Abstract

Probiotic bacteria have a beneficial effect on diarrhea. In this study, we have examined effect of Lactobacillus plantarum Strain IS-10506 (LIS-10506) and IS-20506  (LIS-20506) on inhibition of NFκB activation, downregulation TNF Receptor-1 (TNF-R1) and Apoptosis in Epithelial Brush Border of Rattus novergicus induced with LPS. On the first day, LPS was inducedto Rattus novergicus per oral. At the 3rdday, Lactobacillus plantarum Strain IS-10506 and LIS20506 separately were supplemented for 7 days (the 3rdday up to the 9thday). Rat was sacrificed after anaesthetizing with ether to assess NFκB activation, TNFR1 and apoptosis. Result showed the decreases of activation NFκB in LIS-10506 group as well as in LIS-20506 group, significantly different, in NFκB activation at group with only LPS (average 14.33+4.509) compared to group with LIS-10506 induction (average 2.00+1.732)and LIS-20506 induction (average 1.33+1.528), at p( p<0.000). Downregulation of TNF-R1 was significant  at LPS group compared to LIS-10506 induction as well as LIS-20506 induction. The index of apoptosis showed significant of degradation (p<0.000) after induced by LIS, where LPS group (14.67+2.517), LIS-10506 induction (6.33+2.309) and LIS-20506 induction (6.00+3.000). As a conclusion, supplementation of LIS-10506 and LIS-20506 separately will inhibit theNFκB activation, and although the mechanism was not sure, what by significant degrade the expression TNF-R1( as ekuivalen of activity TNFα), and the mentioned give the implication that happened by the degradation of occurence apoptosis at cell of epitel bush border intestine.

Abstract

BACKGROUND: Risk of acute respiratory infections in children less than 5 years of age is up to 95%. Zinc deficiency is one of the main risk factors. This study aimed to explore the effect of zinc on the bronchial mucosae inflammatory status expressed by nuclear factor (NF)-κB p105/p50, NF-κB p65, interleukin (IL)-8, and IL-1β.METHODS: Twenty-four Wistar rats were divided into 4 groups: normal zinc diet group without zinc supplementation (Z1), normal zinc diet group with zinc supplementation (Z2), zinc deficient diet group without zinc supplementation (Z3), and zinc deficient diet group with zinc supplementation (Z4). NF-κB p105/p50, p-NF-κB p65, IL-8, and IL-1β were measured by immunohistochemical staining.RESULTS: The inflammatory status of bronchial mucosae between Z1 and Z2 groups showed no difference (p=0.055). However, the inflammatory status of bronchial mucosae between Z3 and Z4 groups showed significant difference (p<0.01). Multivariate factorial design showed that zinc supplementation was beneficial when given to zinc deficient diet group with regard to decrease p-NF-κB p65, IL-8 and IL-1β levels (p<0.001) and increase dendritic cell (p=0.022).CONCLUSION: Zinc administration under conditions of zinc deficiency affects the inflammatory status, as shown by the decrease of p-NF-κB p65, IL-8 and IL-1β and the increase of NF-κB p105/p50.KEYWORDS: zinc, NF-κB, p105/p50, p65, IL-8, IL-1β, rat

Abstract

BACKGROUND: Galectin-4 and Myosin-1a are important proteins for normal intestinal brush border structure and composition. Damage of these proteins by inflammation may alter digestion, absorption and barrier function. Probiotic has been widely known in maintaining gut health. However, the molecular mechanism of Lactobacillus plantarum IS-2056 probiotic in repairing intestinal brush border is not well defined. Therefore, current study was conducted by investigating the Galectin-4 and Myosin-1a expressions in a rodent model.METHODS: Male Wistar rats were induced with/without lipopolysaccharide (LPS) and treated with/without L. plantarum IS-2056 probiotic. On the seventh day, duodenum, jejunum, and ileum were collected and analyzed with western blot and immunohistochemistry for Galectin-4 and Myosin-1a expressions.RESULTS: Rats administrated with L. plantarum IS-2056 probiotic showed significant increase of Galectin-4 and Myosin-1a expressions in duodenum, jejunum, and ileum compared to the control group (p<0.05). While in control group, Galectin-4 level tended to increase in more distal of intestinal segment and Myosin-1a level tended to decrease in more distal intestinal segment.CONCLUSION: L. plantarum IS-20506 probiotic may facilitate the repairment of damaged intestinal brush border as demonstrated by significant restoration of Galectin-4 and Myosin-1a expressions in duodenum, jejunum, and ileum of LPS-induced rats.KEYWORDS: Lactobacillus plantarum, IS-20506, probiotic, galectin-4, myosin-1a, duodenum, jejunum, ileum

Abstract

BACKGROUND: Airway epithelium is the first line of defense against a variety of exposures. Inflammatory processes, hyperresponsiveness and zinc deficiency cause epithelial damage. Zinc is involved in apoptosis and microtubule formation. However, its role in the integrity of bronchial mucosa and cilia is unclear.METHODS: To assess the effect of zinc on the integrity of the bronchial epithelium, 24 male Rattus norvegicus strain Wistar rats were randomized into four experimental groups: normal zinc diet group without zinc supplementation, normal zinc diet group with 60 ppm zinc supplementation, zinc deficient diet group without zinc supplementation, and zinc deficient diet group with 120 ppm zinc supplementation. Bronchial mucosal integrity was measured with the number of epithelial cells, and the number and length of cilia.RESULTS: Number of cell in normal zinc diet group was 8.8±1.82, while it was only 8.1±1.08 in zinc deficient diet group (p<0.001). Number of cilia per cell was 4.6±1.08 in normal zinc diet group, compared to 4.0±0.79 in zinc deficient diet group (p<0.001). Ciliary length also differ by 7.68±0.66 μm in normal zinc diet group and only 5.16±0.91 μm in zinc deficient diet group (p<0.001).CONCLUSION: Zinc supplementation of the normal zinc diet group affected the length of bronchial cilia. Zinc supplementation of the zinc deficient diet group affected the integrity of the bronchial epithelium, which was shown by the number and length of cilia, and the number of epithelial cells.KEYWORDS: zinc, bronchial epithelial integrity, cilia length, number of cilia, epithelial cell

Abstract

Background: Norovirus has become a major cause of severe outbreaks of gastroenteritis since the discovery of the rotavirus vaccine, with the main symptom being diarrhea. Until now, research on the epidemiological analysis of norovirus has not been carried out at Regional Public Hospital (RSUD) Dr. Soetomo, Surabaya. Purpose: This study aims to provide clinical epidemiology data and an analysis of norovirus infections in children with diarrhea at Regional Public Hospital (RSUD) Dr. Soetomo, Surabaya, Indonesia, including the prevalence of norovirus infection in each age group and sex, its clinical appearance, and its seasonal variation. Methods: A cross-sectional study was conducted in children aged 1–60 months hospitalized for diarrhea in RSUD Dr. Soetomo between April 2013 and March 2014. Identification of the virus in the stool was done by norovirus enzyme immunoassay Quick NaviTM Noro2. The proportion, age, sex, clinical symptoms, and patterns of the norovirus seasonal data were calculated. Results: Norovirus was detected in 64 samples (19%) of the 340 stool samples, with a mean patient age of 11.75 months; it was mostly found in patients less than 24 months of age (95%), and 64% were male. The monthly pattern of norovirus infection was mostly found in November, followed by May and April. The clinical symptoms were fever (72%), vomiting (66%), bloating (59%), abdominal cramps (34%), perianal inflammation (27%), abdominal distension (16%), and seizures (8%). Conclusion: The prevalence of norovirus was found to be high in patients with diarrhea aged 1–60 months hospitalized in RSUD Dr. Soetomo, with a proportion of 19%. Further research is needed to determine the severity of norovirus infection.

Abstract

Background: Hirschsprung-associated enterocolitis (HAEC) is one of the worst and most common complications of Hirschsprung’s disease (HD). The mortality rate of HD patients with enterocolitis is still considered to be higher compared to that of those without enterocolitis. Purpose: This study aimed to identify and evaluate potential risk factors for HAEC development. Methods: A cross-sectional study was conducted using secondary data from the medical records of HD patients treated from January 2015 to September 2018 at Regional Public Hospital (RSUD) Dr. Soetomo, Surabaya. The inclusion criteria were HD patients who had or had not experienced enterocolitis. The analysis was done by comparing the presence of risk factors between groups of HD patients with and without preoperative and/or postoperative HAEC. The results were presented as the median value and frequency. To evaluate further, a prevalence ratio (PR) with a 95% confidence interval was performed. The Mann–Whitney U test was also performed with a significance level of p < 0.05 for one factor: length of aganglionic intestinal segments. Results: This study showed that 12 of the 40 HD patients studied (30%) had experienced enterocolitis. The risk of developing HAEC was associated with patients who had a history of previous enterocolitis (PR 6.60 [2.94 < PR < 14.80]). Regarding surgical details, patients who had had surgery only once (31.30% compared to 14.30%), surgery with one surgical method (29.40% compared to 20.00%), and a primary procedure had a higher incidence of HAEC (29.40% compared to 27.30%). Conclusion: HD patients with a history of previous enterocolitis were found to have a higher risk of developing HAEC.

Abstract

Introduction: Diarrhea is one of the most common causes of morbidity and mortality of children worldwide, especially in developing countries. Acute diarrhea can occur from a large number of causes. We aim to describe the clinical manifestation characteristics of acute diarrhea on pediatric patients that are presented to the hospital.Methods: A cross-sectional study from all pediatric patients treated in Dr. Soetomo General Hospital Surabaya in 2011-2013.Age, gender, nutritional status, length of hospital stay, duration of breastfeeding, clinical signs and symptoms, as well as laboratory results collected and analyzed descriptively.Results: One hundred and fifty patients were included with characteristics of male(54%), infant (55.3%)with a mean age of 6-7 month. History of being exclusively breastfed(54%) and lack of nutrition(48%). Vomiting is mostly present (72.7%) and commonly come with a combination of two symptoms (42%) which were vomiting and fever. Mild to severe dehydration degree could be found on the patient who had vomiting, seizure, and fever. The most length of stay was frequently >72 hours. The occurrence of acute diarrhea on a child who was treated in Dr. Soetomo General Hospital Surabaya is dominated by vomiting and it could be inferred that the most acute diarrhea has a tendency of being caused by a virus, where liquid acute diarrhea without blood, mucus, or fever is found.Conclusion: The etiology of acute diarrhea is mostly caused by viruses and symptoms dominated by a combination of vomiting, fever, and diarrhea without blood caused by villi damage.

Abstract

Introduction: Cerebral palsy (CP) is a non-progressive neurological motor disorder. The incidence of CP in developed countries is around 2 to 2.5/1,000 births. Drooling is a condition when the saliva comes out of the mouth uncontrollably. The number of drooling cases can be estimated to be 10-37% in children with CP. This study aimed to examine and analyze the clinical profile of drooling in CP patients who visited Physical Medicine and Rehabilitation Department Dr. Soetomo General Hospital Surabaya period January 1st, 2016 - December 31st, 2017 according to their ages, gender, CP types, swallowing disorders, speech disorders, and nutritional status.Methods: This study used a retrospective analytical method, and used the patients' medical record data at Physical Medicine and Rehabilitation Department Dr. Soetomo General Hospital Surabaya period January 1st, 2016 - December 31st, 2017.Results: There were 45 subjects consisted of 23 men and 22 women. The peak incidence occurred in the 1-5 years age range. Whereas the most type of CP were quadriplegic spastic. There were 7 patients (15.6%) who suffered swallowing disorders, while speech disorders dominated the pattern of symptoms in the sample (95.6%). A total of 21 people or 52.5% of patients with CP and having clinical manifestations of drooling had good nutritional status. Conclusion: In this study, we can conclude that peak incidence of CP was mostly in the 1-5 years old range, and quadriplegic spastic was the most popular types of CP. Speech disorders dominated the pattern of symptoms.

Abstract

Introduction: Diarrhea is a collection of symptoms in the form of decreased stool consistency accompanied by more than normal frequency of defecation. Diarrhea is the second largest cause of death in children under five years old. In 2010, East Java was ranked first in the frequency of outbreaks in Indonesia. This study described the profile of diarrhea patients in children less than 5 years old in the Children's Ward of Dr. Soetomo General Hospital Surabaya in 2016-2018. Dr. Soetomo General Hospital Surabaya was chosen for data collection because it is one of the main referral hospitals in Indonesia, thus it has diverse cases. This study aimed to describe the profile of diarrhea patients in children less than 5 years old in the Children's Ward of Dr. Soetomo General Hospital Surabaya in 2016-2018.Methods: This study was a retrospective study by assessing the patient's medical record and analyzed descriptively.Results: There were 650 patients (425 male and 225 female), at most 6-11 months (34.6%). Most patients were classified as malnourished (45.4%). The type of diarrhea was dominated by acute watery diarrhea (94.8%). Mild-to-moderate dehydration (84.2%) was found in most patients. The comorbidities were found in 83.4% of patients, with malnutrition (45.4%) and febrile seizures (30.3%) as the most common contributors. The therapy used was intravenous rehydration (99.7%), antibiotics (45.7%), zinc (98.2%), and probiotics (96.3%).Conclusion: Most diarrhea patients in this study were male, aged 6-11 months, mild-to-moderate dehydration, malnourished, suffering from acute water diarrhea, accompanied by comorbidities, and treated with intravenous rehydration.

Abstract

Introduction: Ileostomy is needed as an important part of the management of several congenital disorders of the intestine. One of the indication of ileostomy in children is short bowel syndrome, which is caused by intestinal atresia such as ileal atresia and intussusception. Ileostomy is a surgery procedure which consists of creating holes on abdominal walls to release stool. The objective of this study was to determine the description of pediatric patients who underwent ileostomy in Dr. Soetomo General Hospital Surabaya. Methods: This was a retrospective descriptive study on medical records in the Central Medical Record Department of Pediatrics and General Surgery Dr. Soetomo General Hospital Surabaya from January 2017 to December 2018. Results: There were 26 pediatric patients who underwent ileostomy in Dr. Soetomo General Hospital Surabaya. Samples from male patients consisted of 57.7%, while female patients consisted of 42.3% with the most common age group was neonates with 57.7%, infants 30.8%, and toddlers 11.5%. There were 30.7% patients with 2.0-2.9 kg body weight, 27% with 3.0-3.9 kg body weight, and 42.3% with more than 4 kg body weight. The outcome showed that repair was needed, or the patient was discharged with periodic follow-up after ileostomy. Conclusion: Ileostomy was performed mostly in neonates with average weight within normal body mass index (BMI) and the highest incidence of pediatric patient with ileum stoma was in pediatric patients.
