Abstract

Background Transfusion-dependent thalassemia (TDT) has a major impact on a child's growth and is associated with stunting, risk of vitamin D deficiency, and decreased insulin-like growth factor-1 (IGF-1). To date, the relationship between vitamin D levels and stunting in TDT remains unclear. Futhermore, the role of vitamin D and IGF-1 in mediating stunting in TDT patients is still unknown. Objective To investigate the relationship between stunting and vitamin D as well as IGF-1 levels in children with TDT. Methods This cross-sectional study involved 50 TDT children aged 5 to 18 years, included consecutively from the Pediatric Hemato-oncology Outpatient Clinic, Dr. Soetomo Hospital, Surabaya, East Java. Subjects were divided into two groups: stunted (S) and not stunted (NS). Vitamin D and IGF-1 were evaluated by antibody competitive immunoassay and sandwich-enzyme-linked immunosorbent assay (ELISA), respectively. Age, sex, and duration of repeated transfusion were analyzed as confounding factors. Results Median IGF-1 levels were 91.43 (13.67-192.86) ng/mL and 161.53 (17.99-363.01) ng/mL in the S and NS groups, respectively (P=0.011). Mean vitamin D levels were 20 (+ 5.71) ng/mL and 20.46 (5.25) ng/mL in the S and NS groups, respectively (P=0.765). The correlation coefficient (r) of vitamin D and IGF-I levels was not significant. Multivariate analysis showed that low IGF-1 levels, male, and longer duration of repeated transfusions were associated with stunting in children with TDT. Conclusion Low IGF-1 level is associated with stunting in children with TDT. Vitamin D is not significantly associated with either stunting or IGF-1 in children with TDT.

Abstract

Puasa Ramadhan dapat meningkatkan kebahayaan komplikasi di pasien Diabetes Melitus (DM) tipe 1, yang dapat dicegah dengankendali glikemik yang baik. Pemeriksaan fruktosamin dan albumin glikat digunakan untuk menggambarkan rerata kadar glukosa darahselama 2-3 minggu sebelumnya, sehingga lebih sesuai digunakan untuk menggambarkan kendali glikemik selama puasa Ramadhan(1 bulan). Tujuan penelitian ini adalah membandingkan dan membuktikan adanya kenasaban antara kadar fruktosamin dengan nilaialbumin glikat sebelum, pertengahan dan akhir bulan puasa Ramadhan di pasien DM tipe 1 di RSUD dr.Soetomo Surabaya. Penelitianini menggunakan desain analitik observasional di 13 pasien DM tipe 1 berusia 9–18 tahun yang menjalankan puasa Ramadhan.Pemeriksaan fruktosamin menggunakan metode nitroblutetrazolium (NBT) dengan alat Cobas Integra. Nilai albumin glikat dihitungsebagai persentase kadar albumin glikat (menggunakan metode enzimatik) terhadap kadar jumlah keseluruhan albumin serum(menggunakan metode bromcresol purple), dengan alat Proline R-910. Hasil dianalisis menggunakan uji statistik t-berpasangan dankenasaban Pearson. Tidak didapatkan perbedaan bermakna antara rerata kadar fruktosamin pertengahan dibandingkan sebelum(p=0,307), akhir dibandingkan sebelum (p=0,249) dan akhir dibandingkan pertengahan bulan puasa Ramadhan (p=0,362). Tidakdidapatkan perbedaan bermakna antara rerata nilai albumin glikat pertengahan dibandingkan sebelum (p=0,478), akhir dibandingkansebelum (p=0,285) dan akhir dibandingkan pertengahan bulan puasa Ramadhan (p=0,247). Kenasaban positif bermakna didapatkanantara kadar fruktosamin dan nilai albumin glikat sebelum (p=0,0001, r=0,952), pertengahan (p=0,0001, r=0,948) dan akhir bulanpuasa Ramadhan (p=0,0001 dan r=0,963). Kadar fruktosamin dan nilai albumin glikat dapat digunakan sebagai tolok ukur kendaliglikemik di pasien DM tipe 1 yang menjalankan puasa Ramadhan.

Abstract

Pendahuluan: Neuropati diabetik perifer merupakan salah satu komplikasi mikroangiopati pada penderita diabetes mellitus. Manifestasi neuropati diabetik perifer paling banyak adalah polineuropati simetris distal, yang menunjukkan gangguan sensorik, motorik, serta penurunan refleks tendon dengan pola length-dependent. Diabetes Mellitus tipe 1 terbanyak diderita pada penderita usia muda, dimana saraf tepi memiliki amplitudo yang relatif masih tinggi, sehingga penurunan amplitudo yang relatif kecil, masih dianggap normal. Perbandingan amplitudo saraf sural dan radial dapat mendeteksi adanya neuropati subklinis. Penderita diabetes mellitus tipe 1 dengan kadar glukosa darah yang tidak teregulasi dengan baik mempunyai risiko 25.6% terkena neuropati diabetik perifer. Tujuan: untuk mengetahui hubungan antara kadar HbA1C dengan kejadian neuropati diabetik perifer pada penderita Diabetes Mellitus tipe 1. Metode: Penelitian ini merupakan penelitian observasional analitik dengan rancangan cross sectional dengan teknik pengambilan subjek penelitian menggunakan consecutive sampling admission yang memenuhi kriteria inklusi dan eksklusi. Subjek penelitian dilakukan pengukuran terhadap kadar HbA1C, amplitudo saraf sural dan radial dan dihitung perbandingan diantara keduanya. Hasil: Pada 38 subjek penelitian didapatkan 31 subjek penelitian dengan Sural Radial Amplitude Ratio (SRAR) rendah dan 7 subjek penelitian dengan nilai SRAR normal. Hasil analisis penelitian didapatkan bahwa pada subjek dengan nilai SRAR rendah lebih besar pada kelompok HbA1C tinggi yaitu 26 subjek (83.87%) dibandingkan dengan kelompok HbA1C normal yaitu 5 subjek (16.13%). Perbedaan tersebut bermakna secara statistik dengan p= 0.021, rasio odd 6.933 (IK 95% 1.173-40.981). Kesimpulan: Terdapat hubungan negatif antara kadar HbA1C dengan kejadian neuropati diabetik perifer pada penderita Diabetes Mellitus Tipe 1.

Highlights:1. There was a significant relationship between bone age calculated by the Tanner Whitehouse II method (TW2-20) and chronological age.2. There was a slowdown in bone age in pediatric patients at Universitas Airlangga Hospital (RSUA). AbstractIntroduction: Bone age is an indicator of biological and skeletal maturity in individuals. It is different from chronological age which is calculated based on the time of birth. The differences in chronological age and bone age can indicate abnormalities in bone development. This study aimed to determine the profile of bone age in pediatric patients at Universitas Airlangga Hospital and the relationship between bone ages examined using the Tanner Whitehouse II (TW2-20) method and their chronological ages.Methods: This was a cross-sectional observational analytic study. Secondary data were collected from medical records and X-Ray examination results from Department of Radiology Universitas Airlangga Hospital (RSUA) Surabaya from January 2018 to December 2019. The data of age, gender, and history of illness were taken. From the collected X-ray results, bone age was examined using the TW2-20 method. Then, a normality test was performed using the Shapiro-Wilk for data less than 50 samples. The results showed that the data were normally distributed (p = 0.419). Data processing of the difference between bone age and chronological age was performed using the parametric paired T-test with a confidence level of 95%.Results: 32 samples were obtained from pediatric patients undergoing X-ray examinations from January 2018 to December 2019. The average bone age difference in male patients was 0.64 years old with the highest average difference found in the age range of 9-11.99 years old. The average bone age difference in female patients was 1.1 years old with the highest average difference found in the age range of 12-14.99 years old. There was a significant difference between bone age calculated using the TW2-20 method and chronological age (p < 0.001).Conclusion: Based on the comparison of the average bone age in pediatric patients and their chronological age, all samples showed deceleration of bone age in pediatric patients at RSUA. Based on the analytical study, the TW2-20 method was not suitable for the sample examined.

Abstract

HIGHLIGHTS 1. Presents a longitudinal case of a child with Organic Central Precocious Puberty (CPP).2. Focused on psychological aspect during the COVID-19 pandemic.   ABSTRACT Objective: To present a longitudinal case of a child with organic Central Precocious Puberty (CPP) that focused on medical, growth and development, and parent’s psychological aspect during the COVID-19 pandemic. Case Report: A 14-month old girl attended with major complaints of breasts enlargement and menstruation. The Tanner's stage was at A1M3P1 and the vagina showed reddish-brown spots. The patient’s bone age was advanced (3 years and 6 months). USG examination showed a corpus uterine: cervix ratio of 2:1. GnRH stimulation test showed an elevated of FSH/LH and estradiol. MRI showed an extra-axial dense mass that leads to Hypothalamic Hamartoma (HH). The definitive diagnosis of this patient was organic CPP with HH. The patient was managed with GnRH analog. Precocious puberty (PP) becomes a financial and psychosocial burden for parents. The COVID-19 pandemic adds a double burden for the parents. Conclusion: During the COVID-19 pandemic, parents with PP children had a good psychological aspect if the child was comprehensively handled with adequate motivation and psychoeducation.

Abstract

Introduction: In the COVID-19 pandemic, social restriction and lock-down policies were implemented. It has far-reaching implications in many sectors of life including health, particularly in patients with chronic diseases, one of which is Type-1 Diabetes Mellitus (T1DM). The purpose of this community service in the form of online gathering is to facilitate patients to ask questions and consult with pediatricians, due to the limitations of patients to consult directly due to the pandemic. Methods: Community service is conducted online for T1DM patients or their relatives on Sunday, September 26, 2021, from 12.00 - 13.30 WIB. The Community service was divided into several stages, including before, during, and after the online gathering.  The T1DM patients or their relatives are participants in this community services. The topic are types of Diabetes Mellitus, clinical presentation, disease progression, the diagnostic process, and its management therapy of type 1 diabetes mellitus. Results: The community service was attended by 54 parents of T1DM patient. The online gathering goes smoothly, without being constrained by something. During the online gathering, subjects actively participate in every session during the online gathering. Several questions were asked by the participants and answered well by the speakers. Conclusion: This community services activities allow participant to be able to ask question about their problem according to diabetes mellitus. Participants can consult online with pediatricians. This online gathering has a limited time, so participants can be divided into several breakout rooms with several accompanying pediatricians, so that the QnA session can be done optimally.

Highlights1. Diabetes mellitus type 1 (DM) is a chronic autoimmune illness characterized by insulin insufficiency and hyperglycemia as a consequence.2. Stable HbA1c levels may be indicative of improved growth rates. AbstractBackground: Type-1 diabetes mellitus (DM) is a chronic auto-immune disease characterized by insulin insufficiency and resultant hyperglycemia caused by the destruction of insulin-producing beta cells in the pancreas. Metabolic control in type-1 DM affects the metabolism of patients, one of which is growth status, which is influenced by growth hormone. Improved growth rates can indicate stable HbA1c levels. However, whether growth is affected by the degree of diabetic control is still debatable. Objective: This study was conducted to determine whether the metabolic control correlates with growth status of children with type-1 diabetes in the Endocrine Polyclinic at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Material and Method: Retrospective observational analysis with a cross-sectional method was conducted on 96 type-1 DM patients at Dr. Soetomo General Academic Hospital Surabaya, Indonesia. The analysis was carried out using Chi-Square test to determine whether there was a correlation. Result: From a total of 96 pediatric patients, the majority was male (54.1%), patients with high-risk metabolic control were 79.2%, and those with z-score normal category were 62.5%. The results indicated no significant correlation between growth status and metabolic control of pediatric patients, with Chi-Square test results showed p=0.421 (p>0.05). Conclusion: There was no significant correlation between growth status and metabolic control of type-1 diabetes pediatric patients at the Endocrine Polyclinic Dr. Soetomo General Academic Hospital, Surabaya, Indonesia.

Abstract

Introduction: The involvement and ability of health workers in Mandangin Island in early managing acute diarrhea in children to reduce infant mortality due to diarrhea are very important. This study aimed to determine the effect of community development Faculty of Medicine Universitas Airlangga - Dr. Soetomo General Academic Hospital Surabaya in a seminar on Early Management of Acute Diarrhea in Children using video conference to educate health care workers (HCWs) at the Mandangin Community Health Center. Material and Methods: This cross-sectional study was designed on 20 HCWs at the Mandangin community health center. Subjects were evaluated for demographic data (gender, age, occupation, last education), and the knowledge was assessed using a pre-test before the seminar was given. We used questionnaire about acute diarrhea containing 10 questions with maximum score 100. The seminar was given through video conferences with form classical lecturer and a case study on acute diarrhea, and subjects attended the seminar in Community Health Center’s Hall. We assessed knowledge after the seminar using the same questionnaire with randomization in question and answer and compared the pre-and post-test using the Wilcoxon Rank sum test. Results: Most of the subjects were midwifery, with a mean age of 32.4 ± 5.75 years old, 45% with D-3 last education and 80% were female. There was a significant difference between pre-and post-test in acute diarrhea in children (47.89±20.7 vs 64.74±22.94) (p=0.008). Abstract

BACKGROUND: Type 1 diabetes (T1D) mostly occurs due to the destruction of pancreatic beta cells due to autoimmune processes. Diagnosis of T1D can be established by examining the c-peptide levels and the markers of pancreatic autoantibodies, including glutamic acid decarboxylase 65 autoantibodies (GAD-65). However, the association between c-peptide and anti-GAD-65 toward patients’ clinical manifestations needs to be further explored. Hence, the aim of current study was to identify the association of anti-GAD65 with c-peptide and clinical characteristics in children with T1D.METHODS: Case-control study involving 47 T1D children (T1D group) and 41 healthy children (control group) younger than 18 years old was conducted. Secondary data regarding subjects’ demographic characteristics and medical history were collected from subjects, and serum blood was drawn from each subject for the anti-GAD65 and c-peptide measurement. Anti-GAD65 and c-peptide levels were measured using an Enzyme-Linked Immunosorbent Assay (ELISA) methods.RESULTS: Anti-GAD65 antibody was detected in 78.7% T1D group, while only 2.43% were detected in control group subject (p=0.0000). The c-peptide level of T1D group was 0.07±0.19 nmol/L and control group was 1.5±0.77 nmol/L (p=0.0000). The total daily dose of insulin in subjects with positive anti-GAD65 was greater than in the negative anti-GAD65 (p=0.012). The sensitivity and specificity of the anti-GAD65 were 85.4% was 66.7%, respectively.CONCLUSION: The results of this study show that anti-GAD65 was associated with total daily dose of insulin requirement in children with T1D.KEYWORDS: diabetes mellitus, type 1 diabetes, anti-GAD65, c-peptide

Abstract

Functional gastrointestinal disorders are problems that are often experienced by children, especially in isolated areas. Mandangin Island is among the secluded islands located in Sampang district.. The Department of Child Health, Faculty of Medicine, Universitas Airlangga - Dr. Soetomo General Academic Hospital held a community development that aimed to increase public knowledge about functional gastrointestinal disorders in children on Mandangin Island, Sampang District. The community service was conducted using a hybrid seminar approach, where speakers participated remotely via Zoom while the local community gathered at the Village Hall on Mandangin Island.. The speakers explained the definition, important signs and symptoms, early management of Functional Gastrointestinal Disorders. All subjects who took part in community service activities were evaluated for knowledge based on pre- and post-tests, each of which consisted of 5 questions with maximum score 100. Subject’s knowledge was considered good if the score was ≥ 60. Based on our data, there were 29 people who participated in community development with the most working as fishermen and entrepreneurs. We found that 69% of the subjects had an increase in the pre-test score. At the time the pre-test was distributed, only 10.4% had good knowledge scores, while in the post-test there were 51.7% who had good knowledge. We also succeeded in making 3 health cadres caring in childhood functional gastrointestinal disorders. Community development in the form of health education for functional gastrointestinal disorders in children using video conference (ZOOM) on Mandangin Island could increase public knowledge and this activity can be implemented on other islands.
