The growing rapid infrastructure jobs have been acknowledged in line with rapid development of instfrastructure project in this nation. PT BBB is a prominent construction enterprise with specialize in both corporate and government project. As such project profoundly depend on human worker, a number of accident occurred which caused delay to finish scheduled task. This study aim to mapping degree of accident risk in order to mitigate risk likelihood. Job Safety Analysis is used as research approach. This research begun in determining working activities breakdown, followed by describing and identifing hazards in each working activity, and finally propose a mitigating scenario in order to eliminate work accidents. The results showed that there were 208 accidents. Those accidents have been calculated from seven working activities breakdown. Furthermore, it is found 22 potential hazards with likelihood risk as follow : 4 potential hazards is consideredaas low level risk, 8 potential hazards is determined in moderate level risk, 6 potential hazard is classified in high level risk and finally 4 potential hazards is considered as extreme level risk.

Zero accident was considered as the primary occupational health and safety value in manufacturing and service enterprises. PT. ABC provided electrical and instrument services, including a low-voltage installation, high-voltage installation and an instrument installation. This company is known to excellent electricity and instrument service providers across the country . Moreover, men’s power is used as a primary source to maintain these kind of services. Installation projects depend not only on the high working speed, but also on high working accuracy within certain timeframes . As a result, work accidents have occurred with a classification varying from minor to fatal accidents. The purpose of this research is to identify potential hazards to assess risk levels and obtain recommendations for preventing accident. The HIRARC method is employed as a research approach. This research is initiated through Hazard Identification, Risk Assessment and Risk Control. This research identified that two hazards were classified as extreme risk, three hazards as high- risk, eight hazards as moderate risk , and two hazards have been identified as low risk . Furthermore, a number of action lists have been suggested in order to minimize accident rates and finally the working conditions are designed to maintain zero accidents.

Fertilization is an action in plant care. Fertilization provides additional nutrients for the soil. Fertilization has a large influence on plant growth and production. Fertilization consists of organic and inorganic fertilizers. Both of these fertilizers must be balanced so that the nutrient content can be maintained properly. The use of fertilizers an organic fertilizer that is widely used, but the use of chemical fertilizers on an ongoing basis will reduce the level of soil fertility. This must be balanced with organic fertilizer. One of the organic fertilizer is guano fertilizer. This fertilizer is fertilizer made from animal waste, namely bats. This fertilizer has a very good content including nitrogen, phosphorus, and potassium. Knowledgethis fertilizer will still be low, so that an introduction and practice about guano fertilizer is needed. Therefore a community service activity was carried out on the development of the manufacture of guano fertilizer. Guano fertilizer development is carried out in the form of granules (granules). The targets of this service are vocational students who are related to agriculture. Vocational students are selected as the young generation to know about organic fertilizers and are able to contribute to the implementation of the agricultural industry. Community service activities were carried out with a presentation of the theory and practice of making guano fertilizer. As a result of this activity, students learned about organic fertilizer, namely guano fertilizer and its contents, nutrient content in the soil, balance in maintaining nutrient content in the soil, the practice of making guano fertilizer, and the creation of granule form from guano fertilizer.

Secara geografis, Desa Banjarmadu terletak di wilayah Kecamatan Karanggeneng Kabupaten Lamongan, Secara umum mata pencaharian warga Desa Banjarmadu teridentifikasi ke dalam beberapa sektor yaitu petani, pedagang,dan sektor industri. Mayoritas pekerjaan masyarakat desa adalah sebagai petani.Selain itu terdapat Unit Kegiatan Masyarakat (UKM) yang dijalankan masyarakat desa Banjarmadu. Jika dilihat dari sektor kependudukan jumlah penduduk desa Banjarmadu berjumlah 3456 jiwa dengan total 1152 KK yang tercatat didesa. Dengan melihat banyaknya kepala keluarga yang ada di desa Banjarmadu maka bisa diperkirakan banyaknya sampah yang dihasilkan setiap harinya. Hasil dari program kerja yang diadakan oleh Tim Dosen Universitas Muhammadiyah Gresik dengan membuat alat “Komposter Sederhana” dan membuat plakat “Dilarang membuang sampah sembarangan” agar masyarakat bisa menyadari cara pengolahan sampah agar tidak menimbulkan dampak yang tidak baik untuk masyarakat lainnya.

The rised in number of four-wheeled vehicles in Indonesia has a positive impact on elevating business opportunities for otomotive repair services. This type of business is reside in all over region with service level vary from conventional services to advance services. Hari Jaya Teknik is a medium-sized service enterprise which focus on otomotive repairements in Gresik city. The business is experiencing with decreasing customers’ demand caused by conventional business promotion techniques and a number of business competition around bussiness location. This study aims to reformulate a marketing strategy to maintain the existence of a business that has been built for more than twenty years. SWOT and QSPM are the approaches used in this study. The research method begins by identifying internal and external factors obtained from six expert respondents, followed by the calculation of the SWOT and QSPM matrix. The results of this study found eight factors as primary consideration in order to redevelop marketing strategy. The highest factor namely strategy to both maintain and enhance the quality of service. These factors followed by calculating reasonable service costs in order to maintainbussiness reputation as well as customer loyalty. The reformulation of the eight strategies is also expected to be able to bring in new customers in the workshop in order to sustaining bussiness existence.

Zero accident merupakan faktor kunci dalam pengelolaan keselamatan dan kesehatan kerja (K3). Berbagai metode K3 diterapkan di perusahaan jasa dan manufaktur guna menghilangkan dan meminmalisir risiko pekerjaan. PT Shell Indonesia merupakan salah satu perusahaan multinasional yang menerapkan prinsip zero accident. Semua karyawan mendapatkan pelatihan secara berkala tentang K3 dalam berbagai aktivitas proyek di onshore maupun offshore. Proyek Reaktivasi terminal Shell Indonesia merupakan salah satu aktivitas onshore di Gresik. Permasalahan yang teridentifikasi yaitu tinginya potensi bahaya dan kecelakaan kerja pada aktifitas Loading dan unloading Bahan Bakar Minyak (BBM). Aktivitas tersebut memiliki potensi membahayakan pekerja dan infrastruktur perusahaan. Penelitian ini bertujun untuk menganalisis potensi bahaya sebagai upaya mitigasi kecelakaan kerja. Job Safety Analysis digunakan sebagai metode penyelesaian permasalahan. Explanatory study merupakan pendekatan yang digunakan dalam penelitian ini dengan menggunakan proyek revitalisasi sebagai studi kasus. JSA Work Sheet merupakan instrument yang digunakan dalam observasi secara langsung. Obyek peneltiian adalah karyawan sub kontraktor pada pekerjaan loading dan unloading BBM. Tahapan pengolahan data dimuali dari identifikasi penyebab kecelakkan kerja, penilaian risiko potensi bahaya dan selanjutnya adalah usula pengendalian potensi bahaya. Hasil penelitian ini didapatkan bahwa teridentifikasi empat potensi bahaya dengan klasifikasi bahaya medium dikarenakan faktor human error. Peningkatan keselamatan kerja dapat dilakukan dengan melakukan tindakan – tindakan perbaikan terhadap kemungkinan human error tersebut.

PT Semen Indonesia Plant Gresik merupakan pabrik manufaktur yang memproduksi semen di kabupaten Gresik. Pada tahun 2019, bahan baku yang diproduksi 50% sudah dilakukan proses pengolahan dari Tuban yang dikirim dalam bentuk klinker. Permasalahan yang teridentifikasi yaitu Pertama, sejak PT Semen Gresik (Persero) Tbk bertransformasi menjadi PT Semen Indonesia (Persero) Tbk, dan berperan sebagai strategic holding company membuat PT. Semen Gresik yang sebelumnya dalam memproduksi produk semen secara terus menerus, akan tetapi pada saat ini tahun 2019 aktivitas strategi proses produksi yang diterapkan oleh perusahaan adalah berubah dengan sistem Make To Order (MTO). Kedua, banyaknya waktu menganggur pada operator packer dan loader. Ketiga, tidak tercapainya jumlah nilai release yang direncanakan oleh perusahaan dengan hasil release yang sudah tercpai saat ini. Tujuan penelitian ini yaitu menganalisis beban kerja yang diperlukan untuk digunakan sebagai dasar dalam alternatif penentuan kebutuhan jumlah tenaga kerja yang optimal dengan metode Work Sampling Dan Work Load Analysis. Dari hasil perhitungan metode work sampling berdasarkan data sampling kerja random dan sampling kerja random dengan rumus excel, jumlah tenaga kerja yang dibutuhkan operator packer dan operator loader berdasarkan hasil waktu standar yaitu sama-sama membutuhkan 1 orang. sedangkan hasil dari metode work load analysis berdasarkan data sampling kerja random mampu memberikan alternatif 7 operator packer per shift. Untuk alternatif jumlah operator loader sebanyak 9 operator loader per shift. Sedangkan berdasarkan perhitungan data sampling kerja random dengan rumus excel mampu memberikan alternatif 5 operator packer per shift. Untuk alternatif jumlah operator loader sebanyak 6 operator loader per shift.

Preventive maintenance is considered as keypoint in maintenace management in order to maintain logistics’ steadiness along with pre and post production process. PT. Siam Maspion Terminal is one of the leading port’s service company which is provide logistics services on terminal port. Grab heavy eguipment is considered as the main equipment during looading and unloading process. Initial instigation found that inspite of periodically maintenance have been implemented, however, a high number of machine’s downtine have been identified. This study aims to analyze the cause and effect risk failure of grab’s components in order to mitigate alternative preventive maintenance using the FMEA approach. This research begins with downtime identification of five types of Grab heavy equipment, followed by identify the cause and effect such failure to determine ranking values based on the severity, frequency of occurrence, and detection. A number of espondents from the maintenance department were chosen as expert who filled out the FMEA questionnaire. This study found that there are six Grab components which are identified as critical component from Grab no 2 and Grab no 5. It was found that two highest failure risk are pulley and axle components. Regular maintenace recommendations based on these critical failure components are recommended in order to prevent such failure occur in the future.

Kjpp Edi Rianto and Partners is a service enterprise engaged in appraisal business services as well as consultants in property appraisal. There was problem identified in term of workers’ excess workload which causes performance to be felt to be less than optimal. Along with the development of the company and requests for appraisal from the assignor, this results in the occurrence of Overtime in the ongoing work process. Workload Analysis is a method that is often used by companies or institutions in measuring and determining the optimal amount of workforce, after observing and determining the level of productivity with the Work Sampling calculation method, from the observations, the workload data received by Surveyor 1 is obtained. (132%), Surveyor 2 (141%), and Surveyor 3 (135%), the suggestion given is to add 3 workforces of surveyors, this can reduce the workload of workers, originally Surveyor 1, whose workload was originally 132%. to 66%, Surveyor 2 whose workload was originally 141% to 70.5%, and Surveyor 3 whose workload was originally 135% to 67.5%.

Pengelolaan rantai pasokan secara tepat merupakan suatu upaya meminimalisasi potensi risiko pada suatu perusahaan jasa atau manufaktur. CV. Palet Jaya merupakan produsen palet kayu juga mengalami permasalahan yang mengakibatkan kendala pada pencapaian proses bisnis pada sepanjang aliran rantai pasok. Permasalahan yang teridentifikasi yaituketidakstabilan pasokan bahan baku dan mengakibatkan proses produksi yang kurang optimal, dan keterlambatan pengiriman produk.Penelitian ini bertujuan untuk mengidentifikasi potensi kejadian dan penyebab risiko serta menghitung dampak dari penyebab risiko di konfigurasi aliran rantai pasokan palet kayu. House of Risk merupakan metode yang digunakan dalam menyelesaikan permasalahan tersebut. Penelitian dimulai dengan melakukan konfigurasi rantai pasokan palet kayu. Penelusuran potensi dan penyebab risiko pada tahap perencanaan, proses produksi, pengiriman, pemasok bahan baku, dan pengembalian produk dilakukan menganalisis risiko yang terjadi sepanjang rantai pasokan tesebut.Hasil penelitian menunjukkan 18 potensi kejadian risiko dan 20 penyebab risiko sepanjang aliran rantai pasok produksi. Strategi mitigasi dengan menitikberatkan pada perbaikan ketrampilan karyawan, perbaikan pada proses inspeksi bahan baku dan perbaikan pada proses inspeksi alur produksi merupakan langkah tepat guna meminimalisasi risiko yang terjadi di sepanjang aliran informasi dan aliran barang rantai pasokan produksi palet kayu.
