Abstract

 Abstract.Background : The maternal mortality rate in Indonesia is dominated by preeclampsia as the second highest cause after bleeding. Several predictors can be categorized as risk factors that can be used to increase alertness in the care of pregnant women with severe preeclampsia to avoid complications. The purpose of this study was to study the relationship between risk factors for severe preeclampsia and its complications. Methods: This study used an observational analytic method with a case control design. The sampling technique is simple random sampling. The number of samples in this study were 80 samples, with 40 samples in each group. The case group was severe preeclampsia with one complication and the control group was severe preeclampsia without complications. Statistical analysis used was chi square and multiple logistic regression analysis. Results: The results showed that there was a significant relationship between age variables (p value = 0.001 < 0.05; OR = 5.318; CI = 2.118 – 13,356), BMI (p value = 0.002 < 0.05; OR = 6.000; CI = 1.958 – 18.384 ) and gestational age (p value = 0.039 < 0.05; OR = 2.636; CI = 1.040 – 6.685). Multiple logistic regression analysis showed that BMI (p = 0.003), age (p = 0.001) and gestational age (p = 0.048) were variables included in the final modeling of the regression analysis with the incidence of severe preeclampsia complications. Conclusion: BMI is the variable that has the strongest relationship with the incidence of severe preeclampsia complications.

Abstract

AbstractBackground: The MCH (Maternal Child Health) Handbook is a mandatory handbook for mother and children that has function as information especially for pregnant women until childbirth and also newborn to toddlers. One of the utilization of MCH Handbook is by an active reading role is considered to be able to  achieve the goal so that mothers can carry out health care independently so that pregnant women can detect complications as early as possible. Method : The research method is analytic Observational with cross-sectional design. The total sample is 114 pregnant women, using purposive sampling technique. The independent variable is the ownership of MCH Handbook which is categorized in 3 groups of reading activities and the dependent variable is the level of knowledge. Data analysis using Kruskal Wallis test. Result : The result showed that pregnant women who read entire contents of the MCH Handbook had a good level of knowledge is 47 respondents and none had sufficient or insufficient knowledge, while pregnant women who read some of the contents of the MCH Handbook had a good level of knowledge is 55 people and 2 others had enough knowledge.For the group of pregnant women who did not read the contents of MCH Handbook, there were 10 people and all of them had a good level of knowledge. The result of Kruskal Wallis test is p values = 0,365 ( p>0,05). Conclusion : There is no different of the knowledge level on 3 reading group.

Abstract

Introduction: Diabetes Mellitus is a disease caused by an increase in blood glucose levels, which is an increase in levels at an abnormal level, or called hyperglycemia. Diabetes is a metabolic disease related to various factors such as physical activity, lifestyle, habits, diet, etc. Diabetes mellitus is classified as type 1 diabetes mellitus, type 2 diabetes mellitus, and gestational diabetes. Type 2 Diabetes Mellitus (T2DM) is caused by the body's inability to use insulin effectively (insulin resistance). This study aims to determine the relationship between physical activity and fasting blood glucose levels of T2DM patients to provide social change by increasing physical activity to control fasting blood glucose in patients with T2DM. Methods: This study is quantitative research of analytic observation with a cross-sectional design. The sample in this study was the outpatient T2DM at Endocrine Unit RSUD Dr Soetomo Surabaya from June until September 2018 consecutive sampling. The independent variable was physical activity, and the dependent variable was fasting blood glucose (FBG). Methods of collecting primary data by interview using IPAQ (International Physical Activity Questionnaire) and secondary data by looking at the results of FBG from the patient's medical records of laboratory tests. Collected data were entered into Microsoft excel and then statistically analyzed using IBM SPSS 22. Spearman’s Rho Correlation Test analyzed the data. Results: Sixty three respondents became in inclusion criteria. Most patients have a moderate physical activity of 28 people, or around 44.4%. Thirty-two people had poor fasting blood glucose levels of about 50.8%. The analysis result was there is a significant correlation between physical activity and fasting blood glucose levels of T2DM patients and a low correlation coefficient in the opposite direction (not in the same order). Conclusion: Physical activity impacts fasting blood glucose levels and has a negatively correlated direction. The increase in physical activity showed by the value of the IPAQ score, followed by a decrease in fasting blood glucose level.

Abstract

Introduction: Allergy is one of the most common diseases for children in the world. The incidence of allergy has increased sharply in Indonesia in the last 20 years. At any time, 30% of people develop allergy manifestations, more than 40% of school-aged children have experienced an allergy manifestation, 20% have experienced asthma, and 6 million people have suffered atopic dermatitis. Considering the incidence number has kept increasing, a prevention effort has become a crucial aspect. One of the easiest methods is exclusively giving breast feeding. Experts have supported that exclusive breastfeeding can reduce children's allergy risk. Methods: This study was analytical-observational research with a case-control study design. The subject of this research is patients in the pediatrics outpatient unit of Dr. Soetomo General Academic Hospital. They are suspected of suffering from allergies and have a familial atopic history. Data were collected by reviewing the patient's medical records as secondary data. After matching the subject, the data were analyzed using the chi-square test. Results: From the total of 118 samples, it was found that in the case group, 13 children had been given exclusive breastfeeding, and 46 children had not been given exclusive breastfeeding. Meanwhile, in the control group, it was found that 31 children had been given exclusive breastfeeding, and 28 children had not been granted exclusive breastfeeding. A bivariate analysis found that exclusive breastfeeding can reduce the risk of allergy (p-value = 0.001; OR= 0.255 CI = 0.115 – 0.468). Conclusion: This research concludes that children breastfed exclusively will have a risk of allergy 0.255 times more than those who had not been breastfed exclusively.

Abstract

Introduction: About 300.000 – 350.000 Indonesian people suffer from cardiac arrest each year. Basic Life Support (BLS) is the first immediate response needed by cardiac arrest people, especially the out of hospital cardiac arrest (OHCA) patient. BLS that given from public to OHCA could increase the survival rate up to 2 – 3x higher. Unfortunately, the Indonesian still did not know what is cardiac arrest, how to recognize it, and how to do BLS. Objective: The objective of this study was to identify the effect of BLS training in the knowledge and skill level of community in Sidodadi Village, Lawang, Indonesia. Method and Material: This experiment used a quasi-experimental design with 102 of the random sampling of people age 20-40 in Sidodadi Village, Lawang, Indonesia. The sample was given pre-test and post-test knowledge questioner before and after joining BLS training. The sample's skill was scored by an assistant doctor using a skill checklist. The data then analysed with a t-paired sample test, Wilcoxon sum rank test, and Pearson correlation. Result and Discussion: The score of women is higher than men both in pre-test and post-test, with the significance of p=0,039 for pre-test and p=0,001. 99% who joined BLS training have a significant increase in knowledge with p=0,001. Conclusion: The increase in knowledge and skill is correlated for an excellent and good level of the score, but not correlated in a bad level. About 99% of people who have joined BLS training have a significant increase in knowledge and skill. These trained people have the willingness to be a bystander for cardiac arrest people near them.

Abstract

Introduction: Triage is a system to sort patients based on their urgency for care during an emergency. In addition, to determine patient's severity, triage reduces time wasted and overcrowding in the emergency room.1 Triage errors such as over-triage or under-triage must not exceed 35% and 5% of the total numbers of patients respectively. Dr. Soetomo General Hospital of Surabaya with its status as one of type "A” hospital in Indonesia that has good amenities, facilities, and infrastructures which is also the main referral hospital from Primary or Secondary Health Care, is prone to patients overcrowding. The high number of patients and the demands for high-quality service as the main referral hospital require higher qualification from health care professionals in term of cognitive, skills, and attitude. Objective: This study conducted to evaluate the level of knowledge of the ER (Emergency Room) nurses at Dr. Soetomo General Hospital in 2019 towards triage. Methods and Materials: This study is a quantitative descriptive study using a validated questionnaire with 32 nurses as the samples. Results and Discussion: Demographic profile showed that majority of nurses are: female (52%), age 26 – 35 years old (46 %), graduated from D3 (61%), have been working in this field for >15 years (46%), attended Basic Life Support Training (33%), and have moderate knowledge in triage (61%). Conclusion: Emergency room nurses of Dr Soetomo General Hospital had adequate knowledge in triage. The researcher considers there might be some factors influencing nurse's knowledge in triage.

Abstract

Penderita kusta tahun 2015 di Kota Surabaya tertinggi pertama adalah Kecamatan Kenjeran, disusul Kecamatan Semampir, dan Kecamatan Tandes. Ada beberapa faktor risiko yang memengaruhi kejadian kusta, salah satunya adalah tingkat ekonomi. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat penghasilan dengan kejadian kusta. Jenis penelitian observasional analitik dengan rancangan penelitian case control matching untuk mengetahui hubungan tingkat penghasilan dengan kejadian penyakit kusta MB pada warga yang tinggal di wilayah Kecamatan Kenjeran, Semampir dan Tandes. Penelitian ini dilakukan antara bulan Januari-April 2017. Jumlah responden kasus sebanyak 28 orang dan kontrol sebanyak 28 orang, Pengumpulan data dilakukan dengan cara wawancara dan penelusuran rekam medis. Analisis bivariat dilakukan dengan menggunakan SPSS 16. Hasilnya didapatkan P value 0.003 < 0.05 menunjukkan bahwa ada hubungan antara tingkat penghasilan dengan kejadian kusta dengan nilai OR 5.200 (1.427-18.948). Simpulan : Terdapat hubungan antara tingkat penghasilan dengan kejadian kusta di Kecamatan Kenjeran, Tandes dan Semampir. Responden yang memiliki tingkat penghasilan rendah berisiko 5.2 kali terkena kusta dibandingkan responden berpenghasilan tinggi.

Abstract

Introduction: Diabetes mellitus is a non-transmitted pandemic disease which had spreaded on a global scale. The prevalence of diabetes mellitus in the world according to the International Diabetes Federation (IDF) is 1.9% and diabetes mellitus is ranked seventh as the cause of death in the world, around 95% in the world is type 2 diabetes mellitus. According to Riset Kesehatan Dasar (Riskesdas) in 2013, the prevalence of diabetes mellitus in Indonesia is 6,9%. Type 2 diabetes mellitus that is not well controlled will increase the risk of chronic complications, both microangiopathies such as nephropathy, and macroangiopathy such as hypertension. The aim of the study was to determine correlation between HbA1c levels with eGFR (Estimated Glomerulus Filtration Rate) and blood pressure (systolic and diastolic).Methods: An analytic observational cross-sectional study, collecting the data from 94 patients with type 2 diabetes mellitus, Dr. Soetomo General Hospital, Surabaya, from January to May 2018. The variables were collected from patient's medical records. Analysis using Spearman's Rank Correlation test. To determine normality of the data, we use Kolmogorov-smirnov test.Results: The results showed that there are 134 (57,8%) patients with type 2 diabetes mellitus are female more than male. Patients with greater than or equal to 45 years old were 203 (87,5%) patients, more than under 45 years old. The correlation test showed that there was no significant correlation between HbA1c and systolic blood pressure (r=-0,127; p=0,054). Also there was no significant correlation between HbA1c and diastolic blood pressure (r=-0,111; p=0,093). Analysis on other factor showed a significant correlation between HbA1c and eGFR (r=0.341; p=0.000).Conclusion: There was no significant correlation between HbA1c and systolic blood pressure, also there was no significant correlation between HbA1c and diastolic blood pressure. But, there was a significant correlation between HbA1c and eGFR. This is because the data retrieval does not consider the drug consumption and therapy that has been done by the patient, as well as examination of HbA1c, serum creatinine eGFR, and blood pressure not at the same time.

Abstract

Riskesdas result in 2013 showed that the prevalence of WUS population who were pregnant and at risk of CED was 24,2%. CED in pregnant women can cause abortion and premature birth, low birth weight babies and infant disability, children become malnourished and brain development is hampered, and children are at risk of developing metabolic diseases. In Bajulmati Village there were 15 pregnant women. 40% of 15 pregnant women suffer from CED. The purpose of this study was to determine differences between groups based on maternal age, maternal occupation, family income, previous pregnancy history, history of chronic illness, and maternal knowledge about nutrition in the incidence of pregnant women with CED in Bajulmati Wongsorejo Banyuwangi and differences in knowledge levels and attitudes towards behavior about nutrition after intervention. An observational analytic, cross sectional research design using questionnaires and observations. The sample selection technique uses total sampling technique. There were significant differences between groups based on family income in the incidence of CED in pregnant women and previous pregnancy history in the incidence of CED in pregnant women. Level of knowledge gained after the intervention was in the form of counseling to Prevent CED significantly.

Abstract

The use of digital technology has increased rapidly. In Indonesia, gadget use by children has become very high and requires special attention. Many effects may occur due to gadget use, one of which is sleep quality disturbance. This study aims to analyze the correlation between gadget addiction and sleep quality in children grades 4-6 SDN 01 Srigading Lawang. This research is an analytic observational study with a cross-sectional design in a population of 4-6th graders of SDN 01 Srigading Lawang. A total of 126 students over 130 students were included. Data were collected using two questionnaires, which are Smartphone Addiction Scale and The Pittsburgh Sleep Quality Index, and tested using RxC Contingency. Gadget addiction has a significant correlation with sleep quality with a weak correlation. Children with mild levels of addiction to gadgets had a potential of 2,013 times to have good sleep quality, compared to children with moderate levels of addiction. In contrast, children with severe levels of addiction to gadgets were at risk 12.04-fold to have poor sleep quality compared to the mild level of addiction. There is a significant correlation with a weak correlation between gadget addiction and sleep quality in children grades 4-6 SDN 01 Srigading Lawang.
