Abstract

Diabetes mellitus (DM) is a chronic condition that occurs when there is an increase in blood glucose levels because the body cannot produce insulin or use insulin effectively. This study aims to evaluate more in-depth information about compliance with Type-2 DM undergoing diet therapy from the perspective of the Theory of Planned Behavior. The informants in this study consisted of 17 informants consisting of 1 Doctor, 1 Nurse, and 15 Type 2 DM patients. This type of research was qualitative using in-depth interviews related to dietary compliance with Type 2 DM patients, then analyzed by triangulation. The results showed that diet-adherent behavior was significantly influenced by attitudes, subjective norms, individual behavior control, and intentions. The amount of intention and obedient attitude and behavior possessed by Type 2 DM patients will make it easier for Type 2 DM patients to carry out diet therapy. The level of adherence to diet therapy in this study with a high category of 10 patients with a percentage of 66.7%, patients with a level of adherence to diet therapy with a moderate category of 4 patients with a percentage of 26.7%, and patients with a level of adherence to diet therapy with a low category as many as 1 patient with a percentage of 6.7%. The amount of intention and obedient attitude and behavior possessed by Type 2 DM patients will make it easier for Type 2 DM patients to carry out diet therapy.

Abstract

This study aims to determine the factors that influence BPJS users of the Tiered Referral System in the Work Area of Pokenjior Health Center, Padangsidimpuan City. This research is an analytical research type with explanatory research type. The populations in this study were all BPJS users or patients undergoing health services at the Pokenjior Public Health Center in Padang Sidempuan City with a sample of 98 patients participating in the Health BPJS. Data were collected using a questionnaire and analyzed using the Pearson Correlation test and multiple logistic regression tests. The results showed that there was an influence of BPJS user knowledge (p = 0.007), officer attitudes (p = 0.003), availability of infrastructure and human resources (p = 0.002), and referral information (p = 0.003) to the tiered referral system at Pokenjior Public Health Center. The results of multivariate analysis and modeling show that BPJS user knowledge, availability of infrastructure, and human resources and referral information are the dominant factors influencing the tiered referral system. Thus it was concluded that the knowledge factor of BPJS users, the availability of infrastructure and human resources as well as the referral information had a significant effect on the tiered referral system in Pokenjior Public Health Center, and the availability of infrastructure and human resources was the biggest or most dominant factor affecting the tiered referral system in the work area Pokenjior Public Health Center in Padangsidimpuan City

Abstract

Cancer is one of the leading noncommunicable diseases leading to death worldwide. Cancer treatment has progressed but has side effects including nephrotoxic effects. The toxicity of doksorubisin has been widely known which is likely to be chained by the metabolic conversion of doxorubicin to doxorubicinol. The main mechanism of doxorubicinol toxicity occurs due to its interaction with iron and the formation of ROS that damages cell macromolecules. One of the plants that are widely used by the community to deal with various diseases and prevention is bay leaves. Bay leaves contain secondary metabolites that are antioxidant in nature. Antioxidants are needed to protect the body's cells from oxidative damage, preventing various degenerative diseases such as cancer, cardiovascular disease. The study aimed to determine the content of nephroprotective EEDS chemical compounds in phytochemical screening, EEDS characterization and EEDS nephroprotective activity in doxorubicin-induced mice based on uric acid, ureum and creatine levels. The study was experimentally laboratory using mice as test animals. The treatment group consisted of CMC-Na 0.5%, Nature E, EEDS doses of 100 mg/kgBB, 300 mg/kgBB and 500 mg/kgBB followed by administration of doxorubicin. The results showed phytochemicalally EEDS contains chemical compounds that are nephroprotective namely flavonoids, steroids / triterpenoids, saponins . EEDS has a water soluble sari content value of 21.64%, total ash content of 1.79%, water content of 9.98%. Ethanol soluble juice levels were 45.38% and acid insoluble ash levels were 0.39%. EEDS doses of 100 mg/kgBB, 300 mg/kgBB and 500 mg/kgBB can lower uric acid, ureum and cretinine levels and can picture the histopathology of doxorubicin-induced mouse kidneys with an effective dose of 500 mg/kgBB. It can be concluded that ethanol extract of bay leaves has nephroprotective activity.

Abstract

Indonesia is the largest archipelago in the world (17,504 islands). In addition, Indonesia has abundant natural resources and fertile soil that allows a wide variety of plants to live. Curcuma domestica val, known as turmeric, is one of Indonesia's medicinal plants. A number of active ingredients found in turmeric have been shown in studies to have potential health benefits, including anti-diabetic anti-inflammatory, anti-cancer, cough medicine, liver medicine, and Alzheimer's disease. Based on the results of phytochemical research, flavonoids, alkaloids, tannins, triterpenoids, steroids, saponins, and glycosides are present in the ethyl ethanol extract of turmeric leaves (Curcuma domestica Val). Diabetes mellitus is a metabolic condition characterised by increased blood glucose levels caused by disruption of the insulin hormone which functions to lower blood glucose levels to maintain body homeostasis. Because of the close relationship between diabetes mellitus and lifestyle, diet, sleep, work, daily routine activities and others must be balanced. This study was conducted in the laboratory and was experimental in nature using male white rats as research subjects. Rats were induced using Alloxan at a dose of 100 mg/kgBB. Rats were divided into 6 groups, each consisting of 5 rats. Group 1 is a normal group, Group 2 is a negative group only given aquadest and induced with Alloxan.

Abstract

Highlights:1. Given the suboptimal implementation of clinical practice guidelines at Mandau District General Hospital, it is important to conduct research on potential obstacles to this problem.2. The findings of this study can encourage hospitals to develop and apply effective clinical practice guidelines while also addressing the obstacles to their implementation.   Abstract Clinical practice guidelines are procedures followed by physicians to optimize healthcare services and achieve more effective outcomes. Conducted in October 2022, this study aimed to analyze the factors that hinder the implementation of clinical practice guidelines among physicians at Mandau District General Hospital, Bengkalis, Indonesia. This study used an analytical survey design and a cross-sectional approach. This study included all 85 individuals who were employed as general practitioners at the Mandau District General Hospital. The data were subjected to univariate, bivariate, and multivariate analyses. The independent t-test was used in the data analysis, with a value of p<0.05 considered significant. The results showed that there was an association between the implementation of clinical practice guidelines and several variables, including the levels of ability in recognizing and explaining clinical pictures of diseases (p=0.008), diagnosing diseases (p=0.004), carrying out initial management (p=0.000), and carrying out independent and directed management (p=0.001). Upon conducting the follow-up analysis on logistic regression, it was found that the variables that were associated with the implementation of clinical practice guidelines were the ability to carry out initial management (p=0.000, OR=65.512, 95% CI=22.048–98.835). In conclusion, physicians' insufficient ability to perform initial management hinders the implementation of clinical practice guidelines. This study suggests incorporating the findings as input in the evaluation and development of training for physicians on the implementation of clinical practice guidelines.

Abstract

The problem of stunting is a disturbance in the growth of a child's height or body length, where the height growth does not match with increasing age, which is also an abnormal condition of the body caused by more than one factor (multifactor). So this re-search discusses many things, including analyzing the influence of clean water access, latrine access, hand washing behavior with soap (CTPS), history of exclusive breastfeed-ing and MP-ASI on the growth and development of toddlers aged 24-59 months. This research used a cross sectional study design. The research was conducted in March-April 2020. Respondents were 118 children under five (aged 24-59 months) who lived in Og-an Ilir. The selection of respondents was taken using a simple random sampling tech-nique. Stunting, a condition characterized by impaired growth and development in chil-dren due to poor nutrition, recurrent infections, and inadequate psychosocial stimulation, is a significant public health problem. Statistical analysis was carried out to identify cor-relations and potential causal relationships between these variables and the incidence of stunting. The results show that low socio-economic status and maternal education are significantly related to high stunting rates. Inadequate food intake and poor sanitation practices are also important factors. Frequent infections, especially gastrointestinal dis-eases, are known to worsen the risk of stunting in toddlers.The study concludes that a multifaceted approach is needed to address stunting, involving improved economic con-ditions, educational initiatives for mothers, improved nutrition programs, better access to health services, and improved sanitation facilities. These findings highlight the im-portance of integrated public health strategies to reduce stunting and improve the wel-fare of children in South Nias District. Further research is recommended to explore long-term interventions and their effectiveness in various settings.

Abstract

Tingkat kejadian kematian yang terjadi karena kesalahan medis telah meningkat secara signifikan selama 10 tahun terakhir. Salah satu kesalahan medis yang sering terabaikan adalah pencatatan rekan medis khususnya pada asesmen awal pasien. Pencatatan Rekam Medis yang lengkap yang baik adalah komponen penting dalam pengelolaan informasi kesehatan di Rumah Sakit. Mengidentifikasi apakah faktor kelengkapan pencatatan Rekam Medisdapat mempengaruhi pencapaian International Patient Safety Goals. Penelitian ini menggunakan penelitian survei yang bersifat analitik deskriptif dengan menggunakan desain cross sectional study. Sampel diambil dari Rekam Medis pasien rawat inap di Rumah Sakit Royal Prima Medan periode Januari 2024 yang berjumlah 218 pasien. Pada penelitian yang dilakukan di RS Royal Prima Medan pada periode Januari 2024, didapatkan hasil kelengkapan pencatatan Rekam Medis mencapai 82% dan standarisasi JCI 7th edition yang menggunakan IPSG di RS Royal Prima Medan pada periode Januari 2024 mencapai 84,17%. Dari penelitian ini dapat disimpulkan sementara bahwa semakin tinggi nilai pencatatan Rekam Medis, maka nilai IPSG juga akan meningkat pada faktor identifikasi pasien, komunikasi yang efektif, dan keamanan penggunaan obat.Kendati demikian, harus ada perubahan dalam penyusunan tujuan utama akreditasi JCI, karena tidak dapat diletakkan pada seluruh lapisan Rumah Sakit.

Abstract

Queues arise due to the need for services exceeding the capacity of services or service facilities so that users of facilities cannot immediately get services due to busy services. Very long queues and taking too long to get a turn for service are very dissatisfying for patients with the Self-Registration Kiosk (ADM) method at Batugana Health Center aims to maximize the outpatient registration process in order to create easy, fast and comfortable services for patients. However, in its implementation there are still many elderly people who do not understand its use and sometimes there are problems with the system, thus slowing down the service process at Batugana Health Center. The purpose of this study was to evaluate the effectiveness of the use of the Self-Registration Kiosk Method (ADM) at Batugana Health Center. The method in this study is qualitative analytic. The sample in this study consisted of 4 informants including: Head of the Health Center, Head of the Self-Registration Kiosk Division, Self-Registration Kiosk Officers and Outpatients. Data were obtained through in-depth interviews with informants, then the data was analyzed by data reduction through Data Triangulation and sources, then displayed in narrative form. The results of the study showed that there was an influence of human resources, SOPs and facilities and infrastructure on the use of the Self-Registration Kiosk (ADM) method at Batugana Health Center on the evaluation of the effectiveness of the use of the Self-Registration Kiosk (ADM) Method at Batugana Health Center., has not fully implemented the use of APM effectively because the expected results have not been in accordance, namely creating fast service. At Batugana Health Center, the efforts that will be made are to re-evaluate the performance of officers on APM, determine officers who really understand the APM machine. And what will be done is to create an SOP on the use of the APM machine, check the function of the APM machine, add internet quality and add electric generators.

Abstract

Electronic cigarette (e-cigarette) use has rapidly increased among adolescents, raising concerns about its impact on oral health. This study investigated the association between e-cigarette and gingival pigmentation among adolescents in a community setting. A cross-sectional study was conducted in November 2024 among members of the Vapers City of Medan community. Data were collected through a Google Form questionnaire and photographic documentation of participants' gingiva. The questionnaire assessed e-cigarette use patterns, including frequency and duration, and any concurrent use of conventional cigarettes. Gingival pigmentation was assessed through photographs, noting the presence and extent of pigmentation. Data were analyzed using SPSS, employing Chi-Square, correlation, and multivariable tests to determine the association between e-cigarettes and gingival pigmentation. E-cigarette use was significantly associated with gingival pigmentation (p=0.026). Adolescents who used e-cigarettes had a higher prevalence of gingival pigmentation compared to those who did not. Correlation analysis revealed a moderate positive correlation between e-cigarette use and gingival pigmentation severity (r=0.54, p=0.01). Duration and frequency of e-cigarette use were also weakly positively correlated with gingival pigmentation severity. Multivariable analysis confirmed that e-cigarette use (OR=4.5, 95% CI=1.2-16.8, p=0.024), duration of use (OR=2.1, 95% CI=1.1-4.0, p=0.028), and frequency of use (OR=1.8, 95% CI=1.0-3.2, p=0.046) were independently associated with gingival pigmentation. In conclusion, e-cigarette use is significantly associated with gingival pigmentation in adolescents. This finding underscores the potential adverse effects of e-cigarettes on oral health, highlighting a growing public health concern.

Abstract

This study examines the relationship between Hospital Healthcare Service Quality (HCSQ) and patient loyalty at Tgk. Chik Ditiro Sigli Regional General Hospital, with a focus on the mediating role of patient satisfaction and the influence of leadership styles. Employing a qualitative approach, data were collected through in-depth interviews, focus group discussions, and document analysis to capture the perspectives of patients, healthcare professionals, and hospital administrators. The findings reveal that key dimensions of HCSQ—such as responsiveness, empathy, communication, and facility environment—significantly impact patient satisfaction, which in turn mediates patient loyalty. Leadership styles emphasizing collaboration, communication, and patient-centeredness were found to enhance service quality and foster stronger patient-hospital relationships. Conversely, authoritarian leadership styles or those lacking effective communication negatively affected both employee performance and patient perceptions. The study highlights the importance of empowering employees, fostering teamwork, investing in infrastructure, and prioritizing patient-centered care to improve service quality and loyalty. These findings provide actionable insights for hospital management to enhance organizational performance and build long-term trust with patients. Future research is recommended to explore these relationships quantitatively across diverse healthcare settings and examine the role of cultural factors in shaping patient expectations and loyalty behaviors.
