Abstract

Background: Basal cell carcinoma (BCC) is the most common skin malignancy found in nonmelanoma skin cancer. There are multiple factors contribute to risks for BCC but the underlying mechanism is genetic alteration. The Sonic hedgehog (SHH) pathway is implicated in the etiology of the most common BCC. We reported BCC nodular type in the tip of nasal region with increase SHH. Case Illustration: 50 years old man, presented with hyperpigmented tumor on the nasal tip which easily bleed since 6 months ago. Dermatological examination: the tip of nasal region: a solitaire irregular immobile hyperpigmented tumor, with the size of 2x1x0,3 cm, pearly border with rubbery consistency. Dermoscopy examination found multiple blue-gray globule and telangiectasia feature. Histopathology examination found nodule and micronodule with palisade feature in the periphery and “slit like retraction” in central. Immunohistochemistry examination found more than 50% cell show SHH expression. Wide surgical excision was done with tumor margin 3 mm. Discussion: The hedgehog (Hh) pathway known have important role in embryonic development, that responsible for patterning development organ and tisuue. SHH is one protein ligand that most study and well known in Hh pathways. Conclusion: Activating SHH pathway can resulting in unrestricted SMO activity that causes uncontrollable cell proliferation and eventually carcinogenesis. Increased expression of SHH can be used as a marker in aggresivity of BCC.

Abstract

Background: Xerosis and pruritus (XP) are the most common skin conditions in the elderly, manifesting clinically as roughness, scales, fissures, and mild-to-severe itching. Vitamin D improves skin hydration, transepidermal water loss (TEWL), and regulates the immune system. We aimed to determine the efficiency of topical 7-dehydrocholesterol (DHC)-vitamin D3 complex cream in elderly patients with XP. Methods: An experimental phase III study was performed on elderly women with XP from December 2021 to March 2022 at the Geriatric Dermatology Clinic of Dr. Mohammad Hoesin General Hospital, Palembang. Inclusion criteria were age older than 55 years, healthy or with comorbidity. The exclusion criteria include the use of moisturizer within two weeks before the study. DHC-vitamin D3 cream was applied on subject’s volar arms once a day. Overall dry skin score (ODSS), visual analog scale (VAS) for pruritus and adverse effects, skin hydration, TEWL, sebum level, serum vitamin D level, and dermatology life quality index (DLQI) were assessed at baseline and after 4 weeks. Statistical analysis was done with Wilcoxon and paired T-test. Results:Eighteen elderly women were included. Most subjects were 60 to 74 years old (83,3%). Half the subjects (50%) had comorbidity. There was a significant difference in skin hydration (p Conclusion: Topical 7-DHC vitamin D3 complex cream is proven to improve skin barriers, increase serum vitamin D levels, well-tolerated, and provided satisfaction and comfort in elderly women with XP.

Abstract

Empowering health workers at primary health facilities in Marga Sungsang Village: introduction to skin cancer. Skin cancer is a skin malignant tumor derived from keratinocytes and melanocyte cells, known as keratinocyte carcinoma and cutaneous melanoma, the most common is keratinocyte carcinoma consisting of Basal cell carcinoma(BCC) and Squamous cell carcinoma (SCC) and rarely cutaneous melanoma (MM). The incidence has increased sharply around the world, especially in Palembang. The study in Palembang at RSMH (2017-2019) found that there was an increase of skin cancer is five times compared to previous studies. Generally, skin cancer patients referred to tertiary hospitals are in the advanced stage and accompanied by complications, increasing morbidity and mortality, making therapy difficult. so the cost of hospital care is high and becomes a burden to the state. The problem for health workers in primary health facilities is that knowledge about skin cancer is still low and this knowledge is only obtained during education level. The purpose is to improve and develop the knowledge of primary health care workers by Oncologist and skin surgeon and lecturer in Dermatology and Venereology FK Unsri / RSMH Palembang, about skin cancer and its problems, reducing the incidence and finding new cases of skin cancer, preventing skin cancer an advanced stage in primary health facilities. The counseling in the village of Marga Sungsang, Sungsang sub-district, 27 participants consist of 26 women (96,3%) and 1 male (3,7%) participants, with varied in age, the youngest was 18 years and highest was 44 years old. Education level: SMA as 20 participants (74.0%) and S1 / AMK level as 7 participants (25.9%). The results of the pretest and posttest knowledge showed a 5-fold increase posttest compared to the pretest. The knowledge of skin cancer health workers is still low. Counseling is needed for all health workers and community leaders in the Banyuasin district.

Abstract

Diabetes mellitus (DM) is a chronic and complex disease that affects various parts of the body. It can lead to multiple systemic complications and also cutaneous manifestation. Diabetic foot ulcer (DFU) is one of the most devastating complications of DM in dermatology. The main etiology is an increase in plasma glucose, risk factors, or comorbidities due to DM itself. Neglected DFU can lead to further complications, including high amputation and mortality rates; thus, the healing of ulcers is the main objective of the treatment. Management is divided into the standard of care and adjuvant therapies. This study aims to optimize DFU management, so it can provide proper treatment and prevent complications.

Abstract

Mucocutaneous manifestations are the first recognized human immunodeficiency virus (HIV) infections. More than 90% of HIV-infected patients develop at least one or more types of typical or atypical skin lesions during the disease. Mucocutaneous manifestation can also indicate the progression of HIV infection and cluster of differentiation 4 (CD4) cell count as an indicator of the development of acquired immunodeficiency syndrome (AIDS). It is important to know mucocutaneous manifestations and their correlation with the patient’s immune status to provide appropriate treatment to improve the patient’s quality of life.

Abstract

Urtikaria Kronis Spontan (UKS) berdampak signifikan pada kualitas hidup pasien. Autologous Plasma Skin Test (APST) dan C-reactive protein (CRP) merupakan skrining praktis pada UKS. Hasil positif disebab- kan oleh sitokin inflamatorik saat degranulasi sel mast dan dapat berkorelasi dengan keparahan UKS. Studi analitik observasional dengan desain potong-lintang dilakukan selama bulan Juli-September 2021 melibatkan lima puluh pasien UKS berusia 17-51 tahun. Keparahan penyakit dinilai menggunakan Urticaria Activity Score (UAS7). Hasil APST dinilai berdasarkan kriteria Sabroe. Nilai serum CRP diukur menggunakan high sensitiv- ity CRP (hsCRP). Hasil APST positif ditemukan pada hampir semua pasien (95%) dan hanya empat pasien dengan CRP positif (8%). Keparahan berkorelasi bermakna terhadap diameter urtika (P = 0,038) dan nilai CRP (P = 0,041). Analisis odds ratio (OR) menunjukkan bahwa pasien dengan hasil APST positif memiliki risiko 32 kali mengalami UKS sedang-berat. Tidak ditemukan korelasi bermakna antara nilai APST dan serum CRP pada UKS.

Abstract

Background: Snakehead fish (Channa striata, CS) has traditionally been used in Indonesia as a nutrient to promote wound healing. It contains protein highly similar to human protein, which may be involved in wound healing via biological pathways. We aim to determine the effectiveness of the topical 5% CS extract gel in promoting wound healing. Methods: A double-blind, placebo-controlled randomized clinical trial was conducted from June to August 2022 at the Department of Dermatology, Venereology, and Aesthetics of Dr. Mohammad Hoesin Hospital. We allocated 32 post-skin surgery participants with grade I acute superficial wounds on the face, neck, trunk, and extremities into 2 groups with one receiving 5% CS (n=16) and the other receiving placebo (n=16). Both groups applied the treatments twice daily. Assessments of wound healing using modified REEDA (redness, edema, ecchymosis, drainage, approximation) score, wound size, and adverse effects were done at follow-up days 1, 3, and 7. Results: Both groups showed REEDA score improvements on days 1, 3, and 7, with significant difference between both groups identified on day 3 (p=0.038). Wound size improved at each follow-up in all participants, with the 5% CS group being slightly better than the placebo group on day 3 (p>0.05). A mild adverse event was found in only one participant in the 5% CS group (3.1%). Conclusion: Both 5% CS extract gel and placebo had good safety profile and were effective in promoting wound healing. 5% CS extract gel was better at reducing REEDA score.

Abstract

Background: Post-acne hypertrophic scars and keloids result from abnormal wound healing within the pilosebaceous unit. Ruptured microcomedones create wounds that can lead to prolonged inflammation, increased collagen synthesis, and fibrinolysis inhibition, resulting in elevated scar tissue in acne lesions. Vitamin D deficiency can lead to prolonged inflammation and damage to dermal collagen. High total cholesterol levels can lead to excessive sebum production, exacerbating inflammation and fibrosis. This study aimed to determine the association between serum vitamin D and total cholesterol levels with scar severity in patients with post-acne hypertrophic scars and keloids. Methods: This analytical cross-sectional study was conducted at Dr. Mohammad Hoesin General Hospital, Palembang, South Sumatera, Indonesia, among patients who met the inclusion and exclusion criteria. Scar severity was assessed using the Vancouver Scar Scale (VSS). Blood samples were collected for examination of serum vitamin D and total cholesterol. The data were analyzed statistically. Results: Chi-square test of serum vitamin D with VSS scores showed a significant association (p-value = 0.007), while total cholesterol with VSS showed no association (p-value = 1.000). Odd-ratio (OR) for serum vitamin D and VSS was 60, concluding that vitamin D deficiency increased the risk of high VSS by 60-fold in study patients. Conclusion: There was a significant association between serum vitamin D levels and VSS score, while there was no association between total cholesterol and VSS score. Deficiency of serum vitamin D is associated with a higher risk of developing a higher VSS score in patients with post-acne hypertrophic scars and keloids.
