Abstract

The relative contributions of individual components of multimodal diabetes management to glycemic control remain unclear in real-world primary care settings. This observational pre–post study enrolled 100 adults with type 2 diabetes in a 12-week, nurse-led four-pillar program integrating patient education, physical activity guidance, dietary counseling, and medication adherence monitoring. Multiple linear regression examined the simultaneous associations between behavioral predictors and changes in HbA1c (ΔHbA1c). Mean HbA1c reduction was 2.72 ± 1.25% (95% CI: 2.48–2.96), exceeding the clinically meaningful threshold (>0.5%). The regression model was statistically significant (F(4,95) = 5.16; p = 0.0008; R² = 0.178). Medication adherence independently predicted ΔHbA1c (unstandardized B = 0.327; standardized β = 0.216; p = 0.023), whereas diabetes knowledge, physical activity, and dietary adherence were non-significant (p > 0.05). A structured, nurse-coordinated four-pillar program achieved substantial short-term glycemic improvement among adults with type 2 diabetes, with medication adherence emerging as the strongest behavioral predictor. While comprehensive, multidimensional care remains essential for sustainable self-management, systematic adherence monitoring may optimize metabolic outcomes in routine clinical practice. Causal inference is limited by the observational pre–post design and potential residual confounding; future randomized controlled trials with extended follow-up and objective adherence measures are warranted to clarify component-specific effects and long-term sustainability

Abstract

Climate change is increasingly amplifying global morbidity and mortality, disproportionately affecting vulnerable populations and placing sustained pressure on health systems. Nurses, as the largest segment of the healthcare workforce, are strategically positioned to mitigate climate change-related health impacts; however, the scope and empirical basis of these roles remain insufficiently consolidated. This scoping review aimed to map and critically examine the multifaceted roles of nurses in addressing climate-related health consequences. A comprehensive search of SCOPUS, PubMed, ProQuest, and ScienceDirect identified 15 articles eligible for inclusion in this review. Four core nursing roles were identified: (1) integrating climate change into nursing curricula and community health education, (2) advancing research on planetary health, (3) influencing climate-related health policies, disaster preparedness, and resilience strategies, and (4) providing climate-responsive care to vulnerable populations. A key finding was the predominance of normative and conceptual literature, revealing a substantial gap between expected nursing roles and empirically supported practices, particularly in direct caregiving interventions.

Abstract

Introduction: Workplace accidents are a serious problem in industrial settings, particularly in high-risk production sectors such as pharmaceutical manufacturing. This community service activity aims to improve employee knowledge and skills in providing First Aid through theory-based and practical training. Methods: The training was conducted at a pharmaceutical company in East Java on August 28, 2025, and lasted 6 hours. Participants consisted of 25 employees purposively selected by company management to represent various divisions. The method used was a one-group pre-test and post-test design, utilizing a closed-ended questionnaire containing 15 multiple-choice questions to assess first aid knowledge, as well as a practical assessment to evaluate participants’ skills. The training covered basic first aid principles and focused on three core skills: basic life support, bandaging, and casualty transportation, delivered through interactive lectures and hands-on simulations using standard first aid equipment. Results: A significant improvement in both participants’ knowledge and practical skills after the training. The mean knowledge score increased from 12.36 (pre-test) to 14.27 (post-test), representing an increase of 1.95 points. Conclusion: First aid training was effective in improving both the knowledge and practical skills of employees. A notable increase in knowledge scores, along with enhanced competencies in basic life support, bandaging, and casualty transportation, indicates that the training successfully strengthened participants’ preparedness and ability to respond to emergency situations. These findings highlight the importance of integrating structured first aid training programs in the workplace to improve emergency response capacity. Abstract

Introduction: Hypertension commonly impairs sleep quality among older adults because persistent sympathetic nervous system activation interferes with the physiological relaxation required for sleep initiation and maintenance. Although antihypertensive therapy controls blood pressure, sleep disturbances often persist, highlighting the need for effective non-pharmacological nursing interventions in primary care. This case report aimed to describe the implementation of lavender aromatherapy, guided by Kolcaba's Comfort Theory, in managing sleep pattern disturbances among hypertensive older adults. Methods: A descriptive case report was conducted involving two hypertensive older adults receiving nursing care at the Gadingrejo Health Center, Pasuruan City, Indonesia. Nursing assessment and care were based on the Indonesian Nursing Diagnosis, Intervention, and Outcome Standards (SDKI, SIKI, and SLKI). Lavender aromatherapy was administered using a diffuser containing 50 mL of water and 4–5 drops of lavender essential oil for approximately 60 minutes before bedtime over seven consecutive days, accompanied by sleep hygiene education. Data were collected through interviews, direct observation, physical examination, and nursing documentation. Results: Both participants experienced fewer nocturnal awakenings and reported improved subjective sleep quality after the intervention. One participant also demonstrated resolution of neck pain and dizziness, accompanied by normalization of blood pressure. In the second participant, nocturia remained a persistent factor limiting optimal sleep despite overall improvement. Conclusion: Lavender aromatherapy, integrated within Kolcaba's Comfort Theory, showed potential as a feasible, theory-based nursing intervention for improving sleep comfort in hypertensive older adults in primary care settings. Further studies with larger samples are warranted to confirm its effectiveness.

Abstract

Chronic pain is a common yet under-recognized complication of hypertension, resulting from sustained vascular stress, sympathetic overactivity, and impaired tissue perfusion. Celery juice (Apium graveolens) has been proposed as a nonpharmacological adjunct to interrupt this pain–pressure cycle, but community-based nursing evidence remains limited. This study evaluated the effect of home-administered celery juice on chronic pain and blood pressure among hypertensive older adults in the Gadingrejo Community Health Center, Pasuruan City, using the Gate Control Theory of pain as the theoretical framework. A quasi-experimental, non-equivalent two-group pretest–posttest design compared one adherent and one non-adherent patient over seven days. Both met identical inclusion criteria and received the same education and intervention offers. The intervention consisted of 200 mL of celery juice administered 1–2 times daily alongside standard nursing care. Pain was assessed using the Numeric Rating Scale (NRS), and blood pressure was measured with a calibrated sphygmomanometer before and after the intervention. Both patients had baseline pain scores of 6 and blood pressure of 160/100 mmHg. The adherent patient improved to a pain score of 3 with reduced blood pressure and better functional comfort, whereas the non-adherent patient improved only to a pain score of 5 with minimal blood pressure change. Regular celery juice consumption was associated with greater reductions in chronic pain and blood pressure, supporting its use as a safe, low-cost nursing adjunct when combined with sustained patient adherence.

Abstract

Background: Hypertension, or persistently high blood pressure, generally leads to a decline in sleep quality due to dizziness and reduced physical comfort. The sedative and anxiolytic properties of apigenin and the flavonoids found in chamomile aromatherapy help relax the body, making it a potential complementary therapy for addressing sleep disturbances in patients with hypertension. Objective: To describe the application of nursing care using chamomile aromatherapy for a patient with hypertension experiencing sleep disturbances. Methods: This study used a descriptive case study design on a patient with hypertension at Dr. R. Soedarsono Regional General Hospital for 3 days. The intervention was performed using a humidifier with 5-8 drops of chamomile essential oil diluted in 70-100 mL of water for 10-15 minutes. Daily sleep quality was assessed using the Pittsburgh Sleep Quality. Results: A significant improvement was observed as indicated by a decrease in the PSQI score from 12 to 5. The patient also appeared much more physically relaxed and to awaken less often at night. Conclusion: The study showed that chamomile aromatherapy effectively improved sleep quality in hypertensive patients. This method is worthy of recommendation as a safe complementary intervention to accompany conventional medical therapy.

Abstract

Stroke masih menjadi salah satu penyebab utama kematian dan kecacatan di Indonesia. Keterlambatan mengenali tanda awal stroke menyebabkan banyak penderita tidak memperoleh penanganan pada masa emas sehingga meningkatkan risiko kecacatan maupun kematian. Keluarga merupakan pihak pertama yang menyaksikan munculnya gejala stroke, namun pengetahuan yang masih terbatas sering kali menyebabkan keterlambatan dalam mencari pertolongan medis. Kader Posyandu dan anggota PKK memiliki posisi strategis sebagai penggerak kesehatan masyarakat karena berinteraksi langsung dengan keluarga. Program pengabdian kepada masyarakat ini bertujuan meningkatkan kapasitas kader Posyandu dan PKK sebagai health educator keluarga melalui Program DESA FAST. Kegiatan dilaksanakan di Desa Mojoparon dengan melibatkan 50 peserta yang terdiri atas kader Posyandu dan anggota PKK. Metode kegiatan meliputi identifikasi kebutuhan, pretest, penyuluhan interaktif, demonstrasi, simulasi metode FAST (Face drooping, Arm weakness, Speech difficulty, Time to call emergency services), pendampingan praktik edukasi keluarga, serta evaluasi melalui posttest. Peserta juga dilatih menggunakan media edukasi sebagai sarana untuk menyampaikan informasi kepada masyarakat. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan dan keterampilan peserta dalam mengenali gejala stroke, memberikan respons awal yang tepat, serta menyampaikan edukasi kepada keluarga dengan lebih percaya diri. Skor pengetahuan peserta meningkat dari 63,20 menjadi 89,40, atau meningkat sebesar 41,46%, dan hasil uji Wilcoxon Signed-Rank Test menunjukkan perbedaan yang signifikan (p=0,001). Program DESA FAST menjadi salah satu strategi pemberdayaan masyarakat dalam memperkuat deteksi dini stroke melalui peningkatan kapasitas kader kesehatan sehingga diharapkan mampu mempercepat pengambilan keputusan keluarga untuk segera membawa pasien ke fasilitas pelayanan kesehatan.
