Abstract

Malnutrition is a nutritional problem determined based on anthropometric indicators of body weight according to height or body length (BW / TB) with a z-score of <-3 SD and the presence or absence of edema. There are many problems with malnutrition in poor and developing countries such as Indonesia. Based on the 2015 Indonesian Health Profile, as many as 26,518 toddlers suffer from malnutrition, with a prevalence of 3.8% malnutrition in Indonesia. The current management of nutrition problems cannot only be handled by the government but requires the involvement and support of other stakeholders, such as elements of higher education. This activity aims to increase the knowledge of doctors and officers who work in Puskesmas throughout Tanah Datar Regency in overcoming malnutrition. The method used is training (capacity building). The training was given to Puskesmas stakeholders on efforts to improve children's nutritional status through mothers under five classes, measuring the nutritional status of children attending posyandu, providing nutrition education for mothers under five through mothers under five classes, and monitoring nutritional and nutritional intake of children under five. The main targets in implementing this activity are doctors, nutrition officers, and MCH officers in all puskesmas in Tanah Datar District. This activity is expected to increase health workers' knowledge in overcoming stunting in the Tanah Datar district. There was an increase in health workers' knowledge with an average pre-test score of 8.9, increasing to 9.3 in the post-test. With this activity, it is hoped that it can reduce malnutrition and mortality in children starting from the Tanah Datar district's scope.

Abstract

This study employed a longitudinal case study design aimed at describing the clinical course, respiratory recovery, nutritional status, developmental progress, and social dynamics of a child with aspiration pneumonia accompanied by cardiac arrest. The research subject was a one-year-old girl who experienced baked potato aspiration while living in a correctional facility with her mother. The patient received intensive treatment at Andalas University Hospital and was subsequently monitored for three consecutive semesters after discharge. Data collection was conducted through multiple sources to obtain comprehensive information. Clinical data during the acute phase were obtained through a review of the patient’s medical records, including physical examination findings, laboratory and radiological results, and records of medical interventions provided during hospitalization. Follow-up data were collected through direct clinical observation during routine control visits, focusing on respiratory recovery, nutritional status, and developmental progress. Child development was assessed using standardized developmental screening tools, namely the Denver Developmental Screening Test II (Denver II) and the Capute Scales (CAT-CLAMS). Additional contextual data were obtained through in-depth interviews with the child’s parents, correctional facility health workers, and healthcare providers at the referral hospital to understand the caregiving environment and health management after discharge. Home visits were also conducted after the mother returned from the correctional facility to evaluate the child’s living conditions, caregiving practices, and environmental factors that might influence recovery and development. Data analysis was carried out using a descriptive and thematic approach. Clinical and developmental data were analyzed longitudinally to identify patterns of recovery from the acute phase to the follow-up period across three semesters. The analysis also explored the interaction between medical factors (such as treatment response and clinical outcomes) and social factors (including caregiving environment and family support) in influencing the child’s recovery and development. All research procedures adhered to pediatric ethical principles, including maintaining the confidentiality of the patient’s identity and obtaining informed consent from the family for documentation and analysis of the case. This approach allows for a comprehensive understanding of the child’s recovery process, integrating both clinical and social perspectives.

Abstract

Background: Adolescent rifampicin-resistant tuberculosis is usually reported through microbiological and treatment-outcome endpoints, while nutritional and psychosocial recovery is described mainly in narrative terms. Objective: To describe the 18-month course of five-site disseminated rifampicin-resistant tuberculosis with marasmic severe acute malnutrition and assess agreement among nutritional and psychosocial recovery instruments. Methods: This CARE-guided longitudinal case report followed a 14-year-old Indonesian girl using serial clinical, laboratory, anthropometric, imaging, and psychosocial assessments during an all-oral bedaquiline-based regimen with nutritional support. Results: Tuberculosis involved the lung, ovary, peritoneum, sacrum, and temporal bone. Weight increased from 31 to 45 kg, body-mass-index-for-age z score from −3.27 to −0.68, haemoglobin from 10.1 to 13.5 g/dL after a nadir of 8.7 g/dL, and quality of life from 61.25 to 85.75. Nutritional indices were irreconcilable at the same visit, and two behavioural screens remained normal despite a psychiatric diagnosis and impaired quality of life. Conclusion: Microbiological, nutritional, and psychosocial recovery did not proceed synchronously, and instruments within the same domain could disagree. The absent mid-point height prevented temporal ordering of recovery; a ten-item minimum longitudinal data set is proposed.
