Abstract

Case-based learning (CBL) is a newer modality of teaching healthcare, combining medical theory and practices. Many researchs reported effectivity of CBL to improve student-centered learning. The aim of this study is to evaluate how CBL improved students’ clinical reasoning compared with other method such as problem-based learning. An interventional research involved 4th grade medical students receiving PBL and CBL methods supervised by a tutor or instructure. An alternative method combining CBL and structured role play (in an Integrated-Patient Management method) also performed using clinical scenario with medical practice setting. The result showed that student received CBL had better clinical reasoning indicated from better miniquizz result compared with PBL students (p 0,05, Mann Whitney U test. Students exposed with CBL had final MCQ and essay exam grade compared with unexposed students. Students exposed with CBL also had better clinical skill indicated from Objective Structure Clinical Examination (OSCE) result (p 0,05, independent t test). Students’ acceptance level on CBL method is 79, 4%. It is concluded that compared with PBL, CBL is more effective in improved students’ performance, learning outcomes and clinical reasoning and also had high acceptance among medical students.

Abstract

Kafein dipergunakan secara luas dalam konsumsi sehari-hari dan di bidang pengobatan. Kafein meningkatkan kadar katekolamin, merangsang pertumbuhan dan proliferasi otot polos dan dapat merangsang sistem saraf simpatis. Tujuan penelitian adalah mengetahui pengaruh pemberian kafein terhadap ketebalan tunika muskulartis eksterna rectum embrio Gallus-gallus strain ISA brown. Metode penelitian adlah eksperimental dengan menggunakan tigapuluh butir telur Gallus-gallus strain ISA brown fertil dengan berat 60-70 gram dikelompokkan dalam 2 kelompok,  15 butir dalam perlakuan “caff” yaitu diberi kafein 0,1 mg/ml setiap hari sejak hari ke-4 masa inkubasi sampai hari ke-8 inkubasi dan  kelompok kontrol mendapat aquabidestilasi dalamn waktu yang sama. Hari ke-19 inkubasi, hewan coba dibedah dan diambil rectum, diukur ketebalan tunika musculartis eksterna (stratum sirkuler dan stratum longitudinal) dibawah mikroskop. Hasil penelitian menunjukkan rerata ketebalan tunika muskularis rektum adalah 54,5313 µm (stratum longitudinal,“caff”)  dibanding 42,0833 µm (stratum longitudinal, “kontrol”) dan  74,2969 µm (stratum  sirkular “caff”) dibanding  72,7083 µm  (stratum sirkular, “kontrol”) tetapi tidak ada perbedaan yang signifikan antara kelompok perlakuan dengan ketebalan tunika muskularis rektum stratum longitudinal (t=0,442) dan stratum sirkular (t=0,135) secara statistik. Kesimpulan terdapat peningkatan ketebalan tunika muskularis eksterna rektum embrio Gallus-gallus strain ISA brown setelah pemberian kafein walaupun  secara statistik tidak signifikan.

Abstract

Diabetes mellitus is a disease that often causes diabetic nephropathy complications due to persistent hyperglycaemia. Phaleria macrocarpa (scheff. Boerl)  is one of the plants that has been widely used in the treatment of diabetes mellitus, but its effect on the risk of diabetic nephropathy is still unknown. A dose of 300mg / 200gbb / day is known as an effective dose that can significantly reduce blood sugar levels in diabetic rat. The Streptozotocin Effect (STZ) causes damage to the pancreas and causes hyperglycaemia. This study aims to compare the effect of Phaleria macrocarpa (scheff. Boerl) fruit extracts on urea and creatinine levels as a parameter of kidney function in hyperglycaemic mouse models. The study was carried out experimentally using the post-test only with control group design. Fifteen white rats were divided into 3 groups. All rats were induced with 40 mg / 200gbb of Streptozotocin (STZ) to experience hyperglycaemia. Group I as negative control was given aquades. Group II was given ethanol extract of Phaleria macrocarpa (scheff. Boerl) with a dose of 300 mg / 200gbw / day. Group III was given Metformin at a dose of 150 mg / 200gbw / day. On the 22nd day of treatment, blood specimens were taken for examination of urea and creatinine levels. The research data were statistically analyzed with α <0.05; CI95%. The mean of urea levels in Group I (79.80 ± 25.09 mg / dl), II (76.00 ± 22.59 mg / dl and III (59.60 ± 6.35 mg / dl). Kruskal-Wallis test results showed no significant difference in urea levels between treatment groups (p value = 0.273; CI95%). The mean of creatinine levels in Group I (0.68 ± 0.07 mg / dl), II (0.63 ± 0.14 md / dL) and III (0.98 ± 0.25 mg / dL). One Way Anova and Post hoc test results showed a significant difference in mean creatinine levels between Groups I and III (p = 0.014; 95% CI) and II with III ( p value = 0.006; CI95%). the results of this study can be concluded that the extract of Phaleria macrocarpa (scheff. Boerl) fruit flesh at a dose of 300 mg / 200gbb has better effectiveness than metformin dose 150 mg / 200gbb in repairing the kidney function of hyperglycaemia rats.

Abstract

Background: Nowadays, cardiovascular disease caused by hypercholesterolemia has become the main cause of death. Propolis has been used widely to reduce plasma cholesterol levels.Objective: The aims of this research was to study the effect of propolis on lipid profile of hypercholesterolemic Sprague Dawley rats.Method: This was an experimental study with pre-post test. Twenty four (24) male Sprague Dawley rats aged 12-16 week old, weighing 125-200 g were allocated into 4 groups. Group I received standard meal + aquadest-gavage; group II received high cholesterol meal + PTU 0,01 + aquadest gavage; group III received high cholesterol meal + PTU 0,01 + 0,027 g propolis gavage; group IV received high cholesterol meal + PTU 0,01 + 0,054 g propolis gavage. Total cholesterol, triglycerides, HDL cholesterol and LDL cholesterol levels before and after treatment were measured. The data were then analyzed with One Way Anova.Results: The study showed that there were no significant differences in changes of body weight. There were significant differences in total cholesterol levels between all groups of treatment. Triglyceride levels were significantly different among all groups, except between group I and IV. Furthermore, the HDL cholesterol levels of group I vs III and group I vs IV were significantly different. However, there were no differences found in LDL cholesterol levels among all groups of treatment.Conclusion: Provision of 0,027 g and 0,054 g propolis improve lipid profile (total cholesterol, triglyceride and HDL cholesterol levels) of hypercholesterolemic rats.

Abstract

Phaleria macrocarpa (Scheff.) Boerl. has been widely used in the treatment of diabetes mellitus but its effectiveness is still inconclusive so it needs to be compared with standard drugs such as metformin. This study aims to analyze the effects of P. macrocarpa fruit extracts on blood sugar levels and the repairing of the pancreatic Langerhans islets of diabetic rat models. The study was conducted experimentally by pre and post-test design with a control group. Thirty white rats were divided into 6 groups. All rats were treated with 40 mg/200 g BW of streptozotocin (STZ) to induce hyperglycemia, except group I. Group II as a negative control was given with aquadest. Group III, IV, and V were given with ethanol extract of P. macrocarpa pericarps at doses of 200, 250, and 300 mg/200 g BW/day, respectively. Group VI was given with metformin at a dose of 150 mg/200 g BW/day. On the 22nd day of treatment, blood specimens were taken for examination of fasting blood sugar levels and pancreatic organs were analyzed for histopathological examination of the islets of pancreatic Langerhans. The data were statistically analyzed with α<0.05; CI95%. Paired sample t-test results showed a significant decrease in blood sugar levels before and after treatment in group V (p-value=0.048). One way anova and post hoc Least Significant Difference (LSD) test results showed a significant difference in mean blood sugar levels after the final treatment between groups I and II (p-value=0.013), I and III (p-value=0.004), I and IV (p-value=0.002), I and VI (p-value=0.000), II and VI (p-value=0.031), and V with VI (p-value=0.005); one way anova and post hoc LSD test results showed a significant difference in the mean area of islets of pancreatic Langerhans between group I and all treatment groups (p-value=0.000), II with V and VI (p-value=0.000 and 0.009), III with V and VI (p-value=0.000 and 0.001), and IV with V (p-value=0.003). It can be concluded that the extract of P. macrocarpa pericarps showed a hypoglycemic effect and repaired the damage in Langerhans islets of STZ-induced rat in a dose-dependent manner.

Abstract

Aging is a complex biological process involving molecular, cellular and organic changes. Aging process is not merely caused by chronological age but it can be accelerated by environmental factors contributes to oxidative stress. Aging in male reproductive system is characterized by many conditions such as terticular atrophy, decreases testicular function of spermatogenesis, decreased testicular function of testosteron production which can lead to a serious clinical condition, infertility. Management of male infertility with excellent outcome is still a challenge and therefore new strategies need to be examined. Because of difficulties and ethical issues in human research, animal models of male reproductive aging are useful research tools for this purpose. Rodent models of male reproductive aging are the first choice because similarity of metabolism system and short lifespan. D-galactose animal model for aging seems to be closer to human aging of male reproductive system. It is an advantageous model for testing potencies of pharmacological agent and natural compounds on aging.

Abstract

Background: Global prevalence of Diabetes Mellitus (DM) is predicted to be increasing with worldwide population increase, longer life expectancy, urbanization, change in lifestyle and higher incidence of obesity. Diabetes Mellitus which is characterized by increasing level of blood glucose can induce increasing activity of ROS (Reactive Oxygen Species) which can further lead to abnormality in male reproductive system, especially testis. Mahkota dewa (Phaleria macrocarpa (Scheff.) Boerl.=PM) contain flavonoid, alkaloid and saponin which can prevent further negative effect on ROS increase in male reproductive system. The aim of this study was to know effect of PM mesocarp infusion on testicular damage and sperm count in alloxan induced diabetic rat.Design and Method: The design of this study was post test only with control group design. Rats in this study were divided into 4 groups (6 rats each group): control group, diabetic group, diabetic group treated with PM mesocarp infusion (9% and 13.5%). Diabetes was induced by alloxan 50 mg/BW single dose injection intramuscularly. PM mesocarp’s infusion was given in 4 weeks. Testicular damage was observed in hematoxylin-eosin stain using Johnsen criteria for spermatogenesis level and tubulus seminiferous’ diameter. Sperm count was performed on the semen taken from vas defferens using Giemsa stain.Result: PM mesocarp infusion group has higher spermatogenic level (p < 0,05), higher tubulus seminiferus diameter and sperm count (p < 0,05) than diabetic group.Conclusion: Mahkota dewa mesocarp infusion improved testicular damage and sperm count in alloxan-induced diabetic rat (Sains Medika, 4(2):115-123).

Abstract

BackgroundHyperglycemia interrupts wound healing, causing persistent and non-healing wounds. Phaleria macrocarpa extract (PME) has anti-diabetic, anti-inflammatory, antimicrobial, and antioxidant properties. This study aimed to assess P. macrocarpa activity on skin wound healing in diabetic rats. MethodsAn experimental study performed on 25 male Wistar rats. Ointments were prepared by adding vehicle (w/w) to PME at the desired concentration. Diabetes was induced by injecting rats with nicotinamide (NAD) 230 mg/kg and streptozotocin (STZ) 65 mg/kg. After hyperglycemia was confirmed, animals were randomly grouped into: i) normal rats, ii) diabetic rats; iii) diabetic rats + 2.5% ointment; iv) diabetic rats +5% ointment; and v) diabetic rats +10% ointment. Full-thickness skin wounds were induced on the dorsum and treatment was applied daily for 3 and 7 days, respectively. On days 4 and 8, wound closure was measured and animals were sacrificed for tissue samples. Wound healing was evaluated by measuring malondialdehyde (MDA) in tissue homogenates of the dermal wounds and analyzing histological changes by hematoxylin-eosin and Sirius-red staining. ResultsPME 10% ointment improved MDA levels and wound closure of inflammatory and proliferation phases. In inflammatory phase, 5% and 10% ointment reduced inflammation severity compared with diabetic rat group (p<0.05). In proliferation phase, PME 10% ointment group had a higher wound histological score (characterized by epidermal regeneration, fibroblast count, granulation tissue, and angiogenesis), and higher collagen bundle density compared with untreated groups (p<0.05).  ConclusionsTopical P. macrocarpa improves inflammatory and proliferation phases of excision wound healing in type 2 diabetes.

Abstract

Purpose: Retinoblastoma is the most common primary intraocular tumour in children. Its prognosis varies depending on the stage of diagnosis. However, in developing countries such as Indonesia, retinoblastoma still has a poor prognosis as most patients present at advanced stages. This research aims to explore the factors influencing the delay in patients with retinoblastoma reaching referral health facilities from the perspective of their families. Methods: A qualitative study using a phenomenological approach was conducted to gather family perspectives through in-depth interviews via WhatsApp calls. Each interview lasted 20-25 minutes and included families of retinoblastoma patients who had visited the eye clinic at Sardjito General Hospital at a late hour. “Late” was defined as tumour spread beyond the eyeball (intraorbital, spread to the optic nerve, or intracranial spread) or involvement of more than one eye. Audio was transcribed verbatim using a mobile phone voice recorder, and open coding was performed using Open Code 4.02 software to identify meaning units, codes, and categories. Triangulation and peer debriefing were used to ensure validity and trustworthiness. Results: Several categories emerged, including reasons for seeking help, trust in medical professionals, family compassion, and challenges in health facilities. Challenges in health facilities, particularly diagnostic capabilities, communication, and education about eye tumours, along with negative emotions like denial from families, contributed to delays in seeking treatment for retinoblastoma patients. Conclusion: Family compassion plays a positive role in encouraging families to seek medical help. However, challenges in health facilities, particularly in diagnosis, communication, and education about eye tumors, as well as family denial, contribute to the delays in retinoblastoma patients reaching referral health facilities.

Abstract

Prevalensi global diabetes melitus yang semakin meningkat memunculkan masalah kesehatan, sosial dan finansial. Kondisi hiperglikemia pada diabetes akan memicu serangkaian proses yang akan menyebabkan timbulnya komplikasi. Karena isu etik pada penelitian dengan subyek manusia, model hewan coba untuk studi penyembuhan luka diabetik menjadi perangkat penelitian yang sangat bermanfaat. Telaah literatur ini dilakukan untuk membahas mengenai model hewan coba diabetes serta komplikasi diabetes pada ginjal dan kulit. Tinjauan pustaka dilakukan dari beberapa jurnal yang terbit mulai tahun 2014-2024. Pencarian dilakukan dengan kata kunci: “animal model”, “diabetes mellitus”, diabetic nephropathy”, “wound healing” dan “diabetic complication”. Sumber dari penelitian kepustakaan ini terdiri dari artikel dengan kriteria inklusi: naskah dalam Bahasa Inggris, memiliki Digital Object Identifier (DOI), merupakan studi mengenai diabetes dan komplikasinya. Literatur dieksklusi apabila tidak dalam bentuk naskah lengkap, dan merupakan tulisan pra-cetak. Kondisi diabetik dapat diserupakan dengan melakukan modifikasi genetik, pembedahan dan induksi kemikalia ataupun biologis. Kondisi nefropati diabetik dapat diserupakan dengan menambahkan teknik pengangkatan ginjal unilateral atau mempertahankan kondisi hiperglikemia selama beberapa waktu hingga muncul penurunan fungsi ginjal. Luka diabetik dapat diinduksi dengan bermacam teknik seperti metode insisi dan eksisi. Yang menjadi simpulan dari tinjauan pustaka ini adalah terdapat beraneka banyak ragam metode untuk menyerupakan kondisi diabetes melitus pada model hewan coba. Baik diabetes tipe 1 dan 2 dapat diserupakan dengan baik pada hewan model maupun komplikasi jangka panjang berupa nefropati diabetik dan gangguan penyembuhan luka Kata kunci model hewan coba; diabetes melitus; komplikasi; nefropati diabetik; luka diabetik
