Abstract

Amputation is the removal of a member of the body which involves cutting a part or all of the limb. Problems arising from amputation include surgical wound pain, decreased joint motion, decreased muscle strength, and decreased functional ability. The purpose of this study was to determine physiotherapy interventions in cases of sinistra transtibial amputation due to chronic limb ischemia. This research is a case study conducted at the Gatot Subroto Army Hospital in February-March 2018. Data obtained from interviews, observation, assessment, and management of Physiotherapy. In this case study the limitations of the problem to be discussed are pain, stiffness, spasm, limited scope of joint motion and decreased muscle strength. The modalities that will be applied are Trans Electrical Nerve Stimulation (TENS) and exercise therapy in the form of active motion exercises, stretching, strengthening, and core stability exercises. The results of this case study show that the TENS modality can reduce silent pain, tenderness and motion pain in amputees. Active motion exercises, stretching, strengthening, and core stability exercises can reduce stiffness, spasm, and increase the scope of joint motion and increase muscle strength so as to be able to prepare patients to walk using aids.

Abstract

Bronchiectasis is a chronic disorder characterized by irreversible bronchial dilatation accompanied by an inflammatory process in the walls of the bronchi and the surrounding lung parenchyma. Bronchiectasis can be caused by infection with the bacterium Mycobacterium Tuberculosis which is the bacteria that causes pulmonary tuberculosis (pulmonary TB). The main characteristic of bronchiectasis is the accumulation of mucus, chronic bacterial infections, and persistent bronchial inflammation. The purpose of this case study was to determine the effectiveness of the Active Cycle Of Breathing Technique (ACBT) on increasing functional capacity in bronchiectasis patients with pulmonary tuberculosis. The research method used is in the form of a case report with 1 patient given physiotherapy intervention and evaluation 4 times. To overcome some of the problems that arise, namely shortness of breath, and sputum retention due to bronchiectasis after pulmonary tuberculosis in this study, physiotherapy interventions were used, namely exercise therapy in the form of ACBT. The results obtained after 4 times of therapy were a decrease in tightness as evidenced by the Borg scale from 4 to 1, and a reduction in sputum retention with the auscultation results in the form of rhonchi in the posterior apical segment of the bilateral upper lobe to rhonchi in the posterior apical segment of the right upper lobe. Based on these results, it can be concluded that physiotherapy interventions with ACBT effectives in patients with post pulmonary tuberculosis bronchiectasis.

Abstract

Stroke is a clinically defined syndrome of acute, focal nerve discharge caused by vascular injury (infarction, hemorrhage) of the central nervous system. Punches can cause damage to the brain and affect a person's ability to walk, balance and coordinate movements. Impaired gait is common in stroke cases, often resulting in poor recovery of mobility and inability to carry out daily activities without assistance. One of the interventions that can be used is a soft exosuit. This method can help overcome walking problems in stroke patients where soft exosuits are used to increase walking speed and walking endurance. The purpose of this study was to determine the effectiveness of giving a soft exosuit to stroke patients to improve gait speed and gait endurance This research is a literature review study in narrative form, in which 6 articles were found that were reviewed after searching and searching four databases. The results of the review found that 40 patients with a 6MWT score of 0.5 m/s aged >55 years were given intervention in the form of exo gait training with a frequency of 5 times/week. After 8 weeks of intervention, evaluation data analysis found an increase in walking speed in stroke patients. Based on the results of this review, it can be interpreted that the soft exosuit provides a physiological effect that is in accordance with the stroke conditions of patients with walking disorders so that it can be used as an additional intervention option to improve speed and gait endurance.

Abstract

Background: Children with cerebral palsy (CP) often require various forms of support to fulfill their basic rights and achieve a good quality of life. This research aimed to identify strategies to enhance the resilience and sustainability of children with CP. Methods: This research used the analytic hierarchy process (AHP) developed by T. Saaty. We identified key elements, such as social, economic, and environmental factors, that enhance resilience in children with CP. This study used expert respondents, including the government, academics, the community, the private sector, society, and people with CP. Results: The research results indicated that the main strategies were strengthening central and regional regulations, updating integrated data, enhancing the ecosystem of inclusive disability services, increasing the quantity and quality of medical and educational personnel, massive social movements related to disabilities, and building inclusive areas. The government plays a key role in determining policies that would have an overall impact. The potential for the sustainability of children with CP could be realized by paying attention to the key factors crucial for the sustainability of children with CP. Conclusion: According to this study, local governments should be better equipped to create policies based on reliable disability data. Teachers and medical staff should also get continual training on how to help children with CP. To ensure the strategy's durability, stakeholder collaboration using the Penta Helix approach must be institutionalized. This study suggested that we should increase the public advocacy through social movements and the media to end stigma and promote inclusive public space design.

Abstract

Background: Children with spastic diplegia cerebral palsy often showed impaired trunk control and upper limb function, which limited their daily activities and independence. Neurodevelopmental treatment (NDT) was used as a physiotherapy approach to improve motor control and reduce abnormal muscle tone. However, the evidence on its effectiveness for upper limb function and trunk control remained limited. This study aimed to examine the effect of non-invasive treatment based on NDT on trunk control and upper extremity functional ability in children with spastic diplegia. Methods: This pre-experimental one-group pre-test–post-test study involved 20 children aged 5–12 years with spastic diplegia cerebral palsy (gross motor gunction classification system (GMFCS) Level III) who received NDT-based physiotherapy twice a week for eight weeks at Yayasan Ramah Cerebral Palsy (RCP), Bogor. The study excluded children with spastic hemiplegia, quadriplegia, athetoid CP, comorbid conditions such as heart disease, non-active members, and those who did not undergo therapy at RCP. Researchers measured upper extremity function using the quality of upper extremity skills test (QUEST) and assessed trunk control using the trunk control measurement scale (TCMS). They analyzed the data using paired sample t-tests to compare pre- and post-intervention results and applied Pearson’s correlation to examine the relationship between trunk control and upper limb function. Results: Post-intervention evaluation revealed a significant improvement in upper extremity function (QUEST mean increase: 22.45±3.05, p< 0.001) and trunk control (TCMS mean increase: 16.15±5.01, p< 0.001). The relationship between improved trunk control and enhanced upper extremity function was significant, as indicated by Pearson's correlation of 0.696, p < 0.001. Conclusion: In children diagnosed with spastic diplegia CP, an 8-week NDT intervention significantly improved upper extremity functional capacity and trunk control. The findings emphasized that the application of NDT in enhancing motor coordination and independence, thereby contributing to an improved quality of life for this population.
