Abstract

Background: Hysterectomy is the most common non-obstetric surgery in adult, reproductive-age women. Hysterectomy with or without ovarian conservation is known to increase the risk of cardiovascular disease. However, only a few studies regarding its immediate and short-term effects on hypertension are available. This study aimed to determine changes in blood pressure after a hysterectomy procedure. Methods: This study is a prospective cohort study of patients who underwent a total hysterectomy procedure with or without ovarian conservation at Cipto Mangunkusumo Hospital, Indonesia, from July 2018 to July 2020. Samples were grouped into patients with total hysterectomy only or hysterectomy with ovarian conservation (HT/HTSOU) and bilateral salpingo-oophorectomy hysterectomy (HTSOB). Statistical analysis was done using paired t-test and Wilcoxon test. Results: There were 80 patients included in this study (40 for each group). A significant increase in all blood pressure components occurred at 12 months after the HT/HTSOU and HTSOB procedures (p < 0.05). After six months of the HTSOB procedure, the increase in blood pressure components only occurred in systolic blood pressure and mean arterial pressure (MAP) (p < 0.05).Conclusions: There was a significant increase in all blood pressure components in the HT/HTSOU and HTSOB group at 12 months following hysterectomy while there was a significant increase in systolic blood pressure and MAP in the HTSOB group at 6 months following hysterectomy.

Abstract

Background: Secondary amenorrhea presents a significant diagnostic challenge in gynecology. While Polycystic Ovary Syndrome (PCOS) is a common cause, rare etiologies like tuberculous endometritis can be overlooked, especially in patients with features suggestive of PCOS. This phenomenon, known as diagnostic overshadowing, can lead to delayed diagnosis and irreversible reproductive consequences. Case presentation: A 35-year-old nulligravid woman with a diagnosis of Class I Obesity presented with a five-year history of secondary amenorrhea and a desire for fertility. Her clinical and initial ultrasonographic findings were suggestive of PCOS. However, persistent amenorrhea despite hormonal therapy prompted further investigation. Hysteroscopy revealed a fibrotic uterine cavity with micropolyps, and subsequent endometrial biopsy with histopathology, polymerase chain reaction (PCR), and culture confirmed a diagnosis of tuberculous endometritis. The patient was successfully treated with a six-month anti-tuberculosis regimen, cyclical progestin therapy, and a structured lifestyle modification program. Following treatment, she experienced a significant weight loss, resumption of regular menstrual cycles, and marked improvement in endometrial and ovarian ultrasound parameters. Conclusion: This case underscores the critical importance of a comprehensive diagnostic evaluation for secondary amenorrhea, including endometrial sampling, even when a common diagnosis like PCOS is suspected. It highlights the potential for tuberculous endometritis to mimic PCOS and demonstrates the efficacy of a multidisciplinary approach in restoring menstrual and potentially reproductive function. Clinicians in tuberculosis-endemic regions must maintain a high index of suspicion for this insidious pathology to prevent long-term morbidity.

Abstract

Introduction: Uterine Fibroids (UFs) are benign smooth-muscle neoplasms typically originating from the myometrium. Hysterectomy is the most common non-pregnancy-related surgical procedure performed on women. Laparoscopy Hysterectomy (LH) is currently accepted as a safe and efficient way to manage benign uterine disease with a lower risk of trauma and morbidity. A qualitative approach, which focuses on the Quality of Life of each patient, should help navigate which treatment is more beneficial.Case Illustration: A 56-year-old woman (G0P0) came for a health screening. It was known that about 10 years ago, the patient had a history of uterine myoma. Currently, the patient has no complaints, no abdominal pain, no history of bleeding, history of regular menstruation, and no dysmenorrhea. The patient underwent abdominal laparotomy bilateral salpingo-oophorectomy hysterectomy. Methods: We searched through 3 databases: Pubmed, Cochrane, and ProQuest with the keywords: “Abdominal Hysterectomy” OR “Laparoscopy Hysterectomy” AND “UFS-QOL” with critical review based on the criteria of the Oxford Centre for Evidence-Based Medicine for Qualitative-Studies. Conclusion: Studies show that laparoscopic hysterectomy increased patients’ QOL after the procedure, which revolves around symptom elimination and pain reduction. To ensure holistic and best-practice management, an individual assessment of each patient’s condition according to their symptoms and demographics is needed. Fibroid Uterus Asimptomatik: Penanganan Terbaik untuk Meningkatkan Kualitas Hidup Wanita dengan Membandingkan Histerektomi Laparoskopi dan Laparoskopi (Evidence-Based Medicine)AbstrakPendahuluan: Fibroid Uterus (UFs) merupakan neoplasma jinak otot polos yang biasanya berasal dari miometrium. Abstract

Contraceptives play an undeniable important role in woman’s lives. It is widely accepted that the availability of both hormonal and non-hormonal modern contraceptives have improved 0 women’s lives worldwide by allowing women to control their own fertility. Indirectly, contraceptives have been shown to reduce maternal mortality, unwanted pregnancy, and overpopulation. However, the contraceptives available today may not be suitable for all users. Furthermore, there is still need to expand available current contraceptives choices especially long-term contraceptives in order to improve acceptability. As a result, several novel products such as implants, contraceptive vaginal rings, and transdermal patches have recently been introduced in family planning programs. Additional issue of contraceptive needed to be addressed is the adverse effects of hormonal contraceptives. Therefore, new combinations with an improved metabolic profile is currently being researched. Generally, the introduction of new methods with additional health benefits and less side effects would help women with their unmet needs to obtain access to a wider range of contraceptives.

Abstract

AbstractObjective: To evaluate the accuracy of office hysteroscopy in assessing tubal patency compared with hysterosalpingography. Methods: This cross-sectional study was conducted from January 2021 to May 2023 and involved 26 subjects who underwent office hysteroscopy as part of a subfertility work-up. Tubal patency was assessed using the bubble test (Parryscope test) and by measuring changes in the amount of fluid in the pouch of Douglas after the procedure. All subjects subsequently underwent hysterosalpingography as the standard method. Due to technical limitations, diagnostic laparoscopy the gold standardwas not performed. Agreement between office hysteroscopy and hysterosalpingography was evaluated using concordance rates and kappa values, along with sensitivity, specificity, positive predictive value, and negative predictive value. Pain scores during the procedure and post-procedural symptoms were also analyzed. Results: The concordance between the two modalities in assessing tubal patency was 67% (moderate agreement), with a kappa value of 0.28 (fair agreement). Office hysteroscopy resulted in lower pain scores and a reduced incidence of post procedural lower back pain and abdominal bloating. Conclusion: Office hysteroscopy demonstrates fair accuracy in assessing tubal patency compared with hysterosalpingography, with the advantages of minimal pain and no serious side effects. Keywords: HSG, office hysteroscopy, tubal patency.

Abstract

ABSTRACT Background: Acne vulgaris is a health problem experienced by 85% of people in Indonesia. The highest prevalence is in women, who attempt to treat acne vulgaris. Unsuccessful acne vulgaris therapy is associated with hormonal influences, the stimulation of the sebaceous glands by androgen hormones. Hyperandrogenism is experienced by 10% of women in Indonesia. The main therapy for hyperandrogens is ciproterone acetate. Drospirenone combined with ethinyl estradiol, apart from sparing pregnancy, is also thought to be effective in treating clinical symptoms of hyperandrogenism. Currently, there is no study on drospirenone for acne and hyperandrogen therapy in Indonesia. Objective: Determine the efficacy of treatment combination 3-mg drospirenone and 20-microgram ethinyl estradiol as management of choice for moderate-severe acne vulgaris and symptoms of hyperandrogenism (hirsutism, secondary amenorrhea, and oligomenorrhea) for 3 cycles. Method: A randomized clinical trial with a double-blind study was done. Consecutive sampling is based on a random allocation table. A total of 42 subjects were diagnosed with moderate-severe acne vulgaris with hirsutism, secondary amenorrhea and/or oligomenorrhea at RSCM, divided into 2 groups, 21 subjects using drospirenone 3 mg combined with 0.030 mg ethinyl estradiol (DRSP/EE) and 21 subjects using 2 mg ciproterone acetate and 0.035 mg ethinyl estradiol (CRPN/EE), followed with examination of free testosterone check in laboratory. There was 1 subject who dropped out (Cyproterone group). The duration of therapy was 3 months then evaluation of acne lesions, Ferriman Gallwey score (FG score), and menstrual period. Results: Characteristics of study subjects with the majority of subjects aged 25-35 years, 51,3%. The education level of most subjects about 57,1%, was a bachelor's degree. Testosterone levels at the start of the study were 1,41±0,64 for the DRSP/EE group and 1,32±0,48 for the CRPN/EE group. For the degree of acne, 85.7% of subjects using DRPS/EE and 85% of subjects using CPRN/EE had reduced acne lesions with a P value of 0.645. For hirsutism by calculating the FG score, 61.9% of the subjects using DRSP/EE and 50% of the subjects consumed CPRN/EE had a decreased FG score with a value of P value 0.443, and their menstrual period became regular for 85.7% of the subjects using DRSP/EE and 90% of the subjects using CPRN/EE with P value 0.645. Conclusion: Drospirenone and ethinyl estradiol are as effective as cyproterone acetate combined with ethyl estradiol as treatment of moderate to severe acne vulgaris, hirsutism, secondary amenorrhea, and oligomenorrhea. Keywords: acne vulgaris, cyproterone acetate, drospirenone, hirsutism, secondary amenorrhea
