Abstract

Kelengkapan pengisian rekam medis sangat penting sehingga dapat berdampak pada mutu kualitas rekam medis dan pelayanan kesehatan. Berdasarkan studi pendahuluan, pengisian rekam medis rawat inap di Puskesmas Umbulsari Jember masih belum terisi secara lengkap pada bulan Juni-Agustus 2022 dengan persentase ketidaklengkapan bulan Juni sebesar 6,70%, bulan Juli sebesar 8,92%, dan bulan Agustus sebesar 7,72%. Penelitian ini bertujuan untuk menganalisis penyebab ketidaklengkapan pengisian rekam medis rawat inap di Puskesmas Umbulsari Jember. Jenis penelitian ini menggunakan kualitatif dengan teknik pengumpulan data yaitu wawancara mendalam, observasi, dan dokumentasi. Subjek penelitian ini yaitu 1 kepala puskesmas, 1 dokter, 2 perawat rawat inap, dan 1 petugas rekam medis. Uji keabsahan data menggunakan triangulasi sumber dan triangulasi teknik. Hasil penelitian menunjukkan penyebab ketidaklengkapan berdasarkan faktor motivation (motivasi) yaitu tidak adanya reward bagi petugas yang mengisi rekam medis rawat inap. Faktor opportunity (kesempatan) yaitu belum pernah diadakannya pelatihan terkait pengisian rekam medis, belum terdapat SOP pengisian rekam medis, belum pernah diadakan sosialisasi SOP penilaian kelengkapan rekam medis. Faktor ability (kemampuan) yaitu kurangnya pengetahuan petugas dan adanya petugas dengan masa kerja baru masih dalam tahap penyesuaian lingkungan

Abstract

Incomplete filling of medical records (KLPCM) is an indicator of the quality of nursing care. Citra Husada Hospital experienced an 83.5% medical record incompleteness rate in the fourth trimester of 2022, with the highest form of incompleteness occurring in the pre-surgical assessment at 59.68%. Furthermore, the KLPCM review process still relies on Excel, which presents several drawbacks, such as non-specific columns, a lack of user-friendliness, and the need for high precision due to small input fields. As a result, the KLPCM review process is time-consuming and prone to input errors. Based on these issues, a KLPCM information system is needed at Citra Husada Hospital in Jember. The system development method used is the Prototype method, with data collection techniques including interviews, observation, and documentation. The system is web-based, developed using the PHP programming language with the Laravel framework and a MySQL database. The system's features include user management, doctor management, form and form content management, patient management, KLPCM medical record analysis, reports, and incompleteness notifications. The system testing results, conducted using black box testing, showed that all functionalities work properly.

Abstract

Existing learning and administrative conditions at the Nurul Yaqin Islamic Boarding School in Jember are currently still entirely conventional, with low levels of digital literacy among teachers and students. Based on these issues faced by our partners, this community service program aims to accelerate the digital transformation of Islamic boarding school education through the implementation of an e-learning system based on the Technological Pedagogical Content Knowledge (TPACK) framework. The activity involved four key stages: needs assessment, training on digital tools (Google Classroom, Canva, Google Forms), implementation, and mentoring. The results showed that teachers’ digital competence increased from 20% to 100%, and students’ participation in e-learning activities rose from 40% to 90%. Moreover, the digital administration system, including QR code-based attendance and digital archiving, reduced administrative time by approximately 50%. This program contributes to the development of technology-integrated pedagogy in Islamic education, the strengathening of teachers’ digital competence and instructional innovation, and the promotion of sustainable digital transformation in pesantren-based education systems.

Abstract

Health is one of the sectors that has rapidly advanced through the application of information and communication technology (ICT) under the concept of e-Health. Nutrition services at the Teaching Factory Nutrition Care Center (TeFa NCC) are still conducted manually, requiring technological innovations to enhance efficiency and accessibility. This community service aims to digitalize nutrition services to facilitate public access to information and support staff operations more effectively. The method involves a participatory approach with three main steps: application development, socialization, and monitoring and evaluation to improve the quality of nutrition services and user satisfaction. The development of the Android-based E-Stagi application has proven successful in increasing service efficiency, with 34% of users finding the application very easy to operate. Monitoring results also indicate that the application simplifies data management for staff and provides easier access to nutrition information for the public compared to traditional methods. Despite technical challenges and user unfamiliarity, further training and application updates are necessary to optimize its utilization.
