Abstract

Penderita diabetes melitus rentan terhadap infeksi kaki yang menyebabkan kematian jaringan oleh obstruksi pembuluh darah yang memberikan nutrisi kepada jaringan yang biasa terjadi pada ekstermitas bawah atau disebut gangren pedis. Perawatan luka dengan menggunakan hydrogel merupakan salah satu intervensi yang dapat dilakukan untuk mengatasi gangren pedis. Karya ilmiah ini bertujuan mengetahui efektifitas asuhan keperawatan perawatan luka gangren pedis menggunakan bahan hydrogel pada pasien diabetes melitus. Asuhan keperawatan dilakukan selama 3 hari dengan intervensi utama yakni perawatan luka dengan observasi hasil intervensi menggunakan Bates-Jensen Wound Assessment Tool (BWAT). Setelah dilakukan intervensi, hasil menunjukkan adanya peningkatan kondisi luka dan penurunan keluhan nyeri pasien. Pada hari pertama perawatan pasien mengeluh nyeri skala NRS 4 (sedang), ukuran luka lebar 2,5 cm dan panjang 4 cm, terdapat jaringan nekrotik berwarna hijau kehitaman tidak keras, berbau, kulit sekitar luka berwarna gelap, tepi luka terlihat. Pada hari ketiga perawatan, pasien mengatakan nyeri berkurang menjadi skala 2 (ringan), ukuran luka lebar 2,5 cm dan panjang 4 cm, jaringan nekrotik berkurang banyak, bau berkurang, kulit sekitar luka berwarna pucat/putih, tepi luka terlihat menyatu dengan dasar luka. Perawatan luka menggunakan hydrogel pada luka gangren efektif dilakukan karena menunjukkan perubahan dalam 3 hari perawatan. Diharapkan untuk analisis selanjutnya dapat memberikan intervensi lebih lama agar dapat menunjukkan efektifitas perawatan luka lebih rinci dan baik.

Abstract

A diabetic foot ulcer is one of the diabetes mellitus complications. The farmer is a job that has a risk for developing diabetic ulcers, especially farmers with diabetes. Farmers pay less attention to protecting their feet, so they risk foot trauma. This study aimed to describe the risk of diabetic foot ulcers in farmers. This was a descriptive observational study conducted at the Palengaan Pamekasan Health Center working area. As many as 76 respondents were chosen using the purposive sampling technique. All respondents were examined using inlow's 60-second diabetic foot instrument. The data were analyzed with univariate analysis. This study found that 72.4% of farmers had a moderate risk of developing diabetic ulcers, and 11,8% of patients were at risk for severe foot ulcers. This data showed that most farmers are at high risk of developing diabetic foot ulcers. Therefore, farmers with diabetes need intensive attention from medical practitioners to provide education on the importance of foot protection while working.

Abstract

Intracerebral Hemorrhage (ICH) occurs due to the presence of cerebral hemorrhage as a result of rupture of blood vessels in the brain tissue. Clinically Intracerebral Hemorrhage can be characterized by the presence of a decrease in consciousness. So that bed rest is one of a person's efforts in restoring his condition by lying in bed for a long time. It is at risk for experiencing impaired skin integrity. One of the skin integrity disorders that occurs in clients with bed rest is a decubitus ulcer. Skin care is one of the efforts to improve pressure sores by providing back massage therapy. This study aimed to analyze the nursing care of Skin Integrity Disorder patients by giving back massage in ICH cases in the Jasmine room of Dr. Soebandi Jember Hospital. The design of this study is descriptive in the form of a case study in patient who experienced redness of the skin of his back due to prolonged bed rest. After being given a back massage for 2 x 24 hours for 3 days, the patient experienced a change in skin integrity as evidenced by reduced redness of the skin. The administration of back massage therapy is effective in reducing the problem of skin integrity disorders in intracerebral hemorrhage patients who are lying down

Abstract

The problem that is often faced by people with bipolar disorder is that they cannot control the emotions and feelings they feel because they do not know the cause and also how to cope or respond when patients experience both episodes. The purpose of this study is to identify problems associated with predisposing and precipitation factors in order to get the right diagnosis. The research design used in this study is a case study. Predisposing factors include biological factors, loss of objects, personality, learning, and behavior. Precipitation factors in patients include loss, life events, roles, and physiology. Diagnosis The problem is based on complaints experienced by patients and those closest to them, anxiety and low self-esteem. The intervention that has been done by researchers to patients or participants for several weeks the patient is quite independent. Efforts can be made to improve the health quality of patients themselves by providing psychoeducation and nursing interventions. Humans are a denial of depression, individuals who experience mania are dominated by id with few manifestations of the super ego until the individual experiences a cycle back to the normal state or depressive phase of bipolar disorder.

Abstract

Coronavirus Disease 2019 (COVID-19) becomes a health crisis in the world. The main route of spreading COVID-19 is through the respiratory tract, but this virus also spread through extra respiratory route. This literature review aimed to describe the ocular transmission as an alternative route of COVID-19 transmission. This literature review study used sources of article obtained through ProQuest, Springer, ScienceDirect, PubMed, and Google Scholar with range 2019-2020 publication years and were traced using a combination of search term for ocular transmission, virus receptor, eye manifestation and COVID-19. Previous research used Experimental design, Case study, and Cross-sectional study, and published from 2019 to 2020. Then analyzed by PRISMA diagram method with four stages starting from identification, screening, eligibility, and included. 4 articles used Experimental study design, 4 articles used Cross-sectional study design, and 2 articles used case study design. This study showed that COVID-19 RNA was found on ocular (conjunctiva and cornea) in a low percentage (2.23%-24%). Patients with confirmed COVID-19 could have ocular manifestations. COVID-19 enter the human body through the eye by spike protein binds to eye receptors that are: ACE2, CD147/Basigin/EMMPRIN, TMPRSS2, ANPEP and AGTR2. Then this virus transferred to the respiratory tract through the nasolacrimal duct. The study showed that ocular transmission can be an alternative route of COVID-19 transmission even with a low percentage level, so it is important for health worker to wear eye protector.

Abstract

At the end of the COVID-19 Pandemic, as it is today, people tend to lose their self-awareness regarding implementing their health protocols. On the other hand, a new variant of Covid 19 threatens public health, as evidenced by the recent increase in the number of cases in Indonesia. The community needs to get educated about mitigating non-natural disasters of the Covid 19 outbreak, so they can prepare themselves when the danger of this outbreak re-emerges in the community through self-awareness of the Covid 19 Protocol. The time of health education is about 60 minutes. Health education methods are used through lectures, discussions, and demonstrations using information media such as LCD projectors, leaflets, posters, and X-banners. Understanding evaluation methods using pre-test and post-test. The community showed enthusiasm during the activity, as evidenced by the many questions asked. Participants could also practice washing their hands properly and wearing a mask. When the verbal evaluation was carried out, participants could also answer the questions the teaching team posed correctly. The results of the written test evaluation showed an increase in the average value, namely from the pre-test score of 70.5 to 90.1 for the post-test average value. The community expressed their commitment to implementing the COVID-19 protocol in their daily lives to prevent the spread of COVID-19 cases.

Abstract

Kerusakan saraf perifer merupakan komplikasi diabetes melitus (DM) yang dapat menyebabkan kecacatan secara permanen. Kabupaten Jember memiliki angka DM yang tinggi dan penderitanya memiliki resiko untuk mengalami kerusakan saraf perifer. Desa Mayang merupakan mitra sasaran yang membutuhkan informasi untuk mencegah kerusakan saraf perifer. Pengabdian masyarakat ini bertujuan untuk mencegah kerusakan saraf perifer pada penderita diabetes melitus tipe 2 di Kabupaten Jember. Metode pelaksanaan dilakukan dengan ceramah, diskusi, adanya evaluasi berdasarkan nilai pretes dan postes yang diberikan kepada peserta. Hasil kegiatan ini menunjukkan nilai pretes yang didapatkan dari rata-rata jawaban benar yaitu 41,79%. Sedangkan nilai benar rata-rata peserta postest yaitu 69,5%. Meskipun jawaban benar peserta belum mencapai 100%, dapat dikatakan bahwa terdapat peningkatan secara pengetahuan terkait diabetes melitus dan pencegahan komplikasi kerusakan saraf perifer pada peserta.Berdasarkan hasil tersebut, terdapat peningkatan nilai rata-rata jawaban benar dari peserta pengabdian kepada masyarakat. Sehingga dapat disimpulkan bahwa edukasi dengan metode diskusi dan ceramah efektif meningkatkan pengetahuan masyarakat.

Abstract

Diabetes Mellitus (DM) influence on psychological and physical condition of the patients. Patients arerequired to maintain self-care and to prevent complication which can cause psychological pressure. Thiskind of pressure may affect hardiness of patients in performing self-care. Hardiness may be influenced byspiritual well being. The aim of this study was to analyse the relationship between spiritual well-being andhardiness in patients with type 2 DM. This research applied correlational design with cross-sectionalapproach. A total of 112 respondents were selected by using consecutive sampling technique. The data werecollected by administering questionnaire of Spiritual Well-Being Scale (SWBS) and Hardiness Scale. Dataanalysis was performed by Spearman-rank correlation test with the significance level of 0.05. The resultsshowed that the median score of spiritual well being was 91 (72-100) while the median value of hardinesswas 104 (84-122). There was a significant positive correlation between spiritual well-being and hardiness (pvalue : 0.001 ; r : 0.303). The higher the spiritual well-being the better the hardiness of patients with type 2DM. This research reveals on the importance of assessing spiritual well-being to enhance hardiness inpatients with type 2 DM.

Abstract

Chronic bronchitis is an inflammatory disease of the bronchial respiratory tract that lasts more than 3 months or several times in 2 years. Symptoms in patients with chronic bronchitis can affect breathing patterns, including dyspnea, respiratory frequency, depth of breathing, and oxygen saturation. Providing Pursed Lip Breathing therapy can create resistance to the air coming out of the lungs and open the airways to improve breathing patterns. This study aimed to analyze the effectiveness of Pursed Lip Breathing therapy on breathing patterns in patients with chronic bronchitis. This descriptive, experimental research was conducted on a Chronic Bronchitis patient. Patient in the Inpatient Room of a government hospital. The implementation of Pursed Lip Breathing therapy was carried out for 4 days, where each day, there was one session of 8-10 repetitions of abdominal breathing followed by Pursed Lip Breathing therapy for 8 minutes. Data analysis was explained descriptively daily to compare the improvement in breathing patterns between pre and post-therapy values. The study results showed significant improvement in breathing patterns, including the level of dyspnea, depth of breathing, orthopnea, use of accessory muscles for breathing, and vital signs, including RR, SPO2, and the patient's blood pressure. This study concluded that Pursed Lip Breathing therapy effectively improved breathing patterns in Chronic Bronchitis patients. The results of this study indicate that pursed lip breathing therapy is safe to apply to patients undergoing treatment. Nurses can apply pursed lip breathing therapy and implement siki for patients with breathing pattern disorders.

Abstract

Background: Multiple Fractures or broken bones are a condition where the continuity of the bones is broken in a complex manner, this is caused by trauma caused by frequent traffic accidents. Objective: To analyze the effect of a combination of non-pharmacological therapies Quantum Spiritual Touch Therapy" (QST) and Finger Holding in multiple fraktur patients with acute pain at RSD dr. Soebandi Jember. Method: This research is an observational study with a case study design. Primary data obtained through anamnesis and physical assessment. Assessment of the effect of QST therapy and finger holding was carried out a total period of 35 minutes, 20 minutes for QST therapy and 15 minutes for finger holding therapy, based on initial, process, and final data. Results: The results showed a reduction in pain levels with a Numerical Rating Scale (NRS) was carried out for 6 meetings with the implementation of therapy once a day. Pain levels decreased in the pre-implantation measurements with an average of 5,4 to 4,7 at the time of post-implementation. Discussion: The combination of pharmacological therapy from QST therapy and finger holding aims to help the effectiveness of analgesic drugs by improving the psychological structure with implementations that contain spiritual elements.
