Abstract

CORRELATION ANALYSIS OF TOTAL SYMPTOM SCORE, PAIN, AND QUALITY OF LIFE POST HIGH DOSE VITAMIN B1, B6, DAN B12 TREATMENT IN PERIPHERAL NEUROPATHYABSTRACTIntroduction: Peripheral neuropathy (PN) is a clinical condition in which nerves of the peripheral nervous system are damaged and is associated with various symptoms affecting the patients’ quality of life (QoL).Aims: To understand the  effect of Vitamin B1, B6, and B12 combination in mild to moderate PN.Methods: This is a prospective, open label, multicenter, single arm observational study involved 399 subjects with PN of different etiology. Subjects received the vitamin B1, B6, and B12 combination tablet orally once daily and were observed for 3 months. Total symptom score (TSS), visual analog score (VAS) and QoL were assessed, and the correlation between these parameters was analyzed.Results: Clinically significant reductions were observed from baseline to subsequent visits for TSS and VAS. Positive correlation  was observed between TSS and components of VAS. The study treatment was associated with a significant improvement in QoL parameters. Inverse correlation was observed between QoL and TSS as well as QoL and components of VAS. The study treatment was found to be well tolerated.Discussion: The correlation  analysis between different outcome measures demonstrated the beneficial effect of combination of vitamin B1, B6, and B12 in relief from symptoms and improvement in QoL of PN.Keyword: Correlation analysis, peripheral neuropathy, SF-8, TSS, VAS, vitamin B1, B6, and B12ABSTRAKPendahuluan: Neuropati perifer (NP) merupakan kondisi klinis akibat kerusakan pada sistem saraf tepi yang memengaruhi kualitas hidup (quality of life/QoL) pasien.Tujuan: Mengetahui efek pemberian kombinasi vitamin B1, B6, and B12 pada NP ringan hingga sedang.Metode: Penelitian observasional dan prospektif secara open label, multisenter, dan single arm, yang melibatkan 399 subjek penderita neuropati perifer dengan etiologi yang berbeda-beda. Subjek mengonsumsi tablet kombinasi vitamin B1, B6, and B12 secara oral satu kali sehari dan diamati selama 3 bulan. Dilakukan penilaian skor gejala total (total symptom score/TSS), visual analog scale (VAS), dan QoL, serta korelasi antara parameter-parameter tersebut.Hasil: Terdapat penurunan TSS dan VAS yang bermakna secara klinis antara baseline (awal) dengan kunjungan berikutnya. Terdapat korelasi yang positif antara TSS dengan komponen-komponen VAS. Pemberian perlakuan dalam penelitian berkorelasi secara bermakna dengan perbaikan parameter dalam QoL. Teramati juga adanya hubungan terbalik antara QoL dengan TSS serta QoL dan komponen VAS. Pengobatan pada penelitian ini juga terbukti dapat ditoleransi dengan baik.Diskusi: Analisis korelasi antara berbagai macam metode pengukuran yang berbeda menunjukkan manfaat dari pemberian kombinasi vitamin B1, B6, and B12 dalam mengurangi gejala dan perbaikan QoL pada pasien PN.Kata kunci: Analisis korelasi, neuropati perifer, kualitas hidup, SF-8, TSS, VAS, vitamin B1, B6, and B12

Abstract

Background: This study aimed to evaluate the effects of neuromuscular taping decompression technique (NMT) on modifying pain scale in diabetic patients with painful distal symmetrical polyneuropathy (DSPN) at Wahidin Sudirohusodo Hospital, Makassar, Indonesia. Using a pure experimental design with a pre-test and post-test control group method, 30 patients were included: 15 in the treatment group and 15 in the control group. The treatment group received decompression NMT on the dorsal and plantar aspects of the foot twice a week for four weeks, along with vitamin B complex supplementation. The control group received only vitamin B complex. Result: The Mann-Whitney test showed significant improvement (p <0.05) in the pain scale of the treatment group on day 28, with the Numeric Pain Rating Scale (NPRS) score being lower than that of the control group. Conclusion: This study demonstrated the significant pain-reducing effect of NMT in DSPN patients. The combination of vitamin B1, B6, and B12 supplementation with 8 NMT sessions over 28 days reduced pain to 0 on the NPRS scale. Treatment twice a week for 4 weeks, alongside vitamin B, led to near-zero pain, with pain-free results expected after 5 treatments over 17 days. NMT combined with vitamin B reduced pain approximately 2 times faster than vitamin B alone. Abstract

Pain is a health problem phenomenon that most often brings patients to see a doctor. Pain intensity is the most often domain which assessed in clinical and research settings. Loeser suggests that four elements need to be considered when evaluating patients with pain. Based on the biopsychosocial approach and the concept of pain from Loeser, a pain assessment was made and called "Alternative Method of Quantitative Pain Assessment (MAPKN)". This study was conducted on 415 pain patients consisting of 163 nociceptive pain, 119 mixed pain, and 133 neuropathic pain. In the nociceptive pain group, MAPKN showed sensitivity 92.59%, specificity 96.43%, AUC 0,959 and a p value <0.001. In the mixed pain group, MAPKN showed sensitivity 98.17%, specificity 80.00%, AUC 0,836 and a p -value of 0.001. In the neuropathic pain group, MAPKN showed sensitivity 92.06%, specificity of 85.71%, AUC 0,951 and a p<0.001. The AUC value of 0.959 (96%) indicates that the diagnostic value of the MAPKN score in assessing pain intensity in the nociceptive pain group is excellent (>90%-100%). AUC value of 0.836 (83%) indicates that the diagnostic value of the MAPKN score in assessing pain intensity in the mixed pain group is good (> 80%-90%). The AUC value of 0.951 (95%) indicates that the diagnostic value of the MAPKN score in assessing pain intensity in the neuropathic pain group is excellent (>90%-100%). This is in accordance with research conducted by Eka Pranata 2004 that MAPKN has strong discriminant validity in assessing pain intensity. MAPKN is valid and has excellent diagnostic value in assessing nociceptive and neuropathic pain intensity, MAPKN is valid and has good diagnostic value in assessing mixed pain intensity.

Abstract

The prevalence of adolescents who spend >5 hours per day playing games has a higher risk of developing sleep disorders. Gaming addiction can cause sleep deprivation and the inability to concentrate, which can lead to attention disorders and impulsive behavior. This study is an analytical observational study with a cross-sectional design that aims to determine the relationship of sleep quality in adolescent gamers to impaired attentional function at SMP Negeri 30 Makassar which was conducted in January 2023. The total subject in the study was 64 people. Most of the subject had a duration of playing games of 3-6 hours per day, which totaled 35 (54,6%) people. Gamers who have abnormal sleep quality are 43 (67,2%). We found that the longer duration of gaming caused the sleep quality to decrease (p-value=0,032). There was a significant relationship between the Pittsburgh Sleep Quality Index (PSQI) score with the visual attention test score (p-value=0,001) and the digit span word test score (p-value=0,006). This study concludes that poor sleep quality is associated with impaired attentional function in the accuracy and reaction domain and impaired attentional function in the working memory domain.
