Abstract

Stunting is a nutritional problem that needs attention because of its risk to future generations. The causes of stunting are related to environmental factors such as sanitation and drinking water quality. Intervention efforts are carried out to improve sanitation and drinking water quality, which is regarded as a risk factor. This article reviewed literature relevant to the topic from Science Direct, Scopus, EBSCO (CINAHL, MEDLINE), and Proquest databases from 2020-2024 publications with the keywords Water Quality and Stunting, then analyzed using a synthesis matrix. The inclusion criteria for this study were articles in English with a correlational design and full text. The results of the journal review found six articles that stated that sanitation factors, especially the quality of drinking water, were the cause of the stunting problem. The quality of drinking water is related to bacterial contamination due to disasters (floods), risk factors for unhealthy behavior, and open toilets. Poor water quality causes infection, which indirectly affects other factors that affect linear growth (weight and height), which are indicators of stunting. Interventions reduce the prevalence of stunting by improving sanitation, especially the quality of proper drinking water, and improving clean living behavior in the community.

Abstract

Stunting is one of global nutrition problems, especially in developing countries including Indonesia. The prevalence of stunting in Demak Regency, Indonesia, in 2021 was 4.34%, and in 2022 it was 2.99%. However, stunting problem remains a priority because the impact of stunting can hinder efforts to enhance the health of the society and the development of the quality of human resources. Risk factors for stunting are influenced by behavioral aspects of the environment, like sanitation and drinking water. This study aimed to determine the content of bacteriological factors that contribute to the risk pattern of drinking water consumption in cases of stunting toddlers in Temuroso Village, Working Area of Guntur I Health Center, Demak Regency, Indonesia. This study was a quantitative observational case study. The population included families and toddlers in Temuroso Village, totaling as many as 854 people. The sampling technique was purposive sampling, so the sample size was 182 people for cases and controls. Data analysis used Chi-Square test and linear regression test. The bacteriological content was predominantly negative (66.5%), with the majority of subjects showing no infections (67%). Most participants fell into the high drinking water consumption category (62.1%). Stunting incidence was significantly associated with bacteriological content (p <0.0001) and drinking water consumption patterns (p <0.0001). Among these, bacteriological content was the most dominant factor influencing stunting (exp B =0.199). In summary, the primary risk factors for stunting among toddlers in Temuroso Village, within the Guntur I Health Center working area, Demak Regency, were bacteriological content (p <0.000) and drinking water consumption patterns (p <0.000).

Abstract

Introduction: The widespread use of pesticides in agriculture has been linked to an increased risk of diabetes mellitus. Long-term exposure to pesticides can disrupt glucose metabolism, trigger insulin resistance, and raise susceptibility to diabetes mellitus. This study investigates the association between pesticide exposure and the incidence of type 2 diabetes mellitus. Methods: This analytical observational study with a cross-sectional design involved 162 female farmers selected by purposive sampling from a population of 2,187, using the Lemeshow formula. Independent variables included work duration, types and frequency of pesticide use, spraying duration and direction, PPE usage, and cholinesterase levels. The dependent variable was type 2 diabetes mellitus. Data were collected through questionnaires, observation, and clinical tests for blood glucose and cholinesterase levels. Logistic regression was performed at 95% CI and α = 0.05. Result and Discussion: The results showed that 69.1% of female farmers exposed to pesticides had diabetes with lower cholinesterase levels as a significant indicator (p<0.001; OR=3.897; 95% CI=2.087-7.277). Factors such as working duration ≥10 years (p=0.010; OR=3.564; 95% CI=1.359-9.346), number of pesticides types (p=0.023; OR=3.370; 95% CI=1.187-9.570), spraying duration (p=0.020; OR=3.083; 95% CI=1.197-7.940), and PPE usage (p=0.007; OR=3.601; 95% CI=1.416-9.159) were associated with type 2 diabetes mellitus. Conclusion: Pesticides, particularly organophosphates and carbamates, act as endocrine-disrupting chemicals that impair pancreatic function and increase oxidative stress, contributing to insulin resistance. Female farmers more vulnerable to these effects due to hormonal sensitivity, making gender specific approaches essential. Strict regulations and farmer education are crucial to reduce long-term health risks.
