 people (100%). There is a relationship between dentist instructions on the level of patient compliance with a correlation value of 0.053 (strong relationship level) and there is a relationship between patient knowledge and patient compliance with a correlation value of 0.514 (strong relationship level). Conclusion: Dentist instructions and patient’s knowledge are related to the level of patient compliance while experience has no correlation with the level of patient compliance. Complications and impaired wound healing can be minimized by increasing the effectiveness of instructions to patients. KEY WORDS: Dentist's instructions, knowledge, experience, level of compliance, complication.

Abstract

Background: Reversible pulpitis is mild pulp inflammation. The treatment is direct pulpcapping. Tricalcium silicate cement has been better at inducing reparative dentin than other materials. Tricalcium silicate cement can cause inflammation 1-2 weeks after application, that natural ingredients need to improve the quality. Tricalcium silicate cement can be combined with galam leaf extract which contains anti-inflammatory properties by accelerating lymphocyte cells. Purpose: To analyzed the effect of gelam leaf extract (Melaleuca cajuputi) concentration of 100% and Tricalcium Silicate Cement on the number of lymphocyte cells on day 3 and 5 in direct pulp capping treatment. Methods: This research is a true experimental study with a post test-only with control design. This study used 24 Wistar rats, divided into 3 treatment groups with 2 different days, namely the group given a combination of Gelam leaf 100% + tricalcium silicate cement, tricalcium silicate cement as positive control, and without treatment as negative control. Results: One-way ANOVA test obtained p value=0.00<0.05, which indicates there is a significant difference between each group. Post Hoc Bonferroni test showed significant difference between galam leaf extract and tricalcium silicate cement, tricalcium silicate cement, and no treatment on days 3 and 5. Conclusion: The combination of galam leaf extract and tricalcium silicate cement can increase the number of lymphocyte cells on day 3 of pulp inflammation and reduce the number of lymphocyte cells on day 5 more effectively than tricalcium silicate cement and without treatment on days 3 and 5 so that the healing process is faster.
