Abstract

Kerusakan saraf perifer merupakan komplikasi diabetes melitus (DM) yang dapat menyebabkan kecacatan secara permanen. Kabupaten Jember memiliki angka DM yang tinggi dan penderitanya memiliki resiko untuk mengalami kerusakan saraf perifer. Desa Mayang merupakan mitra sasaran yang membutuhkan informasi untuk mencegah kerusakan saraf perifer. Pengabdian masyarakat ini bertujuan untuk mencegah kerusakan saraf perifer pada penderita diabetes melitus tipe 2 di Kabupaten Jember. Metode pelaksanaan dilakukan dengan ceramah, diskusi, adanya evaluasi berdasarkan nilai pretes dan postes yang diberikan kepada peserta. Hasil kegiatan ini menunjukkan nilai pretes yang didapatkan dari rata-rata jawaban benar yaitu 41,79%. Sedangkan nilai benar rata-rata peserta postest yaitu 69,5%. Meskipun jawaban benar peserta belum mencapai 100%, dapat dikatakan bahwa terdapat peningkatan secara pengetahuan terkait diabetes melitus dan pencegahan komplikasi kerusakan saraf perifer pada peserta.Berdasarkan hasil tersebut, terdapat peningkatan nilai rata-rata jawaban benar dari peserta pengabdian kepada masyarakat. Sehingga dapat disimpulkan bahwa edukasi dengan metode diskusi dan ceramah efektif meningkatkan pengetahuan masyarakat.

Abstract

Diabetes Mellitus (DM) influence on psychological and physical condition of the patients. Patients arerequired to maintain self-care and to prevent complication which can cause psychological pressure. Thiskind of pressure may affect hardiness of patients in performing self-care. Hardiness may be influenced byspiritual well being. The aim of this study was to analyse the relationship between spiritual well-being andhardiness in patients with type 2 DM. This research applied correlational design with cross-sectionalapproach. A total of 112 respondents were selected by using consecutive sampling technique. The data werecollected by administering questionnaire of Spiritual Well-Being Scale (SWBS) and Hardiness Scale. Dataanalysis was performed by Spearman-rank correlation test with the significance level of 0.05. The resultsshowed that the median score of spiritual well being was 91 (72-100) while the median value of hardinesswas 104 (84-122). There was a significant positive correlation between spiritual well-being and hardiness (pvalue : 0.001 ; r : 0.303). The higher the spiritual well-being the better the hardiness of patients with type 2DM. This research reveals on the importance of assessing spiritual well-being to enhance hardiness inpatients with type 2 DM.

Abstract

Chronic bronchitis is an inflammatory disease of the bronchial respiratory tract that lasts more than 3 months or several times in 2 years. Symptoms in patients with chronic bronchitis can affect breathing patterns, including dyspnea, respiratory frequency, depth of breathing, and oxygen saturation. Providing Pursed Lip Breathing therapy can create resistance to the air coming out of the lungs and open the airways to improve breathing patterns. This study aimed to analyze the effectiveness of Pursed Lip Breathing therapy on breathing patterns in patients with chronic bronchitis. This descriptive, experimental research was conducted on a Chronic Bronchitis patient. Patient in the Inpatient Room of a government hospital. The implementation of Pursed Lip Breathing therapy was carried out for 4 days, where each day, there was one session of 8-10 repetitions of abdominal breathing followed by Pursed Lip Breathing therapy for 8 minutes. Data analysis was explained descriptively daily to compare the improvement in breathing patterns between pre and post-therapy values. The study results showed significant improvement in breathing patterns, including the level of dyspnea, depth of breathing, orthopnea, use of accessory muscles for breathing, and vital signs, including RR, SPO2, and the patient's blood pressure. This study concluded that Pursed Lip Breathing therapy effectively improved breathing patterns in Chronic Bronchitis patients. The results of this study indicate that pursed lip breathing therapy is safe to apply to patients undergoing treatment. Nurses can apply pursed lip breathing therapy and implement siki for patients with breathing pattern disorders.

Abstract

Background: Multiple Fractures or broken bones are a condition where the continuity of the bones is broken in a complex manner, this is caused by trauma caused by frequent traffic accidents. Objective: To analyze the effect of a combination of non-pharmacological therapies Quantum Spiritual Touch Therapy" (QST) and Finger Holding in multiple fraktur patients with acute pain at RSD dr. Soebandi Jember. Method: This research is an observational study with a case study design. Primary data obtained through anamnesis and physical assessment. Assessment of the effect of QST therapy and finger holding was carried out a total period of 35 minutes, 20 minutes for QST therapy and 15 minutes for finger holding therapy, based on initial, process, and final data. Results: The results showed a reduction in pain levels with a Numerical Rating Scale (NRS) was carried out for 6 meetings with the implementation of therapy once a day. Pain levels decreased in the pre-implantation measurements with an average of 5,4 to 4,7 at the time of post-implementation. Discussion: The combination of pharmacological therapy from QST therapy and finger holding aims to help the effectiveness of analgesic drugs by improving the psychological structure with implementations that contain spiritual elements.

Abstract

Background: The government has made vaccination the primary strategy to control the COVID-19 pandemic. However, the public still needs to implement COVID-19 prevention behavior even though they have been vaccinated because COVID-19 infection with or without symptoms can still occur. Health literacy is the basis for behavior change. Purpose: This study aimed to determine the relationship between health literacy and post-vaccination COVID-19 prevention behavior of the community in the work area of "‹"‹the Patrang Public Health Center, Jember Regency. Methods: A descriptive correlation study with a cross-sectional approach was performed in May 2022. The sample in this study was 435 selected by purposive sampling with the inclusion criteria of those aged >17 years old and receiving a full primary dose of vaccination. The data were collected using the Health Literacy Survey Coronavirus Disease Questionnaire 22 (HLS-COVID-Q22) and COVID-19 prevention behavior questionnaire. The data were analyzed by Spearman with a significance level of 0.05. Results: The results showed that the majority of health literacy was inadequate health literacy (43,2%), and the post-vaccination COVID-19 prevention behavior was in the moderate category (42.7%). There was a significant moderate positive correlation between health literacy and post-vaccination COVID-19 prevention behavior (p < 0.001; r = 0.513). Conclusion: The higher the health literacy, the better the post-vaccination COVID-19 prevention behavior. The nurse was importance to provide health education by paying attention to the health literacy ability. Focusing on improving health literacy is an important strategy to enhance COVID-19 prevention behavior and reduce transmission of the coronavirus.

Abstract

Diabetic pruritus is a skin complication that is often found in diabetes mellitus patients which is characterized by complaints of itching. One of the complementary therapies that can reduce itching is to do massage and VCO combination therapy. The study aim to analyze the effectiveness of combination massage therapy and VCO to reduce itching in diabetic. This research is a descriptive study using a case study approach. Participant in this study was patient with type II diabetes mellitus with complaints of itchy skin. The intervention was carried out for 3 days of treatment with a frequency of action twice a day in the morning and evening after bathing. The pre-test and post-test were assessed using two instruments including the Overall Dry Skin Score (ODSS) to assess the dryness of the patient's skin and the Itch Numeric Rating Scale (Itch NRS) to assess the patient's itch level. The results showed a decrease in the level of skin dryness from the scale 3 to scale 2 on day 3 of treatment and there was a decrease in the itching scale from scale 7 to scale 4 on day 3 of treatment. Combination therapy of massage and VCO can increase the level of skin moisture as evidenced by a decrease in the level of skin dryness so that it will also reduce the level of skin itching. With these results, it can be considered to make combination massage therapy and VCO as therapeutic modality to overcome the problem of itching in DM patients.

Abstract

Background: Heart Failure is a condition that the heart can not pump enough blood to meet the circulatory needs used in the body's metabolic processes. Patients with heart failure disease may experience swelling (oedema) in certain parts of the body, especially the lower extremities. Oedema that is not treated will exacerbate damage to the circulatory system. One of the treatments for lower extremity oedema in heart failure patients can be done by giving compression therapy. Purpose: The purpose of this study is to determine the effectiveness of compression therapy on lower extremity oedema in heart failure patients. Method: This research is a case study that managed one patient with heart failure disease who has lower extremity oedema. Data collection was carried out by observation, interview and provision of interventions carried out for 4 days using moderate pressure (20-30 mmHg). This research use compression stocking for 18 hours within 24 hours. Compression therapy is given once a day that the stocking is used at 14.00 WIB and removed at 08.00 WIB on the following day. The instrument used in this study was the pitting oedema observation sheet. Result: The results of this study were a decrease in the degree of pitting oedema in patients from a value of +3 with a depth of 5 mm in a return time of 65 seconds to a value of +1 with a depth of 1 mm in a return time of 2 seconds. Conclusion: Compression therapy is effective in reducing the degree of pitting oedema in patients with heart failure. Compression therapy has optimal effectiveness on the third day of intervention. Therefore, this therapy is highly recommended to be applied in the inpatient room, especially in patients with heart failure who have oedema in lower extremity.

Abstract

Nurse caring behavior is essential for the quality of healthcare services and patient well-being, yet several internal and external factors influence this behavior. This study analyzes the effects of work rewards, workload, motivation, and personality on nurses’ caring behavior. This cross-sectional study used a quantitative approach. Data were collected from 217 nurses in hospitals using structured questionnaires measuring variables of work rewards, workload, motivation, personality, and caring behavior. Data analysis was performed using descriptive statistics and multiple regression analysis. The findings show that motivation has the most significant influence on caring behavior (β = 0.45, p < 0.01), followed by work rewards (β = 0.30, p < 0.01) and personality (β = 0.20, p < 0.01). Workload has a significant negative effect (β = -0.28, p < 0.01). An R-squared value of 0.65 indicates that these four variables can explain 65% of the variability in caring behavior. These findings support Gibson’s theory that environmental factors (work rewards, workload) and individual factors (motivation, personality) affect caring behavior. Motivation and work rewards drive caring behavior, while a high workload hinders it. Policies that enhance motivation and work rewards, along with balanced workload management, are necessary to support caring behavior in nurses Keywords: Caring behavior, Work rewards, Workload, Motivation, Personality.

Abstract

Background: Stroke is a neurological disorder frequently associated with reduced cerebral perfusion and impaired consciousness. While conventional interventions are widely implemented, their effectiveness may be enhanced by evidence-based approaches such as Familiar Auditory Sensory Training (FAST), a non-pharmacological intervention that utilizes meaningful auditory stimuli to activate neural pathways and support consciousness recovery. Objective: This study aims to evaluate the effectiveness of FAST in addressing the nursing problem of risk for ineffective cerebral tissue perfusion in an ischemic stroke patient. Methods: A case study was conducted involving a 73-year-old male patient in a soporous state. FAST was administered over three consecutive days, three times daily, with each session lasting 10 minutes. Data were collected through direct observation and Glasgow Coma Scale (GCS) measurements before and after each session. Patient outcomes were evaluated using the Nursing Outcomes Classification (NOC), focusing on indicators including consciousness level, blood pressure, and agitation. Results: The patient’s level of consciousness improved from sopor (6) to somnolence (9). Improvements were also noted in blood pressure, heart rate, and respiratory rate, although an increase in restlessness was observed. FAST supported cerebral responsiveness and cognitive function recovery, contributing to the resolution of the nursing problem: risk for ineffective cerebral tissue perfusion. Conclusion: FAST can be utilized as a therapeutic nursing intervention to enhance hemodynamic stability in patients with decreased consciousness

Abstract

Background: Spiritual distress is a problem with the patient's response to health. Spiritual distress is a disturbance in the meaning of life about oneself, others, and God. Spiritual distress occurs due to spiritual needs not being met. Purpose: This study aims to determine spiritual distress in post-operative orthopedic patients in the surgical room at dr. Soebandi Regional Hospital Jember. Methods: This study employed a descriptive, consecutive sampling method involving 150 respondents from both major and minor post-operative orthopedic patients in 2023. Results: The majority of respondents in this study were men with varying ages and types of surgery, 80.7% experienced major surgery, and 19.3% underwent minor surgery. Data collection was conducted using the Spiritual Distress Scale questionnaire, and the results showed a median value of 74.00, with a minimum value of 33 and a maximum value of 103. This indicates that the resulting value is close to the maximum value of the questionnaire, suggesting a high level of spiritual distress. This can be caused by the high value placed on self-relationship, which encompasses the strengths that exist within a person, including knowledge and attitudes about oneself. This high value can occur due to a lack of knowledge about oneself and a lack of confidence in the future, leading to a loss of peace of mind. Conclusion: This research provides additional data and knowledge regarding spiritual distress in post-operative orthopedic patients. Patients with spiritual distress require maintenance of spiritual well-being, which helps them find meaning in life and experience a sense of peace. The results of this research make it possible for nurses to develop and understand the importance of spiritual care for patients
