Abstract

Background: Leopard syndrome is a rare case, only around 200 cases has been reported worldwide.  Leopard syndrome is abbreviation for multipel Lentigines, Electrocardiographic conduction, Ocular hypertelorism, Pulmonary stenosis, Abnormality of genitalia, Retardation of growth, and sensorineural Deafness. This disorder suggests a possible relation between PTPN11 gene mutations and distinct clinical features. Purpose: This case is presented so that ENT specialists could identify signs and symptoms of Leopard Syndrome which manifest as sensorineural hearing loss (SNHL). Case: We report a 29 year old woman with multiple lentigines, scoliosis and atrial septal defect. She has 4 year old twin boys, one of them has cryptorchidism and a 10 month old girl with asymmetric septal hypertrophy and they also have multipel lentigines. They were referred to ENT Department for auditory function screening since  Leopard syndrome is suspected. The audiometry of the mother reveals mild conductive deafness (40 dB)  in right ear due to tympanic membrane perforation. Audiometry of the twin boys reveals sensorineural hearing loss above 4000 Hz frequency. Brainstem Evoked Response Audiometry (BERA) of the daughter reveals mild sensorineural hearing loss (40 dB )on right ear. Genogram shows that the disorders is dominant autosomal inherited. Management: Periodic auditory examination for sensorineural hearing loss is recommended since delayed onset could occur. Conclusion: Sensorineural hearing loss is a mani-festation of Leopard syndrome that should be assessed early and periodically to detect delayed onset. Keywords: Sensorineural hearing loss (SNHL), Leopard syndrome, generalized lentiginosa.    Abstrak :  Latar belakang: Sindroma Leopard merupakan kasus yang jarang ditemukan dan dari publikasi yang ada, hingga saat ini hanya terdapat 200 kasus di seluruh dunia. Sindroma Leopard merupakan singkatandari Lentigines multipel, Electrocardiographic conduction, Ocular hypertelorism, Pulmonary stenosis,Abnormality of genitalia, Retardation of growth and sensorineural Deafness. Kelainan ini disebabkan olehmutasi gen PTPN11 dengan gambaran klinis yang khas. Tujuan: Kasus ini diajukan agar spesialis THTmengenali gejala sindroma Leopard yang dapat melibatkan gangguan pendengaran berupa sensorineuralhearing loss(SNHL) sehingga tidak terjadi keterlambatan dalam penatalaksanaan. Kasus: Perempuanberusia 29 tahun dengan lentiginosa multipel, skoliosis dan defek septum atrium. Ia memiliki 2 anaklaki-laki kembar yang salah satunya mengalami kriptorkismus dan anak perempuan usia 10 bulan yangmenderita hipertrofi septum asimetris dan juga menderita multipel lentiginosa. Mereka dikonsulkan ke THT dari bagian kulit RSCM untuk pemeriksaan fungsi pendengaran dengan kecurigaan sindromaLeopard. Pada pemeriksaan audiometri diperoleh hasil pada ibu berupa tuli konduktif ringan (40 dB)telinga kanan akibat perforasi membran timpani. Dua orang anak kembar menunjukkan hasil tuli sarafdi atas frekuensi 4000 Hz. Pemeriksaan Brainstem Evoked Response Audiometry (BERA) pada anakperempuan menunjukkan tuli saraf ringan (40 dB) di telinga kanan. Genogram menunjukkan kelainanautosom dominan. Penatalaksanaan: Tindak lanjut berupa pemeriksaan pendengaran secara berkaladianggap penting untuk mendeteksi terjadinya awitan lambat. Kesimpulan: SNHL merupakan salah satumanifestasi sindroma Leopard yang perlu diperiksa untuk mendeteksi terjadi awitan lambat. Abstract

Latar belakang: Anak tuli prelingual akan kehilangan fungsi mendengar dan bicara, sehingga akan berpengaruh pada komunikasi, psikologis, dan kualitas hidupnya. Implan koklea hadir sebagai alat habilitasi terutama pada anak dengan tuli derajat berat dan sangat berat. Tujuan: Membahas karakteristik anak 6-12 bulan pasca implantasi koklea, evaluasi perkembangan auditori anak, serta faktor-faktor yang memengaruhi hasil keluaran. Metode: Bersifat deskriptif potong lintang, pada 36 subjek yang dievaluasi dengan metode penilaian berupa pengamatan yang bersifat global yaitu Categories Auditory Performance (CAP)-II. Hasil: Median waktu saat evaluasi 8,9 bulan, median CAP-II pada subjek ialah 3 (minimal 2-maksimal 7), dengan pencapaian 33,3% subjek dalam kategori CAP tinggi (skor 5 atau lebih). Kesimpulan: anak pasca implantasi koklea memiliki peningkatan persepsi auditori CAP-II dibandingkan sebelum penggunaan implan koklea yang dapat mulai dievaluasi sejak 6 bulan pasca implan. ABSTRACT Background: Prelingual deaf in children will cause the sufferers to loose their function of hearing and speaking, thus, are unable to hear and speak, and impacting on their ability to communicate, to grow psychologically, and on their overall life quality. Cochlear implant comes as a habilitating device especially mainly for children with severe and profound deafness. Purpose: This study described the children’s characteristics at 6-12 months after cochlear implantation, by evaluating their auditory development, and other factors influencing the output. Method: A descriptive cross sectional study, conducted on 36 subjects, evaluating the subjects by using the global observation method of Categories Auditory Performance (CAP)-II. Results: The median hearing age was 8.9 months, the median score of CAP-II was 3 (minimum 2-maximum 7), and 33.3% of subjects reached high CAP scores (CAP score of ≥ 5). Conclusion: CAP-II score would increase in children

Abstract

Background: Bartter syndrome is a rare inherited case characterized by autosomal recessive and has few different types. Diagnosis is established by laboratory findings, namely hypokalemic metabolic alkalosis, and normotensive. Hearing loss indicates Bartter syndrome type IV. Purpose: To report a case of Bartter syndrome with delayed speech. Case report: A seven years old girl with delayed speech and recurrent hypokalemia was referred to the Otolaryngology Head and Neck Surgery Department, Dr. Cipto Mangunkusumo Hospital, in order to evaluate the hearing level and treatment needed. Based on the Otoacoustic Emission (OAE), Brainstem Evoked Response Audiometry (BERA), and Auditory State Steady Response (ASSR), the diagnosis was profound bilateral sensorineural hearing loss and proceeded with hearing aid and also speech occupational therapy. Clinical question: Is there a relationship between Bartter syndrome and the incidence of hearing loss? Review method: Literature review through PubMed, Cochrane, and EBSCO, using keywords such as the impacts of Bartter syndrome on hearing loss, and sensorineural hearing loss in Bartter syndrome case. Result: Following screening of double publication and based on clinical questions over the past five years, only one relevant literature was found. Conclusion: Audiological assessment should be done in all Bartter syndrome’s cases. Early intervention and timely audiological rehabilitation could improve the quality of life of such children.ABSTRAK Latar belakang: Sindrom Bartter merupakan kasus jarang, yang diturunkan secara autosomal resesif dan terdiri atas beberapa tipe. Diagnosis ditegakkan berdasarkan temuan laboratorium yaitu hipokalemi, alkalosis metabolik dan tekanan darah normal. Adanya gangguan pendengaran merupakan sindrom Bartter tipe IV. Tujuan: melaporkan kasus Sindrom Bartter dengan keterlambatan bicara. Kasus: Dilaporkan satu kasus anak perempuan berusia tujuh tahun dengan gangguan bicara dan terdapat riwayat hipokalemia berulang yang dirujuk ke Departemen Telinga Hidung Tenggorok-Bedah Kepala Leher, Rumah Sakit Cipto Mangunkusumo untuk menilai ambang dengar serta tatalaksana selanjutnya. Berdasarkan hasil pemeriksaan Otoacoustic Emission (OAE), Brainstem Evoked Response Audiometry (BERA) dan Auditory State Steady Response (ASSR) ditemukan tuli sensorineural sangat berat bilateral yang kemudian ditatalaksana dengan pemakaian Alat Bantu Dengar dan terapi wicara dan okupasi. Pertanyaan klinis: Apakah ada hubungan antara sindroma Bartter dengan insidens gangguan pendengaran? Telaah literatur: Pencarian literatur melalui, PubMed, Cochrane dan EBSCO dengan kata kunci hubungan gangguan pendengaran pada sindrom Bartter, gangguan pendengaran sensorineural pada sindrom Bartter. Hasil: Setelah dilakukan skrining yakni publikasi ganda dan sesuai pertanyaan klinis dari lima tahun terakhir hanya didapatkan satu literatur yang relevan. Kesimpulan: Pemeriksaan pendengaran harus dilakukan pada kasus dengan sindrom Bartter. Intervensi dini dan rehabilitasi audiologi yang tepat waktu dapat meningkatkan kualitas hidup anak dengan sindrom Bartter.Kata kunci: sindroma Bartter, tuli sensorineural.

Abstract

Latar belakang: Variasi performa pasca implan koklea menunjukkan adanya variasi patologi yang belum dapat dijelaskan melalui pemeriksaan standar pra-operasi. Untuk itu dibutuhkan pemeriksaan yang dapat menunjukkan variasi integritas jaras auditori sebagai petunjuk variasi potensi internal tiap individu. Tujuan: Mendapatkan variasi respons jaras auditori terhadap stimulus listrik pasca implan berdasarkan pemeriksaan Implanted Electrically evoked Auditory Brainstem Respons (Imp-eABR), serta korelasinya dengan faktor internal pra-implan lain berupa usia, luas area n. koklearis dan kemampuan komunikasi pra-operasi. Metode: Kriteria subjek penelitian adalah pasien operasi implan koklea berusia kurang dari 10 tahun, memiliki koklea dan n. koklearis normal, dan pada pasien berusia lebih dari 4 tahun sudah memakai alat bantu dengar minimal 1 tahun. Pemeriksaan Imp-eABR dilakukan terhadap 5 elektrode yang mewakili area apikal, medial dan basal koklea. Dicari hubungan respons dari stimulasi terhadap ketiga faktor internal.Hasil:Pemeriksaan Imp-eABR dilakukan terhadap 28 telinga dari 19 pasien yang dilakukan operasi secara unilateral dan bilateral. Rerata skor total respons Imp-eABR adalah 6,3 + 2,63; dengan 60,7% telinga memiliki skor total kurang dari 8. Uji regresi linear menunjukkan faktor usia, luas area n. koklearis dan kemampuan komunikasi berkorelasi secara bermakna dan dapat memprediksi variasi skor Imp-eABR sebanyak 49,2%. Kesimpulan:Variasi skor total Imp-eABR pada pasien dengan faktor internal pra-operasi menunjukkan adanya patogenesis lain yang mendasari kemampuan transmisi jaras pendengaran retrokoklea terhadap stimulasi listrik dari implan koklea. Faktor internal pra-operasi dapat menjelaskan kurang dari 50% variasi respons Imp-eABR. Kata kunci: Imp-eABR, luas area n. koklearis, implant koklea, integritas jaras auditori ABSTRACT Background:Variations in performance after cochlear implant indicates pathology variations that cannot be explained through standard preoperative examination. Therefore, an examination is needed to show variations in the integrity of auditory pathways as a guide to internal potential variations of each individual. Objectives: To show variations of auditory pathways responses to electrical stimulation from cochlear implant by performing Implanted Electrically evoked Auditory Brainstem Response (Imp-eABR) examination and to find its correlation to age, cochlear nerve area and preoperative communication skills. Methods: Subjects were cochlear implant patients with age less than 10 years, had normal cochlear and cochlear nerve,and patients over 4 years old with history of wearing hearing aids for at least 1 year. The Imp-eABR examination was performed to 5 electrodes representing the apical, medial and basal areas of the cochlea. Results:Imp-eABR examination was performed on 28 ears from 19 patients who underwent unilateral and bilateral surgery. The average total score of Imp-eABR responses was 6.3 + 2.63; with 60.7% of the ears having a total score of less than 8. Linear regression tests showed age,cochlear nerve area,andcommunicationabilitysignificantlycorrelate,it canexplain49.2%ofthevariationinthetotal Imp-eABR score. Conclusion: The variation in the total Imp-eABR score in patients with preoperative internal factors indicated that another pathogenesis also underlines the ability of transmission of the retrocochlear auditory pathway to electrical stimulation from the cochlear implant. Preoperative internal factors can only explain less than 50% of the variation of the Imp-eABR response.

Abstract

Latar belakang: Implan koklea merupakan pilihan utama untuk habilitasi pendengaran dan berbicara pada anak tuli sensorineural berat bilateral. Pengaturan comfortable dan threshold level berdasarkan nilai evoked compound action potential (ECAP) direkam dengan neural responses imaging (NRI) saat pemetaan. Tujuan: Memperoleh nilai ECAP sebagai acuan pemetaan berdasarkan jarak elektroda intrakoklea ke modiolus, jarak terpanjang elektroda nomor satu dengan elektroda berhadapan, jarak marker dengan lubang kokleostomi dan faktor lainnya. Metode: Anak tuli sensorineural usia 2-10 tahun, menggunakan implan koklea dengan desain contour atau straight terdiri dari 16 elektroda, 120 channel sebagai subjek penelitian. Subjek penelitian sebanyak 46 telinga (39 anak), terpasang implankoklea diperoleh secara konsekutif dengan desain potong lintang. Perekaman ECAP elektroda 3-5, 8-10, 13-15 mewakili daerah apeks, medial dan basal. Hasil tomografi komputer resolusi tinggi koklea dengan program OsiriX dilakukan rekonstruksi 3D untuk menilai posisi dan jarak elektroda. Analisis data diawali dengan univariat dan uji korelasi Spearman ‘s pada bivariat. Kandidat faktor yang berperan disertakan pada regresi ganda untuk mendapatkan faktor determinan ECAP. Comfortable zone untuk populasi diperoleh dari analisis area pada distribusi normal menggunakan comfortable level. Hasil: Diperoleh persamaan yaitu: (rerata ECAP)=-21,19+5,87 rerata jarak elektroda ke modiolus (mm)+1.31, rerata threshold level (cu)+0.48 lama penggunaan implan koklea (bulan). (R square=0.60). Comfortable zone diperoleh dengan ECAP yang berada pada variasi 84-87,5% comfortable level. Kesimpulan: Jarak elektroda ke modiolus, lama penggunaan implan koklea dan t level merupakan faktor determinan ECAP. Nilai ECAP dapat digunakan untuk mengidentifikasi penyimpangan jarak elektroda dan memperoleh comfortable zone.Kata kunci : ECAP, implan koklea, lokasi elektroda, tuli sensorineuralABSTRACT Background: Currently cochlear implant remains a preferred choice in hearing and speechhabilitation in children with bilateral profound SNHL. Comfortable and threshold level setting based on ECAP value is recorded by NRI during mapping. Purpose: To obtain ECAP value as mapping guidance based on the distance between electrode to modiolus, the longest distance between electrode number one with the ones it faces, the distance between marker and cochleostomy and other factors. Methods: Research subject were children with SNHL, between 2-10 years old using CI with 16 electrodes, 120 channels. There were 46 ears (39 children) with CI chosen consecutively by cross sectional design. Using NRI, ECAP was recorded on electrode 3-5, 8-10, 13-15 that represent the apex, medial and basal area. Their cochlears were examined with HRCT then 3D reconstruction with OsiriX programto determine the electrode position and calculate the distance. Data analysis started with univariat 1 and bivariat with Spearman’ correlation. Candidates’ factor were analysed with multiregression test to gain ECAP determinant factor. Comfortable zone for population was gained from area analysis in normal distribution using comfortable level. Results: The equation found were: y (average ECAP)=21.19+5.87 the average electrodes to modiolus distance (mm)+1.31, threshold level (cu)+0.48 CI length use (months). (R square=0.60).Comfortable zone was acquired with ECAP between 84-87,5% comfortable level variation. Conclusion: The electrode to modiolus distance, duration of CI use and t level are ECAP determinant factor. The value of ECAP can be used as guidance to identify electrode distance deviation and to gain comfortable zone.Keywords: cochlear implant, ECAP, electrode location, sensoryneural hearing loss

Abstract

Latar belakang: Corpus callosum (CC) merupakan jalur utama yang menghubungkan hemisfer cerebral. Fungsinya adalah mentransfer informasi dari satu hemifer ke hemisfer lainnya dengan menyediakan sarana untuk mengintegrasi informasi dari setiap hemisfer untuk menerima, memahami dan bertindak sepenuhnya atas masukan sensori termasuk auditori yang berdampak pada proses perkembangan bicara. Tujuan: Mengetahui adakah gangguan pendengaran dan proses perkembangan bicara pada anak sebagai Agenesis Corpus Callosum (AgCC) serta penanganannya. Laporan kasus: Dilaporkan dua kasus yang terdiagnosis sebagai agenesis corpus callosum dan dirujuk untuk mengetahui adakah gangguan pendengaran yang diperkirakan akan berdampak pada perkembangan bicara. Pada kasus pertama terdapat Otitis Media Efusi bilateral dengan adanya keterlambatan bicara dan motorik, sedangkan kasus kedua tidak ditemukan adanya gangguan pendengaran. Metode: Berdasarkan telaah literatur berbasis bukti melalui Pubmed, Science Direct, Springer Link, Clinical Key dengan kata kunci agenesis corpus callosum, pemeriksaan pendengaran, gangguan bicara diperoleh 161 literatur. Skrining dilakukan dan diperoleh 12 literatur yang relevan. Pemilihan jurnal 5 tahun terakhir dengan memasukkan kata kunci maka diperoleh 2 jurnal yang sesuai. Kesimpulan: Penegakan diagnosis agenesis CC sering luput karena gejala yang ditimbulkan tidak khas, sehingga kebanyakan kasus baru terdiagnosis bila ada riwayat kejang yang perlu dicari etiologinya, atau adanya dugaan hidrosefalus. Gangguan pendengaran tidak selalu terjadi tetapi permasalahan yang utama adalah tidak terjadinya integrasi informasi sensorik antar hemisfer yang memengaruhi perkembangan bicara. Oleh karena itu kedua kasus tersebut perlu dievaluasi lanjut. Penanganan kasus agenesis CC bervariasi tergantung kelainan yang dialami.Kata kunci: Agenesis corpus callosum, gangguan pendengaran, perkembangan bicaraABSTRACT Background: The corpus callosum is the main pathway connecting the cerebral hemispheres. Its function is to transfer information from one hemisphere to another by providing a means to integrate information from each hemisphere to receive, understand and act fully on sensory input including auditory which has an impact on the process of speech development. Objective: To identify hearing loss and speech development impairment in children as agenesis CC (AgCC) and provide recommendations regarding the treatment needed. Case Report: Two AgCC cases were reported and referred to identify hearing loss that could interfere with speech development. On the first case, bilateral otitis media with effusion was found along with speech and motoric delay. On the second case, hearing problem was not identified. Methods: Evidence-based literature review was performed through Pubmed, Science Direct, Springer Link, Clinical Key with agenesis corpus callosum, hearing examination, speech disorders as keywords produced 161 literatures. Screening was carried out, resulting in 12 relevant literatures. Selection of the last 5 years by entering keywords produced 2 appropriate journals. Conclusion: Diagnosis of CC is often missed because the symptoms are not typical. Most cases are diagnosed when a history of seizures appeared that need to be sought for etiology or the presence of hydrocephalus. Hearing loss is not always the case but the main problem is its impact on speech development. Therefore both cases need to be further evaluated. Treatment of AgCC cases varies depend on the abnormality found.

Abstract

Latar belakang: Sindrom Waardenburg adalah suatu kelainan yang bersifat autosomal dominan yang ditandai oleh gangguan pendengaran sensorineural dan kelainan pigmen pada mata, rambut, dan kulit. Sindrom auditori-pigmen ini terjadi karena tidak adanya melanosit dari kulit, rambut, mata, serta stria vaskularis pada koklea. Tujuan: Memberikan informasi mengenai manifestasi klinis Sindrom Waardenburg sehingga dapat melakukan deteksi dan penanganan sedini mungkin, terutama untuk gangguan pendengaran. Kasus: Seorang anak laki-laki berusia 7 tahun dengan keterlambatan deteksi gangguan pendengaran. Orang tua anak tidak mengetahui bahwa gangguan pendengaran tersebut merupakan salah satu manifestasi sindrom Waardenburg, sehingga pada anak ini terdapat gangguan perkembangan wicara. Metode: Penelusuran literatur menghasilkan 14 jurnal, dan terdapat 2 jurnal yang relevan. Hasil: Dari 2 jurnal yang didapat, ditemukan 1 artikel penelitian dan 1 laporan kasus sindrom Waardenburg yang memaparkan berbagai manifestasi klinis yang menyertai pasien. Kesimpulan: Deteksi dini dan penatalaksanaan untuk gangguan pendengaran yang merupakan manifestasi klinis tersering dari sindrom Waardenburg, penting dilakukan karena mempengaruhi perkembangan individu dan perkembangan sosial pasien. Kata kunci: Sindrom auditori-pigmentasi, kelainan pigmen, gangguan pendengaran sensorineural, sindrom Waardenburg ABSTRACT Background: Waardenburg Syndrome (WS) is an autosomal-dominant disorder, characterized by sensorineural hearing loss and pigmentary abnormalities of the eyes, hair, and skin. Auditory-pigmentary syndromes are caused by physical absence of melanocytes from the skin, hair, eyes, and the stria vascularis of the cochlea. Purpose: To provide information regarding clinical manifestation of Waardenburg Syndrome for early detection and prompt treatment particularly for hearing impairments. Case: A 7-year-old boy with late detection of hearing impairment. Previously, his parents didn’t recognize that this condition was one manifestation of Waardenburg syndrome, so that this patient developed speech delay. Method: Searching for evidence produced 14 journals and only 2 journals were relevant. Result: Of the 2 relevant journals, there were 1 research article and 1 case report that explained several clinical manifestations of Waardenberg Syndrome. Conclusion: Early detection and management of hearing impairment as one of frequent clinical manifestation of Waardenberg Syndrome is significantly important since this condition could influence patient’s individual and social developments. Abstract

ABSTRACTBackground: The appropriate management of patients with congenital deafness is installing hearing aids, either external hearing aids or implanted in the ear (cochlear implant), aiming to reduce the medical and social burden, besides improving the quality of life of the sufferers. Objective: To ascertain the cost of hearing aids in patients with congenital deafness, in the form of external hearing aids or cochlear implants. Method: A descriptive study with cross-sectional design using questionnaires through interviews. The sample size was 535 mothers whose children had congenital deafness at 24 hospitals with facilities for establishing a diagnosis of congenital deafness in 17 provinces in Indonesia. Result: Most respondents were aged 30-39 years (55%), occupations were housewives (71.8%), and education level was high school (52.5%). The type of hearing aid used mostly was external (92.7%), with 45.9% paid by personal expense. The surgically planted hearing aids in 22 children was mostly cochlear implants (95.5%), which were financed by the Indonesian Healthcare and Social Security Agency (BPJS) plus personal costs (50%). Discussion: This study found that the most common type of hearing aid used by children with hearing impairments was external hearing aids (92.7%) through independent financing (45.9%). Only 7.3% of patients chose surgery in hearing habilitation, and 95.5% were cochlear implants. The small percentage of surgery were due to the high-priced of cochlear implants, and the government did not cover all financial expenses. Conclusion: Most external hearing aids were paid independently-out-of-pocket, while cochlear implant surgeries were funded by BPJS, plus extra costs independently. ABSTRAKLatar belakang: Penatalaksanaan terbaik untuk penderita tuli kongenital adalah pemasangan alat bantu dengar (ABD), baik berupa ABD eksternal maupun ABD yang ditanam dalam telinga (implan koklea), dengan tujuan untuk mengurangi beban medis dan sosial, serta meningkatkan kualitas hidup penderita. Tujuan: Untuk mengetahui seberapa besar biaya pemasangan ABD pada penderita tuli kongenital, baik berupa ABD eksternal maupun implan koklea. Metode: Penelitian deskriptif dengan rancangan cross sectional study menggunakan kuesioner melalui wawancara. Besar sampel 535 ibu yang anaknya menderita tuli kongenital pada 24 rumah sakit yang memiliki fasilitas penegakkan diagnosis tuli kongenital di 17 provinsi di Indonesia. Hasil: Sebagian besar responden berusia 30-39 tahun (55%), pekerjaan terbanyak adalah ibu rumah tangga (71.8%), dan tingkat pendidikan SMA (52.5%). Jenis ABD yang terbanyak adalah ABD eksternal (92,7%) dengan pembiayaan secara mandiri 45,9%. Pemasangan ABD dengan tindakan operasi dilakukan pada 22 anak, yang terbanyak adalah implan koklea (95,5%) yang dibiayai oleh Badan Penyelenggara Jaminan Sosial (BPJS) ditambah dengan biaya sendiri (50%). Diskusi: Penelitian ini mendapati bahwa ABD yang terbanyak digunakan oleh anak dengan gangguan pendengaran adalah ABD eksternal (92,7%) dengan biaya mandiri (45,9%). Habilitasi pendengaran dengan tindakan operasi hanya dilakukan pada 7,3% pasien, berupa implantasi koklea 95,5%. Kecilnya persentase habilitasi bedah dikarenakan tingginya harga implant koklea, dan bantuan dari BPJS tidak meliputi keseluruhan biaya. Kesimpulan: Sebagian besar pembiayaan alat bantu dengar eksternal secara mandiri, sedangkan operasi implan koklea menggunakan biaya BPJS ditambah biaya sendiri.

Abstract

ABSTRACTBackground: Lennox-Gastaut syndrome (LGS) is a severe pediatric epilepsy syndrome characterized by multiple seizure types including tonic, atonic, atypical absence, and generalized tonic-clonic seizures. LGS is also associated with cognitive decline and various neural disturbance, including hearing loss. Purpose: To report a case of Lennox-Gastaut syndrome with unilateral hearing loss and delayed speech. Case report: Four years old boy with LGS and delayed speech was referred to the Otolaryngology Outpatient Clinic, Dr Cipto Mangunkusumo Hospital for hearing and speech ability evaluation. He had had repeated seizures since he was three weeks old, and also had cortical lesion and mild atrophy in the left hemisphere brain. Hearing tests with Otoacoustic Emission (OAE), Brainstem Evoked Response Audiometry (BERA), and Auditory Steady State Response (ASSR) revealed profound unilateral hearing loss in the left ear, which required hearing aid and also speech therapy. Clinical question: Is there any correlation between brain abnormality/damage with unilateral hearing loss and delayed speech in Lennox-Gastaut syndrome? Method: Using Pubmed, Proquest Database, and Hand Searching to search the evidence. The evidence selected will be appraised by at least two members of our group using Oxford Center of Evidence-Based Medicine (CEBM) worksheet. Result: Following screening of double publication and its suitability to our clinical questions over ten years, no literature was found. Conclusion: The patient was given antiepileptic drugs (AEDs) and postural control training. For the hearing and speech problems, as the best comprehensive treatment the patient was suggested to use hearing aids and undergo speech therapy program. ABSTRAKLatar belakang: Lennox-Gastaut syndrome (LGS) adalah kumpulan gejala epilepsi berat pada anak dengan ciri khas kejang multipel, tonik, atonik, kejang absans atipik dan tonik klonik menyeluruh. LGS juga berkaitan dengan penurunan kognitif dan berbagai gangguan saraf, termasuk gangguan pendengaran. Tujuan: melaporkan kasus sindrom Lennox-Gastaut dengan gangguan pendengaran satu sisi dan keterlambatan bicara. Laporan Kasus: Seorang anak laki-laki berusia 4 tahun dengan sindroma Lennox-Gastaut mengalami keterlambatan bicara dirujuk ke Departemen Telinga Hidung Tenggorok, Rumah Sakit Dr.Cipto Mangunkusumo untuk evaluasi ambang dengar dan penanganannya. Riwayat penyakit menunjukkan kejang berulang sejak berusia tiga minggu, yang ditemukan juga lesi di korteks serebri dan atrofi ringan pada hemisfer otak kiri. Berdasarkan Otoacoustic Emission (OAE), Brainstem Evoked Response Audiometry (BERA), dan Auditory Steady State Response (ASSR) ditemukan adanya gangguan pendengaran sangat berat unilateral pada telinga kiri dan membutuhkan Alat Bantu Dengar dan terapi wicara. Pertanyaan klinis:Apakah terdapat korelasi antara abnormalitas otak atau kerusakan otak dengan gangguan pendengaran unilateral dan keterlambatan bicara pada LGS? Metode: Pencarian bukti dilakukan melalui PubMed, Proquest, dan pencarian manual. Artikel yang didapatkan kemudian ditelaah oleh setidaknya dua anggota kelompok dengan menggunakan lembar kerja yang diperoleh dari Oxford Center of Evidence-Based Medicine (CEBM) Hasil: Setelah dilakukan skrining publikasi ganda dan kesesuaian dengan pertanyaan klinis dari sepuluh tahun terakhir tidak ditemukan literatur yang sesuai. Kesimpulan:  Pasien ditangani dengan obat antiepilepsi dan latihan psikomotorik. Untuk gangguan pendengaran pasien dianjurkan untuk menggunakan Alat bantu Dengar dan terapi wicara. Penanganan kasus LGS harus menyeluruh untuk memperoleh hasil yang baik.

Abstract

ABSTRACTBackground: Congenital hypothyroidism remains as one of the most common causes of hearing loss in children, considering that thyroid hormone plays an essential role in the development and maturation of auditory organs. Purpose: To report a case of hearing loss in a child with congenital hypothyroidism who had received thyroid hormone replacement therapy. Case report: Presenting a 3-year-old boy with congenital hypothyroidism, growth and developmental delays, and a very severe bilateral sensorineural hearing loss. He was diagnosed at 6 months old, and even after subsequent therapies, the hearing impairment of the child remained profound. Clinical question: Could thyroid hormone replacement therapy improve the prognosis of hearing function in congenital hypothyroid patients accompanied by sensorineural hearing loss? Review method: Literature review through database PubMed, ProQuest, EBSCO, EBSCOhost and Embase, using keywords: “congenital hypothyroidism”, “hearing loss”, and “thyroid hormone replacement therapy”. Result: Screening on 1088 articles found no studies that were relevant with the clinical question and inclusion criteria. Conclusion: This report supported the evidence on the effects of congenital hypothyroidism on hearing loss, and the importance of early diagnosis and prompt treatment in congenital hypothyroid children. A missed diagnosis of congenital hypothyroidism in neonates may result in unfavorable effects on the child’s growth and development. Further studies are needed to assess the improvement of hearing function in congenital hypothyroid patients accompanied by sensorineural hearing loss that received thyroid hormone replacement therapy. ABSTRAKLatar belakang: Hipotiroidisme kongenital merupakan salah satu penyebab paling umum gangguan pendengaran pada anak-anak, terutama mengingat hormon tiroid berperan penting dalam perkembangan dan maturasi organ pendengaran. Tujuan: Melaporkan satu kasus gangguan pendengaran pada anak dengan hipotiroidisme kongenital yang menetap setelah terapi sulih hormon tiroid. Laporan kasus: Seorang anak laki-laki berusia 3 tahun dengan hipotiroidisme kongenital yang mengalami keterlambatan pertumbuhan dan perkembangan, dan gangguan pendengaran tipe sensorineural bilateral derajat sangat berat. Pasien didiagnosis pada usia 6 bulan, dan walaupun setelah terapi, gangguan pendengaran pada anak tersebut menetap. Pertanyaan klinis: Apakah terapi sulih hormon tiroid mempengaruhi prognosis perbaikan fungsi pendengaran pada pasien hipotiroid kongenital yang disertai gangguan pendengaran sensorineural? Tinjauan literatur: Melalui database PubMed, ProQuest, EBSCO, EBSCOhost dan Embase, menggunakan kata kunci: “hipotiroidisme kongenital”, “gangguan pendengaran”, “terapi sulih hormon tiroid”. Hasil: Setelah skrining 1088 artikel, tidak ada penelitian yang sesuai dengan pertanyaan klinis dan kriteria inklusi. Kesimpulan: Laporan ini menambah bukti tentang efek hipotiroidisme kongenital pada gangguan pendengaran, serta pentingnya diagnosis dini dan pengobatan segera pada anak hipotiroid kongenital. Diagnosis hipotiroidisme kongenital yang tidak terdeteksi pada neonatus, dapat mengakibatkan efek buruk pada pertumbuhan dan perkembangan anak. Diperlukan penelitian lebih lanjut untuk menilai prognosis peningkatan fungsi pendengaran pada pasien hipotiroid kongenital dengan gangguan pendengaran sensorineural, yang mendapatkan terapi sulih hormon tiroid.
