Abstract

Penelitian ini bertujuan untuk mengidentifikasi keinginan dan kebutuhan konsumen untuk membuat perencanaan dan pengembangan produk batu onix untuk lantai. Analisis yang digunakan adalah dengan penerapan quality function deployment (QFD). Setelah melakukan survey serta wawancara pendahuluan selanjutnya dibuat suatu kuisioner untuk konsumen untuk mengetahui Voice of customer serta kuisioner utuk manajemen perusahaan. Selanjutnya data diolah sesuai dengan langkah-langkah yang ada di QFD. Hasil yang diperoleh dari penelitian ini adalah : 1) Keinginan dan kebutuhan konsumen akan produk lantai dari batu Onix berdasarkan urutan tingkat kepentingan atributnya adalah : tembus Cahaya, tahan goresan, mudah di rawat, tahan cuaca, ukuran bervariasi, mengkilat, mudah di pesan, warna bervariasi dan tahan Benturan. 2) Berdasarkan kemampuan teknis yang dimiliki perusahaan dalam memenuhi Customer need ialah  kualitas bahan baku utama, kualitas bahan baku penolong, type mesin gergaji circle,volume bahan baku utama, peralatan & mesin pendukung, type mesin poles dan ketrampilan pengrajin. 3) Pada perencanaan desain, karakteristik respon teknis yang menjadi prioritas utama dalam merencanakan suatu pengembangan dan peningkatan kualitas produk lantai dari Onix yang dapat dilakukan oleh pengrajin Batu Onix adalah sebagai berikut : kualitas bahan baku Onix dan bahan baku penolong, type mesin gergaji circle yang digunakan, volume/jumlah batu yang dipergunakan, serta peralatan dan mesin. Kata Kunci : QFD, Voice of Customer, respon teknis, house of quality.

Abstract

In carrying out its business processes, PT Barata Indonesia requires raw material supply and services from related partners to be able to complete work on time. But increasingly complex business competition makes the company inseparable from a risk that can disrupt the normal operation of the system. Therefore supply chain risk management is needed in managing supply chain activities to reduce the impact of the risk. This study uses the House of Risk (HOR) method with the GSCOR approach to minimize the occurrence of risks and potential risk events that can occur in the company's business processes. The results of the identification with the tools why why analysis and the calculation of the house of risk method, identified 82 risk events, 22 risk agents and 22 risk minimization actions. Of the 22 risk agents, 10 risk agents are prioritized based on the largest ARP value in the calculation of HOR1 and 10 risk minimization actions on HOR2 based on the greatest value of Effectiveness to Difficulty Ratio. Based on the results of the analysis of 10 priority risk minimization actions from the calculation of HOR2 obtained 5 risk mitigation strategies namely, 1) implementing the HR development strategy, 2) conducting information disclosure and a customer-focused approach, 3) Determining supplier selection policies that are green certified, 4) procurement planning inverter, 5) Good communication with third parties and partnersek).

Abstract

Pelayanan merupakan salah satu faktor kunci pada keberhasilan bisnin kuliner. Kuma merupakan usaha rintisan kafe berkonsep kembali ke alam dengan target penjualan minuman 75 porsi dalam sehari. Saat ini usaha tersebut sedang mengalami permasalahan yaitu banyaknya keluhan pelanggan yang disebarkan di media sosial. Adanya banyak persepsi dan harapan pelanggan yang tidak bisa dipenuhi oleh usaha tersebut. Untuk meningkatkan kualitas pelayanan dengan baik maka peneliti melakukan suatu penelitian dengan pendekatan Integrasi Servqual – Six Sigma. Penelitian ini bertujuan untuk melakukan evaluasi kualitas pelayanan sekaligus menentukan akar permasalahan kesenjangan antara persepsi dan harapan konsumen serta menentukan strategi perbaikan secara efektif. Hasil penelitian diharapkan bermanfaat bagi manajemen perusahaan dalam upaya perbaikan kualitas pelayanan. Define, Measure, Analyze, Improve, dan Control merupakan pendekatan yangdigunakan dalam penelitian ini. Hasil penelitian ini ditemukan 31 atribut pelayanan yang memiliki gap negatif. Atribut X24, barista selalu memberikan sikap senyum, sapa, dan santun, merupakan atribut dengan skor  gap dan sigma terkecil yaitu  -1,14 dan 1,74. Faktor manusia dan lingkungan merupakan dua hal kunci yang dapat digunakan dalam peningkatan kualitas layanan bisnis kuliner tersebut.

Abstract

CV. Citra Buana Mandiri is a service enterprise  which provide service such as retreading of used tires. During  retread production process, there were found defects which caused by external and internal factors so the final product  are not in accordance with the quality standards set by the firm.   The purpose of this research is to identify the causes of ring 20 tire defects, analyze whether the product defects are still within control limits and provide recommendations for improvements to improve the quality of ring 20 tires. Tseven tools was used as research approach with final result as follows: identifying factors that cause 1000 tire defects.  ring 20 is the factor of human, machine, raw material, environment and method, then we get 13 possible root causes of the problem.  The proportion of defects in the 1000 ring 20 tire product, it can be concluded that the product is still within control limits, but based on the company's standard tolerance, the percentage of defects that are still outside the predetermined standard is 1 percent and 13 improvement along production stages in order to maintaining excellent quality.

Abstract

Mempertahankan kesinambungan proses produksi tidak hanya bergantung pada aliran produksi tetapi juga pada kinerja peralatan di semua ukuran perusahaan. Industri rumah tangga Tahu mengalami tantangan dalam mempertahankan kelangsungan usaha karena kerusakan mesin. Kerusakan mesin penggiling menyebabkan penurunan produktivitas. Penelitian ini bertujuan untuk mengevaluasi efektivitas dari mesin penggiling tersebut. Overall Effectiveness Equipment dan Failure Mode and Effect Analisis digunakan sebagai pendekatan penelitian. Hasil penelitian memperlihatkan bahwa availability rate tertinggi adalah 89,46%, dan terendah adalah 88,24%. Nilai performance rate tertinggi adalah 94,69%, dan terendah 78,55%. Nilai OEE tersebut lebih rendah dari nilai standar rujukan 85%. Tiga peringkat skor tertinggi dari Risk Priority Number adalah 448, 336, dan 294. Skenario perbaikan adalah melatih operator untuk meningkatkan keterampilan perawatan untuk mendapatkan kinerja waktu perawatan yang lebih baik.

Abstract

Integrated maintenance management is an important factor in maintaining the stability of machine operations in manufacturing companies. PT AAAA Factory Gresik is a cement manufacturing company, where the Section Division of Gresik Finish Mill Packer Operation has a problem of downtime cement packing machine 40 kg and causes production delay. This study aims to evaluate the effectiveness of the 40kg cement packing machine as well as to calculate losses due to unproductiveness of the packing machine. Overall Equipment Effectiveness (OEE), Six Big Losses, and Failure Mode and Effect Analysis (FMEA) were used as research method. This study found that OEE value for January-December 2019 of 41.25% are considered as lower percentage in compare with the ideal value of international standards which is 85%. Moreover, the largest average losses are found, namely reduce speed losses, idle and stoppages minor losses and defects in process with percentage in following order, 4.49%, 4.49% and 2.76%.Result from FMEA analysis have been identified 3 highest Risk Priority Number (RPN), namely transportation with an RPN value of 294, the second RPN value of 288 is a blockage in the packing machine, finally the engine button is jammed with an RPN value of 252. The proposed improvement scenario is the start of the engine operating operators checking and cleaning machines regularly and the marketing division expands the sales market.

Abstract

Small and Medium sized enterprises are recognized as national safety net during crisis era in this nation. Gangsar is a small-sized enterprise and recognized as songkok home industry in Lamongan City. Maintaining sustainable quality is known as both challenge and weakness in order to sustain its business. The research purpose is to identify defect and analyze it in order to improve product quality. Failure Mode and Effect Analysis and Fault Tree Analysis were used as research method.  This research begun with mapping flow production process in order to identified defect product. Once defect product was recognized, the next stage was calculating risk priority number. Mapping fault tree was the next stage to detect failure mode. The results showed three defects identified namely trimmed with Risk Priority Number144, disorderly sewing process with RPN 126 and non-precision cutting fabric with RPN 86. These defects have been analyzed using FTA and it was found factors such as human errors, no-good material, and broken sewing machines need to be eliminated. Improving work facilities and regular training as well as working supervision are considered as better scenario in order to sustaining product quality.

Abstract

Quality control is an activity to certify that the process meets the enterprise's standards. The Quality Division of CV GGG has been encouraged to find suitable methods to decrease current larger than common defects. This research aims to evaluate the quality of the carton box production line based on the new seven tools approach. This approach begins with affinity diagrams followed by interrelationship diagrams, tree diagrams, matrix analysis diagrams, activity network diagrams, and process decision program charts. The study identified two defects: untidy paint quality and fragmented fractions throughout the production line. Furthermore, implementing the new seven tools has been distinguished primary factors causing defects. The first factor is the unavailability of manual Standard Operational procedures for maintenance schedules. The next factor is unnecessary movement during storage and transfer of materials and improper working environment on the production workshops. Several scenarios have been proposed to eliminate such defects and increase productivity.

Abstract

A service provider is known as a job that profoundly depends on human work. Musculoskeletal disorder is considered a risk in such work. PT. ABA is a contractor service company that provide some services. The fire brick installation is a part of the refractory project, which is carried out continuously at the customer's request. Early results using the Nordic Body Map questionnaire found severe risk, which may lead to wellbeing difficulty.  This study aims to evaluate musculoskeletal disorder risk due to inappropriate posture in fire brick installation activities. Rapid Upper Limb Assessment and Work Ergonomic Risk Assessment are ergonomic risk approaches used in this study. This research found two final scores using RULA and WERA calculation. RULA final score is 44, classified as a medium score. The evaluation results of the RULA method obtained a final score of 7 with an extreme condition. An urgent improvement is a necessity in order to improve posture and work positioning of fire brick installation.

Abstract

As an industrial manufacture , xyz is an enterprise  that produces mineral water should take into account in detail the processes in each production line in order to minimize the cost of activities of valueless activities. The impact of these activities is that they are excessive and inadequate to meet customer orders. The research focused  on analysing  the activities leading to waste to a glass-type production line based on a lean production approach. A case study based explanation  is a research approach. According to the current state map, according to the current state map of  wasteacross the value stream, the 'ham-filtering' of speakers and the 'cause of process  failure  on production lines' remains to be determined . Research shows that the dominant waste of the  higher sequence is deliberate, the lead time value is 4320 seconds and the cycle time is 60 seconds. Next, the average consumer product of 7.7% exceeds the company's maximum standard. Based on value stream flow map, improvement recommendations as follows: minimize waste, disposal of worthless activities,  5W1H enhancement on the fish edge  diagram and preventative maintenance activities to prevent unavoidable defects along production process activities.
